Abstract
血液中のヘモグロビン濃度を安定的に目標値に維持し、且つ目標ヘモグロビン濃度に対する変動幅を小さくなし得るESA投与量の決定方法を提供する。 血液中の目標ヘモグロビン濃度を設定して、その目標ヘモグロビン濃度を達成する目標ヘモグロビン産生速度を算出すると共に、ヘモグロビン産生速度とESAの濃度との関係から、かかる目標ヘモグロビン産生速度が達成される血清中のESA濃度を算出し、そしてその血清中のESA濃度を達成するESAの投与量を、ESA濃度とESA投与量との関係から算出して、目標ヘモグロビン濃度を達成するESAの投与量を決定するようにした。
Description
本発明は、赤血球造血刺激因子製剤の投与量決定方法に係り、特に、血液中の目標ヘモグロビン濃度を安定的に維持することの出来る赤血球造血刺激因子製剤の投与量を有利に決定する方法に関するものである。
従来から、骨髄における赤血球の産生は、腎臓で主として産生されるエリスロポエチンによって刺激されるものであるところから、腎臓が荒廃している腎不全患者においては、エリスロポエチンが生成されないか、或いは生成が極めて低下し、そのために、腎不全患者では、赤血球の産生が抑制されて、高度の貧血、所謂腎性貧血が生じることが、知られている。
そこで、透析患者等、腎性貧血の患者に対して、エリスロポエチンを補充するために、遺伝子組み換えにより作製されたエリスロポエチン製剤である赤血球造血刺激因子製剤(ESA;erythropoiesis-stimulating agent)が投与されている。このESAには、エポエチンαやエポエチンβ(EPO)の如き第一世代の薬剤とダルベポエチンα(DA)等の第二世代の薬剤があり、これらの薬剤は、例えば、透析患者においては、透析終了時に血液回路より静注により投与されている。なお、その際のESAの投与量は、医師の経験に基づき、学会のガイドラインで決められている適正な血中ヘモグロビン濃度(10~11g/dL)が実現されるように、適宜に決定されているのであるが、医師の経験に基づくものであるために、ESA投与量が多くなり過ぎたり、少なかったりして、血液中のヘモグロビン濃度が大きく変動することが避けられず、その変動幅を小さくすることは、困難なことであった。
ところで、かかるESAの投与量が多くなって、血液中のヘモグロビン濃度が高くなった場合においては、高価なESAの使用量が増加することによる治療コストの増大という問題だけでなく、むしろ死亡のリスクが増大するという問題も生じる。一方、ESAの投与量が少ないために、血液中のヘモグロビン濃度が低くなり過ぎると、やはり死亡のリスクが増大するという問題があり、更に、血液中のヘモグロビン濃度の変動率が大きい程、死亡のリスクが増大するという報告もなされている(非特許文献1参照)。
上記の医師の経験に基づく方式における問題を解決するために、目標ヘモグロビン濃度と現時点におけるヘモグロビン濃度との差と、過去のESA投与量とから、次回のヘモグロビン濃度の測定時において、目標ヘモグロビン濃度と実際に測定されたヘモグロビン濃度とが一致するようにESA投与量を定めるというアルゴリズムが作成された。即ち、目標ヘモグロビン濃度よりも現時点におけるヘモグロビン濃度が低ければ、ESAの投与量をその差に応じて増やし、逆に目標ヘモグロビン濃度よりも現時点におけるヘモグロビン濃度が高ければ、ESAの投与量をその差に応じて減らすこととするものである(非特許文献2参照)。しかし、実際には、そのような手法では、実測のヘモグロビン濃度は、目標ヘモグロビン濃度を挟んで上下に大きく変動することとなり、その変動幅を小さくすることは困難であった。
