Abstract
Description
タキサン系抗腫瘍剤は、細胞分裂に必要な微小管の脱重合を阻害することで、癌細胞の分裂を阻害することが知られている。パクリタキセルは卵巣癌、非小細胞肺癌、乳癌、胃癌等の治療に、ドセタキセルは乳癌、非小細胞肺癌、胃癌、頭頚部癌、卵巣癌、食道癌、子宮体癌等の治療に使用されている。
臨床の場においては、薬効の増強や抗癌スペクトラムの拡大を目的に、タキサン系抗腫瘍剤と作用機序の異なる他の抗腫瘍剤を組み合わせた多剤併用療法が行われている。例えば、パクリタキセルとカルボプラチンの併用(Cancer, 1996, 77:2458-63.)や、ドセタキセルとシスプラチン等の併用が知られている(Eur. J. Surg. Oncol. 2006 32 (3) 297-302)。また、アメリカ食品医薬品局(FDA)が提供する各種データベース、例えば、"Drugs@FDA" (http://www.accessdata.fda.gov/scripts/cder/drugsatfda/index.cfm)や欧州医薬品庁(EMA)のホームページ(http://www.ema.europa.eu/)には、パクリタキセルやドセタキセルと他の抗腫瘍剤を併用する臨床レジメンが開示されている。
非特許文献9及び10には、ヒト卵巣癌細胞や頚頭癌細胞において、低濃度のNOはシスプラチン及びパクリタキセルによるアポトーシスを抑制する傾向を示し、高濃度のNOはアポトーシスを促進することが記載されている。これらの文献には、iNOS阻害剤1400WとサーバイビンRNAiの併用によってパクリタキセルだけでなくシスプラチンによるアポトーシスを促進することも記載されており、NOの細胞保護効果はパクリタキセル選択的な作用ではないといえる。
即ち、本発明は、
(1) タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と組み合わせて併用投与するように用いられることを特徴とする、FK330又はその塩を有効成分として含有する癌治療用組成物に関する。
(2) パクリタキセル又はドセタキセルと組み合わせて併用投与するように用いられることを特徴とする(1)記載の組成物。
(3) パクリタキセルと組み合わせて併用投与するように用いられることを特徴とする(2)記載の組成物。
(4) ドセタキセルと組み合わせて併用投与するように用いられることを特徴とする(2)記載の組成物。
(5) 60~1200mg/日のFK330又はその塩を含有する経口投与用組成物である(1)乃至(4)記載の組成物。
(6) 60~200mg/日のFK330又はその塩を含有する経口投与用組成物である(5)記載の組成物。
(7) 更に、プラチナ系抗腫瘍剤、アントラサイクリン系抗腫瘍剤、シクロフォスファミド、フルオロウラシル、トラスツズマブ及びカペシタビンから選択される1以上の抗腫瘍剤と組み合わせて併用投与するよう用いられることを特徴とする(1)乃至(6)記載の組成物。
(8) タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の1投与サイクル中の少なくとも1日において併用投与するように用いられることを特徴とする、(1)乃至(6)記載の組成物。
(9) タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の投与日の少なくとも1日において併用投与するように用いられることを特徴とする、(8)記載の組成物。

(17)タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の併用治療に用いる場合の治療有効用量、並びに、FK330又はその塩の併用治療に用いる場合の治療有効用量を、同時に、別々に、連続して、或いは間隔をあけて、対象に投与することを特徴とする癌の治療方法。
なお、「対象」とは、その予防若しくは治療を必要とするヒト又はその他の動物であり、ある態様としては、その予防若しくは治療を必要とするヒトである。
(18)タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と組み合わせて併用投与するように用いられることを特徴とする癌治療用組成物を製造するための、FK330又はその塩の使用。

乳癌の別の態様としては、例えば、HER2陽性転移性乳癌、HER2陰性転移性乳癌、HER2陽性乳癌、或いは、転移性乳癌が挙げられ、別の態様としては、HER2陰性転移性乳癌が挙げられ、また別の態様としては、HER2陽性転移性乳癌が挙げられる。
非小細胞肺癌の別の態様としては、例えば、局所進行性非小細胞肺癌、或いは、転移性の非小細胞肺癌が挙げられ、別の態様としては、非小細胞肺癌における非扁平上皮癌が挙げられ、また別の態様としては、転移性の非小細胞肺癌における非扁平上皮癌が挙げられ、さらに別の態様としては、転移性の非小細胞肺癌における扁平上皮癌が挙げられる。
胃癌の別の態様としては、転移性胃癌、転移性胃食道接合部癌等が挙げられる。
前立腺癌の別の態様としては、転移性去勢抵抗性前立腺癌、ホルモン抵抗性前立腺癌等が挙げられ、別の態様としては、ホルモン抵抗性前立腺癌が挙げられ、また別の態様としては、転移性去勢抵抗性前立腺癌が挙げられる。
本発明で使用されるFK330又はその塩としては、ある態様としてはフリー体無水物である。別の態様としてはフリー体2水和物である。例えば、国際公開第02/055541号パンフレットの実施例41に記載の方法により得られるFK330フリー体を、アルコール/水溶媒系、例えばエタノール/水、又はイソプロパノール/水等にて再結晶を行い、得られた結晶を減圧下乾燥させるとフリー体無水物を得ることができる。また、得られたフリー体無水物に対して調湿操作を行うことにより、フリー体2水和物結晶を得ることができる。
