Abstract
Método in vitro para modificar el perfil de depleción de células Treg presentes en una población total de esplenocitos de una muestra biológica que comprende aislar, cultivar y exponer los esplenocitos a un medio de ATP y polimixina B, donde los esplenocitos a concentraciones de Polimixina B en el rango de 0,1 □g/ml a 100 □g/ml, y uso combinado de Polomixina B y ATP para preparar un medicamento útil en el tratamiento de tumores o cáncer en mamíferos, donde el cáncer es melanoma.

La presente invención se relaciona con el uso Polimixina B en la depleción de linfocitos T reguladores CD4+CD25 in vitro. Específicamente, como coadyuvante en la inhibición de la proliferación de tumores en ratones. Más particularmente, el uso de polimixina B y ATP en el tratamiento del cáncer.
Campo de Aplicación de la Invención
La presente invención está dirigida al uso de Polimixina B y ATP. Mas particularmente, la presente invención está dirigida al uso de Polimixina B y ATP en el tratamiento del cáncer orientada a la depleción de linfocitos T reguladores CD4+CD25+. Este uso es complementario a aquel de inmunoterapia oncológica aplicada en cáncer de próstata, pulmón, biliar, colon, riñon, páncreas, mama, ovario, piel, recto, hígado, encéfalo, vejiga urinaria, estomago, endometrio, bucofaríngeo, laringe, esófago, cuello uterino.
Descripción del arte previo
Durante la última década se ha registrado un incremento de la incidencia del cáncer a nivel mundial, representando un importante problema en salud pública (ver Redondo, P. (2000). Update on melanoma: incidence, development and biological aspects. An. Sist Sanit Navar. 23 (1): 67-92). En nuestro país, el cáncer, representa la segunda causa de muerte en la población mayor de 20 años, lo que se traduce en 17.266 muertes anuales (ver Medina E., Kaempffer, A. (2000). Mortalidad del adulto en Chile. Rev. Méd. Chile. 128: 1144-49). Modernas técnicas de cirugía y transplantes, nuevos y más efectivos fármacos y mejores métodos de irradiación han permitido tratar con resultados promisorios en algunas de estas patologías (ver Silverman, L.B., Weinstein, H.J. (1997). Treatment of Childhood Leukemia Curr. Opin. Oncol. 9: 2660-8. Review). Actualmente, además de las terapias convencionales para el tratamiento del cáncer, se está utilizando la aproximación inmunoterapéutica, cuyo objetivo es estimular al sistema inmune para montar una respuesta específica contra el tumor a través de vacunas terapéuticas o de
tratamiento, produciendo así su eliminación o reducción. Aunque hay evidencia de que las células tumorales son eficientemente reconocidas por células del sistema inmune después de los tratamientos con vacunas, muchas veces este reconocimiento no es suficiente para eliminar la patología. Este hecho se debe a que distintos canceres humanos son capaces de inhibir activamente el correcto desarrollo de la respuesta inmune, a través de distintos mecanismos como la generación y reclutamiento de linfocitos T reguladores CD4+CD25+ (Tregs) hacia el infiltrado tumoral que inducen inmunosupresión hacia los linfocitos T efectores (ver Regulatory T cells in cáncer, Beyer, 2006 y Identification of a New Subset of Myeloid Suppressor Cells, Filipazzi, 20074,5). En la actualidad existen muchos estudios clínicos en fases avanzadas cuyo fin es modular la respuesta inmunológica de los pacientes con el fin que puedan montar una respuesta inmunológica contra las células tumorales, estos tratamientos se administran con otras formas de "terapia dirigida" como las terapias basadas en reforzar las respuestas inmunitarias contra el cáncer usando modificadores de la respuesta biológica o bien inhibidores de la respuesta inmune regulatoria. Entre estos últimos, varios esfuerzos han sido desarrollados con el fin de generar una terapia dirigida mediante drogas o moléculas las cuales buscan depletar a esta población Tregs CD4+CD25+ en humanos.