さらに、ヘモグロビン濃度が月に1回、測定される場合において、1ヶ月前に測定されたヘモグロビン濃度と現時点におけるヘモグロビン濃度とから、1ヶ月後のヘモグロビン濃度を推定し、この1ヶ月後の推定ヘモグロビン濃度が目標ヘモグロビン濃度よりも低ければ、ESA投与量をその差に応じて増やし、逆に、この1ヶ月後の推定ヘモグロビン濃度が目標ヘモグロビン濃度よりも高ければ、ESA投与量をその差に応じて減らすという方法も報告されている(非特許文献3参照)。しかし、この1ヶ月後の推定ヘモグロビン濃度と目標ヘモグロビン濃度との差から、両者の差を縮小するESA投与量を定める方法をもってしても、目標ヘモグロビン濃度を挟む実測のヘモグロビン濃度の変動を十分に小さくすることは出来なかった。

ここにおいて、本発明は、かかる事情を背景にして為されたものであって、その解決課題とするところは、血液中のヘモグロビン濃度を安定的に目標値に維持して、その変動幅を小さくなし得るESA投与量の決定方法を提供することにある。
そして、本発明は、上記した課題を解決するために、以下に列挙せる如き各種の態様において、好適に実施され得るものであるが、また、以下に記載の各態様は、任意の組み合わせにて採用可能である。なお、本発明の態様乃至は技術的特徴は、以下に記載のものに何等限定されることなく、明細書全体の記載及び図面に開示の発明思想に基づいて、認識され得るものであることが、理解されるべきである。
(1)血液中の目標ヘモグロビン濃度を設定する第一の工程と、その目標ヘモグロビン濃度を達成する目標ヘモグロビン産生速度を算出する第二の工程と、現時点におけるヘモグロビン産生速度と現時点までのESAの濃度との関係から、前記目標ヘモグロビン産生速度が達成されるESAの濃度を算出する第三の工程と、かかるESAの濃度を達成するESAの投与量を、ESA濃度とESA投与量との関係から算出して、前記目標ヘモグロビン濃度を達成するESAの投与量を決定する第四の工程と、を含むことを特徴とするESAの投与量決定方法。
(2)前記目標ヘモグロビン産生速度が、目標総ヘモグロビン量を赤血球の平均寿命にて除した値として、求められる前記態様(1)に記載のESAの投与量決定方法。
(3)前記ヘモグロビン産生速度が前記ESAの濃度の対数値に比例するという関係式を用いて、前記第三の工程におけるESAの濃度の算出が、行なわれる前記態様(1)又は前記態様(2)に記載のESAの投与量決定方法。
(4)前記ヘモグロビン産生速度が前記ESAの濃度に対して直線的に比例するという関係式を用いて、前記第三の工程における血清中のESAの濃度の算出が、行なわれる前記態様(1)又は前記態様(2)に記載のESAの投与量決定方法。
(5)前記第三の工程が、現測定時点におけるヘモグロビン産生速度を求める工程と、前回測定時点から現測定時点までの間における赤血球造血刺激因子製剤の投与量から、前回測定時点から現測定時点までの間の平均赤血球造血刺激因子製剤濃度を算出する工程と、該平均赤血球造血刺激因子製剤濃度と前記ヘモグロビン産生速度とから、ESAの濃度に対するヘモグロビン産生速度の感度aを算出する工程と、かかる感度aと前記目標ヘモグロビン産生速度とから、前記目標ヘモグロビン濃度を達成する平均ESA濃度を算出する工程と、を含んでいる前記態様(1)乃至前記態様(4)の何れか1つに記載のESAの投与量決定方法。
(6)前記算出された目標ヘモグロビン濃度を達成する平均ESA濃度から、前記第四の工程における前記目標ヘモグロビン濃度を達成するESAの投与量が、決定される前記態様(5)に記載のESAの投与量決定方法。
(2)前記目標ヘモグロビン産生速度が、目標総ヘモグロビン量を赤血球の平均寿命にて除した値として、求められる前記態様(1)に記載のESAの投与量決定方法。
