Abstract
La demande concerne un dispositif de réaxation d'un genou affecté d'une déformation en varus. L' implant est caractérisé par une cage d'ostéotomie métallique (1) pourvue d'une importante zone de rhéabitation osseuse, et de correction angulaire ajustable. Le design de la prothèse est déterminé par le profil des faces FS et Fl médio latérales L.5 (figure 1) et L.8 (figure 2) , puis des dites faces antéro postérieures L.4 (figure 1) et L.7 (figure 2) . Le dit corps est prolongé par une patte P.1 , et une patte P.2. Les dites pattes coulissant l'une contre l'autre verticalement, actionnent ainsi l'ouverture et la fermeture des faces FS et Fl, afin d'effectuer la correction angulaire adéquate. La patte P.1 est pourvue d'une gorge GOC dans laquelle est introduite une vis cortico spongieuse, la dite vis étant également introduite dans un deuxieme orifice ORV de la patte P.2, puis dans le tibia, bloquant ainsi la correction angulaire. Les charnieres C sont pourvues de plots d'ancrage PLT.1 et PLT.2 solidaires de la face inférieure Fl, les dits plots étant introduits dans les orifices 0.1 et 0.2 solidaires de la face supérieure FS, pour actionner les dites charnières C.
Description
CAGE D'OSTEOTOMIE TlBIALE A ANGULATION AJUSTABLE
L'invention concerne un dispositif de réaxation d'un genou affecté d'une déformation en varus, et susceptible d'être insérée dans l'ouverture d'une ostéotomie. L'indication pour laquelle ce type d'implant est utilisé, a généralement pour origine une gonarthrose qui concerne la partie interne de l'articulation fémoro tibiale.
La correction de cette déformation, impliquant un rétablissement de l'axe de la jambe préalablement calculé en pré opératoire, est effectuée à l'aide de divers matériaux. Les prothèses jusqu'à présent proposées pour obtenir la correction angulaire pour les ostéotomies tibiales réalisées par voie interne, sont des greffons osseux ne permettant pas un appui précoce, des cales d'ostéotomies tibiales générant un encombrement important dans l'ouverture de l'ostéotomie, réduisant d'autant la zone de réhabitation osseuse.
D'autre part, les systèmes de correction internes centrés sur l'axe des faces internes et externes du tibia avec implantation de greffons osseux associés, n'offrent qu'un appui partiellement satisfaisant sur la face interne, et une possibilité de perte de correction après explantation du matériel.
L'objet de la présente invention est de proposer un dispositif implantable bio compatible, caractérisé par une cage d'ostéotomie métallique, pourvu d'une importante zone de réhabitation osseuse, de correction angulaire ajustable au niveau de sa hauteur maximale, et maintien de la correction pendant la consolidation osseuse, permettant ainsi un appui précoce.
Ce dispositif rend possible selon sa conception, le passage d'un plot d'ancrage d'une prothèse totale de genou, en cas de reprise ultérieure de l'indication, et en principe une implantation définitive.
La prothèse peut supporter une charge pour des contraintes en compression d'au moins 140 kg.
Le design de la prothèse qui est déterminée en fonction du profil des faces médio latérales et antéro postérieures du tibia, génère un bon appui sur l'os compact.
Par rapport à la première demande de brevet déposée le 2 Mai 2005 sous le n°0504435, l'objet de la présente demande est de proposer un mode de réalisation de l'implant simplifié en ce qui concerne le mécanisme articulant les deux faces de l'implant précédemment décrit, et induisant également une variation d'angulation entre les dites faces de 8 à 14°.
La matière préférentiellement utilisée pour sa réalisation est l'inox médical 316L. La cage d'ostéotomie fait l'objet d'un traitement de surface créant des micro porosités apte à permettre la fusion entre l'os et les parties métalliques. Les faces supérieures et inférieures sont percées de trous d'un diamètre d'environ 1 mm, afin de favoriser la recolonisation osseuse, et l'adhésion des surfaces.
La prothèse qui est très ajourée, représente un volume dans l'ouverture de l'ostéotomie peu conséquent, et peu susceptible de générer des doléances de la part des patients. Le descriptif et le mode de réalisation qui vont suivre sont donnés à titre indicatif et non limitatif.
Selon le descriptif :
Figure 1 : La figure 1 est une vue schématique de dessus sur laquelle est représentée la face supérieure FS du corps prothétique 1. L'épaisseur de la dite face, comme la patte P.1 sont d'environ 1 mm. Le dit corps a une hauteur finale de l'ordre de 12 mm, correspondant sensiblement à une angulation de 14°. La longueur correspondant à l'encombrement médio latéral L.5 est d'environ 38 à 45 mm, et la largeur L.4 qui correspond à l'encombrement antéro postérieur, a une valeur de l'ordre de 36 à 42 mm.
Les valeurs L.4 (figure 1) et L.7 (figure 2), ont été majorées dans l'axe antéro postérieur par rapport à la première demande de brevet, dans la mesure où les charnières de la cage ne sont plus articulées par le même système. Par conséquent, les charnières C. de la cage doivent se situer au plus près de la charnière osseuse, afin que les angulations soient exactes.
L'encombrement L.5 et L.4 font l'objet de 3 tailles de cage d'ostéotomie respectivement pour le tibia gauche et pour le tibia droit proposées en fonction du gabarit du patient.