投与は錠剤、丸剤、カプセル剤、顆粒剤、散剤、液剤等による経口投与、又は、関節内、静脈内、筋肉内等の注射剤等による非経口投与のいずれの形態であってもよい。ある態様としては、錠剤、丸剤、カプセル剤、顆粒剤、散剤、液剤等による経口投与である。
経口投与のための液体組成物は、薬剤的に許容される乳濁剤、溶液剤、懸濁剤、シロップ剤又はエリキシル剤等を含み、一般的に用いられる不活性な希釈剤、例えば精製水又はエタノールを含む。当該液体組成物は不活性な希釈剤以外に可溶化剤、湿潤剤、懸濁剤のような補助剤、甘味剤、風味剤、芳香剤、防腐剤を含有していてもよい。
FK330と併用可能なタキサン系抗腫瘍剤の具体的な別の態様としては、パクリタキセル(pactitaxel)、ドセタキセル(docetaxel)、カバジタキセル(cabazitaxel)、アブラキサン(abraxane)、テセタキセル(tesetaxel)、オルタタキセル(ortataxel)等が挙げられ、また別の態様としては、パクリタキセル(pactitaxel)、ドセタキセル(docetaxel)、又はアブラキサン(abraxane)が挙げられ、さらに別の態様としては、パクリタキセル(pactitaxel)又はアブラキサン(abraxane)が挙げられ、またさらに別の態様としては、パクリタキセル(pactitaxel)が挙げられ、別の態様としては、ドセタキセル(docetaxel)が挙げられる。
カバジタキセルは、また、国際公開96/30355号パンフレットに開示される製造方法又はそれらの変法によって容易に入手可能である。テセタキセルは、当業者にとって自明である方法、或いは、国際公開01/27115号パンフレットに開示される製造方法又はそれらの変法によって容易に入手可能である。オルタタキセルは、当業者にとって自明である方法、或いは、米国特許5705508号明細書に開示される製造方法又はそれらの変法によって容易に入手可能である。
例えば、パクリタキセルの用法・用量の例としては、日本の医薬品医薬機器情報提供ホームページのタキソール注(登録商標:パクリタキセルの商品名)の添付文書には、「[効能又は効果]卵巣癌、非小細胞肺癌、乳癌、胃癌;[用法及び用量];1.通常、成人にはパクリタキセルとして、1日1回210mg/m2を3時間かけて点滴静注し、少なくとも3週間休薬する。これを1クールとして投与を繰り返す。なお、投与量は、年齢、症状により適宜減量する。」と記載される。
また、ドセタキセルの用法・用量の例としては、日本の医薬品医薬機器情報提供ホームページのタキソテール注(登録商標:ドセタキセルの商品名)の添付文書には、「1.乳癌、非小細胞肺癌、胃癌、頭頸部癌;効能又は効果毎の用法及び用量;1.通常、成人に1日1回、ドセタキセルとして60mg/m2を1時間以上かけて、3~4週間間隔で点滴静注する。なお、症状により適宜増減すること、ただし、1回最高用量は70mg/m2とする」と記載される。
FK330とタキサン系抗腫瘍剤を併用投与する場合の投与形態としては、それぞれに適した投与経路、投与頻度及び投与量を採用する限りは特に限定されず、例えば、(1)FK330とタキサン系抗腫瘍剤とを含有する組成物、即ち、単一の製剤としての投与、(2)FK330とタキサン系抗腫瘍剤とを別々に製剤化して得られる2種の製剤の同一投与経路での同時投与、(3)FK330とタキサン系抗腫瘍剤とを別々に製剤化して得られる2種の製剤の同一投与経路での時間差をおいての投与(例えばFK330、タキサン系抗腫瘍剤の順序での投与、あるいは逆の順序での投与)、(4)FK330とタキサン系抗腫瘍剤とを別々に製剤化して得られる2種の製剤の異なる投与経路での同時投与、(5)FK330とタキサン系抗腫瘍剤とを別々に製剤化して得られる2種の製剤の異なる投与経路での時間差をおいての投与(例えばFK330、タキサン系抗腫瘍剤の順序での投与、あるいは逆の順序での投与)等が挙げられる。ある態様としては、本発明の癌治療用組成物はタキサン系抗腫瘍剤とは別に製剤化し、タキサン系抗腫瘍剤は静注、点滴静注若しくは経口により投与され、本発明の癌治療用組成物は経口若しくは非経口で投与される。別の態様としては、タキサン系抗腫瘍剤は静注、点滴静注により投与され、本発明の癌治療用組成物は経口で投与される。
2種類の製剤は、例えば、各製剤のバイオアベイラビリティー、安定性等を考慮し、それぞれの製剤に適した製剤処方、投与経路、投与頻度等の投与条件下にて、同一又は異なる投与ルートで同時、別々に、連続して、或いは間隔を空けて投与されるものである。ここで、「同時に」とは、第一製剤と第二製剤を一緒に同じ投与経路で投与することを意味し、「別々に」とは、第一製剤と第二製剤を同一若しくは異なる投与経路で、同一若しくは異なる投与頻度又は投与間隔で、別々に投与することを意味する。「連続して」とは、第一製剤と第二製剤を同一若しくは異なる投与経路で、第一製剤に続いて第二製剤の順序、あるは逆の順序で投与することを意味する。「間隔を空けて」とは、第一製剤と第二製剤を同一若しくは異なる投与経路で、第一製剤に続いて一定の間隔を空けて第二製剤の順序、あるは逆の順序で投与することを意味する。
また、本発明のFK330又はその塩は、多くの抗腫瘍剤に報告されるような重篤な副作用を有さず、安全性の点からも有用である。
