Abstract
System zur Formgebung einer auf einen Knochen, insbesondere auf einen Femurkopf, aufgebrachten formbaren Masse aufweisend eine Formkappe (30) mit einem die formbare Masse formenden Innenraum und aufweisend ein Führungselement, das eine axiale Führung der Formkappe in Richtung der formbaren Masse ermöglicht, wobei die Formkappe (30) einen Kappenboden (31) aufweist, in den Auslassöffnungen (33) zum Austritt von Luft und von überschüssiger formbarer Masse eingebracht sind, wobei in den Kappenboden (31) Ventilöffnungen (34) eingebracht sind, die den Eintritt von Luft in den Innenraum der Formkappe (30) beim Abheben der Formkappe von der formbaren Masse ermöglichen.
Description
ZEMENTAPPLIKATOR MIT FORMKAPPE
Die vorliegende Erfindung betrifft ein System zur Formgebung einer auf einen Knochen, insbesondere auf einen Femurkopf, aufgebrachten formbaren Masse, insbesondere Knochenzement, aufweisend eine Formkappe mit einem die formbare Masse formenden Innenraum und aufweisend ein Führungselement, das eine axiale Führung der Formkappe in Richtung der formbaren Masse ermöglicht, wobei die Formkappe einen Kappenboden aufweist, in den Auslassöffnungen zum Austritt von Luft und überschüssiger formbarer Masse eingebracht sind.
Ein solches System zum Einsatz in der Hüftendoprothetik ist aus der DE 10 2007 024 083 A1 bekannt. Aus dieser Anmeldung sind Hilfswerkzeuge und Applikatoren bekannt, die den Operateur in die Lage versetzen, Zement kontrolliert, gleichmäßig und glatt auf dem Knochen zu verteilen, um eine definierte Zementpenetration in den Knochen zu bewirken. Trotz der großen Vorteile der bekannten Vorrichtung hat es sich als schwierig erwiesen, die Formkappe von der letztendlich geformten Masse zu lösen, ohne dass es zu Verwerfungen auf der Oberfläche der an sich glatten Oberfläche der geformten Masse kommt.
Aufgabe der Erfindung ist es daher, ein in der Praxis einfach und entsprechend komfortabel handhabbares System zur Formgebung der auf den Knochen, insbesondere auf den Femurkopf, aufgebrachten formbaren Masse vorzuschlagen, mit der sich die formbare Masse in ausgezeichneter Formgebung und besonders glatter Oberfläche aufbringen lässt.
Diese Aufgabe wird gelöst durch ein System mit den Merkmalen des Anspruch 1 Die Merkmale der Unteransprüche betreffen besonders vorteilhafte Ausgestaltungen der Erfindung.
Der erfindungswesentliche Grundgedanke besteht darin, neben den für die Funktion wesentlichen Auslassöffnungen, aus denen beim Formprozess zunächst die eingeschlossene Luft und letztendlich der überschüssige Knochenzement austreten kann, auch Eintrittsöffnungen in Form von Ventilöffnungen vorzusehen, die beim Abheben der formschlüssig auf der formbaren Masse anliegenden Kappe den Lufteintritt zu ermöglichen. Auf diese Weise kann ein Druckausgleich zwischen dem Luftdicht abgeschlossenen Innenraum innerhalb der Formkappe und der Umgebung stattfinden. Diese Ventilöffnungen sind im Gegensatz zu den Auslassöffnungen so auszubilden, dass sie für den Austritt von Luft und formbarer Masse beim Formprozess verschlossen sind. Erfindungsgemäß werden somit in den Kappenboden Ventilöffnungen eingebracht, die den Eintritt von Luft beim Abheben der Formkappe von der formbaren Masse ermöglichen. Dabei umfasst das Merkmal „Luft" in diesem Sinne jegliches Fluid, was den Druckausgleich ermöglicht. Im Normalfall wird der Druckausgleich durch einströmende Umgebungsluft geschaffen.