Actualmente existen dos "terapias dirigidas" que han probado tener una buena eficiencia en el proceso para depletar a esta población Tregs CD4+CD25+ en humanos. El primero contempla el uso de la Ciclofosfamida, la cual es una droga comúnmente utilizada en inmunoterapia debido a que tiene un efecto citotóxico directo hacia las células tumorales. Además de esta propiedad, recientemente fue descubierto que cuando esta droga es administrada en bajas dosis tiene un efecto inmunomodulador, depletando Tregs sin afectar las poblaciones linfocitarias T helper ni T citotóxica (ver Different mechanisms for anti-tumor effects of low- and high-dose cyclophosphamide. Motoyoshi. 2006). Otro mecanismo de depleción de estas células reguladores fue desarrollado mediante la generación de un fármaco específico llamado ONTAK® o Dinileukin Diftitox. Este corresponde a una proteína de fusión entre IL-2 humana y los dominios activos enzimáticamente de la toxina de la difteria, la cual actúa sobre células que presentan el receptor de alta afinidad por IL-2 (constituido por CD25, CD122 y CD132) en su membrana celular. Una vez que
ONTAK® ¡nteractúa con la membrana, es internalizado por endocitosis e inhibe la síntesis proteica, llevando finalmente a la apoptosis celular (ver IL-2 immunotoxin denileukin diftitox reduces regulatory T cells and enhances vaccine-mediated T-cell immunity. Litzinger. 2007). En ambos casos mayores dosis pueden afectar las poblaciones efectoras de linfocitos disminuyendo a su vez también la respuesta antitumoral.
Recientemente, se ha determinado que las células Tregs CD4+CD25+ expresan diferencialmente una molécula llamada receptor P2X7 (P2X7R) en forma funcional a diferencia de otros linfocitos. El P2X7R es un miembro de los canales de iones P2X ATP-dependientes. Una de las características de este receptor es que a bajas concentraciones de ATP se induce una formación de poros, que se ha asociado a la inducción de muerte celular por apoptosis (ver Aswad F, Kawamura H, Dennert G. High sensitivity of CD4+CD25+ regulatory T cells to extracellular metabolites nicotinamide adenine dinucleotide and ATP: a role for P2X7 receptors. J Immunol. 2005 Sep 1 ;175(5):3075-83). En estos tipos celulares se ha demostrado que dos agonistas no específicos del receptor P2X7, ATP y recientemente NAD, son capaces de inducir la muerte celular mediada por la activación de P2X7, tanto in vivo como in vitro (ver Ferrari D, Pizzirani C, Adinolfi E, Forchap S, Sitta B, Turchet L, Falzoni S, Minelli M, Baricordi R, Di Virgilio F. The antibiotic polymyxin B modulates P2X7 receptor function. J Immunol. 2004 Oct 1 ;173(7):4652-60 y F. Aswad, , Kawamura H., Dennert, High sensitivity of CD4+CD25+ regulatory T cells to extracellular metabolites nicotinamide adenine dinucleotide an ATP: a role for P2X7 receptors, J. Immunol. 175 (2005), pp. 3075-3083). En este contexto se han descrito varios péptidos cíclicos y catiónicos, como la gramicidina y la polimixina B, que son capaces de sensibilizar la activación del receptor P2X7R. Estos polipeptidos en sí son capaces de inducir la producción de interleuquina 1 beta en macrófagos y células dendríticas derivadas de medula ósea de ratón, a niveles comparables con los inducidos por ATP, en forma independiente del receptor P2X7 (ver Allam R, Darisipudi MN, Rupanagudi KV, Lichtnekert J, Tschopp J, Anders HJ. Cyclic Polypeptide and Aminoglycoside Antibiotics Trigger IL-1 b Secretion by Activating the NLRP3 Inflammasome. J Immunol. 2011 Mar 1 ;186(5):2714-8). Por otra parte solo el antibiótico Polimixina B (PMB), un péptido cíclico y catiónico obtenido de la bacteria