実施例に示した動物モデルでは、本発明の癌治療用組成物はタキサン系抗腫瘍剤選択的、かつ用量依存的に抗腫瘍作用を増強することが確認された。即ち、FK330又はその塩のみの場合は、有意な抗腫瘍効果が見られず、また、FK330又はその塩をタキサン以外の抗腫瘍剤と併用しても、格別顕著な抗腫瘍作用増強作用は確認されないが、タキサン系抗腫瘍剤と組み合わせた場合は良好な抗腫瘍作用増強作用が確認された。殊に、FK330は複数サイクル処置によるタキサン系薬剤の抗腫瘍効果の減弱を改善し、腫瘍増殖を良好に抑制することが示された(図7参照)。これはFK330がタキサン系抗腫瘍剤の耐性化を抑制することを示唆する。
また、本発明者らは、NOは、タキサン系抗腫瘍剤によるアポトーシスを介した癌細胞増殖抑制作用を抑制し、NOによる化学療法剤の減弱効果は、タキサン系抗腫瘍剤に選択的に認められることも見出している。
従って、本発明者らは、FK330によるタキサン系抗腫瘍剤の抗腫瘍作用増強のメカニズムとして以下を明らかにした。すなわち、タキサン系抗腫瘍剤は、腫瘍組織でiNOS蛋白のを亢進し,その結果NO産生を増強する。産生されたNOは、タキサンの微小管脱重合阻害作用を直接的に抑制することでタキサン系抗腫瘍剤の抗腫瘍効果の減弱を引き起こす。一方、FK330は、タキサン系抗腫瘍剤によって増強されたNO産生を阻害することにより、タキサン系抗腫瘍剤の効果の減弱を抑制して抗腫瘍作用を増強できることを明らかにしたものである。
以上のことから、FK330又はその塩は、微小管脱重合阻害作用をターゲットとするすべてのタキサン系抗腫瘍剤の作用を増強できるといえる。
実施例1
1) 被験物質
FK330としてはフリー体2水和物を用いた。以下、FK330の投与量は当該2水和物の重量で表示した。ドセタキセル水和物注射剤(タキソテール(商標)点滴静注用)はサノフィ-アベンティス株式会社から購入し、ドセタキセルとして使用した。パクリタキセル注射剤(パクリタキセル注射液「サワイ」)は沢井製薬株式会社から購入し、パクリタキセルとして使用した。塩酸イリノテカン(トポテシン(商標)注)は第一三共株式会社から購入し、イリノテカンとして使用した。注射用ゲムシタビン塩酸塩(ジェムザール(商標)注射用)は日本イーライリリー株式会社から購入し、ゲムシタビンとして使用した。注射用塩酸ドキソルビシン(アドリアシン(商標)注用10)は協和醗酵工業株式会社から購入し、ドキソルビシンとして使用した。カルボプラチン注射液(パラプラチン(商標)注射液)はブリストル・マイヤーズ株式会社から購入し、カルボプラチンとして使用した。
2) 被験物質の調製及び投与
FK330は0.5%メチルセルロース水溶液(0.5% MC)に懸濁した(20mg/mL)。ドセタキセルは添付溶解液にて4mg/kgに調製した後、生理食塩液(大塚製薬)で1mg/mLに希釈した。パクリタキセル、イリノテカン、ゲムシタビン、ドキソルビシン及びカルボプラチンは注射用生理食塩水にてそれぞれ、1mg/mL、3mg/mL、16mg/mL、1mg/mL及び6mg/mLに用時調製した。
3) 細胞
ヒト肺癌由来Calu 6 (HTB-56)はAmerican Type Culture Collection (VA, USA)より入手した。細胞は10%の加熱不活性化したウシ胎仔血清(FBS)が添加されたRPMI1640培地を用いて、37℃、5% CO2の条件で培養した。トリプシンを用いて回収した細胞をPBSで6x107 cells/mLに懸濁し、等量のMatrigel(商標) Basement Membrane Matrix (Becton Dickinson Co.製, Bedford, MA, USA)と混合した。
5週齢の雄性ヌードマウス(CAnN Cg-Foxn1nu/CrlCrlj(nu/nu))は日本チャールスリバー社(日本、神奈川)より購入した。動物は試験期間を通じて特定病原体未感染(SPF)条件下で標準的な飼料と飲水を与えて飼育した。培養した細胞を3x106 cells/0.1 mL/mouseでヌードマウスの背部皮下に移植した。腫瘍体積(短径 x [長径]2 x 0.5)が111.9 から 369.3 mm3になったヌードマウスを群間および群内の腫瘍体積ばらつきが小さくなるようにSASを用いて群分けした。
5) 薬物投与および測定
投与初日をDay1としてDay29まで観察を行った。それぞれの群 (n=5又は6)を以下の様に処理した。FK330は1日2回、6から8時間間隔で強制経口投与(5mL/kg)し、各化学療法剤は尾静脈より静脈内ボーラス投与(10mL/kg)した。
対照群:未処置

FK330は1日2回、6から8時間間隔で強制経口投与(5mL/kg)した。各化学療法剤はFK330投与後2~4時間後に尾静脈より静脈内ボーラス投与(10mL/kg)した。
3~4日ごとに体重とデジタルノギスを用い腫瘍径を測定した。腫瘍体積は(短径 x [長径]2 x 0.52)の楕円体積の計算式より算出した。
結果は平均値(Mean)±標準誤差(SEM)で示した。試験期間中、触診限界以下を完全消失(CR)として判定し、測定値は0とした。試験終了時の腫瘍体積の実測値について、単独投与群と併用群で対応のない2群間の差の検定(Student's t-test)を行い、5%未満を有意差ありと判定した。データ処理にはSASまたはGraphpad Prism version 5.03を用いた。
7) 結果
FK330と各種化学療法剤を併用投与した時の抗腫瘍作用を検討した。結果は図1から図6に示した。即ち、FK330とドキソルビシンの併用効果は図1に、ゲムシタビンとの併用効果は図2に、イリノテカンとの併用効果は図3に、カルボプラチンとの併用効果は図4に、パクリタキセルとの併用効果は図5に、ドセタキセルとの併用効果は図6にそれぞれ示した。FK330単独では抗腫瘍効果は認められなかったものの、抗腫瘍効果の有意な増強がパクリタキセル(Day29でのパクリタキセル単独群に対する腫瘍体積減少率57.6%)及びドセタキセル(Day29でのドセタキセル単独群に対する腫瘍体積減少率65.2%)で確認された。一方、FK330はゲムシタビン及びカルボプラチンの抗腫瘍効果には影響を与えず、抗腫瘍効果の若干の増強がドキソルビシン(Day29でのドキソルビシン単独群に対する腫瘍体積減少率34.5%)及びイリノテカン(Day29でのイリノテカン単独群に対する腫瘍体積減少率34.1%)で観察されたが、有意な作用ではなかった。
