Abstract
Description
本願は、2010年7月29日に、日本に出願された特願2010-171214号に基づき優先権を主張し、その内容をここに援用する。
このほか、特許文献2には、ポリエチレンテレフタレート(PET)等からなるバリアフィルムを備えたフィルム状の経皮投与薬用支持体が記載されている。特許文献3には、アルミニウム等からなる蒸着層を設けてバリア性を付与したフィルム状の貼付剤用支持体が記載されている。特許文献4には、伸縮性の基体フィルム層と基体フィルムの伸長に応じ幅が変化する溝部を有するポリエステル樹脂フィルム層を積層した貼付剤用支持体が記載されている。
すなわち、柔軟性に優れた材質からなる支持体を用いて医療用貼付剤を製造すること自体は可能であるが、貼り付け時や貼付後に医療用貼付剤が伸長すると、バリア層が支持体の伸長による形状変化に充分に追随することができず、バリア層にクラック等を生じることがある。
一方、特許文献2のように、PET等からなるバリアフィルムを備えた構成では、柔軟性の低いPET等の存在によって、支持体の柔軟性が充分に発揮されないという問題がある。
また、特許文献3に記載のような蒸着層からなるバリア層は、さらにクラック等が生じやすく、やはり医療用貼付剤に好適であるとは言いがたい。
支持体11は、柔軟性を有し、寸法にして10パーセント(%)以上増加する所定の最大伸長率まで伸長可能である。具体的な最大伸長率の値は、医療用貼付剤1の用途や貼付部位等に基づいて適宜設定されてよい。本実施形態において、支持体11を形成するポリウレタンは、特に限定されず、従来ポリウレタンフィルムに用いられているものが利用でき、用途によって適宜選択できる。たとえばポリエーテル系ポリウレタン、ポリエステル系ポリウレタン、ポリカーボネート系ポリウレタン等が挙げられる。耐水性が必要な用途では、ポリエーテル系ポリウレタンまたはポリカーボネート系ポリウレタンが好ましい。
また、ウレタン結合を形成するイソシアネートの種類や、黄変タイプ、無黄変タイプ等についても特定のものには限定されず、用途、使用に際して保管する期間や方法、使用する可塑剤の種類等に応じて適宜選択できる。
支持体11の厚さは、10マイクロメートル(μm)~200μmであり、15μm以上100μm以下とされるのが好ましい。厚さが10μm未満では薄すぎて取扱いが困難であり、200μmより厚いと柔軟性が低下して本来の柔軟性が充分発揮されない。
離型フィルムの材質は特に限定するものではないが、一般にシリコーン処理したPETフィルムや、剥離性の良いポリオレフィンフィルム、紙とポリエチレンの積層体など、伸び縮みせず後で剥離可能なものを使うことが出来る。
モンモリロナイトは、鉱物学的には2八面体型含水層状珪酸塩鉱物で、理想的には次の化学式で表現される。
(Al2-yMgy)Si4O10(OH)2 . (M+,M1/2 2+)y . nH2O ただし、y=0.2~0.6、M:交換性陽イオンNa、K、Ca、Mg、Hなど、n :層間水の量。
モンモリロナイトの結晶構造は、2枚の四面体シートと1枚の八面体シートからなる3層を基本とした層状構造を形成する。四面体シートの陽イオンはSiのみで、八面体シートの陽イオンのAlが一部Mgに置換されている。このため、単位結晶層は負の電荷を帯びており、その負電荷に見合うように結晶層間にNa+、K+、Ca2+、Mg2+、H+などの陽イオンが入り込み補償されている。本発明では、これら陽イオンの種類は特に限定されることなく使用できる。
粘着層20に用いられる粘着剤としては、アクリル系高分子またはゴム系の高分子を基剤とする粘着剤を用いることができる。
アクリル系高分子としては、2-エチルヘキシルアクリレート、メチルアクリレート、ブチルアクリレート、ヒドロキシエチルアクリレート、2-エチルヘキシルメタアクリレート等に代表される(メタ)アクリル酸誘導体の少なくとも一種を含む共重合体等が挙げられる。
