- a predetermined value for example, 200 mmHg
- a predetermined algorithm is applied to the pulse wave amplitude superimposed on the cuff pressure to measure (calculate) the maximum blood pressure and the minimum blood pressure. .
- the speed at the time of pressurization is at least faster than the speed at the time of depressurization.
- the pressurization measurement processing unit 108 is connected to the oscillation circuit 33 and the adjustment unit 50, and performs a blood pressure measurement process in the pressurization process.
- the pressurization measurement processing unit 108 starts processing when it is determined by the determination processing unit 104 to measure blood pressure by the pressurization measurement method.
- the processing by the pressure measurement processing unit 108 may also be realized by a known method.
- the maximum blood pressure and Measure (calculate) diastolic blood pressure for example, in a pressurizing process at a slow speed, by applying a predetermined algorithm to the pulse wave amplitude superimposed on the cuff pressure based on the output signal from the oscillation circuit 33, the maximum blood pressure and Measure (calculate) diastolic blood pressure.
- pressurization is stopped and rapid exhaust is performed.
- Blood pressure values measured by the reduced pressure measurement processing unit 106 and the pressurization measurement processing unit 108 are output to the measurement result storage unit 43B and the display unit 40.
- each functional block may be realized by executing software stored in the memory unit 42, or at least one may be realized by hardware.
- FIG. 6 is a flowchart showing the flow of blood pressure measurement control in Embodiment 1 of the present invention.
- the processing shown in the flowchart of FIG. 6 is stored in advance in the memory unit 42 as a program, and the blood pressure measurement control function is realized by the CPU 100 reading and executing this program.
- selection receiving unit 102 receives a user selection (step S102). Specifically, for example, the selection receiving unit 102 displays a plurality of pieces of user identification information (for example, users A, B,%) On the display unit 40 and causes the user to select any one.
- a user selection for example, the selection receiving unit 102 displays a plurality of pieces of user identification information (for example, users A, B,%) On the display unit 40 and causes the user to select any one.
- the determination processing unit 104 refers to the measurement method management table 431 and the measurement subject at this time, that is, the measurement subject specified by the selected user identification information, sets the measurement method switching function to be effective. It is determined whether or not (step S104). That is, it is determined whether or not information “valid” is stored in the item 72 of the measurement method switching function corresponding to the selected user identification information.
- step S104 If it is determined that “valid” is set (“valid” in step S104), the process proceeds to step S106. On the other hand, if it is determined that invalidity is set (“invalid” in step S104), the process proceeds to step S114.
- step S106 the determination processing unit 104 receives an input of presence / absence of pregnancy from the user.
- FIG. 7 is a diagram showing an example of the screen displayed in step S106 of FIG. Referring to FIG. 7, for example, a message “Are you pregnant?” Is displayed on display unit 40, and button 401 indicating “YES” and button 402 indicating “NO” are displayed. .
- the measurement method according to the measurement subject is a pressurization measurement method. Determination is made (step S110). This is because, as described above, in the case of a pregnant woman, the pressure measurement method can measure blood pressure with higher accuracy.
- the measurement method is determined to be a decompression measurement method (step S112).
- the reason why the pressure reduction measurement method is used is that the default (initial setting) measurement method of the sphygmomanometer 1 according to the present embodiment is the pressure reduction measurement method.
- the method according to the desired measurement method may be determined as the current measurement method.
- step S114 the determination processing unit 104 sets the measurement method set in advance, that is, the measurement method stored in the desired measurement method item 73 of the measurement method management table 431 as the measurement method suitable for the measurement subject. judge.
- step S110 When the process of step S110, S112, or S114 is completed, a control switching process is executed (step S116), and a blood pressure measurement process using the determined method is executed (step S118).
- step S116 for example, insertion / deletion of filter processing in the pressurization process and change of control parameters are performed.
- the filtering process (removal of noise from the driving sound of the pump 51) is not necessary in the pressurization process, but in the case of the pressurization measurement method.
- pressurization is performed at a gradual speed, but in the case of the depressurization measurement method, it is necessary to pressurize at a higher speed than in the case of the pressurization measurement method, and thus the control parameter is changed.
- step S118 if the determined method is the reduced pressure measurement method, the process by the reduced pressure measurement processing unit 106 is executed. If the determined method is a pressure measurement method, the process by the pressure measurement processing unit 108 is executed.