Bacillus polymyxa, es capaz de sensibilizar de manera que se requieren menores concentraciones de ATP para conseguir el efecto apoptótico (ver Hubert S, Rissiek B, Klages K, Huehn J, Sparwasser T, Haag F, Koch-Nolte F, Boyer O, Seman M, Adriouch S. Extracellular NAD+ shapes the Foxp3+ regulatory T cell compartment through the ART2-P2X7 pathway. J Exp Med. 2010 Nov 22;207(12):2561-8. Epub 2010 Oct 25) en líneas celulares que expresan P2X7R. Anteriormente, un efecto similar había sido descrito por el grupo de trabajo de Mihich, en líneas celulares, sin embargo, el mecanismo no había sido aclarado. Ellos determinaron que PMB induce muerte celular en líneas de células eucariontes, como las líneas tumorales de linfoma murino EL4 y EL4/ADM, las líneas celulares tumorales C1498 y REH (células de leucemia mieloide aguda) (ver Verstovsek S, Maccubbin DL, Ehrke MJ, Mihich E. Polymyxin B-mediated lysis of tumor cells. Int Arch Allergy Immunol. 1993,100(1 ):47- 52). La gramicidina es un antibiótico polipeptídico producido por la bacteria gram positiva Bacillus brevi, actúa de forma similar a la polimixina como detergente catiónico, alterando la permeabilidad de la membrana citoplasmática bacteriana, lo que produce cambios en la concentración intracelular de los cationes, especialmente de potasio. La Gramicidina se inactiva en suero y en los líquidos corporales y sólo es eficaz por vía tópica, además no es utilizado de forma sistémica, debido a su elevada toxicidad sobre las células eucariontes.
El mecanismo por el cual actúa la polimixina B solo es claro en procariontes, en estos la membrana externa de la pared celular de bacterias Gram negativas está compuesta principalmente por lipopolisacarido (LPS). El LPS está constituido por un motivo lipídico, llamado Lípido A, que está unido covalentemente con un polisacárido. El lípido A es el responsable del alto efecto endotóxico e inmunogénico del LPS en mamíferos. La polimixina B es un péptido catiónico que posee una carga neta igual a 15 y actúa selectivamente sobre las bacterias Gram negativas (ver Hancock REW. Peptide antibiotics. Lancet 1997; 349: 418-22). La polimixina ejerce su actividad bactericida con un mecanismo similar a un detergente, ya que el anillo del péptido policatiónico se une a la membrana externa desplazando el calcio y el magnesio que estabilizan el LPS (15), generando cambios en la permeabilidad (ver Verstovsek S, Maccubbin DL, Ehrke MJ, Mihich E. Polymyxin B-mediated lysis of tumor cells. Int Arch Allergy Immunol. 1993;100(1):47-52) y la disrupción
fisicoquímica de la membrana (ver Hancock REW. Peptide antibiotics. Lancet 1997; 349: 418-22) que permiten el paso de moléculas incluyendo compuestos hidrofóbicos y pequeñas proteínas (15), generando la salida de componentes celulares (ver Tam VH, Schilling AN, Vo G et al. Pharmacodynamics of polymyxin B against Pseudomonas aeruginosa. Antimicrob Agents Chemother 2005; 49: 3624-30) y el ingreso de la polimixina a la célula por una ruta de absorción auto promovida (ver Hancock REW. Peptide antibiotics. Lancet 1997; 349: 418-22) generando la muerte celular (ver Alexandre Prehn Zavascki, Luciano Zubaran Goldani , Jian Li and Roger L. Nation. Polymyxin B for the treatment of multidrug-resistant pathogens:a critical review. Journal of Antimicrobial Chemotherapy (2007) 60, 1206-1215). En eucariontes el mecanismo de acción no es conocido, sin embargo se sabe que seria totalmente dependiente de la cola hidrofóbica (no asociada al ciclo) de este antibiótico (ver Ferrari D, Pizzirani C, Gulinelli S, Callegari G, Chiozzi P, Idzko M, Panther E, Di Virgilio F. Modulation of P2X7 receptor functions by polymyxin B: crucial role of the hydrophobic tail of the antibiotic molecule. Br J Pharmacol. 2007 Feb; 150(4):445-54. Epub 2007).