ゴム系高分子としては、スチレン-イソプレン-スチレンブロック共重合体(以下、SISと略記する)、イソプレンゴム、ポリイソブチレン(以下、PIBと略記する)、スチレン-ブタジエン-スチレンブロック共重合(以下、SBSと略記する。)、スチレン-ブタジエンゴム(以下、SBRと略記する)、ポリシロキサン等が挙げられる。
粘着層20における可塑剤の配合量は、10~70wt%、好ましくは10~60wt%、さらに好ましくは10~50wt%とされる。
粘着層20における粘着付与剤の配合量は、5~70wt%、好ましくは5~60wt%、さらに好ましくは10~50wt%とされる。
吸収促進剤としては、炭素鎖数6~20の脂肪酸、脂肪アルコール、脂肪酸エステル、アミド、またはエーテル類、芳香族系有機酸、芳香族系アルコール、芳香族系有機酸エステルまたはエーテル(以上は飽和、不飽和のいずれでもよく、また、環状、直鎖状分枝状のいずれでもよい)、さらに、乳酸エステル類、酢酸エステル類、モノテルペン系化合物、セスキテルペン系化合物、エイゾン(Azone)、エイゾン(Azone)誘導体、ピロチオデカン、グリセリン脂肪酸エステル類、プロピレングリコール脂肪酸エステル類、ソルビタン脂肪酸エステル類(Span系)ポリソルベート系(Tween系)、ポリエチレングリコール脂肪酸エステル類、ポリオキシエチレン硬化ヒマシ油系(HCO系)、ポリオキシエチレンアルキルエーテル類、ショ糖脂肪酸エステル類、植物油等が挙げられる。
粘着層20における吸収促進剤の配合量は、0.01~20wt%、好ましくは0.05~10wt%、さらに好ましくは0.01~5wt%とされる。
なお、吸収促進剤には、一部可塑作用を併せ持つものもあり、本発明ではそのような吸収促進剤を可塑剤として使用することができる。また、吸収促進剤として挙げた上述の物質は、軟化剤、溶解剤、溶解補助剤、可溶化剤等として用いられてもよい。
このほか、必要に応じて、抗酸化剤、充填剤、架橋剤、紫外線吸収剤、防腐剤等が適宜加えられてもよい。

特に、本発明の医療用貼付剤は、1週間程度の長期間にわたり皮膚に貼付することができるので、毎日貼り換えるなどのわずらわしさをなくし、患者または介護者の負担を軽減することができるため、コンプライアンスの向上にもつながる。
一般に、支持体11を構成するポリウレタンは、可塑剤や薬剤を吸着等しやすく、その場合、前述した諸問題が発生する恐れがあるが、医療用貼付剤1では、支持体11の非伸長時はもちろん、伸長率20%(伸長後の長さが20%増加することを指す)まで伸長されたときにおいてもバリア層12のバリア性が好適に保持される、その結果、使用前の保存時だけでなく、対象物に貼り付けられる使用時においても上記問題を好適に防止することができる。なお、具体的なモンモリロナイト含有率は、使用する可塑剤および薬剤を用いた予備試験等により、容易に設定することができる。一部の可塑剤および薬剤に対するバリア性とモンモリロナイト含有率との関係については、後述する。
(1-1 サンプルの作成)
支持体として、厚さ20μmのポリエーテル系ポリウレタン製のものを用いた。支持体の一方の面に、MNとPVA(ケン化度80%)とを混合したバリアコート剤を、1.0g/m2均一に塗布することによりバリア層を形成した。これを基本構成として、バリア層におけるMNの含有率を、1wt%、2wt%、10wt%、18wt%、22wt%、25wt%、30wt%、および37wt%の8段階設定し、医療用貼付剤の材料としての支持フィルムのサンプルを8種類作製した。
