Die Erfindung betrifft ein transdermales therapeutisches System mit Deckschicht, wirkstoffhaltiger Matrix und fakultativer Abziehfolie und Remifentanil oder Remifentanil-Hydrochlorid als Wirkstoff.
Description
TRANSDERMALES THERAPEUTISCHES SYSTEM MIT REMIFENT ANIL
Die Erfindung betrifft ein transdermales therapeutisches System mit Remifentanil als Wirkstoff.
Bei Remifentanil handelt es sich um Methyl- l-(2-methoxycarbonylethyl)-4-(phenyl- propanoyl-amino)-piperidin-4-carboxylat. Remifentanil ist ein potentes ultrakurz wirkendes synthetisches Opioides Analgetikum. Remifentanil wird in Form seines Hydrochlorids als Injektions- oder Infusionslösung verwendet. Der Wirkstoff wird zum Sedieren als auch in Kombination mit anderen Medikationen bei Generalanästhesie verwendet. Remifentanil ist durch einen raschen Wirkungseintritt und eine kurze Wirkdauer gekennzeichnet. Abgesehen von seiner Verwendung in Form von Injektions- oder Infusionslösungen ist es daher noch nicht für transdermale therapeutische Systeme vorgeschlagen worden.
Aufgabe der Erfindung ist es, ein transdermales therapeutisches System mit Remifentanil oder Remifentanil-Hydrochlorid als Wirkstoff vorzusehen.
Gemäß einer Ausführungsform wird diese Aufgabe durch ein transdermales therapeutisches System mit Deckschicht, beispielsweise einer Abdeckfolie, wirkstoffhaltiger Matrix und fakultativer Abziehfolie und Remifentanil oder Reminfentanil-Hydrochlorid als Wirkstoff gelöst.
Vorzugsweise ist das erfindungsgemäße System mit Remifentanil oder Remifentanil- Hydrochlorid übersättigt. Mit einer Übersättigung kann der kurzen Wirkdauer des Wirkstoffs Rechnung getragen werden.
So kann ein erfindungsgemäßes System einen Übersättigungsstabilisator umfassen. Ein Beispiel für einen Übersättigungsstabilisator stellt Polyvinylpyrrolidon dar. Für
Polyvinylpyrrolidon als Übersättigungsstabilisator kann verwiesen werden auf EP 0 519 926 Bl, WO 95/18603 und WO 96/40355.
Das erfindungsgemäße System kann eine Matrix aufweisen, die aus einem Material besteht, das durch Hautfeuchtigkeit an- oder auflösbar ist. Wird eine derartige Matrix durch die Hautfeuchtigkeit an- oder aufgelöst, so kommt es in Nachbarschaft zur Haut zu einer Erhöhung der Wirkstoffkonzentration, wodurch der kurzen Wirkdauer des Wirkstoffs Rechnung getragen wird. Relevanter Stand der Technik ist WO 99/61007.
Auch kann ein erfindungsgemäßes System eine Matrix aufweisen, die ein Material umfasst, das durch Hautfeuchtigkeit an- oder auflösbar ist. Dabei kann Remifentanil als Lösung oder Dispersion oder Suspension in einem nicht-wassermischbaren Polymer oder Remifentanil- Hydrochlorid als Dispersion oder Suspension in einem nicht-wassermischbaren Polymer vorliegen, und zwar jeweils neben dem durch Hautfeuchtigkeit an- oder auflösbaren Material der Matrix. Relevanter Stand der Technik ist wiederum WO 99/61007.
Unter Dispersion werden insbesondere folgende Systeme verstanden:
molekulardispers: sogenannte „wahre" Lösungen; Moleküle sind im Lösungsmittel dispergiert / gelöst. Teilchendurchmesser < 1 nm.
kolloiddispers: Feststoffteilchen sind im Lösungsmittel dispergiert. Teilchendurchmesser zwischen 1 und 500 nm.
grobdispers: Teilchen im Lösungsmittel > 500 nm.