Um einen homogenen Lufteintritt zu gewährleisten ist es vorteilhaft, wenn mehrere, beispielsweise drei, solcher Ventilöffnungen in einer Kreisteilung, also in gleichem Abstand auf der Ringfläche der rotationssymmetrischen Formkappe in den Kappenboden eingebracht sind. Dabei ist drei sicherlich eine gute, da einfach zu handhabbare Anzahl.
Dies gilt insbesondere, wenn zum anfänglichen Verschließen der Ventilöffnungen eine entsprechende Zahl von Schließelementen vorgesehen sind, die in die insbesondere als Bohrungen ausgebildeten Ventilöffnungen dichtend von Außen einsteckbar und ebenso vor dem Abheben der Kappe wieder herausnehmbar sind. Diese Schließelemente sind vorteilhafterweise in die Bohrungen einschraubbar, wobei die Bohrungen und die schraubenartigen Schließelemente mit entsprechenden Gewinden ausgestattet sind. Die schnelle und sichere Handhabung von viel mehr als dreier solcher Schließelemente würde während der Operation eine große Herausforderung sein.
Besonders vorteilhaft ist es, wenn die Schließelemente eine zum Innenraum gerichtete Bodenfläche aufweisen, die bei eingestecktem Schließelement plan mit der formenden Fläche des Innenraums abschließen. Auf diese Weise kann eine besonders glatte Oberfläche des aufgebrachten Knochenzementes gewährleistet werden. Um dieses zu ermöglichen, ist es weiterhin vorteilhaft, wenn die Bohrungen im Boden der Formkappe zweierlei Durchmesser aufweisen, wobei der außen liegende Durchmesser größer ist und ein Innengewinde aufweist und wobei der innen liegende Durchmesser glatt ist und einen entsprechenden Dichtstift des Schließelementes mit Passung führt. Auf diese Weise kann der Dichtstift in Länge und Form seiner Außenfläche so ausgebildet werden, dass die Außenfläche plan mit der formenden Fläche des Innenraums abschließt. Dabei kann die Außenfläche sogar mit einer leichten Innenwölbung geformt sein.
Statt mit derartigen Schließelementen können die Ventilöffnungen mit flexiblen Ventilklappen verschlossen werden, die sich von der Austrittströmung beaufschlagt an die formende Fläche des Innenraums und über die Mündungen der Bohrungen legen und diese somit verschließen. Solche Ventilklappen werden vorteilhafterweise von einer dünnen Folie gebildet, die an der formenden Fläche des Innenraums so angebracht sind, dass sie die Ventilfunktion gewährleisten können.
Insgesamt ist auch bei diesem System ein Führungselement für die Formkappe vorzusehen, das auf den Knochen aufgesetzt wird und das ein gleitendes Führen der Formkappe bei der Beaufschlagung der formbaren Masse, insbesondere des Knochenzementes, ermöglicht. Mit diesem Führungselement kann die Formkappe in ihrer Richtung und ihrer Andruckkraft definiert bewegt und die formbare Masse geformt werden. Dazu wird die formbare Masse (nachfolgend „Knochenzement") auf die Knochenoberfläche aufgebracht und mit der vom Führungselement geführten Formkappe formend beaufschlagt. Bei der erfindungsgemäßen Vorrichtung bildet das Führungselement somit eine Führung für die Formkappe aus, wobei die Formkappe auf dem Führungselement axial verschieblich ist.
Mit dem Einsatz der Erfindung kann der Knochenzement geglättet und der späteren Prothesenform angeglichen werden. Zudem kann ein definierter Druckgradient, der die Penetration des Knochenzementes in den Kochen hinein begünstigt, aufgebaut werden. Zudem kann der Druckgradient an die Viskosität der formbaren Masse angepasst und eine kontrollierte Penetration der formbaren Masse in den Knochen hinein gewährleistet werden. Beim Aufsetzen der Prothese verbindet der Knochenzement die schwammartige Knochenoberfläche mit den Verankerungselementen der Prothesenunterfläche und härtet aus. Überschüssiger Zement kann auf einfache Weise am Prothesenpol entfernt werden. Es bildet sich ein belastbarer Sockel, der ein Einwachsen des Knochens in die Prothesenunterfläche unnötig macht und der zu einer gleichmäßigen Krafteinleitung ohne Stressspitzen führt.
