Definitions
- the present invention relates to a method or apparatus for determining the stage of chronic kidney disease or an operating method thereof.
- CKD Chronic Kidney Disease
- End-stage renal failure has been increasing all over the world, and as a countermeasure, it is important that early detection and, therefore, early treatment of so-called “hidden kidney disease” be important.
- diabetic nephropathy one of the three major complications of diabetes. Because diabetic nephropathy is CKD, diabetes becomes a CKD reserve. According to the 2006 National Health and Nutrition Survey (Ministry of Health, Labor and Welfare), there are an estimated 8.2 million diabetics and 18.7 million with additional diabetes reserves. Since 30% of these patients are thought to have renal failure, CKD patients are expected to increase in the future. For example, the medical cost of dialysis treatment in Japan is about 5.4 million yen per person per year, which is 1.5 trillion yen per year as a whole. As the number of dialysis patients is steadily increasing, medical costs for dialysis treatment are also increasing, and measures to control medical costs are urgently needed. For this reason, it is important all over the world to detect kidney damage early, treat early so as not to worsen the symptoms, and ultimately reduce even one dialysis patient.

- a blood-derived specimen or a urine specimen can be used, but it is not particularly limited.
- the blood-derived specimen for example, whole blood, serum, or plasma can be used. Among them, serum and plasma are preferable. Further, plasma is more preferable from the viewpoint that the separation time can be shortened or the degradation of the marker to be evaluated is prevented.
- a substance exhibiting an anticoagulant action by chelating action or the like may be added to these blood-derived specimens. Examples of such a substance exhibiting an anticoagulant action include EDTA (Ethylene Diamine-Tetraacetic Acid). Moreover, you may add the substance which shows a protease inhibitory effect for the same purpose. Examples of the substance exhibiting such anticoagulant action include aprotinin.

- the abundance of one marker selected from the group consisting of (1) to (16) or (10) to (16) When quicker or simpler determination is required, it is preferable to evaluate the abundance of one marker selected from the group consisting of (1) to (16) or (10) to (16) . When more reliable determination is required, it is preferable to evaluate the abundance of two or more types of markers selected from these groups. Depending on the degree of certainty required, the abundance of the three types of markers may be evaluated, or the abundance of the four types of markers may be evaluated.
- GFR has been used as an indicator of CKD severity, but it is calculated based on serum creatinine concentration. Since serum creatinine concentration is strongly influenced by muscle mass, accurate determination is made by eliminating that effect. There was a problem that it was difficult to do. In addition, there was a problem that it was difficult to find a mild kidney injury because no increase in serum creatinine concentration was observed until eGFR was 50% or less.
- none of the markers (1) to (16) has been used as a conventional marker. By using these as markers, the stage of chronic kidney disease can be more appropriately determined. In particular, since markers (1) to (9) tend to increase in abundance at a milder stage, milder renal disorders can be found by using these markers.
- Stage determination step of chronic kidney disease is a reference existence in which the abundance of the marker evaluated in step (A-1) is determined for each stage. In comparison with the dose range, when the abundance of all markers is within the reference abundance range of the same stage, it is a step of determining that chronic kidney disease in the subject is in the stage.
- step (A-2) the presence of at least one marker in step (A-2) indicates that the stage of chronic kidney disease in the subject is severe It is a step of determining.
- the classification of disease stage is not limited, but for example, the classification shown in Table 1 can be used. In addition, classifications generally accepted in the field can also be used.

- a standard stage may be determined, and a magnification with respect to the abundance of the marker in the standard stage may be set as a standard abundance range.
- a magnification with respect to the abundance of the marker in the standard stage may be set as a standard abundance range.