(3)前記ヘモグロビン産生速度が前記ESAの濃度の対数値に比例するという関係式を用いて、前記第三の工程におけるESAの濃度の算出が、行なわれる前記態様(1)又は前記態様(2)に記載のESAの投与量決定方法。
(4)前記ヘモグロビン産生速度が前記ESAの濃度に対して直線的に比例するという関係式を用いて、前記第三の工程における血清中のESAの濃度の算出が、行なわれる前記態様(1)又は前記態様(2)に記載のESAの投与量決定方法。
(5)前記第三の工程が、現測定時点におけるヘモグロビン産生速度を求める工程と、前回測定時点から現測定時点までの間における赤血球造血刺激因子製剤の投与量から、前回測定時点から現測定時点までの間の平均赤血球造血刺激因子製剤濃度を算出する工程と、該平均赤血球造血刺激因子製剤濃度と前記ヘモグロビン産生速度とから、ESAの濃度に対するヘモグロビン産生速度の感度aを算出する工程と、かかる感度aと前記目標ヘモグロビン産生速度とから、前記目標ヘモグロビン濃度を達成する平均ESA濃度を算出する工程と、を含んでいる前記態様(1)乃至前記態様(4)の何れか1つに記載のESAの投与量決定方法。
(6)前記算出された目標ヘモグロビン濃度を達成する平均ESA濃度から、前記第四の工程における前記目標ヘモグロビン濃度を達成するESAの投与量が、決定される前記態様(5)に記載のESAの投与量決定方法。
このように、本発明にあっては、目標ヘモグロビン濃度から、この目標ヘモグロビン濃度下におけるヘモグロビン産生速度、換言すれば目標ヘモグロビン産生速度を算出し、次に、かかる目標ヘモグロビン産生速度が得られる血清中のESA濃度を算出し、更に、この血清ESA濃度が得られるESA投与量を算出して、その算出された量のESAを投与することにより、赤血球の寿命であるおおよそ90日後には、血液中のヘモグロビン濃度を目標ヘモグロビン濃度に安定的に到達せしめ得るようにしたものであり、しかも、目標ヘモグロビン産生速度が得られる血清ESA濃度が、現測定時点におけるヘモグロビン産生速度と、前回測定時点から現測定時点までの間の平均ESA濃度との関係に基づいて算出されるため、目標ヘモグロビン産生速度が得られる血清ESA濃度を正確に求めることが出来ることとなるのである。
ところで、ヘモグロビンを運搬する赤血球の寿命は、透析患者では、おおよそ90日である一方、ヘモグロビン濃度の測定間隔は、おおよそ1ヶ月(4週間あるいは5週間)とされており、ヘモグロビン濃度を3回測定する間に、血液中のヘモグロビンは、全て入れ替わることとなる。従って、ヘモグロビンの産生速度が一定であれば、おおよそ90日後には、ヘモグロビン濃度はヘモグロビン産生速度に応じた値で安定することになる。そこで、本発明においては、90日後までには、目標ヘモグロビン濃度が達成される目標ヘモグロビン産生速度が得られるESA濃度を算出し、更に、このESA濃度を達成するESA投与量を算出するようにしたのである。
そこにおいて、毎日同じ量(Gg/日)のヘモグロビンが産生される一方、毎日産生されたのと同じ量のヘモグロビンが除去されている状態を、ヘモグロビン動態の安定状態と定義するならば、ヘモグロビン動態の安定状態では、ヘモグロビン産生速度は、体内のヘモグロビンの総量であるヘモグロビン濃度とヘモグロビンの分布容積(血液量)の積を、ヘモグロビンの寿命、即ちヘモグロビンが産生されてから除去されるまでの日数で割ることにより求められる。これを、以下に更に詳しく説明する。なお、ここで、ヘモグロビンの寿命は、ヘモグロビンの運搬体である赤血球の寿命に等しいところから、以後は、ヘモグロビンの寿命を赤血球寿命と呼ぶこととする。