Dabei ist es grundsätzlich einerlei, ob zunächst das Führungselement aufgesetzt und dann der Knochenzement aufgebracht wird oder ob der zunächst aufgebrachte Knochenzement später vom Führungselement durchstoßen wird. Vorteilhafterweise wird jedoch zunächst das Führungselement auf den Knochen aufgesetzt, bevor der Knochenzement auf die Knochenoberfläche aufgebracht wird. Dadurch ist die zentrale Öffnung im präparierten Knochen vor dem Auftragen des Knochenzementes verschlossen, so dass sein Eindringen des Zements in die Öffnung vermieden wird. Dabei ist der Vorteil der freien zentralen Öffnung, dass beim Aufsetzen der becherförmigen Oberflächenersatzprothese das Entweichen der Luft unter dieser heraus garantiert werden kann.
Der Grundgedanke liegt demnach darin, die Formkappe mittels des Führungselementes sicher in eine Position zum Knochen zu führen, die der später eingenommenen Position der Prothesenkomponente entspricht. Dabei ist durch die gleitende Führung der Formkappe die Formung des Knochenzementes zügig durchführbar. Nach dem Entfernen der Formkappe verbleibt vor dem Aushärten des Knochenzementes genügend Zeit für das Entfernen eventuell überstehenden Materials und für das Verpressen der Prothese. Untersuchungen zeigten, dass eine Zementpenetrationstiefe von 2-3 mm ausreicht um eine Verzahnung mit
transversal verlaufendem Knochentrabekel zu erreichen. Für eine optimale Prothesenverankerung wird daher eine Zementpenetration zwischen 2 und 5 mm Tiefe empfohlen. Mit Hilfe der Erfindung können diese Voraussetzungen sicher und komfortabel erreicht werden.
Das Verfahren und die Vorrichtung sind nicht auf die Anwendung an einem Femurkopf oder auf Oberflächenersatzprothesen einzelner Hersteller beschränkt, sondern prinzipiell auch für Anwendungen an anderen Gelenken und für Prothesen anderer Hersteller einsetzbar.
Dabei umfasst das erfinderische Verfahren nach der Präparation des Femurkopfes, der Reinigung und der Trocknung insbesondere folgende Verfahrensschritte:
In einem ersten Verfahrensschritt wird das Führungselement auf den Knochen aufgesetzt, so dass es eine gleitende Führung für die Formkappe ausbilden kann. Dabei ist es vorteilhaft, wenn das Führungselement als metallischer Stift ausgebildet ist, der in eine vorbereitete Öffnung im Knochen eingesetzt wird. Im nächsten Verfahrensschritt wird der Knochenzement auf die Knochenoberfläche aufgebracht und diese mit dem Knochenzement vollständig bedeckt. Die Aufbringung kann mit einer Zementpistole geschehen. Dabei ist es nicht notwendig, den Knochenzement mit den Fingern zu berühren oder anzupressen. Darauffolgend kann die vom Führungselement gleitend geführte Formkappe gleichmäßig auf den Knochenzement gepresst werden und den Knochenzement zu einer homogenen Schicht möglichst gleichmäßiger Stärke ausformen
In einer besonders bevorzugten Ausführungsform weist das Führungselement axiale Abschnitte auf, wobei ein erster axialer Abschnitt zumindest teilweise einer Form eines im Kopfteil der Prothese vorhandenen Zentrierstiftes entspricht. Auf diese Weise kann das Führungselement in die für den Zentrierstift vorgesehene zentrale Öffnung im präparierten Knochen eingesetzt und so ausgerichtet werden. Das Führungselement wird dabei optimal auf dem Knochen zentriert, wobei die Handhabung besonders praxistauglich ist. Zudem kann durch die zumindest
teilweise Formgleichheit die Abmessungen der zentralen Öffnung im Knochen, also die Maßhaltigkeit und die Freigängigkeit der Öffnung für den Zentrierstift der Prothese noch vor dem Aufsetzen der Prothese überprüft werden.