作製した8種類のサンプル100を、図2に示すように25ミリメートル(mm)×120mmの大きさに切り出し、引張試験機での操作をしやすくするため、長手方向両端部の両面に厚さ50μmのポリエチレンテレフタレート(PET)製のシート101を両面テープで貼り付け、評価片100Aを作製した。シート101の評価片100Aの長手方向における寸法は10mmとし、各評価片100Aにおいて、シート101に覆われない部分の長手方向の寸法は100mmに設定した。
IPM、TA、およびSMOについては、MN含有率1wt%、10wt%、18wt%、22wt%、25wt%、30wt%、および37wt%の評価片100Aを用いて検討し、MGISについては、MN含有率2wt%、10wt%、および22wt%の評価片100Aを用いて検討した。
結果を表1に示す。IPM、TA、およびMGISについては、MN含有率が22wt%以下の場合、すべての伸長率において支持体の膨潤を認めず、可塑剤の移行が抑制されていた。一方、SMOについては、MN含有率、伸長率にかかわらず支持体に膨潤が認められ、本発明のフィルム材に用いる可塑剤としては好ましくない場合があると考えられた。実験に用いた各可塑剤の溶解度パラメータ(SP値、フェダーズ法による)は、IPMで8.5、TAで10.2、MGISで10.76、SMOで11.76であり、SP値が低い可塑剤のほうが好ましい傾向があることが推測された。
図6から図9は、それぞれMN含有率10wt%、18wt%、25wt%、および30wt%の評価片に対して、伸長率20%の伸長操作を行ったあとの支持フィルムの光学顕微鏡写真である。MN10wt%および18wt%では、外観に大きな変化は見られないが、MN25wt%および30wt%では膨潤によるシワが見られる。
(2-1 サンプルの作成)
支持体として、実験1と同様の材料を用い、支持体の一方の面に、MNとPVA(ケン化度80%)とを混合したバリアコート剤を1.0g/m2均一に塗布することによりバリア層を形成した。バリア層におけるMNの含有率を、1wt%、2wt%、4wt%、10wt%、18wt%、22wt%、30wt%、および37wt%の9段階設定し、支持フィルムのサンプルを9種類作製した。
a.非伸長時における安定性試験
10平方センチメートルに切り出した医療用貼付剤サンプルを、伸長操作を行わずに60℃で1週間保存した。
b.伸長時における安定性試験
幅30mm、長さ50mmに切り出したテープ材サンプルを、剥離部材を除去してから長さ方向に伸長率20%の伸長操作を1回行った後、60℃で3日間保存した。
いずれの場合も、保存後の各テープ材サンプルにおいて、実験1と同様に、支持体におけるシワ発生の有無によりバリア性を評価した。実験2における評価は、膨潤によるシワを認めない:◎、わずかに膨潤によるシワを認めるが、品質に問題ない程度である:○、膨潤によるシワが認められ、使用に耐えない:×の3段階とした。
結果を表2に示す。MN含有率が2wt%、10wt%、および18wt%の場合、非伸長時および20%伸長時のいずれにおいても支持体にシワを認めずバリア性が良好に保たれていることが示唆された。
また、実験1で好ましくない場合があると考えられたSMOについても、バリア層におけるMNの含有率や粘着層における可塑剤の含有率等を適切に設定することにより、支持体への可塑剤の移行を充分抑制できることが確認された。
(3-1 サンプルの作成)
本実験のサンプルにおける粘着層の処方を以下に示す。
SIS 25部
脂環族飽和炭化水素樹脂 42部
流動パラフィン 18部
IPM 10部
エスシタロプラム 5部
上記の処方に従い、エスシタロプラム、IPM、および流動パラフィンを十分に混合した。そして、得られた混合物に、SIS、脂環族飽和炭化水素樹脂およびトルエンからなる混合液を加えて、薬物用塗工液を調製した。この薬物用塗工液をPET製の剥離ライナー(剥離部材)に塗布し、溶媒を乾燥除去して薬物を含有する粘着層を形成した。さらに支持体フィルムとして1-1に記載の方法で作成した支持フィルムのバリア層側に粘着層を貼り合わせて医療用貼付剤を得た。
