Abstract
Description
血栓塞栓症の予防および/又は治療薬としては、注射剤としてヘパリン、経口剤としてワルファリンなどが広く用いられてきており、近年では新たに直接型トロンビン受容体阻害剤、直接型活性化血液凝固第X因子(FXa)阻害剤などが開発されてきている。
これらのなかで、ヘパリン、低分子ヘパリン、および未分画ヘパリンのヘパリン類は、静脈内投与用注射剤や皮下投与用注製剤であり、非経口的投与である点で利便性などに問題がある。一方、ワルファリンは、効果発現までに時間がかかる点、投与患者の個体間による効果のばらつきが大きいこと、他の薬剤との薬物相互作用があり、併用するときに注意を要する点、また食餌の影響を受ける点、および投与患者に対してPT-INR(prothrombin time-international normalized ratio:プロトロンビン時間-国際標準比)のモニターリングが必要である等の問題がある。
また、上記のように血液凝固剤を用いた抗凝固薬療法においては、血栓塞栓症の予防及び/又は治療効果の反面、主薬理に基づく副作用として、出血のリスクを有することがひろく認知されており、出血リスクを軽減する薬剤の提供は極めて有用である。
主薬理作用に基づく出血リスクを低減するために、静脈内投与用注射剤、皮下投与用注射剤などの注射剤であるヘパリン類では、投与が必要とされる患者のクレアチニンクリアランス(Ccr)や体重を指標として、投与する薬剤量を調節する薬剤が知られている(例えば、非特許文献3参照。)。
一般的に、注射剤では、投与量の増加にともなった血中濃度の増加が、経口剤に比べて直線的であり、また投与した患者間のバラツキが少ない。また、注射剤は、投与する量を調節しやすいことから、患者の固有因子、例えばクレアチニンクリアランス(Ccr)や体重等によって、薬剤の投与量を調節することが可能な薬剤があるものと考えられる。一方、経口剤では、投与量の増加にともなう血中濃度の増加のバラツキが、投与患者間で大きな薬剤が多い。したがって、経口剤における投与量調節の困難性から、経口抗凝固剤では、クレアチニンクリアランス(Ccr)や体重などの患者個人の固有因子等を指標として薬剤の投与量を調節し、抗凝固作用を維持し、かつ出血のリスクを低減させる薬剤は知られていない。

本明細書における投与薬剤の用量決定因子としては、(A)体重;及び/又は(B)クレアチニン・クリアランス(Ccr)を意味し;それぞれの投与薬剤の用量を決定する基準値としては、
(A)体重においては:55kg~65kgの範囲が好ましく;65kgがより好ましく;60kgが特に好ましく;また
(B)クレアチニン・クリアランス(Ccr)においては:50ml/minが好ましい。
本明細書における用量決定因子による減量の好ましい態様としては以下のものを挙げることができる。例えば、用量決定因子である体重(BW)が、60kg以下に下がった場合、投与量を半量に減量するのを選択することが好ましい。この場合、用量決定因子であるBWが60kg以下に下がった時点から、一週間以上後に体重を再測定して60kg以下であることを確認し、かつ、薬剤の投与を開始した時点の体重(初期体重)に比べて10%以上の体重減少であった場合に半量に減量を選択することがより好ましい。さらに、半量に減量後に用量決定因子であるBWが、60Kgを超えた場合であっても、投与量は減量したままでよい。また、例えば、用量決定因子であるクレアチニン・クリアランス(Ccr)が、50ml/min以下[かつ30ml/min以上(≧30ml/min~≦50ml/min)]になった場合、投与量を半量に減量するのを選択することが好ましい。この場合、用量決定因子であるCcrが50ml/min以下[かつ30ml/min以上(≧30ml/min~≦50ml/min)]になった時点から、一週間後にCCrを再測定して50ml/min以下[かつ30ml/min以上(≧30ml/min~≦50ml/min)]であることを確認し、かつ、薬剤の投与を開始した時点のCCr(初期CCr)に比べて20%以上の減少であった場合に半量に減量するのを選択することがより好ましい。さらに、半量に減量後に用量決定因子であるCCrが、50ml/minを超えた場合であっても、投与量は減量したままでよい。