8) 結論
FK330は、単独では抗腫瘍効果は認められなかったものの、パクリタキセル及びドセタキセルの抗腫瘍効果を増強させた。一方、イリノテカン、ゲムシタビン、ドキソルビシン及びカルボプラチンの抗腫瘍効果には影響を与えなかった。これらの結果は、FK330はタキサン系抗腫瘍剤の抗腫瘍効果を選択的に増強することを示唆するものである。
1) 被験物質
FK330の用量は水和物を含む重量で表示した。パクリタキセル注射剤(パクリタキセル注射液「サワイ」)は沢井製薬株式会社から購入し、パクリタキセルとして使用した。
2) 被験物質の調製及び投与
FK330は0.5%メチルセルロース水溶液(0.5% MC)に懸濁した(20mg/mL)。パクリタキセルは注射用生理食塩水(大塚製薬)にて1mg/mLに用時調製した。
3) 細胞
ヒト肺癌由来Calu 6 (HTB-56)はAmerican Type Culture Collection (VA, USA)より入手した。細胞は10%の加熱不活性化したウシ胎仔血清(FBS)が添加されたRPMI1640培地を用いて、37℃、5% CO2の条件で培養した。トリプシンを用いて回収した細胞をPBSで6x107 cells/mLに懸濁し、等量のMatrigel(商標) Basement Membrane Matrix (Becton Dickinson Co.製, Bedford, MA, USA)と混合した。
5週齢の雄性ヌードマウス(CAnN Cg-Foxn1nu/CrlCrlj(nu/nu))は日本チャールスリバー社(日本、神奈川)より購入した。動物は試験期間を通じて特定病原体未感染(SPF)条件下で標準的な飼料と飲水を与えて飼育した。培養した細胞を3x106 cells/0.1 mL/mouseでヌードマウスの背部皮下に移植した。腫瘍体積(短径 x [長径]2 x 0.5)が126.1 から 299.0 mm3になったヌードマウスを群間および群内の腫瘍体積ばらつきが小さくなるようにSASを用いて群分けした。
5) 薬物投与および測定
投与初日をDay1としてDay83まで観察を行った。それぞれの群 (n=6)を以下の様に処理した。FK330は5mL/kgで強制経口投与を行い、パクリタキセルは10mL/kgで尾静脈内投与を行った。パクリタキセルはDay1からDay7の連続投与を行った後24日間の休薬した(1サイクル)。PTXの2サイクル及び3サイクル目の投与では、それぞれDay31またはDay60から6日間連続投与した。
対照群:未処置

FK330のパクリタキセル複数サイクル投与時の併用作用を検討した。結果は図7に示した。ヒト非小細胞肺癌(Calu-6)担癌マウスにおいて、複数サイクル投与するにつれてパクリタキセルの抗腫瘍効果は減弱する傾向が認められた。FK330はパクリタキセルの抗腫瘍効果を著明に増強し、Day29でのパクリタキセル単独群に対する腫瘍体積減少率は57.6%(P<0.01)、Day60でのパクリタキセル単独群に対する腫瘍体積減少率は86.1%(P<0.01)、Day83でのパクリタキセル単独群に対する腫瘍体積減少率は87.6%(P<0.01)であった。
8) 結論
ヒト非小細胞肺癌(Calu-6)担癌マウスにおいて、パクリタキセルの抗腫瘍効果は、サイクル依存的に減弱した。一方FK330の併用は、複数サイクル投与によるパクリタキセルの抗腫瘍効果を増強し、腫瘍の再増殖を著明に抑制した。この結果は、FK330が複数サイクル処置によるタキサン系薬剤の抗腫瘍効果の減弱を改善することを示唆するものである。
被験物質の調製、及び細胞の調整は実施例2と同様に行った。

2) 薬物投与および測定
投与初日をDay0としてDay59まで観察を行った。それぞれの群 (n=5)を以下の様に処理した。FK330は5mL/kgで強制経口投与を行い、パクリタキセルは10mL/kgで尾静脈内投与を行った。パクリタキセルはDay0からDay5の連続投与後に21日の休薬期間を設定し、これを1サイクルとして2サイクルまで投与した。
対照群:未処置

FK330のパクリタキセルの抗腫瘍効果増強作用の用量依存性を検討した結果を図8に示した。結果は平均値(Mean)± 標準誤差(SEM)で示した(n=4から5)。試験終了時(Day59)の腫瘍体積は、パクリタキセル単独群では1640.1±902.8 mm3、FK330 30mg/kg bid併用群では、955.9±231.0 mm3、FK330 60mg/kg bid併用群では706.2±256.1 mm3、FK330 100mg/kg bid併用群では297.9±101.4 mm3であり、FK330は用量依存的にパクリタキセルの抗腫瘍効果を増強した(パクリタキセル単独群に対する腫瘍体積減少率:FK330 30mg/kg bidでは41.7%、FK330 60mg/kg bidでは56.9%、FK330 100mg/kg bidでは81.8%)。
4) 結論
FK330の併用は、用量依存的にパクリタキセルの抗腫瘍効果を増強した。
被験物質の調製、及び細胞の調整は実施例2と同様に行った。