CN 1706454 divulga un medicamento contra el cáncer con características sinérgicas de extracto de raíz de bupleurum y polimixina siendo la razón peso entre ambos es 26.9-1724.1. La raíz de bupleurum se prepara por inmersión en un solvente obtener un extracto, separando el sobrenadante del precipitado, y secando el mismo. La composición puede inhibir efectivamente el crecimiento de células cancerosas y requiere una menos dosis de polimixina, permitiendo una mejor resistencia al fármaco.
El presente invento se refiere a un uso específico de un compuesto conocido como complementario a tratamientos convencionales contra el cáncer, que permite la inhibición de la respuesta reguladora, depletando los linfocitos T reguladores por facilitación de la activación del receptor purinergico P2X7, usando el efecto sumado de ATP y PMB en la depleción de los linfocitos Tregs CD4+CD25+ como un refuerzo de la inmunoterapia contra el cáncer. El efecto del ATP solo puede relacionarse a muchos efectos adversos en sí.
A continuación se describe la invención en forma detallada y de modo que ella de sustento a todas la características que posteriormente serán reivindicadas.
Uno de los aspectos de la presente invención es el efecto de PMB sobre las subpoblaciones linfocitarias, hasta la fecha solo se había demostrado este efecto sumado en poblaciones de células transformadas, sin una función clara en disminuir la respuesta T reguladora.
El tratamiento con PMB es capaz de inducir muerte en la población CD4+ CD25+, in vitro e in vivo, sin embargo no afecta significativamente a las poblaciones linfocitarias T helper y T citotóxicas. El efecto sobre la población CD4+ CD25+ depende de la dosis, con dosis evaluadas desde 0,001 hasta 1.000 pg/ml.
Los presentes inventores también encontraron que el efecto depletor de PMB ocurre a bajas concentraciones, unas 100 veces menores a las usadas en procedimientos clínicos y terapéuticos, y que es potenciado notablemente in vitro, así como in vivo por la presencia de bajas concentraciones de NAD o ATP, agonistas inespecíficos del receptor P2X7.
La depleción de linfocitos CD4+ CD25+ mediada por PMB, parece ser dependiente en parte de la activación del receptor P2X7. Además es específica para polimixina B, dado que gramicidina otro modulador descrito para P2X7 no induce cambios en la población Treg así como tampoco en la población CD4 convencional.
La invención puede ser administrada en forma de unidades de dosis, inyectadas intramuscular, intradérmica o intraperitonealmente.
El uso de una dosis "efectiva" o "dosis terapéuticamente efectiva" de PMB en los animales en tratamiento no demostró la aparición de efectos secundarios sobre el estado de salud y conducta habitual del modelo animal.
El uso de PMB mejora sustancialmente la inmunoterapia antitumoral basada en vacunas con cuerpos celulares tumorales, incrementando en un 50% la efectividad del tratamiento en un modelo de melanoma murino.
Como se discutió previamente la actividad depletora de PMB sobre la población de células Treg CD4+CD25 puede ser ocupada en forma complementaria a la inmunoterapia contra el cáncer.
Breve Descripción de las Figuras
Las Figuras 1A y 1 B muestran el efecto de PMB sobre las células Treg, CD8 y CD4 presentes en la población total de linfocitos esplénicos. PMB en sí induce muerte de
las células Treg a concentraciones iguales y mayores de 1 pg/ml, sin afectar la población CD8 ni CD4.
Las Figuras 2A y 2B muestran el efecto de gramicidina sobre las células Treg, CD8 y CD4 presentes en la población total de linfocitos esplénicos.
Las Figuras 3A y 3B muestran el efecto de PMB sobre el efecto inducido por ATP. PMB a 1 μg/ml modifica ligeramente el efecto inducido por ATP, desplazando la curva hacia abajo en forma paralela a la curva control (sin PMB), en cambio para PMB a 10 μg /mi, produce un cambio significativo en la curva de respuesta para ATP reflejado en la depleción de las células Treg. Este efecto se observa también sobre la población Foxp3+.