Ein zweiter axialer Abschnitt des Führungselements bildet einen Gleitsitz für eine zentrale Bohrung im Kappenboden der Formkappe aus, wodurch die Formkappe auf besonders einfache Weise durch das Führungselement geführt werden kann. Der Durchmesser der Bohrung ist entsprechend der Anforderungen an den Gleitsitz an den Durchmesser des Führungselementes anzupassen.
In einer weiteren bevorzugten Ausführungsform hat der zweite axiale Abschnitt des Führungselementes einen größeren Durchmesser als der erste axiale Abschnitt des Führungselements, wobei der Übergangsbereich eine Stufe ausgebildet. Diese Stufe setzt beim Einbringen des Führungselements in die zentrale Öffnung am Rand auf der Knochenoberfläche auf. Durch die Stufe wird ein gewisser Sicherheitsabstand um die zentrale Öffnung im Knochen herum geschaffen, der nicht von dem Knochenzement bedeckt wird. Dadurch wird gewährleistet, dass beim Aufsetzen der Prothese die eingeschlossenen Luft durch die freie zentrale Öffnung bis zum vollständigen Prothesensitz entweichen kann.
Um eine komfortable Handhabung durch den Operateur zu ermöglichen, wird die Formkappe in einer bevorzugten Ausführungsform mit einem Handgriff ausgestattet, der vorteilhafterweise lösbar mit der Formkappe verbunden ist. Der Handgriff wird dann an der Formkappe befestigt, bevor diese durch das Führungselement zentriert zur formenden Beaufschlagung des Knochenzementes auf die Knochenoberfläche gepresst wird. Durch die Verbindung mit dem Handgriff ist die Formkappe besonders einfach handhabbar. Die lösbare Befestigung trägt auch zu einer besseren Reinigung der Einzelteile bei. Vorzugsweise bilden Führungselement und Handgriff eine Anzeige aus, die den Abstand des formenden Innenraums der Formkappe von der Knochenoberfläche anzeigt. Weist die Anzeige eine Skalierung auf, ist der Operateur beim Beaufschlagen des Knochenzementes zu jedem Zeitpunkt über die verbleibende Schichtdicke informiert. Bevorzugterweise ist ein Anschlag vorgesehen, der eine Endposition
zwischen der Formkappe und dem Führungselement und damit der Formkappe relativ zum präparierten Knochen definiert. Die Formkappe kann vom Operateur auf die formende Masse gedrückt werden, bis der Anschlag erreicht ist, wobei der Anschlag so eingestellt ist, dass der gewünschte Abstand zwischen der Formkappe und der Oberfläche des Knochens sichergestellt ist.
In einer weiteren besonders bevorzugten Ausführungsform kann während der Beaufschlagung unter der Formkappe eingeschlossene Luft und/oder ein Teil der formbaren Masse durch mindestens eine Auslassöffnung im Kappenboden entweichen. Durch diese Auslassöffnungen werden Einschlüsse der formbaren Masse am Prothesenpol, ein zu hoher Druckgradient und damit eine zu tiefe Penetration der formbaren Masse vermieden. Hierdurch kann eine Druckentlastung und Verminderung der Menge der formbaren Masse im Bereich der Problemzone des Prothesenpols erfolgen. Die Anzahl und Durchmesser der Auslassöffnungen im Kappenboden sind vorzugsweise entsprechend der Viskosität des Knochenzementes auszulegen. Für einen Knochenzement durchschnittlicher Viskosität geeignet sind insbesondere drei symmetrische um die zentrale Öffnung des Kappenbodens herum angeordnete Auslassöffnungen mit einem jeweiligen Durchmesser von etwa 4 mm.
Der äußere Rand der Formkappe ist vorzugsweise konusfömig angefast. Dadurch wird erreicht, dass der beim Aufdrücken der Formkappe verdrängte Knochenzement auf die Knochenoberfläche gedrückt wird. Somit entsteht auch an dem äußeren Rand der Formkappe ein Druckgradient, der zu einer ausreichenden Penetration des Knochenzementes in den Knochen führt.