試験期間中、触診限界以下を完全消失(CR)として判定し、測定値は0とした。試験終了時(Day55)の腫瘍体積は、パクリタキセル単独群では1229.2±289.6 mm3、パクリタキセルとFK330の同時投与では、559.8±161.8 mm3、FK330の順次投与では1150.9±264.9 mm3、FK330の連続投与では168.5±66.0 mm3であり、それぞれ、パクリタキセル単独群に対する腫瘍体積減少率は、54.5%、6.4%及び86.3%であった。
4) 結論
FK330の投与タイミングはタキサン系薬剤の投与開始日よりサイクル期間中連日投与した場合が最も併用効果が高いことが示唆された。
被験物質の調製は実施例2と同様に行った。

ヒト胃癌由来NCI-N87(CRL-5822)はAmerican Type Culture Collection (VA, USA)より、MKN-28(JCRB0253)は医薬基盤研究所(HSRRB)より、AZ-521(RCB2087)は理研BRCより入手したものを使用した。NCI-N87及びMKN-28細胞は10%の加熱不活性化したウシ胎仔血清(FBS)が添加されたRPMI1640培地を用いて、AZ-521は10%の加熱不活性化したFBSを含むDMEM培地を用いて、37℃、5% CO2の条件で培養した。トリプシンを用いて回収した細胞をPBSで6x107 cells/mLに懸濁し、等量のMatrigel(商標) Basement Membrane Matrix (Becton Dickinson Co.製, Bedford, MA, USA)と混合した。

3) 薬物投与および測定
投与初日をDay0として、AZ-521担癌マウスではDay46まで、MKN-28担癌マウスではDay84まで、NCI-N87担癌マウスではDay56まで観察を行った。それぞれの群 (n=5又は6)を以下の様に処理した。FK330は5mL/kgで強制経口投与を行い、パクリタキセルは10mL/kgで尾静脈内投与を行った。パクリタキセルは下記の条件にて複数サイクル投与した。
対照群:未処置
FK330単独投与群:FK330 100mg/kg bid(200mg/kg/day、実験開始から実験終了時まで連続投与)

AZ-521担癌マウス:
パクリタキセルをDay0~Day5まで6日間連続投与を行った後22日間の休薬期間を置き(1サイクル)、パクリタキセルをDay28~Day32まで5日間連続投与した(2サイクル)。
MKN-28担癌マウス:
パクリタキセルをDay0からDay5まで6日間連続投与を行った後22日間の休薬期間の後、Day28からDay32の5日間連続投与し23日間の休薬期間をおいた(2サイクル)。さらに、Day56からDay60の5日間の連続投与をおこなった(3サイクル)。
NCI-N87担癌マウス:
パクリタキセルをDay0からDay5まで6日間連続投与を行った後26日間の休薬期間の後、Day32からDay36の5日間連続投与した(2サイクル)。
併用群:
FK330 100 mg/kg bid(200mg/kg/day、実験開始から実験終了時まで連続投与)+ パクリタキセル 10 mg/kg/day

ヒト胃癌(AZ-521、MKN-28、NCI-N87)担癌マウスにおいて、FK330のパクリタキセルとの併用効果を検討した結果を図10から図12に示した。即ち、AZ-521担癌マウスにおけるFK330とパクリタキセルの併用効果を図10に、MKN-28担癌マウスにおける併用効果を図11に、NCI-N87担癌マウスにおける併用効果を図12にそれぞれ示した。ヒト非小細胞肺癌担癌マウスと同様に、FK330はパクリタキセルの抗腫瘍効果を増強した。AZ-521担癌マウスにおいて、実験最終日のパクリタキセル単独群の腫瘍体積は3285.4±359.4 mm3、パクリタキセルとFK330の併用群では795.0±223.3 mm3であり、パクリタキセル単独群に対して腫瘍体積が75.8%(P<0.01)減少した。MKN-28並びにNCI-N87担癌マウスにおいても、FK330はパクリタキセルの抗腫瘍効果を増強した(MKN-28担癌マウス:454.1±147.5 mm3 vs 164.8±18.5 mm3、63.7%減少、P=0.08、NCI-N87担癌マウス:492.0±48.5 mm3 vs 223.9±85.1 mm3、54.5%減少、P<0.03)。
6) 結論
FK330の併用は、ヒト胃癌担癌モデルにおいてパクリタキセルの抗腫瘍効果を増強し、殊に、複数サイクル処置によるパクリタキセルの抗腫瘍効果の減弱を改善する効果が認められた。
1) 被験物質
FK330の用量は単体(非水和物)重量に換算して表示した。パクリタキセルはLC Laboratories社(Woburn, MA, USA)から購入し、パクリタキセルとして使用した。
2) 被験物質の調製及び投与
FK330は0.5%メチルセルロース水溶液(0.5% MC)に懸濁し(20mg/mL)、経口投与した。パクリタキセルは、エタノール/クロモフォアー溶液(1:1で混合)にて10 mg/mLのストック溶液を調製した後、投与直前に注射用生理食塩水(大塚製薬)にて1mg/mLに希釈して尾静脈内投与した。
3) 細胞
ヒト乳癌由来MDA-MB-231(HTB-26)はAmerican Type Culture Collection(VA, USA)より入手した。細胞は10%の加熱不活性化したウシ胎仔血清(FBS)が添加されたDMEM培地を用いて、37℃、5% CO2の条件で培養した。トリプシンを用いて回収した細胞をPBSで6x107 cells/mLに懸濁し、等量のMatrigel(商標) Basement Membrane Matrix (Becton Dickinson社, Bedford, MA, USA)と混合した。