- control switching process is executed in step S116 on the premise that the control parameters and the like at the previous measurement are stored in a predetermined area of the flash memory 43 (regardless of the person to be measured). This process is not essential. That is, for each measurement, a program corresponding to the determined measurement method may be read from the memory unit 42 and each process may be executed.
- the measurement result is output to the flash memory 43 and the display unit 40 (step S120).
- the blood pressure measurement control in the present embodiment is thus completed.
- the measurement method switching function is enabled, blood pressure is measured by the pressurization measurement method when the measurement subject is a pregnant woman. Therefore, it is possible to accurately measure blood pressure regardless of whether the measurement subject is a pregnant woman or not.
- the user can set in advance whether the measurement method switching function is valid / invalid, and if it is invalid, the desired measurement method can be adopted regardless of the presence or absence of pregnancy. Therefore, the measurement method can be determined flexibly for each person to be measured.
- the validity / invalidity of the measurement method switching function is set in advance, but this is not restrictive. For example, first, the presence / absence of pregnancy may be determined, and when it is determined that the patient is not pregnant, the desired measurement method stored in the measurement method management table 431 may be determined as the current measurement method.
- the measurement method management table 431 itself is not essential, and when it is determined that the patient is not pregnant, it may be selected which one is desired each time.
- the user is allowed to select whether or not she is pregnant every time, but such information may be further stored in the measurement method management table 431.
- information on the presence / absence of pregnancy corresponding to the user is read in step S106, and it is determined whether or not the user is a pregnant woman based on the information (S108). Good.
- the measurement method is determined from the viewpoint of measurement accuracy.
- the measurement method is determined from the viewpoint of measurement time.
- Measurement time refers to the time from the start of cuff pressurization to the end of cuff decompression.
- FIG. 8 is a flowchart showing a conceptual flow of blood pressure measurement control in Embodiment 2 of the present invention.
- the same step number is attached
- steps S106 and S108 in FIG. 6 instead of the processes in steps S106 and S108 in FIG. 6, processes in steps S206 and S208 are executed, respectively.
- the process of step S116A is executed instead of the process of step S116.
- step S104 determines whether it is determined as “valid” in step S104. If it is determined as “valid” in step S104, the determination processing unit 104 executes a simulation of the measurement time based on, for example, the user's past measurement values stored in the measurement result storage unit 43B (step S206). ). If the method with the shorter measurement time is the pressurization measurement method, the process proceeds to step S110. If the method with the shorter measurement time is the depressurization measurement method, the process proceeds to step S112.
- the decompression measurement processing unit 106 estimates the systolic blood pressure in the pressurizing process, and shifts to the decompression control at the slow speed when the systolic blood pressure is estimated.
- the pressurization measurement processing unit 108 is configured to rapidly depressurize, that is, exhaust, when the systolic blood pressure is detected in the pressurization process at the slow speed.
- FIG. 9 is a diagram showing the difference in maximum pressure value between the pressurization measurement method and the depressurization measurement method.
- the control pressure value in the pressurization measurement method is shown along the time axis.
- FIG. 9B shows the control pressure value in the reduced pressure measurement method along the time axis.
- the blood pressure is measured in the pressurization process, and therefore, pressurization control is performed at a slow speed and with a constant amount of pressure change per predetermined time.
- blood pressure values that is, the minimum blood pressure and the maximum blood pressure are measured (calculated) in real time. Then, when the maximum blood pressure is calculated, the pressurization control is stopped, and the process proceeds to rapid pressure reduction.
- the systolic blood pressure calculated in the pressurizing process is expressed as a pressure value SYS
- the maximum pressure value MAXa is a value that is hardly different from the pressure value SYS.
- blood pressure is measured in the reduced pressure process, so the speed in the pressurized process is faster than the speed in the reduced pressure process (pressure change per predetermined time). It is controlled to become.
- the systolic blood pressure is estimated.
- the pressurization control is stopped when the estimated systolic blood pressure + predetermined value (for example, 40 mmHg) is reached, and the process shifts to slow pressure reduction.
- the systolic blood pressure estimated in the pressurizing process is expressed as a pressure value E_SYS, the operation stops at a pressure value MAXb higher than the pressure value E_SYS by a predetermined value.