Die Formkappe kann nach Erreichen des gewünschten Abstandes mit einer Drehbewegung am Handgriff von der Zementoberfläche entfernt werden. Bei der erfindungsgemäßen Vorrichtung bleibt kein Knochenzement an der Formkappe hängen und es entsteht ein verhältnismäßig glatter Mantel aus Knochenzement, der eine insbesondere gleichmäßiger Schichtdicke und homogene Dichte auf der Knochenoberfläche aufweist. Nach dem Entfernen der Formkappe kann der überstehende Knochenzement am Unterrand des Prothesenbettes mühelos unter
Sicht entfernt werden. Ein Überstehen oder Verbleiben von Partikeln des Knochenzementes in der Nähe des späteren Kunstgelenkes wird vermieden.
Die insbesondere gleichmäßig starke Schicht der formbaren Masse auf dem Knochen dient dazu, eine möglichst sichere Verbindung mit der femoralen Prothesen komponente herzustellen und wird beim Setzen durch diese nochmals komprimiert. Ein unvollständiger Prothesensitz aufgrund zu starker oder verschiedener Schichtdicke der formbaren Masse kann damit ausgeschlossen werden.
Das erfindungsgemäße Verfahren hat weiterhin den Vorteil, dass die Druckkraft beim Aufsetzen der Formkappe der Druckkraft entspricht, die im weiteren Operationsverlauf auf die Prothese ausgeübt werden muss. Der Operateur kann erkennen, wie schnell sich die Formkappe unter seiner Handkraft bewegt und auf die formbare Masse setzt. Für den Operateur wird es damit einfacher die richtige Druckbeaufschlagung auf die Prothese zu wählen und aufrecht zu halten.
Das Kappenmaterial sollte genügend fest sein um sich beim Aufpressen nicht zu verformen. Bevorzugt werden die Formkappen komplett aus Teflon gefertigt um ein Anhaften der formbaren Masse zu vermeiden. Aus Kostengründen kann die Formkappe auch aus einem anderen Material hergestellt werden. Das Kappenmaterial sollte es ermöglichen, glatte Innenflächen herzustellen, die optional mit einer Antihaft- oder Teflonbeschichtung versehen werden können. Um einen Verkratzen der Formkappe und damit Anhaften der formbaren Masse vorzubeugen, sollte diese nur wenige male benutzt werden oder als Einmalwegprodukt konzipiert werden.
Das Führungselement besteht bevorzugt aus Edelstahl, hier könnte ebenfalls ein anderes günstigeres Material mit ausreichender Festigkeit verwendet werden.
Mit dem erfindungsgemäßen Verfahren und der erfindungsgemäßen Vorrichtung kann eine sichere Verbindung der Prothese mit dem Knochen durch einen homogenen Zementsockel gewährleistet werden. Dabei zeichnet sich das Verfahren und die Vorrichtung durch die hohe Praxistauglichkeit aus. Auch die
gründliche Reinigung und die Desinfektion der Einzelteile ist problemlos möglich, wobei es, wie oben gesagt, vorteilhaft ist, die Vorrichtung als Einwegartikel herzustellen.
Nachfolgend wird die Erfindung anhand der Figuren 1 bis 4 näher erläutert. Dabei zeigt

In Figur 1 ist als Teil eines Systems zur Formgebung einer auf einen Knochen, insbesondere auf einen Femurkopf, aufgebrachten formbaren Masse, eine erfindungsgemäße Formkappe 30 gezeigt. Das komplette System ist in Figur 3 offenbart. Die Formkappe 30 hat einen die formbare Masse formenden Innenraum 31 mit einer zentralen Bohrung 32. In die Bohrung 32 wird ein zylindrisch geformtes Führungselement eingesteckt, das eine axiale Führung der Formkappe 31 in Richtung der formbaren Masse ermöglicht. Die Formkappe 30 hat einen Kappenboden 33, in den Auslassöffnungen zum Austritt von überschüssiger formbarer Masse eingebracht sind. Zusätzlich sind in den Kappenboden 33 drei als Bohrungen ausgebildete Ventilöffnungen 34 in Kreisteilung eingebracht, die den Eintritt von Luft beim Abheben der Formkappe von der formbaren Masse ermöglichen.
