Abstract
Die vorliegende Erfindung betrifft eine Tablette mit einer Matrix aus einem Gemisch von mindestens zwei Zuckeralkoholen, wobei die Matrix als Wirkstoff Quetiapin oder ein Salz davon enthält.

Quetiapin-Tabletten Beschreibung:
Die vorliegende Erfindung betrifft eine Tablette mit einer Matrix aus einem Gemisch von mindestens zwei Zuckeralkoholen, wobei die Matrix als Wirkstoff Quetiapin oder ein Salz davon enthält. Ferner betrifft die Erfindung ein Verfahren zur Herstellung der erfindungsgemäßen Tablette und deren Verwendung zur Behandlung von Schizophrenie oder von Angstzuständen.
Quetiapin ist ein Dibenzothiazepin-Derivat und ist als atypisches Neuroleptikum in der Behandlung von akuter und chronischer Schizophrenie bekannt. Bisher verfügbare Zubereitungen von Quetiapin sind Filmtabletten, die im Dosisbereich von 25 mg bis 300 mg, 2- bis 3-mal täglich verabreicht werden (Handelsname: Seroquel®, Seroquel XR®, Seroquel® Prolong, AstraZeneca). Der Dosisbereich von Quetiapin liegt zwischen 150 mg bis 750 mg/Tag. Höhere Dosen sind im Behandlungsverlauf oft notwendig. Die Zusammensetzung der bekannten Tablette ist in Beispiel 1 eingegeben.
Bei der Applikation der konventionellen Tabletten gibt es Probleme. Ein Nachteil dieser Tabletten für den Patienten ist die Gefahr, dass der Blutspiegel sehr stark schwankt, da Quetiapin eine pH-abhängige Löslichkeit zeigt. Die damit zusammenhängenden Löslichkeitsprobleme führen zu Problemen mit der Patientencompliance und zu unerwünschten Nebenwirkungen.
Die Retardtablette ist für die 2- bis 3-mal tägliche Gabe unbefriedigend, weil erstens die Arzneimittelfreisetzung zu langsam verläuft, so dass anfänglich kein ausreichend hoher, therapeutisch wirksamer Blutspiegel erreicht wird, und zweitens der erst im unterem Darmabschnitt (Dickdarm) freigesetzte Arzneimittelanteil dort nicht mehr resorbiert wird, also unwirksam bleibt.
Ein weiteres Problem ist die Gefahr des Auftretens, des sogenannten„dose dumping". Beim„dose dumping" kommt es bei Retardarzneiformen, bei denen
zur Erzielung der Retardierung ein funktioneller Filmüberzug eingesetzt wird, zur spontanen Freisetzung der gesamten Arzneistoffdosis auf Grund von Beschädigungen der Arzneiform. Es kann bei den konventionellen Umhüllungsverfahren (Wirbelschicht- und Dragierkesselverfahren) auch nicht regelmäßig verhindert werden, dass beim Umhüllen von Tabletten oder Kapseln die Filmdicke an den Kanten und an Orten mit kleinem Wölbungsradius unterdurchschnittlich gering ist. Das „dose dumping" ist besonders bei hochwirksamen Arzneistoffen befürchtet, da es zu schweren Nebenwirkungen führen kann, wenn die für eine längerfristige, kontinuierliche Abgabe vorgesehene Arzneistoffdosis auf einmal freigesetzt wird.
Aufgrund der dargestellten Probleme beschäftigt sich die Fachwelt mit der Suche nach verbesserten Arzneiformen von Quetiapin.
WO 01/21179 (EP 1 218 009) offenbart eine wässrige Lösung oder Suspension, bei der Quetiapin Fumaratgranulate dispergiert oder gelöst werden.
WO 97/45124 (EP 0 907 364) offenbart eine SR-Formulierung, dadurch gekennzeichnet, dass die Formulierung eine Matrix aus Cellulosederivaten hat.
WO 2007/133583 beschreibt eine Modified Release Formulierung, bei der ein Matrixkern und ein Coating verwendet werden.
WO 2004/002445 offenbart Zweischichttabletten. Die erste Schicht besteht aus Cellulosederivaten und die zweite Schicht besteht aus einer Matrix aus verschiedenen Polymeren.
WO 2007/090091 offenbart die mikropartikuläre Pellets.
EP 0 131 485 offenbart Formulierungen mit einem Freisetzungsprofil nach nullter Ordnung, worin der Wirkstoff mit Zucker und/oder Zuckeralkoholen
gemischt wird. EP 0 131 485 zeigt jedoch keine Möglichkeit auf, wie ein Wirkstoff mit einer pH-abhängigen Löslichkeit formuliert werden kann.