- step S116A since the blood pressure detection process is executed in the pressurization process in any measurement method, the filter need not be switched in the pressurization process.
- FIG. 10 is a diagram illustrating the difference in measurement time between the pressurization measurement method and the decompression measurement method when the systolic blood pressure is high.
- FIG. 10A the control pressure value when measured by the pressurization measurement method is shown along the time axis.
- FIG. 10B shows the control pressure value along the time axis when measured by the reduced pressure measurement method.
- FIG. 11 is a flowchart showing the flow of specific example 1 of the blood pressure measurement control in the second embodiment of the present invention.
- steps S216 and S218 are executed in place of steps S206 and S208 in FIG. 8, respectively.
- step S216 for example, the previous systolic blood pressure is read from measurement result storage unit 43B. More specifically, the systolic blood pressure data included in the measurement result data of the latest date and time is read out from the measurement result data stored in association with the user identification information selected in step S102.
- step S218 it is determined whether or not the read systolic blood pressure is less than a predetermined value, for example, 170 mmHg. If the maximum blood pressure is less than 170 mmHg, the process proceeds to step S110, and if the maximum blood pressure is 170 mmHg or more, the process proceeds to step S112. That is, if the maximum blood pressure is 170 mmHg or more, it is determined that the measurement time is shorter in the reduced pressure measurement method, and the current measurement method is set as the reduced pressure measurement method.
- a predetermined value for example, 170 mmHg.
- the pressurization measurement method is used because the default measurement method of the sphygmomanometer 1 is the pressurization measurement method in the specific example 1 of the present embodiment. .
- the pressurization measurement method as in the case of not being a pregnant woman in the first embodiment, basically, there is no difference in measurement results in any measurement method, and the measurement time does not become excessively long. May be.
- FIG. 12 is a diagram illustrating the difference in measurement time between the decompression measurement method and the pressurization measurement method when the pulse pressure (difference value between the highest blood pressure and the lowest blood pressure) is large.
- the control pressure value when measured by the reduced pressure measurement method is shown along the time axis.
- the control pressure value when measured by the pressurization measurement method is shown along the time axis.
- the pressure When a reduced pressure measurement method is applied to a subject with high pulse pressure, the pressure continues to be reduced at a constant speed until a minimum blood pressure is detected in the pressure reduction process, so the restraint time in the pressure reduction process is lower than that of a person whose pulse pressure is not high. Measurement time becomes longer. In addition, there is a possibility that the influence of blood stasis is reflected in the measurement value. Specifically, the minimum blood pressure may be overestimated.
- the measurement time is shortened by switching to the pressurized measurement method. Further, this can improve the measurement accuracy.
- FIG. 13 is a flowchart showing the flow of specific example 2 of blood pressure measurement control in the second embodiment of the present invention. In FIG. 13, instead of steps S206 and S208 of FIG. 8, steps S226 and S228 are executed, respectively.
- the previous pulse pressure is calculated based on, for example, the previous measurement result data in measurement result storage unit 43B. More specifically, the systolic blood pressure data and the diastolic blood pressure data included in the measurement result data of the latest date and time are read out from the measurement result data stored in association with the user identification information selected in step S102. The difference between the read systolic blood pressure and the diastolic blood pressure is taken as the previous pulse pressure.
- step S2208 it is determined whether or not the calculated pulse pressure is a predetermined value, for example, 100 mmHg or more. If the pulse pressure is 100 mmHg or more, the process proceeds to step S110, and if the pulse pressure is less than 100 mmHg, the process proceeds to step S112. That is, if the pulse pressure is 100 mmHg or higher, it is determined that the measurement time is shorter in the pressurization measurement method, and the current measurement method is set as the pressurization measurement method.
- a predetermined value for example, 100 mmHg or more.
- the reduced pressure measurement method is used because the default measurement method of the sphygmomanometer 1 is the reduced pressure measurement method in the specific example 2 of the present embodiment.
- the measurement time does not become excessively long. Also good.
- the measurement method is determined such that the measurement time is shortened based on the past measurement results of the measurement subject.
- the measurement method is not limited to the past measurement results, and the measurement method may be determined so that the measurement time is shortened based on information on the biological condition of the measurement subject.