5週齢の雌性ヌードマウス(CAnN Cg-Foxn1nu/CrlCrlj(nu/nu))は日本チャールスリバー社(日本、神奈川)より購入した。動物は試験期間を通じて特定病原体未感染(SPF)条件下で標準的な飼料と飲水を与えて飼育した。培養した細胞を3x106 cells/0.1 mL/mouseでヌードマウスの背部皮下に移植した。腫瘍体積(短径 x [長径]2 x 0.5)が148.7から312.5 mm3になったヌードマウスを群間および群内の腫瘍体積ばらつきが小さくなるようにSASを用いて群分けした。
5) 薬物投与および測定
投与初日をDay1としてDay66まで観察を行った。それぞれの群 (n=6)を以下の様に処理した。FK330は5 mL/kgで強制経口投与を行い、パクリタキセルは10 mL/kgで尾静脈内投与を行った。パクリタキセルは、Day1からDay4までの4日間連続投与を行った後、25日間の休薬を行った(1サイクル)。PTXの2サイクル目の投与では、Day30から4日間連続投与した。

ヒト乳癌担癌マウスにおけるFK330のパクリタキセルとの併用効果を検討した結果を図13に示した。ヒト乳癌(MDA-MB-231)担癌マウスにおいて、FK330はパクリタキセルの抗腫瘍効果を著明に増強し、Day28でのパクリタキセル単独群に対する腫瘍体積減少率は59.6%(P<0.01)、Day66でのパクリタキセル単独群に対する腫瘍体積減少率は82.8%(P<0.01)であった。
8) 結論
ヒト乳癌(MDA-MB-231)担癌マウスにおいて、FK330の併用は、複数サイクル投与によるパクリタキセルの抗腫瘍効果を増強し、腫瘍の再増殖を著明に抑制した。

NO供与体であるS-nitroso-N-acetyl-D,L-penicillamine(SNAP)はCayman Chemical社(Ann Arbor, MI, USA) から購入したものを使用した。チューブリン重合測定キットはCytoskeleton社(Denver, CO, USA)から購入し使用した。パクリタキセルは、測定キットに同封されているパクリタキセルをパクリタキセルとして使用した。ドセタキセルはSigma-Aldrich社(St. Louis, MO,USA)から購入し、ドセタキセルとして使用した。
2) 試験方法
蛍光法によるチューブリン重合反応測定は、キットの説明書に従い実施した。即ち、精製ブタ脳由来チューブリン(最終濃2 mg/mL)とSNAP(最終濃度200または500 μmol/L)をキットに同封されている緩衝液中で15分以上、室温でインキュベートした。次にパクリタキセル(最終濃度3 μmol/L)またはドセタキセル(最終濃度1μmol/L)、及びGPT(最終濃度1mmol/L)をこの緩衝液に加え、37℃で60分間インキュベートした。このインキュベーションの間、マイクロプレートリーダー(SpectraMax社, Molecular Devices社, Sunnyvale, CA, USA)にて蛍光強度(測定波長:励起350 nm/蛍光435 nm)を1分毎に測定した。蛍光強度の値は、平衡定常期に当たる反応開始60分後の値を用い、SNAP及びタキサン未処置群に対する百分率で示し、3回施行した平均値をその値とした。
結果は、独立した5回の実験の平均値(Mean)±標準誤差(SEM)で示した。パクリタキセルまたはドセタキセル処置群と未処置群の比較は、対応のない2群間の差の検定 (Student's t-test)を行った。タキサン系薬剤処置及び未処置におけるSNAP未処置群(対照群)とSNAP処置群(200及び500 μmol/L)の比較は、Dunnett's多重比較検定を行った。5%未満を有意差ありと判定した。データ処理にはGraphpad Prism version 5.03を用いた。
4) 結果
パクリタキセル誘発チューブリン重合作用に対するSNAPの抑制作用の代表例、及びチューブリン重合開始後60分の集計値を図14のA)及びB)に、ドセタキセル誘発チューブリン重合に対するSNAPの抑制作用の代表例、及びチューブリン重合開始後60分の集計値を図15のA)及びB)に、それぞれ示した。パクリタキセルまたはドセタキセルは、各タキサン未処置に比較してチューブリン重合をそれぞれ、152.7%(P<0.05)及び184.1%(P<0.001)有意に促進した。NO供与体であるSNAPは、パクリタキセル及びドセタキセルにより促進されたチューブリン重合反応を有意に抑制した。また、SNAPは自然に惹起されるチューブリン重合反応も有意に抑制した。
5) 結論
NO供与体は、パクリタキセル及びドセタキセルにより促進されたチューブリン重合反応を直接抑制した。
1) 被験物質
FK330の用量は単体(非水和物)重量に換算して表示した。パクリタキセル注射剤(パクリタキセル注射液「サワイ」)は沢井製薬株式会社から購入し、パクリタキセルとして使用した。ラット抗マウスF4/80抗原モノクローナル抗体、ウサギ抗iNOSポリクローナル抗体、及びウサギ抗ニトロチロシン抗体は、それぞれAbD Serotec社(Oxford, UK)、Santa Cruz Biotechnology社(Santa Cruz, CA, US)、及びMillipore社(Billerica, MA, USA)から購入したものを使用した。
2) 被験物質の調製及び投与
FK330は0.5%メチルセルロース水溶液(0.5% MC)に懸濁し(20mg/mL)、経口投与した。パクリタキセルは、注射用生理食塩水(大塚製薬)にて1mg/mLに用時調製した。
3) 細胞