EP 0 435 450 beschreibt pharmazeutische Zusammensetzungen, die kristallinen Zuckeralkohol, abstammend von mindestens einem Mono- oder Polysaccharid, enthalten, und die in der Kristallmatrix des Zuckeralkohols Teilchen mindestens einer pharmazeutisch wirksamen Verbindung gleichförmig dispergiert enthalten. Nachteilig ist es jedoch die Verwendung von hohen Temperaturen, da Zucker aufgeschmolzen werden. Solche hohe Temperaturen verursachen aufwendige Prozessbedingungen und Probleme mit der chemischen Stabilität des Wirkstoffes.
WO 2005/041935 offenbart Quetiapin-Tabletten mit einer Matrix, worin die Matrix Quetiapin oder ein Salz davon und ein Wachs enthält.
WO 2007/000778 offenbart eine Modified Release Formulierung mit einem Polymersystem, wobei das Polymersystem mindestens zwei pH-unabhängige Polymere enthält.
Im Falle des Wirkstoffs Quetiapin sind bislang noch keine Formulierungen bekannt, bei denen eine pH-unabhängige Freisetzung neben einer retardierten Formulierung vorgesehen ist, die auch gleichzeitig eine einfache Zusammensetzung aufweisen. Zusätzlich weisen die bisherigen Lösungen komplexe Formulierungen auf, die zu erhöhten Prozesszeiten und Kosten führen. Hinzu kommt die Verwendung der Hilfsstoffe in großen Mengen, die ebenfalls zu erhöhten Kosten führt.
Ebenso wenig geeignet für Quetiapin sind Formulierungen, die unter Einsatz bekannter Retardierungsmittel, wie Cellulosederivate oder lipophiler Stoffe, wie Fette oder Wachse, hergestellt werden, da der Wirkstoff selber lipophil ist und unter Einsatz solcher Retardierungsmittel eine sehr langsame Freisetzungsrate resultieren wird, die auch nicht wünschenswert ist.
Die Aufgabe der vorliegenden Erfindung besteht darin, eine Tablette bereit zu stellen, die es ermöglicht, bei einer 2- bis 3-mal täglichen Gabe eine gleichmäßige - pH unabhängige - Freisetzung von Quetiapin oder seinen Salzen zu erlangen.
Eine weitere Aufgabe der vorliegenden Erfindung besteht darin, eine Tablette bereitzustellen, bei der die Wirkstofffreisetzung unabhängig vom pH-Wert, Wirkstärke und Geometrien gesteuert werden kann, wobei als Wirkstoff Quetiapin in Form eines Salzes, beispielsweise Quetiapinhemifumarat, enthalten ist.
Gelöst wird die Aufgabe durch eine Tablette mit einer Matrix aus einem Gemisch von mindestens zwei Zuckeralkoholen, wobei die Matrix als Wirkstoff Quetiapin oder ein Salz davon enthält.
Unter „Matrix" wird verstanden, dass die mindestens zwei Zuckeralkohole homogen gemischt werden und damit eine Grundmasse für die Einbettung des Wirkstoffes zur Verfügung gestellt wird.
Die mindestens zwei Zuckeralkohole sind ausgewählt aus der Gruppe aus Mannitol, Sorbitol, Isomalt (E 953), Inositol, Lactit, Xylitol, Maltilol.
Unter„Mischen" versteht man das Einschieben oder Eindringen von Partikeln eines Stoffes zwischen die Partikel eines anderen Stoffes mit dem Ziel, eine möglichst gleichmäßige Dispersität zu erreichen. Die Homogenität und die Dispersität der Pulvermischung werden nach bestimmten Mischzeiten durch Probeentziehung an verschiedenen Stellen und Analyse dieser Proben festgestellt. Die Analyse dieser Proben erfolgt visuell und durch die Bestimmung der Abweichung der Ist-Konzentration vom gewünschten Zielwert im Endprodukt. Je kleiner die Schwankung der Konzentration und damit die Varianz, desto besser ist die Mischung. Die Pulvermischung aus
Zuckeralkoholen wird mit den herkömmlichen Mischgerätetypen homogen gemischt. Solche Mischgerätetypen sind dem Fachmann bekannt. Einige davon sind z.B. der V-Mischer, der Doppelkonus- oder der Pflugscharmischer.