Zum zeitweiligen Verschließen der Ventilöffnungen 34 sind entsprechend drei Schließelemente 35 vorgesehen, die von Außen dichtend in die Ventilöffnungen vermittels Gewinden 36 einschraubbar sind. Die Schließelemente 35 weisen eine zum Innenraum gerichtete Bodenfläche 37 auf, die bei eingestecktem Schließelement 35 plan mit der formenden Fläche 31 des Innenraums abschließen.
In Figur 2 wird deutlich, dass die Bohrungen 38 zwei verschiedene Durchmesser aufweisen, wobei der außen liegende Durchmesser größer ist und ein Innengewinde 39 aufweist. Der innen liegende Durchmesser ist glatt und führt einen entsprechenden Dichtstift 40 des Schließelementes 41 mit Passung, die ein auf diese Anwendung optimiertes Spiel aufweist.
Die in Figur 3 gezeigte Vorrichtung 1 besteht aus drei Komponenten. Das rotationssymmetrische Führungselement 2 als eine der Komponenten weist zwei axiale Abschnitte 3 und 4 auf. Der erste axiale Abschnitt 3 des Führungselements 2 mit einer leicht konisch zulaufende Spitze 5 entspricht in seiner Geometrie einem Zentrierstift einer Oberflächenersatzprothese, beispielsweise dem Zentrierstift einer ASR (Articular Surface Replacment) Oberflächenersatzprothese für den Femurkopf. Durch die Übereinstimmung der Maße kann der erste axiale Abschnitt 3 vollständig in eine für den Zentrierstift vorgesehene zentrale Öffnung im präparierten Femurkopf eingesetzt werden.
Der zweite axiale Abschnitt 4 des Führungselements 2 ist zylindrisch geformt und weist einen größeren Durchmesser als der erste axiale Abschnitt 3 auf. Der Übergang zwischen dem ersten Abschnitt 3 und dem zweiten Abschnitt 4 ist als Stufe 6 ausgeformt, die beim Einsetzen des ersten axialen Abschnitts 3 in die zentrale Öffnung des Femurkopfes auf den die Öffnung umgebenden Rand des Knochens aufsetzt. Zudem weist der axiale Abschnitt 4 eine oder mehrere in Umfangsrichtung kreisförmig um die Zylinderoberfläche laufende Markierungen 7 auf, die Teil einer Anzeige zur Feststellung der Eindringtiefe ist.
Eine weitere Komponente der Vorrichtung 1 ist die Formkappe 8. Die Formkappe 8 hat einen Kappenboden 9 bestimmter Stärke und eine äußere Wand 10, wobei Kappenboden 9 und Wand 10 eine formende Unterfläche haben, die als Innenraum 11 mit glatter Oberfläche ausgebildet ist. Die formende Unterfläche 11 der Formkappe 8 hat bezüglich ihrer Innenwinkel eine gleiche, jedoch in ihren Durchmesser größere innere Geometrie als die Unterfläche der aufzusetzenden Prothese. Das hat den Zweck, dass der Knochenzement beim Aufsetzen der Prothese noch etwas Potential hat, um komprimiert zu werden was eine sichere Verbindung von Prothesenunterfläche und Zement garantiert. Für eine Prothese der Größe 49 ist eine Zementmantelstärke von 1 mm zu bevorzugen und der Durchmesser der inneren Geometrie des formenden Innenraums 11 etwa um 2 mm größer als der Innenraum der Prothese anzusetzen. Die Formkappe 8 ist aus Teflon gefertigt, wobei es im Prinzip ausreicht die formende Unterfläche mit Teflon oder einer anderen Antihaftbeschichtung auszukleiden.
In den Kappenboden sind Auslassöffnungen 42 zum Austritt von Luft und von überschüssiger formbarer Masse eingebracht. Zudem sind Ventilöffnungen 43 eingebracht, in die ein Schließelement 44 einschraubbar ist.