La Figura 4 muestra un gráfico donde bajas dosis de PMB y ATP son inhibidos por los antagonistas no específicos para P2X7, cada barra corresponde al promedio de un mínimo de cuatro experimentos independientes para los tratamientos con PPADS 10 μΜ y BBG 100nM en conjunto con los tratamientos PMB 1 pg/mL, ATP 60 μΜ, y el conjunto de ambos sobre la población CD4+ CD25+. Las barras corresponden al error estándar de los datos analizados en cada caso.
La Figura 5 muestra un gráfico donde BBG 100nM solo inhibe el efecto inducido por ATP 100 μΜ, pero no de PMB 10 pg/mL sobre la población CD4+ CD25+. Las barras corresponden al error estándar de los datos analizados en cada caso.
La Figura 6 muestra un gráfico donde A740003, un antagonista especifico para P2X7, solo inhibe el efecto inducido por ATP 100 μΜ, pero no de PMB 1 , 10 ni 30 pg/mL sobre la población CD4+ CD25+. Las barras corresponden al error estándar de los datos analizados en cada caso.
Las Figuras 7A y 7B muestra que una dosis de PMB + ATP inducen la muerte de los linfocitos T reguladores sistémicos en bazo del animal tratado disminuyendo más de un 50% entre los días 1 y 3 post tratamiento, al día 7 se produce una recuperación parcial. Las otras poblaciones se ven levemente afectadas
La Figura 8 muestra que PMB tiene un comportamiento lineal en la depleción de linfocitos Tregs a una dosis fija de ATP, alcanzando una máxima depleción tras el tratamiento con PMB 350 pg/Kg.
La Figura 9 muestra la incidencia de aparición tumoral, se grafican los tratamientos control, CA B16 solos y CA B16 más ATP y PMB, cada tratamiento fue realizado sobre un número de doce individuos de forma aleatoria sin seleccionar sexo.
Las Figuras 10A y 10B muestran que PMB hasta 400 g totales o ATP hasta 50 pmoles no inducen la muerte de los linfocitos T reguladores sistémicos en bazo del animal al día 3 post desafío. Solo el tratamiento mayor de ATP 50 pg + PMB 40 pg induce una disminución de los Treg in vivo. Las otras poblaciones se ven levemente afectadas.
La Figura 11 muestra el efecto de PMB sobre las células Treg. PMB y ATP en sí se induce muerte de las células Treg, la suma de ambos compuestos incrementa levemente esta disminución.
Objetivos de la invención
El objetivo de la presente invención es el uso de Polimixina B como coadyuvante con ATP o NAD para la depleción de linfocitos T reguladores en mezclas de poblaciones linfocitarias, sin afectar mayormente las poblaciones linfocitas canónicas.
Otro objetivo de la invención son las condiciones in vitro adecuadas para lograr el efecto de depletar los linfocitos T reguladores sin afectar mayormente el resto de las poblaciones linfocitarias.
Aún otro objetivo de la presente invención son las concentraciones que optimizan el efecto de la Polimixina B cuando se usa en forma complementaria en el tratamiento con ATP, en la depleción de linfocitos T reguladores.
Es aún otro objetivo de la presente invención, el mecanismo por el cual la Polimixina B ejerce su efecto en la depleción de linfocitos T reguladores.
Es aún otro objetivo de la presente invención, las condiciones en que se optimiza el efecto Polimixina B cuando se usa como coadyuvante en el tratamiento con ATP para impedir el crecimiento tumoral en ratones a los que se ha inyectado células tumorales singénicas.