支持フィルムのMN含有率は、1wt%、2wt%、4wt%、10wt%、18wt%、22wt%、30wt%、および37wt%の8段階設定し、計8種類の医療用貼付剤のサンプルを作成した。
a.非伸長時における安定性試験
10平方センチメートルに切り出した医療用貼付剤サンプルを、伸長操作を行わずに剥離部材を除去して粘着層を露出させ、60℃で1週間保存した。この実験は、すべてのMN含有率を用いて検討した。
b.伸長時における安定性試験
幅30mm、長さ50mmに切り出した医療用貼付剤サンプルを、剥離部材を除去してから長さ方向に伸長操作を1回行った後、室温で1日間保存した。伸長率は、0%、5%、10%、20%の4段階とし、MN含有率は、1wt%、10wt%、18wt%、22wt%、30wt%、および37wt%の6段階で検討した。
上記a、bのいずれにおいても保存期間終了後のサンプルに対して薬物放出量を測定した。放出量の測定は、米国薬局方(USP:US27-NF22)の放出試験法(Phisical test /〈724〉Drug Release)に記載の回転シリンダー法(Apparatus 6 (Cylinder))に従い、以下の条件で4時間行った。

粘着層に混合した量に対する総放出量を、放出率(%)として表記した。バリア層としてほとんど薬物を吸着しないPETを用いた点のみが異なる医療用貼付剤サンプルの上記試験における放出率が65%であったため、これを基準として、放出率が低下した場合は、支持体への移行や吸着等が起こっていると仮定し、評価を行った。評価は、支持体への移行を認めない:◎、わずかに移行していると推測されるが、品質に問題ない程度である:○、移行量が多いと推測され、医療用貼付剤として適さない:×の3段階とした。
結果を表3に示す。aの実験においては、MN含有率が4%以上のサンプルでは支持体への移行はほとんど見られなかった。bの実験では、概ね問題ないバリア性が確認されたが、MN含有率が22%を超えるサンプルでは、伸長されたものの一部において薬剤が支持体へ移行していることが示唆された。これは、伸長操作によりバリア層にクラックが発生していることによるものと推測された。
また、いずれの実験においても、放出率が65%を超えるサンプルが見られたが、これは測定誤差によるものと推測された。
(4-1 サンプルの作製)
支持体は実験1と同一とし、バリア層に使用するPVAのケン化度を、80%、90%、95.5%、及び98.5%(完全ケン化)の4段階設定した。各ケン化度のPVAとMNとを混合してバリアコート剤を調製し、実験1と同様の量および方法で塗布してバリア層を形成し、支持フィルムのサンプル120を作製した。バリア層におけるMNの含有率は10wt%とした。
a.粘着テープ122を30mm×100mmに切り出し、長手方向の一方の端部にシリコーンを塗布したPETシート121を貼り付けてから、図11に示すように、サンプル120のバリア層に貼り付ける。
b.粘着テープ122及びPETシート121を、サンプル120ごと図12に示すような25mm×90mmの大きさに切り出す。
c.図13に示すように、アクリル板130に25mm×90mmの両面テープ131を2本並べて貼り付け、切り出した粘着テープ122の支持体側と両面テープ131とを、2本の両面テープ131を覆うように接着する。粘着テープ122の幅方向にはみ出した両面テープ131の一部は切り落としてアクリル板130から除去する。
d.50mm×100mmの補強テープ132を準備し、図14に示すように、サンプル120に接着されていない粘着テープ122の端部に、PETシート121を厚さ方向に挟むように補強テープ132を貼り付けて評価片140を作製する。