Bei dem durch Hautfeuchtigkeit an- oder auflösbaren Material kann es sich um ein wasserlösliches Polymer handeln, beispielsweise Gelatine oder Celluloseester oder Celluloseether. Auch bei einem derartigen erfindungsgemäßen System kommt es zu einer Konzentrationserhöhung an Wirkstoff in Nachbarschaft zur Haut, wenn das durch Hautfeuchtigkeit an- oder auflösbare Material aufgelöst wird.
Anstelle des durch Hautfeuchtigkeit an- oder auflösbaren Materials oder neben diesem durch Hautfeuchtigkeit an- oder auflösbaren Material kann auch ein Verdickungsmittel oder Quellmittel vorgesehen werden, beispielsweise Hydroxymethylcellulose, insbesondere Klucel.
Gemäß einer weiteren Ausführungsform wird die der Erfindung zugrunde liegende Aufgabe durch ein transdermales therapeutisches System gelöst, bei dem es sich um ein Elektrodenpflaster mit einer oder mit mehreren hautnahen Elektroden und mit einer oder mit mehreren deckschichtnahen Gegenelektroden handelt. Ein derartiges System ist beispielsweise EP 99 920 628.7 und EP 03 005 664.2 zu entnehmen.
Gemäß einer weiteren Ausführungsform kann die der Erfindung zugrunde liegende Aufgabe auch durch ein Iontophorese-Pflaster gelöst werden. Relevanter Stand der Technik für iontophoretische Systeme sind DE 37 03 321, WO 87/04936, WO 91/16077, WO 92/04938, WO 00/53256, US 3 991 755, US 4 141 359, US 5 415 629 und US 5 944 685.
Somicjjliclϊ wird die der Erfindung zugrunde liegende Aufgabe gemäß einer weiteren Ausführungsform durch ein transdermales therapeutisches System mit einem wirkstoffhaltigen Reservoir und fakultativer Abziehfolie und Remifentanil oder Remifentanil-Hydrochlorid als Wirkstoff gelöst.
Bei dem erfindungsgemäßen System können Remifentanil oder Remifentanil-Hydrochlorid als Dispersion in einem Hydrogel vorliegen.
Dabei können Hydroxymethylcellulose, Carbopol, Agarose oder Polyvinylalkohol als Hydrogel dienen.
Ferner kann Remifentanil oder Remifentanil-Hydrochlorid in dem Reservoir als Lösung in einem Lösungsmittel vorgesehen sein.
Als Lösungsmittel kann ein organisches Lösungsmittel, beispielsweise Ethanol, vorgesehen werden.
Ein erfindungsgemäßes System mit Reservoir kann hautseitig mit einer Kleberschicht versehen sein, die einen Gehalt an Remifentanil oder Remifentanil-Hydrochlorid aufweisen kann.
Das erfindungsgemäße System kann einen oder mehrere Permeationsforderer enthalten. So kann das erfindungsgemäße System mindestens einen Permeationsforderrer aus der Gruppe Ölsäure, Linolsäure, Linolensäure, Sojaöl, Dodecanol, Harnstoff, Labrafil, Labrasol, Monolaureat, insbesondere Propylenglykol-monolaureat, Glycerol-monolaureat und Polyethylenglykol-monolaureat, Octyldodecylacetat, Transcutol und Tocopherol umfassen.
Die erfindungsgemäßen Systeme können 1 bis 100 mg Remifentanil oder Remifenanil- Hydrochlorid enthalten.
Ferner kann das erfindungs gemäße System durch eine Abgaberate von 100 bis 3000 ug/h Remifentanil oder Remifentanil-Hydrochlorid gekennzeichnet sein. Beispielsweise kann das erfindungsgemäße System durch eine Fläche von 20 cm2 und eine Abgaberate von 5 bis 150 μg/cm2 h gekennzeichnet sein.