Descripción detallada de la invención
Para determinar el efecto in vitro de Polimixina y otro moduladores del receptor: P2X7, se extrajeron bazos de ratones b6 y/o balbc, y los esplenocitos, se aislaron utilizando mediante centrifugación repetida, eliminando los glóbulos rojos por tratamiento con amortiguador de lisis ACK. En general los experimentos fueron
realizados en células obtenidas de animales b6. Los esplenocitos obtenidos se cultivaron en medio RPMI suplementado con antibióticos y suero al 10%. Las células se dejan reposar por 2 horas a una concentración de 1 millón de células/ml, tras lo cual se siembran y se incuban con agonistas purinergicos disueltos en PBS (ATP 0,1 μΜ-1.000 μΜ y NAD 0,01-1.000 μΜ), los moduladores del receptor PMB (0,001 hasta 1.000 pg/ml) y gramicidica (0,001 hasta 100 mg/ml). Para el caso de los antagonistas, estos se usaron a concentraciones que han sido descritas como siendo mas especificas para P2X7 que los otros receptores. Estos se disolvieron en las soluciones sugeridas por los fabricantes y fueron pre-incubados por 30 minutos antes del uso de los agonistas o moduladores.
Las células se cultivaron por diferentes tiempos en medio de cultivo RPMI suplementado con antibióticos (penicilina, estreptomicina y fungizona) con suero fetal bovino al 10% en una estufa de cultivo a 37°C y 5% CO2, lo que llamaremos condiciones estándar. Para evaluar el efecto del tratamiento sobre los linfocitos T reguladores, se tomaron muestras de los tratamientos (independientes) a las 24, 48 y 72 horas post desafío. Los linfocitos recuperados se centrifugaron a 800g y se procesaron para ser marcados con anticuerpos contra CD4, CD8, CD25. En algunos casos fueron permeabilizados para la detección de foxp3 (todas las tinciones se llevaron a cabo médiante la utilización del kit de detección de linfocitos T reguladores de ratón de e-bioscience, de acuerdo a lo sugerido por los fabricantes). Las muestras se analizaron en un clitómetro de flujo y analizaron para las diferentes subpoblaciones a los diferentes tiempos evaluados. Los resultados obtenidos aquí permitieron determinar que PMB in vitro bajo las condiciones descritas, depletan selectivamente los linfocitos T reguladores de esplenocitos.
Para evaluar la actividad complementaria de PMB en la terapia antitumoral, primero se determinó el efecto de esta sobre la población T reguladora in vivo. Para esto, animales de 6-15 semanas y diferentes sexos fueron inyectados subcutáneamente con solución conteniendo PMB sola (entre 4 a 400 pg totales), ATP (entre 0.4 a 40 mg totales) o la mezcla de ambos, en proporciones que no afectaban en sí, la población CD4CD25 foxp3. Los animales se sacrificaron a diferentes tiempos post inyección, se extrajo el bazo, se eliminaron los eritrocitos y se evaluaron las poblaciones de linfocitos T presentes. Los resultados obtenidos aquí permitieron
determinar que PMB inyectado depletan los linfocitos T reguladores de forma transiente.
Para evaluar la actividad complementaria de PMB en la terapia antitumoral, se cultivaron células B16 bajo condiciones estándar en medio de cultivo DMEM con suero fetal bovino al 10% en una estufa de cultivo a 37°C y 5% C02. Las células fueron cultivadas y se sometieron a inducción de muerte por deprivación de medio de cultivo por 96 horas, en la estufa de cultivo. Los cuerpos celulares obtenidos fueron colectados en condiciones de esterilidad y guardados a una concentración estimada considerando 1 millón de células iniciales/ml. Estos cuerpos fueron preparados en conjunto con PMB y ATP que induce disminución de los linfocitos T reg y se inyectaron 3 veces cada 7 días. Después de 7 días, una vez finalizado el protocolo de inmunización, los animales se desafiaron con una 20.000 células b16 vivas obtenidas de cultivos en crecimiento exponencial. El crecimiento tumoral fue evaluado por tacto después de la primera semana diariamente y se midió con un pie de metro. Los resultados obtenidos aquí permitieron determinar que PMB in vivo mas ATP y con cuerpos celulares actúan de forma profiláctica aumentando el numero de animales libres de tumores después del desafío con células b 6 vivas. Los resultados obtenidos permitieron determinar que polimixina B, independiente de su uso como antibiótico para bacterias Gram negativas, es útil en el tratamiento del cáncer actuando como depletor de linfocitos T reguladores en conjunto con ATP o NAD in vitro. Tanto la metodología de administración así como las concentraciones se describen con posterioridad. El uso de esta droga puede ser un mecanismo menos tóxico para la eliminación de los linfocitos T reguladores, lo cual asociado a un tratamiento de inmunoterapia puede inducir una respuesta inmunológica resolutiva contra el cáncer. Esta droga asociada a ATP, permitirá en conjunto con estos nucleótidos disminuir la población de linfocitos T reguladores, y puede ser ocupada con cualquier otra terapia inmune que requiera la depleción de linfocitos T reguladores.