本明細書における用量決定因子の基準値を基に、投与量の適宜減量を選択することとしては、(1)体重(BW)、(2)クレアチニン・クリアランス(Ccr)をそれぞれ測定し、いずれの因子かが上記の基準値以下になったときに半量に減量するのが好ましい。本明細書における用量決定因子としては、体重(BW)が好ましく;の基準値以下になったときに、基準値より大きい患者への投与量の半量に減量するのを選択することがより好ましい。
本明細書における、「用量決定因子の基準値」に基づく投与量調整としては、3/4~1/2に減量するのを選択することが好ましく;1/2に減量を選択することがより好ましい。「投与が必要とされる患者の用量決定因子の測定値を基に、該用量決定因子の基準値を指標として、投与量を選択できる」、のより具体的な態様としては、(1)60mgの投与を30mgに減量;又は(2)30mgの投与を15mgに減量;が、好ましい。
本明細書における、より具体的な好ましい態様としては、以下のものを挙げることができる。
すなわち、(A)投与が必要とされる患者の体重が60kgより重い場合、のフリー体である式(1)の化合物に換算し、1日当たり30mg~60mgの範囲;体重が60kg以下の場合、のフリー体である式(1)の化合物に換算し、1日当たり、体重が60kgより重い患者に対して投与される量の半量投与するのを選択することが好ましく;
(B)投与が必要とされる患者の体重が60kgより重い場合、のフリー体である式(1)の化合物に換算し、1日当たり30mg又は60mgを投与し、体重が60kg以下の場合、のフリー体である式(1)の化合物に換算し、1日当たり、体重が60kgより重い患者に対して投与される量の半量投与するのがより好ましい。
本明細書における血液凝固に起因する疾患としては、血栓症及び/又は塞栓症が挙げられ;血栓塞栓症としてひろく認知されている。
本明細書における、血栓塞栓症としては、例えば下記の
「脳梗塞、脳塞栓、心筋梗塞、狭心症、肺梗塞、肺塞栓、急性冠動脈症候群(ACS)、バージャー病、深部静脈血栓症、汎発性血管内凝固症候群、人工弁/関節置換後の血栓形成、血行再建後の血栓形成及び再閉塞、多臓器不全(MODS)、体外循環時の血栓形成又は採血時の血液凝固、静脈血栓塞栓症(VTE)及び非弁膜性心房細動(NVAF)」等からなる群より選ばれる1以上の疾患或いはそれら疾患に基づく血栓塞栓症;を挙げることができるが、なんらこれらの疾患のみに限定されるものではない。
血栓塞栓症としては、下記の
脳梗塞、脳塞栓、心筋梗塞、狭心症、肺梗塞、肺塞栓、急性冠動脈症候群(ACS)、バージャー病、深部静脈血栓症、汎発性血管内凝固症候群、人工弁/関節置換後の血栓形成、血行再建後の血栓形成及び再閉塞、多臓器不全(MODS)、体外循環時の血栓形成又は採血時の血液凝固、静脈血栓塞栓症(VTE)、及び非弁膜性心房細動(NVAF)に基づく血栓塞栓症;
が好ましく;静脈血栓塞栓症(VTE)及び非弁膜性心房細動(NVAF)に基づく血栓塞栓症がより好ましい。
本明細書中における血栓塞栓症の検査の指標となるマーカーとしては、血清中のD-ダイマー(D-dimer)、F1+2、TAT、可溶性フィブリン、PICなど挙げることができる。例えば、抗Xa活性、D-ダイマーとは、血栓が溶解する過程で生じる分解産
物であり、血栓症の二次線溶において上昇するマーカーであり、TATならびにF1+2はトロンビン生成のマーカーであり、凝固活性の指標である。また、可溶性フィブリンは、フィブリン生成のマーカーであり、凝固能の亢進をより反映する可能性が考えられる。PICは、線溶系亢進の指標であり、抗Xa活性は、主薬理作用を確認するためにも有効
である。
本明細書における短期間としては、薬剤の投与期間として2~4週間;好ましくは2週間以内を意味する。また、長期間としては、薬剤の投与期間として20日を越える期間;好ましくは4週間を超える期間を意味するものである。