そして、ヘモグロビン動態が安定している状態が少なくとも赤血球の生存期間であるTR日間以上続いていると仮定すると、ある任意の時点における血液中の最も古いヘモグロビンは、TR日前に産生されたヘモグロビンである。これは、血液中のヘモグロビンは、全て、ある任意の時点以前のTR日以内に産生されたものであることを意味している。従って、ヘモグロビン動態の安定状態では、ある任意の時点において血液中に存在するヘモグロビンの総量( totalHg)は、以下の式(1)にて表わすことが出来る。

一方、血液中のヘモグロビン濃度(CHg)は、上記のヘモグロビン総量を、体重の8%(男性の場合)又は7%(女性の場合)と推定される血液量(BV)で割ったものとして、次式(2)にて表わすことが出来る。

そこで、もし、産生されるヘモグロビン量よりも除去されるヘモグロビン量が多いか、或いは少ない場合、即ちヘモグロビン動態が非安定状態である場合には、ヘモグロビン濃度は低下し、或いは上昇する。このとき、通常は、除去されるヘモグロビン量が増減する結果、ヘモグロビン濃度が低下し、或いは上昇するのではなく、産生されるヘモグロビン量が増減する結果、ヘモグロビン濃度が上昇し、或いは低下することとなるのである。
従って、1日あたりのヘモグロビン産生量をヘモグロビン産生速度と定義すると、ヘモグロビン動態が非安定状態であって、ヘモグロビン濃度が変動している場合、2時点間における生体内の総ヘモグロビン量の差は、2時点間のヘモグロビン産生速度の差が積み重なって生じたものであると言える。従って、2時点間における生体内の総ヘモグロビン量の差を2時点間の日数で割れば、2時点間のヘモグロビン産生速度の差が得られ、その関係は、以下の式(4)にて示される。なお、以下の式(4)において、ΔGは、2時点の
ヘモグロビン産生速度の差を示し、CHg1は、ある任意の時点におけるヘモグロビン濃度を示し、CHg2は、その時点よりも前に測定されたヘモグロビン濃度を示し、そしてBVは血液量、TTは当該2時点間の日数を、それぞれ示している。

れを言い換えると、2時点でのヘモグロビン濃度が等しい場合(ヘモグロビン動態が安定状態にある場合)に比べて、2時点でのヘモグロビン濃度が異なる場合(ヘモグロビン動態が非安定状態にある場合)には、ヘモグロビン産生速度はΔGだけ増加していることに
なる。そこで、非安定状態におけるヘモグロビン産生速度(Greal)は、前記式(3b)により求めたGにΔGを加えてなる式(5a)にて表わされることとなる。

一方、体内にESAを投与する結果、発生する血清ESA濃度は、図1に示すように、指数関数的に減少していくことが認められている。即ち、ESA濃度の減衰曲線は式(6)により示される。なお、そこにおいて、bは定数であり、tは、ESAを投与してからの経過時間となる。また、CESA (t)は時間tにおけるESA濃度、CESA (0)は、ESAを投与した直後の時点、即ちt=0におけるESA濃度を表わしている。

但し、CESA (0)=DESA /VESA であり、DESA はESAの投与量、VESA はESAの分布容積である。定数bには、ESAの種類に応じて、既に報告されているデータから求められる値が用いられ、例えば、ダルベポエチンαの場合においては、b=0.40943が求められ、またエポエチンαやエポエチンβの場合には、b=1.2053が求められている。
ESAの投与頻度について、通常、エポエチンαやエポエチンβは週に3回、月に12~15回、2~3日おきに投与し、ダルベポエチンαは週に1回、月に4~5回、7日おきに投与される。そして、それぞれのESAをこの時間間隔で投与すると、任意の時点におけるESA投与時には、その前のESA投与に伴う血清ESA濃度はほぼゼロにまで低下している。従って、例えば、前回のヘモグロビン測定時点から現ヘモグロビン測定時点までの間にn回、ESAを投与するなら、前回のヘモグロビン測定時点から現ヘモグロビン測定時点までの間のESA濃度の積分平均値である平均ESA濃度(MCESA )は、以下の式(7a)又は(7b)により算出される。