e.評価片140を40℃の温湯に浸けて30分間放置する。このとき、粘着テープ122の全体を湯の中に位置させる。
f.30分経過したら評価片140を温湯から引き上げ、水分を軽く拭き取ってから引張試験機にセットする。このとき、図15に示すように、一方のチャックにアクリル板130を固定し、他方のチャックに補強テープ132のPETシートに接着されていない側の端部を固定する。
g.評価片を300mm/minの引張速度で牽引し、粘着テープ122が支持体から完全に剥がれた時点で測定を終了する。引張量10mmから30mmの範囲における引張試験機の張力値Nの平均値を耐水密着力とした。サンプルごとに3つの評価片を作製して耐水密着力を評価した。
結果を表4に示す。PVAのケン化度が95.5%以下の評価片では、張力値Nの平均値が10ニュートン(N)以上であり、良好な耐水密着力を示したが、ケン化度98.5%の評価片では、著しく耐水密着性が低下した。したがって、支持体とバリア層との良好な密着性を得るためには、水溶性高分子化合物のケン化度は95.5%以下が好ましいと考えられる。
実験5では、より医療用貼付剤の使用環境に近い評価として、入浴による評価を行った。
(5-1 サンプルの作製)
a.支持フィルムの作製
支持体は実験1と同一とし、バリア層に使用するPVAのケン化度を、80%、90%、95.5%、及び98.5%(完全ケン化)の4段階設定した。各ケン化度のPVAとMNとを混合してバリアコート剤を調製し、実験1と同様の量および方法で塗布してバリア層を形成し、4種類の支持フィルムを作製した。バリア層におけるMNの含有率は10wt%とした。
b.粘着層の作製
本実験のサンプルにおける粘着層の処方を以下に示す。
SIS 100部
脂環族飽和炭化水素樹脂 80部
流動パラフィン 10部
上記の処方に従い、SIS、脂環族飽和炭化水素樹脂および流動パラフィンをトルエンに溶解して塗工液を調製した。この塗工液を上述の支持体フィルムのバリア層側に塗布した後、溶媒を乾燥除去して粘着層を形成した。
以上により、乾燥後の粘着層の密度が約100g/m2のサンプルを得た。
上記のサンプル(10cm2)をヒトの上腕部に貼付した。貼付直後に1回目の入浴を行い、さらに貼付後24時間後に2回目の入浴を行った後、貼付剤の支持フィルムの剥離の有無を目視により評価した。体を洗う等の入浴中の動作は、平常時と同様とした。
評価はn=2で行い、評価基準は次のとおりとした。
○:2名とも支持フィルムの剥離を認めなかった、△:2名中1名で支持フィルムの剥離を認めた、×:2名ともに支持フィルムの剥離を認めた。
結果を表5に示す。ケン化度が95.5%以下では、被検者のいずれにおいてもサンプルの支持体の剥離を認めなかったが、完全ケン化PVAを用いたサンプルにおいては、1名に支持体の剥離を認めた。
実験4および5の結果を総合的に判断すると、本実施形態の医療用貼付剤1において良好な耐水密着性を示すPVAケン化度の好適範囲は、95.5%以下であると考えられた。
例えば、本発明の医療用貼付剤においては、PVAのケン化度を上述の範囲に設定することは必須ではない。したがって、耐水性がそれほど要求されないような部位に貼付される等の場合は、ケン化度の値が上述の範囲外であるPVAがバリア層に用いられてもよい。

Claims (2)
- ポリウレタンを含む支持体の一方の面に、水溶性高分子化合物とモンモリロナイトとを含有したバリア層を積層した支持フィルムと、
さらに、前記支持フィルムのバリア層側に、薬剤および可塑剤を含有する粘着層を積層した医療用貼付剤であって、
前記バリア層における前記モンモリロナイトの含有率が、2重量パーセント以上22重量パーセント以下に設定されていることを特徴とする医療用貼付剤。 - 前記水溶性高分子化合物がポリビニルアルコールであることを特徴とする請求項1に記載の医療用貼付剤。