(1)大出血(Major Bleeding):(A)致死的な出血、(B)2g/dLを超えるヘモグロビン量の低下を伴う、臨床的に明らかな出血、(C)4単位(1単位は約200cc)を超える輸血(貯血した自己血輸血を除く)を要する臨床的に明らかな出血、(D)後腹膜出血、頭蓋内出血、眼内出血又は髄腔内出血、(E)再手術を要する出血;
(2)臨床的に重要な出血(Clinically Relevant Non-Major Bleeding):大出血には該当しない以下の出血を臨床的に重要な出血とする。(A)長径が5cm以上の血腫、(B)外的な要因がなく発現し、5分以上継続する鼻出血或いは歯茎の出血、(C)消化管出血、(D)24時間以降も消失しない肉眼的血尿、(E)その他、治験責任医師又は治験分担医師が臨床的に重要な出血と判断した出血;
(3)小出血(Minor Bleeding):大出血にも臨床的に重要な出血にも該当しないすべての出血事象。
本明細書におけるQDとは、薬剤の投与回数を示す「Quaque Die」の略語であり、一日に一回投与することを意味する。
本明細書における製剤としては、経口製剤を意味し、有効成分及び薬理学的に許容される添加剤とともに、例えば錠剤、顆粒剤、粉末剤、カプセル剤又はシロップ剤などの投与剤形で患者に投与される。薬理学的に許容される添加剤としては、例えば賦形剤、崩壊剤、結合剤、流動化剤、滑沢剤、着色剤、光沢化剤等を挙げることができる。
本明細書における、有意差検定、データの解析手法としては、一般的に汎用されているものである。解析においては、商業的に入手可能な汎用の統計ソフト等を使用してもよい。
(実施例1)
人工膝関節全置換手術を施行した患者を対象とし、化合物(1a)の深部静脈血栓症(DVT)、肺塞栓症(Pulmonary Embolism:PE)の予防効果(有効性)の用量反応性の検証及び安全性について、プラセボを対照とした無作為化二重盲検用量比較試験により検討した。
投与量及び投与方法としては、手術後6~24時間に投与を開始し、翌日以降は原則として朝に投与した。治験薬である化合物(1a)を有効成分として含有する製剤を、1日1回、11~14日間経口投与するものである。
投与群(グループ)としては、プラセボ群、化合物(1a)群(5mg、15mg、30mg、60mg[各投与量はフリー体である化合物(1)の量として換算したものである。])計5群(グループ)に分けた。
治験方法は、治験参加者の同意を取得後、術前14日以内に事前検査を行い、術後6~24時間以内に薬剤或いはプラセボの投与を開始し、11~14日間投与を行った。投与終了後、24時間以内に静脈造影検査を実施し、さらに投与終了後25~35日に事後検査を行った。
主要評価項目としては、治験薬投与開始後から治験薬投与終了時の静脈造影検査実施までに認められた以下の血栓塞栓性イベントを一つ以上発現した症例の頻度を、有効性の主要評価項目として評価した。(1)DVT(治験薬投与終了時の両側下肢の静脈造影によって評価されたDVT)、(2)確定診断された症候性のPE、(3)規定の静脈造影前に確認された症候性のDVT、及び(4)VTE(DVT若しくはPE);を評価した。
安全性としては、大出血、臨床的に重要な出血及び小出血の発現率;を評価した。
バイオマーカーとしては、(1)抗Xa活性;(2)D-ダイマー(D-dimer);
(3)F1+2;(4)TAT;(5)可溶性フィブリン;及び(6)PIC;を評価した。なお、バイオマーカーは、有効性を探索的に検討するために設定した。D-dimerは二次線溶を反映する指標であるため設定した。TATならびにF1+2はトロンビン生成のマーカーであり、凝固活性の指標になることから設定した。また、近年測定可能となった可溶性フィブリンは、フィブリン生成のマーカーであり、凝固能の亢進をより反映する可能性が考えられるため設定した。PICは、線溶系亢進の指標であるため設定した。抗Xa活性は、主薬理作用を確認するため設定した。