ところで、in vitroの実験によると、赤血球を培養している溶液中におけるヘモグロビン産生速度(G)は、例えば、図2に示される如く、ESA濃度(CESA )の対数値に比例しており(永野伸郎他、「腎と透析」、 60(6),1039-1046,2006 参照)、下記の式(9)にて表わすことが出来る。但し、そこにおいて、aは、定数であって、ESA濃度に対するヘモグロビン産生の感度として認識されるものである。

なお、上記の式(9)では、ESA濃度がゼロに近づくと、ESA濃度の対数値はマイナス無限大に向かって低下していき、従って、Gもマイナス無限大に向かって低下していくこととなる。従って、この式は、極めて低いESA濃度では有効ではないという欠点がある。

なお、上記の式(9)では、ESA濃度がゼロに近づくと、ESA濃度の対数値はマイナス無限大に向かって低下していき、従って、Gもマイナス無限大に向かって低下していくこととなる。従って、この式は、極めて低いESA濃度では有効ではないという欠点がある。
尤も、かかるヘモグロビン産生速度とESA濃度との関係は、患者の血清ESA濃度の通常の範囲内では、直線的な比例関係に近似することも可能である。ヘモグロビン産生速度と血清ESA濃度との関係を直線的な比例関係に近似すると、血清ESA濃度の通常の範囲内ではGの計算値にいくらかの誤差は生じるが、これには、低いESA濃度でも有効であるという利点がある。
さて、式(9)は、生体内でも成り立つものであるところから、かかる式(9)に先の平均ESA濃度を当てはめると、下記式(10a)を得ることが出来る。

ところで、透析患者においては、約1ヶ月毎(4~5週間毎)に透析開始時にヘモグロビン濃度が測定されるのであるが、本発明にあっては、そのようにして測定された最新のヘモグロビン濃度と、前回(1ヶ月前)測定時のヘモグロビン濃度と、前回測定時から今回測定時までの間の1ヶ月間に投与されたESA量とから、次回の採血時に目標ヘモグロビン産生速度が得られるESA投与量を決定し、それを患者に投与するようになっている。そして、次の採血時に目標とするヘモグロビン産生速度が得られるESA投与量を決定するためには、次のような方法が用いられる。
目標ヘモグロビン濃度が安定して続いている状態では、血液中のヘモグロビンは、全て過去90日間に均一な速度で産生され、血液中に貯留したものである。したがって、目標ヘモグロビン産生速度は、目標ヘモグロビン濃度と血液量の積である目標総ヘモグロビン量を赤血球の平均寿命である90日で割ることにより得られる。即ち、次の採血時に生体内に存在していて欲しいヘモグロビン量である目標ヘモグロビン濃度をtargetCHg、目標ヘモグロビン産生速度をtargetG、血液量をBV、赤血球の平均寿命をTR(=90日)とすると、目標ヘモグロビン産生速度は、以下の式(11)により示される。

ところで、かかる式(14b)により、目標ヘモグロビン濃度を達成するESA投与量を算出しようとする場合、現測定時点から目的測定時点までの間に投与するべき個々のESA量(targetDESA 1、targetDESA 2、・・・、targetDESA n)が算出されるのではなく、それらの総量が算出されることとなる。従って、式(14b)により算出された量のESAを患者に投与しようとする場合には、これが現測定時点から目標測定時点までの間に投与されるべきESAの総量であることから、これを必要な投与回数で割って、1回投与量を決定しなければならない。一方、ESAはアンプルに入った状態で販売されており、患者には、アンプル単位のESA量を投与することになる。