Definitions
- the invention refers to improvements introduced in a tool used in the operating rooms, whose purpose is the introduction of laryngeal masks, when a patient is intervened and it is necessary to be able to operate inside the oral cavity thereof, without the patient in a reflex act it closes the mouth unexpectedly, orienting said improvements in modifying said tool so that it serves both as a mouthpiece and as a guide means for the introduction of laryngeal masks, as well as other medical devices through the oral cavity.
- Another embodiment of the same type of tool is that shown in WO 2007/087446 which avoids the obstruction of the tongue when a laryngeal mask is inserted, and acts by exerting a lingual pressure, just like the aforementioned Patent ES No. 2,332 .616.
- Another embodiment of the same type of tool is the one that shows patents US 2004060564 and GB 2.436.294, which facilitates the introduction of the laryngeal mask when it reaches the back of the mouth, forcing the curvature thereof with the help of a curved rigid tube.
- the mouth is open, it remains open, without requiring the continued competition of the person who places the laryngeal mask, releasing a hand, in a way that facilitates the manipulation of it.
- the patient's tongue is in the lowest possible plane, giving the oral cavity the maximum space to maneuver.
- the palatal portion of the mouthwash moves the laryngeal mask into the hypopharynx.
- the perfected mouth opener facilitates the introduction of other medical devices in addition to the aforementioned masks, such as airway control devices, fibroscopes, gastroscopes, secretion aspirators and others.
- airway control devices such as airway control devices, fibroscopes, gastroscopes, secretion aspirators and others.
- fibroscopes In the open position, it facilitates the permeability of the airway and the patient's breathing.
- - In the open position prevents a possible bite of the patient from crushing the medical device that passes inside the mouth opener.
- the present invention has as its object a tool that acts firstly as a useful mouthpiece, and secondly as useful to facilitate the introduction of the mouthpiece.
- Laryngeal mask once the patient is open-mouthed, thanks to the introduction into the oral cavity of the tool that we have called as mouth opener.
- the oral cavity for the purposes of the invention is previously limited by the lips and front teeth, and subsequently by the oropharynx and the hypopharynx, superiorly by the palate with its two zones (the anterior the hard palate and the posterior the soft palate) , and lower the tongue. From the hypopharynx are born the ducts that are called esophagus and trachea.
- the mouth opener consists of the following parts:
- Said tool comprises a body formed by a substantially flat plate, with a rectangular perimeter, in the central part of which a large hole is provided, and from the lower base of the body of the plate emerges a rectangular extension similar to a handgrip of the tool.
- a second flat plate, screwed to the handle, is provided on the handle and serves as guide means for the lower intraoral or lingual portion, forming part of the self-locking mechanism, since it is in the working position in contact with the language, so that you can upload or to lower in relation to the body of the main plate in which the upper base of a superior or palatine intraoral portion is joined by being in working position with the palate. That is, the distance between the upper and lower portions can be regulated at the discretion of the anesthetist so that it is perfectly fixed to the patient's mouth, and adapts to the geometry of the patient's oral cavity, based on the premise that not all cavities mouths have the same geometry.
- the upper portion is configured according to a preferably concave profile one of whose ends is joined to the upper part of the body of the plate, while the lower portion substantially equal to the upper portion, is joined to a slide that moves between the two plates previously described.
- an adentellada zone that works together with a trigger that allows the translation in vertical direction of the slide and its immobilization in the appropriate position, and that make up the locking and unlocking mechanism.
- Figure 1 is a schematic sectional view of the human head and more specifically of the oral cavity and its surroundings.
- Figure 2 is a schematic sectional view of the human head and, more specifically, of the oral cavity with the intraoral portion of the mouthwash recommended in the opening position in the oral cavity when the laryngeal mask is inserted.
- Figure 3 is a schematized sectional view of the human head and more specifically of the oral cavity, with the intraoral portion of the mouthwash preconized in the open position with the laryngeal mask fully inserted.
- Figure 4 is a top plan view of a conventional laryngeal mask.
- Figure 5 is a bottom plan view of a conventional laryngeal mask.
- Figure 6 is a plan view of the body of the mouth opener.
- Figure 7 is a side elevational view of the mouthpiece in the open position.
- the invention advocated in one of its embodiments possible is formed by a flat plate (10), whose body (11) has a substantially rectangular perimeter, whose lower base (12) is extended on a handle (13) rectangular perimeter, on which is attached by any known method a small plate (14).
- the central part of the plate (10) has a large opening (17), and a small recess (18) in the central part of the lower base
- the plate (14) incorporates two pivots (36) perpendicular to the plate (14).
- the plate (10) acts as a support medium of two parts, an upper part that we denominate upper intraoral portion (15) or palatal, and a lower intraoral portion (16) that we will call lingual.
- the upper portion (15), also seen in figure 7, is joined to the plate (10) and its body (11), while the lower portion (16) runs between the handle (13) and the plate ( 14).
- the upper part (15) has a configuration, with a first part (15 a) flat that extends in a second part (15b) curve sensibly concave and directed downwards, while the lower portion (16) is substantially flat by one of its ends (16 a), while in the opposite (16 b) it is concave and directed downwards.
- Said lower portion (16) is bent at one of its ends downwards at a right angle, according to a vertical flat part (16c) and runs between the plate (14) and the handle (13) of the plate (10).
- the portion (16) incorporates a pivot (19) perpendicular to the portion vertical (16c).
- a laryngeal mask is formed, as shown in Figures 4 and 5 by a tube (32) one end of which ends in the part (33) which is the one that is attached to the larynx of the patient, such and as illustrated in figure 3.
- the mouth opener (10) operates when the anesthetist catches it in the closed position - that is, with the lingual portion (16) in contact with the palatal portion (15) - with one hand by the handle (13), introducing it into the the oral cavity (22) delimited superiorly between the palate (29) and inferiorly by the tongue (30), in the anterior part by the lips (23) and the tooth line (24), and in the posterior part by the oropharynx (27) and the hypopharynx
- the mouthpiece passes from the closed position to the opening, separating the upper (15) from the lower (16), as shown in figure 2, maintaining in opening position by the self-locking mechanism (20).
- the upper portion (15) rests on the palate (29), while the lower portion (16) seeks the plane of the tongue (30) by pressing it downwards, subsequently placing the laryngeal mask (31) whose formal characteristics can be seen in figure 4, so that its end part (33) is inserted by sliding through the upper portion
- the nature of the mouth opener (10) thanks to the possibility of regulating the distance between the upper (15) and lower (16), allows adapting to the geometry of each patient, not so much in terms of the height of the palate, but rather the curvature of the palate (29) in relation to the curvature of the oropharynx (27) and hypopharynx (28), facilitating that the extreme part does not get stuck in the area (27), as well as that the patient in a movement reflects closure the mouth so that the anesthetist loses sight of the oral cavity (22).
- the removal of the perfected mouthpiece (22) is facilitated by unlocking the portion (16) by activating the trigger (21) and the progressive removal of the intraoral portion (15 and 16) between the lips (23).