主な統計解析としては、血栓塞栓性イベント発現率について、Cochran-Armitage検定を用いて、用量反応性を検証した。また、プラセボ群と各化合物(1a)群の差及びその95%信頼区間を算出し、Shirley-Williams検定手順を用いてプラセボと各化合物(1a)群間の比較を行った。また、大出血の発現率、臨床的に重要な出血の発現率について、プラセボ群との各化合物(1a)群の差及びその95%信頼区間を算出し、χ2検定を用いてプラセボ群と各化合物(1a)群の2群比較を実施した。
上記の治験は、化合物(1a)の後期第2相臨床試験(深部静脈血栓症)(人工膝関節全置換術患者を対象とした無作為化二重盲検プラセボ対照用量比較試験)での治療効果を評価したものである。本治験は、薬事法第14条第3項及び第80条の2に規定された基準ならびに平成9年3月27日付厚生省令第28号「医薬品の臨床試験の実施の基準に関する省令」、(以下GCP省令という)を遵守して実施されたものであり、実施に際しては、ヘルシンキ宣言の倫理的原則を遵守し、被験者の人権、福祉及び安全を最大限に確保したものである。また、本治験は、実施医療機関の治験審査委員会、又はGCP省令第27条に規定する治験審査委員会において審査され、承認された後に実施したものであり、年に1回又は治験審査委員会の求めに応じてそれ以上の頻度で、治験の継続の可否について審査を受けたものである。[本治験のプロトコール番号:文書情報「PRT_CP2P3v1.0J」]
<試験結果> 化合物(1a)投与群、プラセボ投与群(グループ)における各群間の被験者の年齢、体重などに大きな差は無かった。
[図1]及び[図2]に示すように、化合物(1a)は無症候性静脈血栓塞栓症、及び症候性静脈血栓塞栓症を、39~81%、それぞれ化合物(1a)を5mg~60mg[各投与量はフリー体である化合物(1)の量として換算したものである。]の投与量に依存した抑制効果がみられ、抑制効果は、プラセボ投与に比較して有意(P≦0.005)な抑制であった。また、[図3]に示すように、化合物(1a)の投与量(5mg~60mg)に依存して出血のイベントは増加したが、出血イベントの発現率はプラセボ投与群との差がなかった。また、血清生化学的検査結果から、化合物(1a)投与群とプラセボ投与群では肝機能には差がなかった。
(実施例2)
非弁膜性心房細動患者を対象として、ワルファリンカリウム(以下ワルファリン)を対照に、各化合物(1a)投与群の出血性イベント発現率を比較した。また、副次的に有効性評価として血栓塞栓性イベント発現頻度、薬力学的指標及びバイオマーカーを、安全性評価として有害事象と副作用の発現頻度を比較した。治験としては、多施設共同無作為化用量比較試験であり、化合物(1a)投与群は二重盲検であり、ワルファリン(Warfarin)投与群はオープンラベルである。対照薬のワルファリンは、非弁膜性心房細動患者の塞栓症に対する有効性が確立しているが、用量調節が容易でないためオープンラベルでの評価とし、化合物(1a)投与群のみ用量間の二重盲検法とした。また、化合物(1a)投与群である30mg、45mg、60mg[各投与量はフリー体である化合物(1)の量として換算したものである。]の3つの群(グループ)、及びワルファリン投与の合計4群は、原則1日1回12週間経口投与した。なお、ワルファリン投与群では、ワルファリンの0.5mg錠及び1mg錠を使用し、PT-INRを2.0以上3.0以下(70歳以上は1.6以上2.6以下)にコントロールできる用量を原則1日1回12週間経口投与した。
治験は、被験者の同意を取得後、登録前に頭部画像診断を実施し、出血性所見がないことを確認し、頭部画像診断(CT又はMRI)、経胸壁心エコー図検査、誘導心電図検査、血圧、脈拍数及び血液学・生化学的検査を行った。また、中央測定として、化合物(1a)投与群の投与開始前と治療期治験薬投与開始後4週目の治験薬服薬前(トラフ)及び治験薬服薬1~3時間後、8週目の治験薬服薬前(トラフ)、12週目の来院時又は中止時(可能な限り)、及び治療期治験薬投与終了後又は中止後4、8週目の来院時に、下記の薬力学的指標及びバイオマーカー測定のための採血を行った。一方、ワルファリン投与群では投薬開始前と治療期治験薬投与開始後4、8、12週目の来院時又は中止時(可能な限り)、及び治療期治験薬投与終了後又は中止後4、8週目の来院時に下記の薬力学的指標及びバイオマーカー測定のための採血を行った。[薬力学的指標:PT、PT-INR、APTT];[バイオマーカー:F1+2、TAT、D-Dimer]