例えば、エポエチンαとエポエチンβの場合には、750単位入りのアンプル、1500単位入りのアンプル、3000単位入りのアンプルが発売されており、ダルベポエチンαの場合には、10μg入りのアンプル、15μg入りのアンプル、20μg入りのアンプル、30μg入りのアンプル、40μg入りのアンプル、60μg入りのアンプルが発売されている。そして、アンプル単位のESA量は必ずしも、式(14b)により算出された、現測定時点から目標測定時点までの間に投与されるべきESAの総量を必要な投与回数で割ることにより得られた、1回あたりの理論的なESA投与量とは一致しない。
そこで、実際には、式(14b)により算出された、現測定時点から目標測定時点までの間に投与されるべきESAの総量を、必要な投与回数で割ることにより得た、投与すべき1回あたりの理論的なESA量に最も近い量のESAを含むアンプルのESAが投与されることとなるのである。
なお、このような本発明に従うESAの投与量決定方法の具体的な手法を、フローチャートにすると、図3に示されるようになる。即ち、そこでは、先ず、血液中の目標ヘモグロビン濃度が、ガイドラインにて適正とされている血中ヘモグロビン濃度:10~11g/dLの範囲内において、患者に応じて設定され、更にその目標ヘモグロビン濃度、例えば10.5g/dLを達成する目標ヘモグロビン産生速度が、式(11)により求められる。一方、式(5b)により、現測定時点におけるヘモグロビン産生速度を求めると共に、式(8b)によって、前回のヘモグロビン濃度測定時から現ヘモグロビン濃度測定時までの間におけるESA投与量から、前回測定時から現測定時点までの間の平均ESA濃度を算出し、そして、式(10b)により、平均ESA濃度に対するヘモグロビン産生速度の感度aが、算出されるのである。次いで、かかる感度aと目標ヘモグロビン産生速度とから、式(12b)により目標ヘモグロビン濃度を達成する平均ESA濃度が、算出される。その後、目標ヘモグロビン濃度を達成する、現測定時点から目標測定時点までの間のESA投与量が、目標ヘモグロビン濃度を達成する平均ESA濃度から式(14b)により算出されるのである。
このように、本発明に従うESAの投与量決定方法によって算出された量のESA、例えばエポエチンα、βやダルベポエチンαを、目的とする患者に投与することにより、赤血球の寿命であるおおよそ90日後には、目標ヘモグロビン濃度に安定的に到達せしめることが出来ることとなり、以て、患者の貧血の程度を改善し、また、死亡のリスク因子の解消を有利に図ることが出来るのである。
以下に、本発明の代表的な実施例の一つを示し、本発明の特徴を更に明確にすることとするが、本発明が、そのような実施例の記載によって、何等の制約をも受けるものでないことは、言うまでもないところである。
先ず、66歳の男性の患者(体重:53.2kg)に対して、6ヶ月間、従来のアルゴリズムによる投与方法を採用して、目標ヘモグロビン濃度を10.5g/dLとして、ESA(ダルベポエチンα)の投与量をコントロールした。なお、従来のアルゴリズムによるヘモグロビン濃度(Hg)とESA投与量との関係は、以下の通りである。

13.0g/dL≧Hg>12.0g/dL 25~50%減量
12.0g/dL≧Hg>11.0g/dL 25%減量
11.0g/dL≧Hg>10.0g/dL 変更なし
10.0g/dL≧Hg 25~50%増量
そして、その後の6ヶ月間は、同様に、目標ヘモグロビン濃度を10.5g/dLとして、本発明に係る方法に従ってESA(ダルベポエチン)の投与量を決定して、患者に投与した。
かかる患者の1ヶ月毎の採血時に測定されるヘモグロビン濃度(g/dL)とそれに基づいて決定されたダルベポエチン投与量(μg/月)の経過月数による変化を、図4及び図5に示す。