- FIG. 14 is a diagram for explaining the influence of the measurement time due to the difference in arm circumference in the case of the reduced pressure measurement method.
- FIG. 14A the control pressure value in the case of a standard thickness arm is shown along the time axis.
- FIG. 14B shows the control pressure value in the case of the thick arm along the time axis.
- the initial pressurization time becomes long if the arm is thick (that is, the circumference of the measurement site is long). As a result, the entire measurement time becomes longer. Such a problem does not occur when the flow rate of the pump 51 is increased, but this increases the components and leads to an increase in cost. For this reason, when the flow rate of the pump 51 is limited, the measurement time can be shortened by using a pressurization measurement method in which pressurization is performed at a slow speed for a person having a thick arm.
- the measurement time is shortened by switching to the pressurized measurement method.
- the measurement method is determined from such a viewpoint.
- Whether or not the arm is thick can be determined, for example, based on whether or not the arm circumference is a predetermined value (for example, 32 cm) or more.
- a cuff for a large arm (not shown) can be attached instead of the cuff 20 shown in FIG. 1, when it is detected that a cuff for a large arm is attached, the person to be measured You can also judge that your arm is thick. In the latter case, for example, it is assumed that there is another unit (not shown) including a thick arm cuff and a thick arm air tube. In that case, it is possible to detect whether or not the cuff for the thick arm is attached by changing the length of the plug of the air tube for the thick arm and the length of the plug of the standard air tube 31.
- FIG. 15 is a flowchart showing blood pressure measurement control in a modification of the second embodiment of the present invention.
- steps S236 and S238 are executed in place of steps S206 and S208 in FIG. 8, respectively.
- step S236 determination processing unit 104 receives an input of arm circumference information as information indicating the biological condition of the measurement subject from the user.
- FIG. 16 is a diagram showing an example of the screen displayed in step S236 of FIG. Referring to FIG. 16, for example, a message “Is your arm circumference 32 cm or more?” Is displayed on display 40, and button 411 indicating “YES” and button 412 indicating “NO” are displayed. Is displayed.
- step S228 If it is determined that the arm circumference is 32 cm or more, that is, the YES button 411 has been pressed (YES in step S238), the process proceeds to step S110. On the other hand, if it is determined that the arm circumference is less than 32 cm, that is, the NO button 412 has been pressed (NO in step S238), the process proceeds to step S112. That is, when information indicating that the arm circumference is 32 cm or more is input, it is determined that the measurement time is shorter in the pressure measurement method, and the current measurement method is set as the pressure measurement method.
- the reduced pressure measurement method is used because the default measurement method of the sphygmomanometer 1 in the modification of the present embodiment is the reduced pressure measurement method.
- the measurement time does not become excessively long. Also good.
- the determination is made based on the thickness of the arm, but it is only necessary to determine whether the peripheral length of the measurement part is a predetermined value or more. . That is, if the measurement site is the wrist, the determination is made based on the circumference (thickness) of the wrist.
- the measurement method is determined based on the past measurement value of the measurement subject or the biological condition of the measurement subject (whether or not he / she is a thick arm) so that the measurement time does not become long. be able to. Therefore, the restraint time of the person to be measured can be shortened, and as a result, a decrease in motivation to the blood pressure measurement of the person to be measured can be prevented.
- the measurement method is determined after giving priority to the measurement subject's wishes. Therefore, a suitable measurement method can be determined flexibly for each person to be measured.
- the pressure measurement method may be determined as long as one of the conditions during pregnancy and the thick arm is satisfied. Further, when not pregnant, the measurement method may be determined according to the determination result by determining the maximum blood pressure level or the pulse pressure level.
- the decompression measurement method is set for the subject having high systolic blood pressure.
- pressurization is performed.
- the measurement method is preferable. This will be described again with reference to FIG.
- the difference between the pressure value SYS and the maximum pressure value MAXa in the pressurization measurement method is “ ⁇ Pa”, and the difference between the pressure value E_SYS and the maximum pressure value MAXb in the decompression measurement method is “ ⁇ Pb”.
- ⁇ Pa is about an error
- ⁇ Pa is a predetermined value (between 10 and 40 mmHg). Therefore, the maximum pressure value MAXb is larger than the maximum pressure value MAXa. Therefore, if the reduced pressure measurement method is adopted for a person having high systolic blood pressure, excessive pressure is required, which causes pain.