主要評価項目としては、治療期間中に認められた出血性イベント[大出血(Major)、臨床的に重要な出血(Clinically Relevant)、小出血(Minor)]の発現頻度を検討した。
また、副次評価項目として、1)血栓塞栓性イベントの発現頻度;2)有害事象及び副作用の発現頻度;3)治療期間中に認められた有害事象及び副作用の発現頻度を検討した。
薬力学的指標は、心房細動患者における抗凝固能を検討するために設定した。
バイオマーカーであるF1+2ならびにTATはトロンビン生成のマーカー、D-dimerは二次線溶のマーカーであり、心房細動患者において上昇するとともにワルファリンによる抗凝固療法により低下するとの報告がある(N.Veneら,Thromb Haemost 2003;90:1163-72)。また、化合物(1a)の前期第II相試験において、化合物(1a)を投与することにより、これらのバイオマーカーの低下が認められている。したがって、本試験ではこれらのバイオマーカーに対する化合物(1a)及びワルファリンによる影響を検討した。
薬物動態評価項目としては、(A)血漿中の化合物(1)の濃度;(B)母集団薬物動態パラメータ;(C)被験者ごとの薬物動態パラメータ;及び(D)各被験者の薬物動態パラメータと出血性イベント発現との相関;を評価した。これら評価項目の設定根拠としては、心房細動患者集団における薬物動態特性を明らかにするためである。また、出血性イベントと薬物動態パラメータの相関性は、化合物(1a)の臨床推奨用量を決定する際の参考とするためである。
主要評価項目の解析方法としては、主解析は、出血性イベント発現頻度について、ワルファリン投与群との各化合物(1a)投与群の差及びその95%信頼区間をScore法(事前検査において、ヘモグロビンが10g/dL未満或いは血小板数が10×104/μL未満)を用いて算出した。投与群ごとに出血性イベント発現頻度及びその95%信頼区間もScore法[登録時に活動性出血をきたしている;なお、皮下出血においては、長径5cm以上の血腫が1つ以上認められた場合。尿検査においては、肉眼的血尿、尿潜血2+以上、尿潜血+かつ尿沈渣赤血球が10個/HPF以上が認められた場合(ただし、同意日の1年前より継続的に尿潜血2+が認められた場合は除く)。]を用いて算出した。さらに化合物(1a)群間においてもΧ2検定を用いた2群比較を行った。
副次解析としては、出血性イベント発現頻度について、Cochran-Armitage検定を用いて次の仮説(対比)を有意水準片側0.025で検討した。副次評価項目の解析としては、非弁膜性心房細動(NVAF)のリスクを有する患者での、出血性イベント発現頻度について、ワルファリン群との各化合物(1a)群の差及びその95%信頼区間を算出した。また、Cochran-Armitage検定を用いて、化合物(1a)群の用量反応性について検討した。
本治験は、薬事法第14条第3項及び第80条の2に規定された基準ならびに平成9年3月27日付厚生省令第28号「医薬品の臨床試験の実施の基準に関する省令」(以下GCP省令という)を遵守して実施する。実施に際しては、ヘルシンキ宣言の倫理的原則を遵守し、被験者の人権、福祉及び安全を最大限に確保したものである。[治験計画番号:DU-176b-C-J225_Ver.03.05.000]
<試験結果> [図4]に示すように、各群間(グループ間)においては、被験者の年齢、体重、リスクファクターなどに大きな差は無かった。
[図5]に示すように、化合物(1a)の投与量が高くなるにしたがって出血性イベント[大出血(Major)、臨床的に重要な出血(Clinically Relevant)、小出血(Minor)]の発現頻度が高くなったが、その発現頻度はワルファリン(Warfarin)のものと有意差がなかった。