An ihrem Rand 12 ist die Formkappe 8 „ausgefast" und läuft in einem Winkel von etwa 30° konisch aus. Der Rand 12 öffnet sich somit nach außen und weist einen im Vergleich mit der äußeren Wand 10 sich vergrößernden Innendurchmesser auf.
Im Kappenboden 9 ist eine zentrale Bohrung 13 vorgesehen, deren Durchmesser für eine gleitende Führung des zylindrischen zweiten axialen Abschnitts 4 des Führungselements 2 ausgelegt ist.
Ein Handgriff 14 als dritte Komponente der Vorrichtung 1 weist einen zylindrischen Hals 15 und einen Griff 16 auf, die fest miteinander verbunden sind. Am Ende 17 des Griffhalses 15 bilden zwei einander entgegengesetzte radiale Ansätze 18 zusammen mit radial außerhalb der zentralen Bohrung 13 des Kappenbodens 9 angeordneten Aussparungen 19 einen Bajonettverschluss 20 aus, vermittels dem der Handgriff 14 und die Formkappe 8 lösbar verbunden werden können. Im Hals
15 ist eine Bohrung 21 vorgesehen, in die der zweite axiale Abschnitt 4 des Führungselements 2 bis zu einem Anschlag 25 bei Druckbeaufschlagung der Formkappe 8 eingeführt werden kann.
Figur 4 zeigt einen um 90° gedrehten Schnitt durch die erfindungsgemäße Vorrichtung entsprechend Figur 3. Um eine auf dem präparierten Femurkopf aufgetragene Zementmasse formend zu beaufschlagen, werden die drei Komponenten 1 , 8 und 14 der Vorrichtung 1 axial entlang einer gemeinsamen Mittelachse A ausgerichtet. Der T-förmig ausgebildete Handgriff 14 wird hierzu über den Bajonettverschluss 20 durch eine 90°-Drehung mit der Formkappe 8 verbunden. Anschließend wird die Formkappe 8 auf das Führungselement 2 aufgesetzt und der zweite axiale Abschnitt 3 in die zentrale Bohrung 13 der Formkappe 8 eingesetzt.
Das mit seinem ersten axialen Abschnitt 2 in die zentrale Öffnung des präparierten Femurkopfes eingebrachte Führungselement 1 führt die Formkappe 8 über den in ihrer zentralen Bohrung 13 gleitenden zweiten axialen Abschnitt 3, so dass die Formkappe 8 über die zentrale Bohrung 13 zentriert ist. Wird nun über den Handgriff 14 entlang der Mittelachse A Druck auf die Formkappe 8 ausgeübt, wird die Zementmasse durch die von dem zweiten axialen Abschnitt 4 geführte und so auf dem Femurkopf zentrierte Formkappe 8 gleichmäßig formend beaufschlagt.
Durch drei symmetrisch im Kappenboden 9 radial außerhalb der zentralen Bohrung 13 angeordnete Auslassöffnungen 22 kann Luft und/oder überschüssiger Zement während der Beaufschlagung von der Knochenoberfläche entweichen. Wiederum sind Ventilöffnungen 45 eingebracht, in die Schließelemente 46 einschraubbar sind.
Langlöcher 23 im Griffhals 15 ermöglichen dem Operateur den Blick auf die Markierung 7 des in der inneren Bohrung 21 aufgenommenen axialen Abschnitts 4 des Führungselements 2. Eine Skalierung 24 am äußeren Umfang des Griffhalses 15 bildet mit der Markierung 7 eine Anzeige, an der sich die noch vorhandene Stärke der Zementmasse während der Beaufschlagung ablesen lässt.
Die innere Bohrung 21 formt den Anschlag 25 für den zweiten axialen Abschnitt 4 aus. Der Anschlag 25 bestimmt eine Endposition der Formkappe 8 auf der Zementmasse, die entsprechend der gewünschten Stärke der Zementmasse ausgelegt wird. Für eine bevorzugten Schichtstärke der geformten Zementmasse von 1 mm ist der Anschlag 25 für eine Endposition des formenden Innenraums 11 der Formkappe 8 von 1 mm über dem präparierten Femurkopf zu wählen.