以上の結果より、ESA(ダルベポエチンα)の投与量のコントロールを、従来のアルゴリズムを採用して実施した場合には、ヘモグロビン濃度の平均値は10.9g/dLであり、本発明の方法を採用して実施した場合には、10.7g/dLとなり、それらの間に差はほとんど認められなかったが、従来のアルゴリズムを採用した場合には、標準偏差(SD)と変動係数(CV)は、それぞれ、1.31g/dLと0.12であったのに対し、本発明に従う方法を採用した場合には、標準偏差(SD)と変動係数(CV)は、それぞれ、0.22g/dLと0.02であった。このことは、本発明に従う手法を採用した場合には、ヘモグロビン濃度の変動が小さいことを示しており、従来のアルゴリズムを採用した場合のようにヘモグロビン濃度の変動が大きいと、死亡のリスクが増大することとなるのである。
また、ESA(ダルベポエチンα)の使用量は、従来のアルゴリズムを採用した場合には、54μg±6.2/月であったのに対して、本発明の方法を採用した場合には、51μg±1.9/月であり、本発明の方法の方が、従来のアルゴリズムによる方法に比べて、ESAの使用量を少なく出来ることが明らかとなった。
Claims (6)
- 血液中の目標ヘモグロビン濃度を設定する第一の工程と、
その目標ヘモグロビン濃度を達成する目標ヘモグロビン産生速度を算出する第二の工程と、
現時点におけるヘモグロビン産生速度と現時点までの赤血球造血刺激因子製剤の濃度との関係から、前記目標ヘモグロビン産生速度が達成される赤血球造血刺激因子製剤の濃度を算出する第三の工程と、
かかる赤血球造血刺激因子製剤の濃度を達成する赤血球造血刺激因子製剤の投与量を、赤血球造血刺激因子製剤濃度と赤血球造血刺激因子製剤投与量との関係から算出して、前記目標ヘモグロビン濃度を達成する赤血球造血刺激因子製剤の投与量を決定する第四の工程と、
を含むことを特徴とする赤血球造血刺激因子製剤の投与量決定方法。 - 前記目標ヘモグロビン産生速度が、目標総ヘモグロビン量を赤血球の平均寿命にて除した値として、求められる請求項1に記載の赤血球造血刺激因子製剤の投与量決定方法。
- 前記ヘモグロビン産生速度が前記赤血球造血刺激因子製剤の濃度の対数値に比例するという関係式を用いて、前記第三の工程における赤血球造血刺激因子製剤の濃度の算出が、行なわれる請求項1又は請求項2に記載の赤血球造血刺激因子製剤の投与量決定方法。
- 前記ヘモグロビン産生速度が前記赤血球造血刺激因子製剤の濃度に対して直線的に比例するという関係式を用いて、前記第三の工程における赤血球造血刺激因子製剤の濃度の算出が、行なわれる請求項1又は請求項2に記載の赤血球造血刺激因子製剤の投与量決定方法。
- 前記第三の工程が、
現測定時点におけるヘモグロビン産生速度を求める工程と、
前回測定時点から現測定時点までの間における赤血球造血刺激因子製剤の投与量から、前回測定時点から現測定時点までの間の平均赤血球造血刺激因子製剤濃度を算出する工程と、
該平均赤血球造血刺激因子製剤濃度と前記ヘモグロビン産生速度とから、赤血球造血刺激因子製剤の濃度に対するヘモグロビン産生の感度aを算出する工程と、
かかる感度aと前記目標ヘモグロビン産生速度とから、前記目標ヘモグロビン濃度を達成する平均赤血球造血刺激因子製剤濃度を算出する工程と、
を含んでいる請求項1乃至請求項4の何れか1項に記載の赤血球造血刺激因子製剤の投与量決定方法。 - 前記算出された目標ヘモグロビン濃度を達成する平均赤血球造血刺激因子製剤濃度から、前記第四の工程における前記目標ヘモグロビン濃度を達成する赤血球造血刺激因子製剤の投与量が、決定される請求項5に記載の赤血球造血刺激因子製剤の投与量決定方法。