Bei der erfindungsgemäßen Tablette mit einer Matrix aus mindestens zwei Zuckeralkoholen zeigt sich überraschenderweise, dass die Freisetzung von Quetiapin pH-unabhängig erfolgt, d.h. in einem weiten Bereich des Gl-Traktes ein gleichmäßiger Freisetzungsverlauf erreicht wird.
Mit der erfindungsgemäßen Tablette werden überraschender Weise große Blutspiegelspitzen verhindert, die üblicherweise für toxische Nebenwirkungen eines Arzneimittels verantwortlich sind. Durch den Einsatz der erfindungsgemäßen Tablette wird die Patientencompliance verbessert, weil weniger Arzneimittel pro Tag eingenommen werden muss.
Ferner ermöglicht das erfindungsgemäße Verfahren eine vereinfachte technische Durchführbarkeit und damit eine verbesserte Wirtschaftlichkeit, da die erfindungsgemäße Tablette eine einfache Zusammensetzung aufweist.
Die erfindungsgemäße Tablette ist vorzugsweise dadurch gekennzeichnet, dass sie Quetiapin in einer Konzentration im Bereich von 10 bis 80 Gew.% enthält, bezogen auf das Gesamtgewicht der Tablette. Die erfindungsgemäße Tablette kann Quetiapin in einer Konzentration im Bereich von 10 bis 75 Gew.% enthalten, bezogen auf das Gesamtgewicht der Tablette. Die erfindungsgemäße Tablette kann Quetiapin in einer Konzentration im Bereich von 10 bis 70 Gew.% enthalten, bezogen auf das Gesamtgewicht der Tablette. Die erfindungsgemäße Tablette kann Quetiapin in einer Konzentration im Bereich von 10 bis 65 Gew.% enthalten, bezogen auf das Gesamtgewicht der Tablette.
Die erfindungsgemäße Tablette kann Quetiapin in einer Konzentration von 50 Gew.%, 51 Gew.%, 52 Gew.%, 53 Gew.%, 54 Gew.%, 55 Gew.%, 56 Gew.%, 57 Gew.%, 58 Gew.%, 59 Gew.%, 60 Gew.%, 61 Gew.%,
62 Gew.%, 63 Gew.%, 64 Gew.%, 65 Gew.%, 66 Gew%, 67 Gew.%, 68 Gew.%, 69 Gew.% oder 70 Gew.% enthalten, bezogen auf das Gesamtgewicht der Tablette.
Quetiapin kann in Form von Salzen vorliegen. Einige Beispiele sind Quetiapinfumarat oder Quetiapinhemifumarat.
Die erfindungsgemäße Tablette ist vorzugsweise ferner dadurch gekennzeichnet, dass die Matrix ein Gemisch aus Mannitol und Sorbitol enthält.
Die erfindungsgemäße Tablette ist insbesondere ferner dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :1 bis 30:1 enthält.
Die erfindungsgemäße Tablette ist insbesondere ferner dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :2 bis 20:1 enthält.
Die erfindungsgemäße Tablette ist weiterhin vorzugsweise dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 6:4 enthält.
Die erfindungsgemäße Tablette ist vorzugsweise dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 17,5:1 enthält.
Die erfindungsgemäße Tablette ist insbesondere dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :1 ,3 enthält.
Die erfindungsgemäße Tablette ist gemäß einer weiteren Ausführungsform dadurch gekennzeichnet, dass die Matrix ein Gemisch aus Mannitol und Xylitol enthält.
Die erfindungsgemäße Tablette ist vorzugsweise dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Xylitol in einem Gewichtsverhältnis von 1 :1 bis 30:1 enthält.
Die erfindungsgemäße Tablette ist weiterhin vorzugsweise dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 50 Gew.-% beträgt, bezogen auf das Gesamtgewicht der Tablette.
In einer bevorzugten Ausführungsform ist die erfindungsgemäße Tablette dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 40 Gew.-% beträgt. Beispielsweise beträgt die Gesamtmenge maximal 35 Gew.-%, insbesondere maximal 30 Gew-%.
Die erfindungsgemäße Tablette ist weiterhin insbesondere dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 25 Gew.-% beträgt, bezogen auf das Gesamtgewicht der Tablette.
Die erfindungsgemäße Tablette ist gemäß einer weiteren bevorzugten Ausführungsform dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 20 Gew.-% beträgt, bezogen auf das Gesamtgewicht der Tablette.
Nach dem erfindungsgemäßen Verfahren werden die Zuckeralkohole nicht aufgeschmolzen.