Ejemplos
Ejemplo 1
Efecto de PMB sobre las poblaciones linfocitarias T helper y Treg (ratón).
Esplenocitos 2x106 células/ml obtenidos a partir del bazo de ratones C57/BL6 (6 semanas de edad) se cultivaron por 1 hora en placas de 24 pocilios a 37°C, 5% C02. Posteriormente las células fueron tratadas con PMB (Sigma-Aldrich, St Louis, Montana, USA) a distintos tiempos y concentraciones (0-24-48 y 72 horas, concentraciones desde 0,01 a 1.000 pg/ml). Los tratamientos se mantuvieron por 24, 48 y 72 h, en los cuales las células permanecieron en medio RPMI-1640 suplementado a 37°C 5% C02. Luego de los tratamiento las células fueron cosechadas en amortiguador IF (PBS 2% SFB) frío sin arrastrar la población adherente. Las células obtenidas se bloquearon a 4°C por 30 min en amortiguador IF. Para determinar las subpoblaciones linfocitarias CD4+, CD8+ y CD4+ CD25+ FOXP3+, se incubaron aproximada-mente 106 células con Anti-CD8 Ly-2 (BDpharmigen, Mississauga, Ontario, USA) 4°C por 30 min con agitación constante y los Linfocitos T CD4+ y Treg fueron identificados con el Kit de marcación de células Treg #3 (eBioscience, San Diego, California, USA) según el protocolo del fabricante. Luego de la incubación las células se resuspendieron en amortiguador IF y se analizaron por citometría de Flujo en el equipo FACSCantoll (Beckett Dickinson, Mississauga, Ontario, USA). Se repitieron los mismos experimentos descritos con anterioridad evaluando el efecto de gramicidina, ATP y ATP en presencia de dos concentraciones de PMB.
Este ejemplo establece que el efecto de PMB es específico y determina las condiciones requeridas para depletar la población CD4+ CD25+ FOXP3+.
Ejemplo 2
El efecto de PMB en las células Treg es mediado en parte por P2X7R.
Esplenocitos fueron tratados con PMB (Sigma-Aldrich, St Louis, Montana, USA) a distintos tiempos y concentraciones en presencia de dos moduladores negativos de la actividad de P2X7, PPADS y el antagonista específico de P2X7 Brilliant Blue G (BBG) (ver Brilliant Blue G Selectively Blocks ATP-Gated Rat P2X7 receptors. JIANG, 2000. 12) Las células fueron mantenidas por 24 hrs en condiciones de cultivo celular. Posteriormente las células fueron recolectadas, teñidas y analizadas por citometría de Flujo.
El ejemplo permite concluir que el efecto de PMB sobre la depleción de los linfocitos T reguladores depende parcialmente de la activación del receptor P2X7.
Ejemplo 3
PMB induce la depleción de linfocitos Treg in vivo.
Se realizaron inyecciones subcutáneas en ratones C57/BL6 de 6 semanas de edad con por diferentes días sola cada droga o con PMB 150 pg/Kg y 350 pg/Kg en combinación con una dosis fija de ATP (1 g/Kg); lo cual corresponde a dosis descritas como no dañinas para los humanos. Los ratones fueron sacrificados, se extrajo su bazo y posteriormente se procedió a realizar mareajes con anticuerpos a- CD4, aCD25, a-CD8 y a-Foxp3. (*=p<0,05).
De esta manera, PMB puede potenciar la depleción de los linfocitos Tregs tras una exposición de 72 hrs in vivo.
Ejemplo 4
Evaluación de PMB y ATP en la terapia inmune antitumoral.