- the pressure measurement method is more desirable.
- the measurement time should be shortened, the measurement accuracy should be improved, or the pain caused by the pressure should be reduced. Therefore, it is also possible to store desired determination criteria among these in the measurement method management table 431 and determine the measurement method based on the determination criteria desired by the user at the time of measurement. Alternatively, for each measurement, the user may be allowed to select from which viewpoint (determination criterion) the automatic switching (determination) of the measurement method is performed.
- the measurement method used for the measurement may be displayed on the display unit 40.
- the CPU 100 uses the CPU 100 to specify the measurement method (that is, the determination result of the measurement method) that is used for measurement among the pressurization measurement method and the decompression measurement method. May be displayed on the display unit 40.
- the doctor can check which measurement method is used to measure the displayed blood pressure value.
- step S120 of each flowchart information for identifying the determination result of the measurement method may be associated with the measurement value and stored in the flash memory 43.
- the measurement result data Mi shown in FIG. 3 may further include data MMi representing the measurement method.
- the doctor can estimate the reliability of the measured value. For example, if the measurement subject is a pregnant woman and is measured by the reduced pressure measurement method, it can be estimated that the reliability of the measurement value is not so high.
- the measurement method is automatically determined based on the biological condition of the measurement subject (pregnant woman, thick arms) or past measurement values.
- a measurement method suitable for each person to be measured hereinafter referred to as “recommended measurement method”. Such a form will be described as a third embodiment.
- the configuration of the blood pressure monitor in the present embodiment is the same as that in the first embodiment. Therefore, the description will be given also using the reference numerals used in the first embodiment.
- FIG. 17 is a flowchart showing a recommended measurement method determination process according to the third embodiment of the present invention.
- the process shown in the flowchart of FIG. 17 is stored in advance in the memory unit 42 as a program, and the CPU 100 reads out and executes this program, thereby realizing the function of the recommended measurement method determination process.
- This determination process is not performed every measurement, but is performed only when the user turns on the power switch 41A and the user instructs execution of the determination process function.
- the reduced pressure measurement processing unit 106 measures blood pressure by the reduced pressure measurement method (step S302).
- the measured blood pressure value is recorded in the internal memory as a measurement value in the reduced pressure measurement method (step S304).
- the pressurization measurement processing unit 108 measures the blood pressure using the pressurization measurement method (step S308).
- the measured blood pressure value is recorded in the internal memory as a measurement value in the pressurization measurement method (step S310).
- the process proceeds to step S314.
- step S314 it is determined whether or not three sets of measurement using the reduced pressure measurement method and measurement using the pressure measurement method have been completed. If three sets have not been completed (NO in step S314), the process returns to step S302 and the above process is repeated. On the other hand, if it is determined that three sets have been repeated (YES in step S314), the process proceeds to step S316.
- step S316 the CPU 100 compares the measurement values recorded in steps S304 and S310. Specifically, the dispersion of the values measured three times is compared, and the method with the stable measurement value is determined as the recommended measurement method (step S318). Or it compares with the blood pressure value measured by the auscultation method typically by medical staffs, such as a doctor, and determines the measuring method of the one near the blood pressure value as a recommended measuring method. In the latter case, it is assumed that the blood pressure value measured by the auscultation method is recorded in, for example, a predetermined area (not shown) of the flash memory 43. Note that the recommended measurement method may be determined manually by a medical worker by comparing the blood pressure value measured by the auscultation method with the blood pressure value measured by both measurement methods.
- the recommended measurement method determined in this way may be newly added to the measurement method management table 431.
- the determination processing unit 104 makes the recommended measurement method recorded in the measurement method management table 431 suitable for the user. It is determined as a measurement method.
- the measurement method with higher measurement accuracy can be surely set as the recommended measurement method.
- blood pressure can be measured by a measurement method suitable for the measurement subject.
- the blood pressure measurement control method performed by the blood pressure monitor of the present invention can also be provided as a program.
- a program can be recorded on an optical medium such as a CD-ROM (Compact Disc-ROM) or a computer-readable recording medium such as a memory card and provided as a program product.
- a program can also be provided by downloading via a network.

Abstract