Über diese Einrichtung ist es möglich, dass die Druckkraft beim formenden Beaufschlagen vermittels der Formkappe 8 zumindest annähernd der Druckkraft entspricht, die beim Setzen der Prothese ausgeübt wird.
Claims
1. System zur Formgebung einer auf einen Knochen, insbesondere auf einen Femurkopf, aufgebrachten formbaren Masse aufweisend eine Formkappe (30) mit einem die formbare Masse formenden Innenraum und aufweisend ein Führungselement, das eine axiale Führung der Formkappe in Richtung der formbaren Masse ermöglicht, wobei die Formkappe (30) einen Kappenboden (31) aufweist, in den Auslassöffnungen (33) zum Austritt von Luft und von überschüssiger formbarer Masse eingebracht sind, dadurch gekennzeichnet, dass in den Kappenboden (31) Ventilöffnungen (34) eingebracht sind, die den Eintritt von Luft in den Innenraum der Formkappe (30) beim Abheben der Formkappe von der formbaren Masse ermöglichen.
2. System nach Anspruch 1 , dadurch gekennzeichnet, dass mehrere, insbesondere drei, Ventilöffnungen (34) in einer Kreisteilung in den Kappenboden (31) eingebracht sind.
3. System nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass Schließelemente (35) vorgesehen sind, die in die insbesondere als Bohrungen ausgebildeten Ventilöffnungen (34) dichtend von Außen einsteckbar sind.
4. System nach Anspruch 3, dadurch gekennzeichnet, dass die Schließelemente (35) in die Bohrungen einschraubbar sind.
5. System nach Anspruch 3 oder 4, dadurch gekennzeichnet, dass die Schließelemente (35) eine zum Innenraum gerichtete Bodenfläche (37) aufweisen, die bei eingestecktem Schließelement (35) plan mit der formenden Fläche (31) des Innenraums abschließen.

System nach Anspruch 5, dadurch gekennzeichnet, dass die Bohrungen zwei verschiedene Durchmesser aufweisen, wobei der außen liegende Durchmesser größer ist und ein Innengewinde (39) aufweist, wobei der innen liegende Durchmesser glatt ist und einen entsprechenden Dichtstift (40) des Schließelementes (35) mit Passung führt.
7. System nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass Ventilklappen vorgesehen sind, die an der formenden Fläche des Innenraums anliegen und die insbesondere als Bohrungen ausgebildeten Ventilöffnungen in Austrittsrichtung dichtend verschließen.
8. System nach Anspruch 7, dadurch gekennzeichnet, dass die Ventilklappen von einer dünnen Folie gebildet werden.
9. System nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Formkappe (8) eine zentrale Bohrung aufweist, in der das Führungselement (2) einsteckbar ist, wobei die Formkappe (8) auf dem Führungselement (2) axial verschieblich ist.
10. System nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass das insbesondere aus Metall gefertigte stiftförmige Führungselement (2) Abschnitte (3, 4) unterschiedlichen Durchmessers aufweist, wobei ein erster Abschnitt (3) zumindest teilweise die Form des im Kopfteil der Prothese vorhandenen Zentrierstiftes entspricht und wobei ein zweiter Abschnitt (4) einen Gleitsitz für die mit einer zentralen Bohrung (13) ausgestatteten Formkappe (8) bildet.
11. System nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass der formende Innenraum (11) der Formkappe (8) glatt ist und der Geometrie der Oberfläche der Prothese entspricht, wobei er insbesondere um eine Pressdistanz größer ist.
12. System nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Formkappe (8) ein sich konisch öffnenden Rand (12) aufweist.
13. System nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Formkappe (8) aus Teflon gefertigt oder der formende Innenraum mit Teflon oder einer anderen Antihaftbeschichtung ausgekleidet ist.
14. System nach einem der vorherigen Ansprüche, gekennzeichnet durch einen insbesondere lösbar an der Formkappe (8) befestigbaren Handgriff (14) mit einer axialen Bohrung (21), die beim formenden Beaufschlagen den zweiten axialen Abschnitt (4) des Führungselements (2) aufnimmt.