ヒト肺癌由来Calu-6(HTB-56)はAmerican Type Culture Collection(VA, USA)より入手した。細胞は10%の加熱不活性化したウシ胎仔血清(FBS)が添加されたRPMI1640培地を用いて、37℃、5% CO2の条件で培養した。トリプシンを用いて回収した細胞をPBSで6x107 cells/mLに懸濁し、等量のMatrigel(商標) Basement Membrane Matrix (Becton Dickinson社, Bedford, MA, USA)と混合した。
4) 動物
5週齢の雄性ヌードマウス(CAnN Cg-Foxn1nu/CrlCrlj(nu/nu))は日本チャールスリバー社(日本、神奈川)より購入した。動物は試験期間を通じて特定病原体未感染(SPF)条件下で標準的な飼料と飲水を与えて飼育した。培養した細胞を3x106 cells/0.1 mL/mouseでヌードマウスの背部皮下に移植した。腫瘍体積(短径 x [長径]2 x 0.5)が136.8 から 248.5 mm3になったヌードマウスを群間および群内の腫瘍体積ばらつきが小さくなるようにSASを用いて群分けした。
5) 薬物投与
薬物投与は、投与初日(Day 1)より開始した。それぞれの群 (n=5)を以下の様に処理した。FK330は5mL/kgで強制経口投与をDay 1からDay 6まで、パクリタキセルは10mL/kgで尾静脈内投与をDay 1からDay 5まで行った。
対照群:未処置
FK330単独投与群:FK330 100 mg/kg bid(200 mg/kg/day、Day 1からDay 6)
パクリタキセル単独投与群:パクリタキセル10 mg/kg/day(Day 1からDay 5)
併用群:FK330 100 mg/kg/kg bid(200 mg/kg/day、Day 1からDay 6) + パクリタキセル 10 mg/kg/day(Day 1からDay 5)
FK330は1日2回、6から8時間間隔で強制経口投与(5 mL/kg)した。パクリタキセルはFK330投与後1~4時間後に尾静脈より静脈内ボーラス投与(10 mL/kg)した。
6) 組織固定及び凍結切片作成
Day 6にマウスより腫瘍組織を採材した。4%パラホルムアルデヒド-リン酸緩衝液(和光純薬工業株)にて、4℃で12時間浸潤固定を行った。組織固定度、10%、15%及び20%スクロース/リン酸緩衝液を用い,常法に従い、4℃で段階的にスクロース置換した。置換終了後、O.C.T.コンパウンド(Toronto Research Chemicals社, Toronto, Canada)中に癌組織を包埋し、ドライアイス/アセトンにて凍結切片を作成し、-80℃で保存した。
7) 免疫化学染色及び画像解析
5 μmの薄さの凍結切片をクライオスタット(サクラファインテックジャパン株式会社)にて作成し、風乾した。4%パラホルムアルデヒド-リン酸緩衝液で乾燥させた凍結切片を洗浄後、内因性ペルオキシダーゼを失活化させるため、0.3%過酸化水素/メタノールで切片を30分間処理した。マウスマクロファージ,iNOS及びニトロチロシン染色は以下の手順により行った。iNOS及びニトロチロシン染色に関しては、免疫化学染色を行う前に、マイクロウエーブ(3分/500 W)により抗原賦活化を行った。
マウスマクロファージ染色:
一次抗体処理:ラット抗マウスF4/80抗原モノクローナル抗体(1:200希釈)と60分間、室温でインキュベーションした。
二次抗体処理:ビオチン化抗ラットIgG抗体(1:100希釈, vector laboratories社)と30分間、室温でインキュベーションした。
検出系:VECTASTAIN ABC kit(Vector Laboratories社)を用い、切片を30分間,室温でインキュベーションした。
発色:3,3'-ジアミノベンジジンテトラヒドロクロライド(DAB,Dako社)試薬を用い,室温で1分間処理した。
iNOS染色:
一次抗体処理:ウサギ抗iNOSポリクローナル抗体(1:500希釈)と一昼夜、4℃でインキュベーションした。
検出系:EnVision (DAKO;)を用い、切片を30分間,室温でインキュベーションした。
発色:DAB試薬を用い,室温で1分間処理した。
ニトロチロシン染色:
一次抗体処理:ウサギ抗ニトロチロシン抗体(1:500希釈)と一昼夜、4℃でインキュベーションした。
検出系:EnVision (DAKO;)を用い、切片を30分間,室温でインキュベーションした。
発色:DAB試薬を用い,室温で1分間処理した。
染色した画像(F4/80染色画像及びiNOS画像は200倍、ニトロチロシン染色画像は400倍)は顕微鏡を介しコンピュータに取り込み、WinROOF(version 5.7)を搭載した画像解析システム(三谷商事株式会社)により解析した。値は、総面積当たりの陽性面積の比(%)で表した。各切片当たり5視野を計測し、その平均値を値とした。
結果は平均値(Mean)±標準誤差(SEM)で示した。腫瘍組織内iNOS蛋白発現、マクロファージ(F4/80)浸潤、及びニトロチロシン(NOの主な最終生成物)の各測定値について、対照群とパクリタキセル単独群、FK330単独群、またはパクリタキセル及びFK330併用群の比較、並びにパクリタキセル単独群とパクリタキセル及びFK330併用群の比較について、対応のない2群間の差の検定 (Student's t-test)を行った。5%未満を有意差ありと判定した。データ処理にはGraphpad Prism version 5.03を用いた。
9) 結果
ヒト肺癌細胞株Calu-6担癌マウスにおけるパクリタキセル単独及びFK330併用時の腫瘍組織内iNOS蛋白発現、マクロファージ(F4/80)浸潤、及びニトロチロシン(NOの主な最終生成物)発現に対する作用を、図16乃至18に示した。対照群の腫瘍組織内には、iNOS陽性細胞並びにマクロファージは殆ど認められず、ニトロチロシン(NOの最終生成物)も低かった。一方、パクリタキセル投与により、腫瘍組織内へのマクロファージの浸潤が著明に認められた。また、腫瘍組織内iNOS蛋白発現も著明に亢進、その結果、腫瘍組織内ニトロチロシン量も増加した。iNOS阻害剤であるFK330の併用は、腫瘍内マクロファージ浸潤及びiNOS蛋白発現には作用を及ぼさなかったものの、ニトロチロシン産生を抑制した。