[図6]に示すように、化合物(1a)投与群を被験者の体重(BW)(BW≦60kg;及びBW>60kg)によって2つの群に分け、ワルファリン(Warfarin)投与群と出血性イベントの発現率をグラフで示したものであり;化合物(1a)投与群では、化合物(1a)の45mg/kg以上の投与で、被験者の体重(BW)がBW≦60kgの出血性イベントが、被験者のBW>60kgの群に比べて高かった。
[図7]に示すように、化合物(1a)投与群を被験者のクレアチニン・クリアランス(Ccr)(Ccr≦50;及びCcr>50)によって2つの軍に分け、ワルファリン(Warfarin)投与群と出血性イベントの発現率をグラフで示したものであり;化合物(1a)投与群では、45mg/kg以上の投与を行った患者群で、被験者のクレアチニン・クリアランス(Ccr)がCcr≦50の出血性イベントが、被験者のCcr>50の群に比べて高かった。
[図8]に示すように、化合物(1a)の投与を行った被験者の横軸に体重(BW)、縦軸にクレアチニン・クリアランス(Ccr)とした分布を示すものであり;塗りつぶし四角は出血性イベントを示した被験者を示したものである。[図9]は、[図8]の分布図を、(1)BW>60kg、Ccr≦50;(2)BW>60kg、Ccr>50;(3)BW≦60kg、Ccr>50;及び(4)BW≦60kg、Ccr≦50;の4つの群に分けたときの、(1)~(4)の各群での出血性イベント発現率の中央値と幅を示す図である。その結果、(4)BW≦60kg、Ccr≦50が出血性イベントの発現率が最も高く、被験者の体重(BW)がBW≦60kgの群である(3)及び(4)が、被験者のクレアチニン・クリアランス(Ccr)がCcr≦50である(1)及び(2)の群より出血性イベントの発現率が高かった。
さらに、心房細動患者において上昇するとともにワルファリンによる抗凝固療法により低下するとの報告がある、二次線溶のバイオマーカーであるD-dimer(D-ダイマー)は、化合物(1a)を投与後28日で、ワルファリンと同程度の数値を示した。また、ワルファリンの投与経歴のない被験者でも、化合物(1a)を投与後28日でワルファリン投与患者と同程度の数値を示した。

(iv)体重が60kg以下の場合、投与量が、体重が60kgより重い場合の投与量の1/4量~半量;
を選択できることを特徴とする、経口血液凝固抑制剤。 - (iv)体重が60kg以下の場合の投与量が、(iii)体重が60kgより大きい場合の投与量の半量である請求項10に記載の経口血液凝固抑制剤。
- 下記の式(1)
で表されるN1-(5-クロロピリジン-2-イル)-N2-((1S,2R,4S)-4-[(ジメチルアミノ)カルボニル]-2-{[(5-メチル-4,5,6,7-テトラヒドロチアゾロ[5,4-c]ピリジン-2-イル)カルボニル]アミノ}シクロヘキシル)エタンジアミド、薬理上許容される塩、又はそれらの水和物を有効成分として含有する経口血液凝固抑制剤であって、
投与が必要とされる患者の体重の測定値を基に、
(v)体重が60kgより重い場合の投与量として、式(1)の化合物に換算し、1日当たり60mg;又は
(vi)体重が60kg以下の場合の投与量として、式(1)の化合物に換算し、1日当たり30mg;
を選択できることを特徴とする、経口血液凝固抑制剤。 - 下記の式(1)
で表されるN1-(5-クロロピリジン-2-イル)-N2-((1S,2R,4S)-4-[(ジメチルアミノ)カルボニル]-2-{[(5-メチル-4,5,6,7-テトラヒドロチアゾロ[5,4-c]ピリジン-2-イル)カルボニル]アミノ}シクロヘキシル)エタンジアミド、薬理上許容される塩、又はそれらの水和物を有効成分として含有する経口血液凝固抑制剤であって、
投与が必要とされる患者の体重の測定値を基に、
(vii)体重が60kgより重い場合の投与量として、式(1)の化合物に換算し、1日当たり30mg;又は