Die erfindungsgemäße Tablette kann zusätzlich pharmazeutisch verträgliche Hilfsstoffe enthalten.
Die erfindungsgemäße Tablette ist vorzugsweise dadurch gekennzeichnet, dass die pharmazeutisch verträglichen Hilfsstoffe ausgewählt sind aus Bindemitteln, Gleitmitteln und Gemischen davon.
Als Bindemittel seien genannt Dextrin, Cellulosederivate, Gelatine, Gums, Stärke, Polyvinylpyrrolidon (PVP), Eudragite. Bevorzugt seien genannt PVP, Cellulosederivate, Stärke und. Eudragite, besonders bevorzugt Eudragit® L30 D- 55 (TS) (Methacrylsäure-Ethyl Acrylat Copolymer (1 :1 )).
Weitere Beispiele für bevorzugte Bindemittel sind ausgewählt aus der Gruppe Poly(meth)acrylatderivate wie Eudragit® S, Eudragit® E, Eudragit® L, Eudragit® E 30 D.
Als Gleitmittel seien genannt Macrogol 400, Magnesiumstearat, Calciumbehenat, Natriumstearylfumarat, und Glycerinmonostearat (Trecirol®), bevorzugt Natriumstearylfumarat.
Die Erfindung betrifft gemäß einer bevorzugten Ausführungsform ferner ein Verfahren zur Herstellung einer Tablette, umfassend das Granulieren von mindestens zwei Zuckeralkoholen und 10 bis 80 Gew.% Quetiapin oder einem Salz davon, bezogen auf das Gesamtgewicht der Tablette, mit einem Bindemittel, und Verpressen des Granulats zu einer Tablette unter Zusatz von einem Gleitmittel.
Das erfindungsgemäße Verfahren ist dadurch gekennzeichnet, dass Quetiapin oder ein Salz davon in die Matrix aus mindestens zwei Zuckeralkoholen eingebettet wird. Dabei wird die Mischung aus mindestens zwei Zuckeralkoholen nicht aufgeschmolzen.
Die erfindungsgemäße Quetiapin-Tablette wird bevorzugt zur Behandlung der Schizophrenie eingesetzt.
Die erfindungsgemäße Quetiapin-Tablette wird ferner bevorzugt zur Behandlung von Angstzuständen verwendet.
Die Erfindung betrifft ferner ein Arzneimittel, dadurch gekennzeichnet, dass das Arzneimittel die erfindungsgemäße Tablette und einen Beipackzettel enthält.
Die erfindungsgemäße Tablette kann in Kombination mit einem zweiten Wirkstoff verwendet werden. Unter„Kombination" wird verstanden, dass die erfindungsgemäße Tablette entweder mit dem zweiten Wirkstoff formuliert wird, so dass eine Tablette mit zwei Wirkstoffen hergestellt wird oder die erfindungsgemäße Tablette wird mit dem zweiten Wirkstoff, der ebenfalls in Form einer Tablette vorliegt, verabreicht.
Vorteilhafterweise lässt sich die erfindungsgemäße Tablette auch so ausgestalten, dass die Tablette zusätzlich einen weiteren Wirkstoff enthält. Bevorzugte Wirkstoffe seien ausgewählt aus der Gruppe Neuroleptika wie Fluphenazin, Haloperidol, Clozapin, Amisulpirid, Olanzapin, Risperidon, Ziprasidon, Flupentixol, Perazin, Perphenazin, Pimozid, Zotepin. Die erfindungsgemäße Tablette kann einen weiteren Wirkstoff aus der Gruppe der Antidepressiva enthalten, der ausgewählt ist aus Imipramin, Melitracen, Amitriptylin, Clomipramin, Doxepin, Trimipramin, Opipramol.
Figur 1 zeigt Freisetzungskurven der Vergleichsbeispiele 1-4 bei pH 4,5. Wie aus der Darstellung ersichtlich ist, haben die verschiedenen Zuckeralkohole einen unterschiedlichen Einfluss auf die Freigabe des Wirkstoffes (hier: Quetiapinhemifumarat). Die Tabletten mit einer Matrix aus einem Gemisch von Mannitol und Sorbitol zeigen jedoch den gleichen Freisetzungsverlauf.