10) 結論
ヒト肺癌(Calu-6)担癌マウスにおいて、パクリタキセルは、腫瘍組織内へのマクロファージの浸潤、及び腫瘍組織内iNOS蛋白発現を誘発し、その結果、腫瘍内NO産生を誘発した。iNOS阻害剤であるFK330の併用は、パクリタキセルにより誘発されるiNOS由来NO産生を抑制した。
Claims (19)
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と組み合わせて併用投与されるように用いられることを特徴とする、N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を有効成分として含有する癌治療用組成物。
- パクリタキセル又はドセタキセルと組み合わせて併用投与されるように用いられることを特徴とする、請求項1記載の組成物。
- パクリタキセルと組み合わせて併用投与されるように用いられることを特徴とする、請求項2記載の組成物。
- ドセタキセルと組み合わせて併用投与されるように用いられることを特徴とする、請求項2記載の組成物。
- 60~1200mg/日のN2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を包含する経口投与用組成物である請求項1乃至4記載の組成物
- 60~200mg/日のN2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を包含する経口投与用組成物である請求項5記載の組成物
- 更に、プラチナ系抗腫瘍剤、アントラサイクリン系抗腫瘍剤、シクロフォスファミド、フルオロウラシル、トラスツズマブ及びカペシタビンから選択される1以上の抗腫瘍剤と組み合わせて併用投与するよう用いられることを特徴とする請求項1乃至6記載の組成物。
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と同時に、別々に、連続して、或いは、間隔をあけて投与されることを特徴とする、請求項1乃至6記載の癌治療用組成物。
- N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を有効成分として含有する、タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤による治療を受けている対象を治療するための癌治療用組成物。
- タキサン系抗腫瘍剤が用いられている癌の治療用である請求項1乃至9記載の癌治療用組成物。
- タキサン系抗腫瘍剤が用いられている癌が非小細胞肺癌である請求項10記載の癌治療用組成物。
- タキサン系抗腫瘍剤が用いられている癌が乳癌である請求項10記載の癌治療用組成物。
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と組み合わせて併用投与されるように用いられることを特徴とする癌治療用組成物を製造するための、N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩の使用。
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤と組み合わせて併用投与されるように用いられることを特徴とする、癌治療のための、N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩。
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の併用治療に用いる場合の治療有効用量、並びに、N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩の併用治療に用いる場合の治療有効用量を組み合わせて、対象に投与することを特徴とする癌の治療方法。
- タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の併用治療に用いる場合の治療有効用量、並びに、N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩の併用治療に用いる場合の治療有効用量を、同時に、別々に、連続して、或いは間隔をあけて、対象に投与することを特徴とする癌の治療方法。
- N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を有効成分として含有する、タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の抗腫瘍作用増強剤。
- N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を有効成分として含有する、タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の耐性化抑制剤又は耐性化予防剤。
- N2-[(2E)-3-(4-クロロフェニル)-2-プロペノイル]-N-[2-オキソ-2-(4-{[6-(トリフルオロメチル)ピリミジン-4-イル]オキシ}ピペリジン-1-イル)エチル]-3-ピリジン-2-イル-L-アラニンアミド又はその塩を有効成分として含有する、タキサン系抗腫瘍剤から選択される1以上の抗腫瘍剤の感受性増強剤。