Schließlich kann das erfindungsgemäße System für eine Tragedauer von 12 h bis 72 h ausgebildet sein.
Nachstehend wird die Erfindung durch eine Figur und ein Beispiel näher erläutert.
Beispiel 1
Zur Herstellung einer Klebstoffschicht bzw. druckempfindlichen Haftschicht 1 wird ein handelsüblicher druckempfindlicher Kleber (Duro-Tak 2051) mit Hilfe einer geeigneten Beschichtungseinrichtung (beispielsweise reverse roll coater) auf eine silikonisierte
Trägerfolie 2 (aus beispielsweise Polyester und insbesondere Polyterephthalsäureester) derart aufgetragen, daß ein Flächengewicht des getrockneten Klebstoffs von 30 g/m2 resultiert.
Zur Herstellung einer Abdeckfolie, d.h. Trägerfolie 3 mit Verankerungsschicht 4 wird ein handelsüblicher druckempfindlicher Haftkleber (Duro-Tak 2051) mit Hilfe einer geeigneten Beschichtungseinrichtung (beispielsweise reverse roll coater) auf eine Trägerfolie 3 (aus beispielsweise Polyester und insbesondere Polyterephthalsäureester) derart aufgetragen, dass ein Flächengewicht des getrockneten Klebstoffes von 30 g/m2 resultiert.
Zur Herstellung der Wirkstofflösung wird Remifentanil in einem terpenhaltigen Pflanzenöl bis zur Sättigung gelöst.
Außerdem werden 10 g Gelatine, 2,32 g Glycerin, 0,51 g Polysorbat 80 und 26 g Wasser bei etwa 60°C zu einer homogenen Lösung verarbeitet. 8 g der zuvor hergestellten Wirkstofflösung werden bei etwa 75 °C in die Gelatinelösung einemulgiert. Die Emulsion wird mit Hilfe einer geeigneten Beschichtungseinrichtung (beispielsweise reverse roll coater) auf eine intermediäre, d.h. wieder zu entfernende Trägerfolie (aus beispielsweise Polyester und insbesondere Polyterephthalsäureester) derart aufgetragen, dass ein Flächengewicht der getrockneten Gelatine mit der inkorporierten Wirkstofflösung von 100 g/m2 resultiert.
Danach wird auf die nicht abgedeckte Seite der zuvor hergestellten Klebstoffschicht 1 der silikonisierten Trägerfolie 2 die wirkstoffhaltige Gelatineschicht 5 aufkaschiert. Die intermediäre Trägerfolie wird von der Gelatineschicht 5 entfernt. Die Klebstoffschicht 1 der Abdeckfolie 3 wiederum wird auf die von der intermediären Trägerfolie befreite Gelatineschicht 5 aufkaschiert. Das nun fünf-schichtige Laminat, bestehend aus Trägerfolie 3, Verankerungsschicht 4, wirkstoffhaltiger Gelatineschicht 5, Klebstoffschicht 1 und silikonisierter Polyesterfolie 2 wird in einer geeigneten Stanz- oder Schneidvorrichtung zu Transdermalsystemen mit einem geeigneten Durchmesser verarbeitet. Nach Entfernen der
silikonisierten Polyesterfolie 2 kann das System mit der druckempfindlichen Haftschicht 1 auf die Haut aufgeklebt werden.
Claims
1. Transdermales therapeutisches System mit Deckschicht, wirkstoffhaltiger Matrix und fakultativer Abziehfolie und Remifentanil oder Remifentanil-Hydrochlorid als Wirkstoff.
2. System nach Anspruch 1, wobei das System mit Remifentanil oder Remifentanil- Hydrochlorid übersättigt ist.
3. System nach mindestens einem der vorhergehenden Ansprüche, wobei das System einen Übersättigungsstabilisator umfaßt.
4. System nach Anspruch 3, wobei das System Polyvinylpyrrolidon als Übersättigungsstabilisator umfaßt.
5. System nach mindestens einem der vorhergehenden Ansprüche, wobei die Matrix aus einem Material besteht, das durch Hautfeuchtigkeit an- oder auflösbar ist
6. System nach mindestens einem der Ansprüche 1 bis 4, wobei die Matrix ein Material umfaßt, das durch Hautfeuchtigkeit an- oder auflösbar ist.