Figur 2 zeigt die Freisetzung von Vergleichsbeispiel 3 (Mannitol : Sorbitol in einem Verhältnis von 1 : 1) bei verschiedenen pH-Werten, die typischerweise im Verdauungstrakt vorherrschen. Wie aus der Grafik klar und deutlich hervorgeht,
zeigen die Tabletten mit einer Matrix aus Mannitol : Sorbitol in einem Verhältnis von 1 : 1 den gleichen Freisetzungsverlauf bei unterschiedlichen pH-Werten.
Figur 3 zeigt die Freisetzung von Vergleichsbeispiel 3 (Mannitol : Sorbitol in einem Verhältnis von 1 : 1), verpresst auf verschiedene Wirkstärken (50 mg, 200 mg, 300 mg).
Figur 4 zeigt die Freisetzung der Beispiele 4 bis 6.
Die Erfindung wird anhand der nachfolgenden, jedoch nicht einschränkenden Beispiele erläutert:

Beispiel 1 : Zusammensetzung von Seroquel® Prolong
Seroquel® Prolong enthält als Wirkstoff Quetiapinhemifumarat und besteht aus einem Tablettenkern und einer Tablettenhülle.

Mikrokristalline Cellulose, Natriumeitrat 2.H20, Laktose Monohydrat, Magnesiumstearat, Hypromellose
Tablettenhülle:
Hypromellose, Macrogol 400, Titandioxid (E 171), Eisen(lll)-hydroxid- oxidhydrat

Beispiel 3: Die Herstellung der erfindungsgemäßen Tablette
Quetiapinhemifumarat, Füllstoffe und Säuerungsmittel wurden gemischt und mit der Eudragit-Dispersion im Schnellmischer gemischt. Das Granulat wurde getrocknet, gesiebt und in einem Freifallmischer mit dem Natriumstearylfumarat gemischt. Die so hergestellte Mischung wurde auf einer Tablettenpresse zu Tabletten verpresst.

Die Ergebnisse der Versuche zeigen, dass die Freisetzung bei pH-Werten, die im Dünn- und Dickdarm vorherrschen, gleichmäßig verläuft. Außerdem ist ersichtlich, dass auch unterschiedliche Dosierungen und Geometrien keinen Einfluss auf die Freigabe des Wirkstoffes haben.
Die erfindungsgemäße Tablette kann auch überzogen werden. Solche erfindungsgemäße Formulierungen sind in den Beispielen 4-6 angezeigt:

Claims
1. Tablette mit einer Matrix aus einem Gemisch von mindestens zwei Zuckeralkoholen, wobei die Matrix als Wirkstoff Quetiapin oder ein Salz davon enthält.
2. Tablette nach Anspruch 1 , dadurch gekennzeichnet, dass Quetiapin in einer Konzentration im Bereich von 10 bis 80 Gew.% enthält, bezogen auf das Gesamtgewicht der Tablette.
3. Tablette nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Matrix ein Gemisch aus Mannitol und Sorbitol enthält.
4. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :1 bis 30:1 enthält.
5. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :2 bis 20:1 enthält.
6. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 17,5:1 enthält.
7. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass das Gemisch die Zuckeralkohole Mannitol und Sorbitol in einem Gewichtsverhältnis von 1 :1 ,3 enthält.
8. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 50 Gew.-% beträgt, bezogen auf das Gesamtgewicht der Tablette.
9. Tablette nach Anspruch 8, dadurch gekennzeichnet, dass die Gesamtmenge an Zuckeralkoholen in der Matrix maximal 30 Gew.-% beträgt, bezogen auf das Gesamtgewicht der Tablette.
10. Quetiapin-Tablette nach einem der vorangehenden Ansprüche, zur Behandlung von Schizophrenie oder von Angstzuständen.
11. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Tablette zusätzlich einen weiteren Wirkstoff enthält.
12. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass der weitere Wirkstoff ausgewählt ist aus Fluphenazin, Haloperidol, Clozapin, Amisulpirid, Olanzapin, Risperidon, Ziprasidon, Flupentixol, Perazin, Perphenazin, Pimozid, Zotepin.
13. Tablette nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass der weitere Wirkstoff ausgewählt ist aus Imipramin, Melitracen, Amitriptylin, Clomipramin, Doxepin, Trimipramin, Opipramol.
14. Verfahren zur Herstellung einer Tablette, umfassend das Granulieren von mindestens zwei Zuckeralkoholen und 10 bis 80 Gew.% Quetiapin oder einem Salz davon, bezogen auf das Gesamtgewicht der Tablette, mit einem Bindemittel und Verpressen des Granulats zu einer Tablette unter Zusatz von einem Gleitmittel.