(viii)体重が60kg以下の場合の投与量として、式(1)の化合物に換算し、1日当たり15mg;
を選択できることを特徴とする、経口血液凝固抑制剤。 - 血液凝固抑制剤が、活性化血液凝固第X因子阻害剤である請求項1~13のいずれか1項に記載の経口血液凝固抑制剤。
- 血液凝固抑制剤が、血栓塞栓症治療剤である請求項1~13のいずれか1項に記載の経口血液凝固抑制剤。
- 血栓塞栓症が、下記の:
脳梗塞、脳塞栓、心筋梗塞、狭心症、肺梗塞、肺塞栓、急性冠動脈症候群(ACS)、バージャー病、深部静脈血栓症、汎発性血管内凝固症候群、人工弁/関節置換後の血栓形成、血行再建後の血栓形成及び再閉塞、多臓器不全、体外循環時の血栓形成又は採血時の血液凝固、静脈血栓塞栓症及び非弁膜性心房細動;
からなる群より選ばれる1以上の疾患或いはそれら疾患に起因する血栓塞栓症である、請求項15に記載の経口血液凝固抑制剤。 - 血栓塞栓症が、深部静脈血栓症、静脈血栓塞栓症又は非弁膜性心房細動に基づく血栓塞栓症である、請求項15に記載の経口血液凝固抑制剤。
- 薬剤の形態が錠剤である、請求項1~13のいずれか1項に記載の経口血液凝固抑制剤。
- 錠剤が、式(1)の化合物に換算して、1錠あたり30mg又は60mg含み、錠剤中の有効成分を二等分する1本の割線を有する、請求項18に記載の経口血液凝固抑制剤。
- 下記の式(1)
で表されるN1-(5-クロロピリジン-2-イル)-N2-((1S,2R,4S)-4-[(ジメチルアミノ)カルボニル]-2-{[(5-メチル-4,5,6,7-テトラヒドロチアゾロ[5,4-c]ピリジン-2-イル)カルボニル]アミノ}シクロヘキシル)エタンジアミド、薬理上許容される塩、又はそれらの水和物を有効成分として含有する経口血液凝固抑制剤を投与する工程を包含する血栓塞栓症の治療方法であって、
(E)該血液凝固抑制剤の出血リスクに関与する因子を用量決定因子として選択し;
(F)該用量決定因子の基準値を定め;
(G)投与が必要とされる患者の用量決定因子を測定し; 次いで、
(H)該基準値を指標として、投与量が決定されることを特徴とする、方法。 - 下記の式(1)
で表されるN1-(5-クロロピリジン-2-イル)-N2-((1S,2R,4S)-4-[(ジメチルアミノ)カルボニル]-2-{[(5-メチル-4,5,6,7-テトラヒドロチアゾロ[5,4-c]ピリジン-2-イル)カルボニル]アミノ}シクロヘキシル)エタンジアミド、薬理上許容される塩、又はそれらの水和物を有効成分として含有する経口血液凝固抑制剤の投与量の判断基準を決める方法であって、
(I)該血液凝固抑制剤の出血リスクに関与する因子を用量決定因子として選択する工程;次いで
(J)該用量決定因子の基準値を定め、該該基準値を指標として該血液凝固抑制剤の投与量の判断基準が決定される工程;
を包含する、方法。 - 下記の式(1)
で表されるN1-(5-クロロピリジン-2-イル)-N2-((1S,2R,4S)-4-[(ジメチルアミノ)カルボニル]-2-{[(5-メチル-4,5,6,7-テトラヒドロチアゾロ[5,4-c]ピリジン-2-イル)カルボニル]アミノ}シクロヘキシル)エタンジアミド、薬理上許容される塩、又はそれらの水和物を有効成分として含有する経口血液凝固抑制剤の製造のための式(1)で表される化合物の使用であって、
(A)該血液凝固抑制剤の出血リスクに関与する因子を用量決定因子として選択し;
(B)該用量決定因子の基準値を定め;
(C)投与が必要とされる患者の用量決定因子を測定し;次いで、
(D)該基準値を指標として、投与量を選択できることを特徴とする、使用。 - 下記の(a)及び(b)からなる血栓塞栓治療用キット。
(a)請求項1~13の経口血液凝固抑制剤;及び
(b)該用量決定因子の該測定値が該基準値より大きい場合、投与量が、式(1)の化合物に換算し、1日当たり30~60mgであり;又は(ii)該用量決定因子の基準値以下の場合の投与量が、該用量決定因子の基準値より大きい場合の投与量の1/4量~半量の選択できる旨が記載された指示書;