Ratones de 6 a 8 semanas de edad fueron tratados con cuerpos apoptoticos (CA) derivados de células de Melanoma murino B16 (2x104), con CA B16 más ATP 500μΜ, CA B16 más PMB 10 mg/mL y con las mezcla de los tres anteriores, el control se realizó con el vehículo de las inmunizaciones PBS 1X estéril.
Las inmunizaciones se realizaron por inyección subcutánea en el flanco derecho, en un volumen final de 50 μί, al día 0, 7 y 21 de tratamiento. El día 0 marca el inicio de los tratamientos. En el día 28 post inyección se realiza el desafío tumoral. Para ello, se inyectaron 104 células tumorales vivas en el flanco izquierdo del individuo en un volumen final de 50 μί en PBS 1X estéril.
Los ratones fueron observados diariamente registrando actividad, peso y aspecto de acuerdo a los parámetros descritos por Morton y Griffiths, 1985 (ver Morton DB, Griffiths PH. Guidelines on the recognition of pain, distress and discomfort in experimental animáis and an hypothesis for assessment. Vet Rec. 1985 Apr 20;116(16):431-6). La aparición tumoral fue registrada y seguida hasta un volumen tumoral de 0,6 cm3, en el cual los animales fueron eutanasiados por dislocación cervical.
El uso de cuerpos celulares confiere inmunidad protectora en el 20% de los animales al día 20 posterior a la inoculación de las células tumorales. El tratamiento inmunoterapéutico con apoyo consistente en la adición de PBM solamente confiere una protección del 60% de los animales desafiados. La adición de PMB y ATP
permite alcanzar protección del 00% de los animales al día 20. La evaluación al día 60 post inoculación entrega porcentajes de protección de 0%, 20% y 50% respectivamente.
De este ejemplo, se concluye que el tratamiento de los cuerpos celulares más ATP o PMB, inducen profilaxis contra el desarrollo del tumor b 6 en ratones y que la suma de ambos compuestos mejora esta profilaxis.
Ejemplo 5
PMB induce la depleción de linfocitos Treg humanos in vivo.
Se realizaron inyecciones subcutáneas en ratones C57/BL6 de 6 semanas de edad con diferentes concentraciones de PMB, ATP o mezclando concentraciones de ambas. Los ratones fueron sacrificados, se extrajo su bazo y posteriormente se procedió a realizar mareajes con anticuerpos o CD4, aCD25, a-CD8 y a-Foxp3. (*=p<0,05).
Ejemplo 6
Efecto de PMB sobre las poblaciones linfocitarias Treg humanas.
PBML 2x106 células/ml obtenidos a sangre periférica extraídas por centrifugación en gradiente se cultivaron bajo condiciones estándar. Posteriormente las células fueron tratadas con PMB (Sigma-Aldrich, St Louis, Montana, USA). Los tratamientos se mantuvieron por 24 h, en los cuales las células permanecieron en medio RPMI- 640 suplementado a 37°C 5% C02. Luego de los tratamiento las células fueron cosechadas en buffer IF (PBS 2% SFB) frío. Las células obtenidas se bloquearon a 4°C por 30 min en amortiguador IF. Para determinar las sub poblaciones linfocitarias CD4+, CD8+ y CD4+ CD25+ FOXP3+. Luego de la incubación las células se resuspendieron en buffer IF y se analizaron por citometría de Flujo en el equipo FACSCantoll (Beckett Dickinson, Mississauga, Ontario, USA).
Claims
1.- Método in vitro para modificar el perfil de depleción de células Treg presentes en una población total de esplenocitos de una muestra biológica, que comprende aislar, cultivar y exponer los esplenocitos a un medio de ATP y polimixina B.
2.- El método según la reivindicación 1 , que comprende exponer los esplenocitos a concentraciones de Polimixina B en el rango de 0,1 Dg/ml a 1.000 Dg/ml.
3.- Uso combinado de Polomixina B y ATP, que sirve para preparar un medicamento útil en el tratamiento de tumores o cáncer en mamíferos.