7. System nach mindestens einem der vorhergehenden Ansprüche, wobei das System ein Verdickungsmittel oder Quellmittel umfasst, vorzugsweise Hydroxymethylcellulose, insbesondere Klucel.
8. System nach Anspruch 6 und/oder 7, wobei Remifentanil als Lösung, als Dispersion oder als Suspension in einem nicht-wassermischbaren Polymer vorliegt oder wobei Remifentanil- Hydrochlorid als Dispersion oder Suspension in einem nicht-wassermischbaren Polymer vorliegt.
9. System nach mindestens einem der Ansprüche 1 bis 4, wobei das System ein Elektrodenpflaster mit ein oder mehreren hautnahen Elektroden und mit ein oder mehreren deckschichtnahen Gegenelektroden ist.
10. System nach mindestens einem der Ansprüche 1 bis 4, wobei das System ein Iontophorese-Pflaster ist.
11. Transdermales therapeutisches System mit einem wirkstoffhaltigen Reservoir und fakultativer Abziehfolie und Remifentanil oder Remifentanil-Hydrochlorid als Wirkstoff.
12. System nach mindestens einem der vorhergehenden Ansprüche 9, 10 und 11, wobei das System Remifentanil oder Remifentanil-Hydrochlorid als Dispersion in einem Hydrogel umfasst.
13. System und Anspruch 12, gekennzeichnet durch Hydroxymethylcellulose, Carbopol oder Polyvinylalkohol als Hydrogel.
14. System nach Anspruch 11, wobei das Reservoir Remifentanil oder Remifentanil- Hydrochlorid als Lösung in einem Lösungsmittel umfaßt.
15. System nach Anspruch 14, wobei das Reservoir Remifentanil oder Remifentanil- Hydrochlorid als Lösung in einem organischen Lösungsmittel umfaßt, insbesondere Ethanol.
16. System nach mindestens einem der vorhergehenden Ansprüche 11 bis 15, wobei das Reservoir hautseitig mit einer Kleberschicht versehen ist.
17. System nach Anspruch 16, wobei die Kleberschicht, die mit der Haut in Kontakt gebracht werden kann, einen Gehalt an Remifentanil oder Remifentanil-Hydrochlorid aufweist.
18. System nach mindestens einem der vorhergehenden Ansprüche, wobei das System einen oder mehrere Permeationsförderer enthält.
19. System nach Anspruch 18, wobei das System mindestens einen Permeationsförderer aus der Gruppe Ölsäure, Linolsäure, Linolensäure, Sojaöl, Dodecanol, Harnstoff, Labrafil, Labrasol, Monolaureat, insbesondere Propylenglykol-monolareat, Glycerol-monolaureat, und Polyethylenglycol-monolaureat, Octyldodecylactat, Transcutol und Tocopherol umfasst.
20. System nach mindestens einem der vorhergehenden Ansprüche, wobei das System 0,1 bis 100 mg Remifentanil oder Remifentanil-Hydrochlorid enthält.
21. System nach mindestens einem der vorhergehenden Ansprüche, gekennzeichnet durch eine Abgaberate von 100 bis 3000 μg/h Remifentanil oder Remifentanil-Hydrochlorid.
22. System nach Anspruch 21, gekennzeichnet durch eine Fläche von 20 cm2 und eine Abgaberate von 5 bis 150 μg/cm h Remifentanil oder Remifentanil-Hydrochlorid.
23. System nach mindestens einem der vorhergehenden Ansprüche, wobei das System für eine Tragdauer von 12 h bis 72 h ausgebildet ist.