La face supérieure du corps prothétique 1 qui est percée de 40 à 45 trous TR 1 répartis sur sa surface, est prolongée par une patte P.1 d'une longueur L.3 de l'ordre de 30 à 35 mm, et d'une largeur L.6 de l'ordre de 8 à 10 mm en fonction des 3 tailles de cage. Cette patte est repliée depuis la base B 1 , vers le bas, ou autrement dit en direction opposée au plateau tibial. La dite patte P.1 coulisse sur la patte P.2, cette dernière étant décrite sur la figure 2. La superposition de ces 2 pattes qui coulissent l'une contre l'autre, articule les deux faces pour effectuer les variations angulaires de la cage d'ostéotomie.
La surface interne de la patte P.1 correspondant à la longueur L.2 est crantée, et destinée à coïncider parallèlement à la surface externe crantée de la figure 2, pour stabiliser l'angulation des 2 faces, avant que la dite angulation soit définitivement bloquée par une vis cortico spongieuse introduite dans l'orifice GOC. La gorge GOC d'une longueur L.3A de l'ordre de 12,5 à 15 mm et d'une largeur L.3B de 4 à 6 mm, correspond au débattement vertical du réglage d'angulation de 8 à 14° des 2 faces. Les orifices 01 et 02 d'un diamètre de l'ordre d'un peu plus de 2 mm, permettent le passage de plots d'ancrage dans le même axe vertical. Ce système autorise l'articulation des charnières.

La figure 2 est une vue schématique de dessus sur laquelle est représentée la face inférieure F1 du corps prothétique 1. Les dimensions L.7 et L.8 de Ia dite face ont sensiblement les mêmes dimensions que L.4 et L.5 décrite sur la figure 1.
L'épaisseur de cette face, comme la patte P.2 sont également d'environ 1 mm.
Cette face inférieure qui est percée de 40 à 45 trous TR2 répartis comme sur la surface de la face supérieure, est prolongée par une patte P.2 d'une longueur L.9A de 30 à 35 mm, et d'une largeur L.9B de 8 à 10 mm en fonction des 3 tailles de cage, pour chaque tibia gauche et droit.
Cette patte P.2 est repliée dans le même sens que la patte P.1. Les dites pattes respectives s'articulent en glissant parallèlement l'une contre l'autre, en décrivant un rayon de courbure adéquate à la pente du tibia. La longueur L.10 de la surface externe crantée de la patte P.2, est de l'ordre de 8 à
11 mm.
L'orifice ORV d'un diamètre de l'ordre de 4 mm, correspond au passage de la vis cortico spongieuse, Ia dite vis étant introduite ensuite dans l'orifice GOC (figure 1), puis dans Ie tibia, bloquant ainsi l'angulation. L'extrémité de la patte P.2 dite MS d'une longueur de l'ordre de 10 à 13 mm, fait l'objet d'un marquage laser des graduations d'angulation de 8 à 14°, dans Ie même sens que les crans représentés sur la longueur L.10. Par conséquent, la patte P.1 révèle les graduations marquées sur la patte P.2 suivant son déplacement vertical, autrement dit lorsque la cage s'ouvre ou se referme. Ce système autorise une précision particulièrement intéressante du réglage de l'angulation.
Les plots d'ancrage PLT.1 et PLT.2 solidaires de la face inférieure, permettent verticalement l'articulation des 2 faces de l'implant au niveau des charnières, comme précédemment décrit. Leur diamètre est de l'ordre d'environ 1 ,5 mm, et leur hauteur de l'ordre de 2 mm est abrasée de façon à ne pas créer une protubérance ressortant à l'extérieur de la face supérieure, et susceptible de gêner la fermeture.
Figure 3 :
La figure 3 est une vue schématique de profil des 2 faces supérieures FS et inférieures Fl, ainsi que des deux pattes P1 et P2 s'articulant pour effectuer les différentes angulations possibles. Ce dispositif est destiné à être implanté 10 à 15 mm au dessous de l'interligne du plateau tibial. En fonction de la pente tibiale moyenne verticalement, et de l'encombrement vertical de l'implant à ce niveau, le rayon préalablement défini pour la patte P.1 est de 34,3 et 32,5 pour la patte P.2.
L'épaisseur des pattes P.1 et P.2 de l'ordre de 1 mm, autorise cependant un ajustement du rayon de courbure lors de la mise en place de la vis cortico spongieuse venant bloquer le système, dans la mesure où l'épaisseur de 1 mm de l'implant permet une déformation, de telle sorte que les dites pattes se retrouvent parallèles et affleurantes à la corticale du tibia.
La gorge G qui est représentée sur la patte P.2, présente une proéminence externe sur laquelle se trouve une surface crantée qui permet un blocage temporaire comme précédemment exposée sur les figure 1 et 2.
L'axe des charnières C et des plots d'ancrage PLT.1 et PLT.2 assure le débattement nécessaire à l'articulation de la prothèse.
Principe de montage :
Une ostéotomie biplane peut être réalisée pour implanter le corps prothétique 1.
C'est-à-dire qu'elle comprend deux parties :
Une partie horizontale qui coupe le tibia de dedans en dehors jusque sur la corticale externe, en gardant celle-ci comme charnière, et une partie oblique qui se situe à la face postérieure de la tubérosité, et qui rejoint la première ostéotomie. Cette ostéotomie est réalisée à la scie oscillante.
On sent et on teste avec la main la résistance à l'ouverture du tibia. Dès qu'il cède, on arrête de scier, l'idéal étant de garder une charnière quasi-complète. Les petits fragments osseux issus de la section osseuse, sont recueillis afin de les introduire ultérieurement dans la cage d'ostéotomie.
Les 3 gabarits d'essai permettent une évaluation de l'encombrement antéro postérieur et médio latéral du corps prothétique 1 à implanter.
Un davier est introduit dans l'ouverture de l'ostéotomie, afin de maintenir la correction souhaitée.
La prothèse est introduite dans la dite ouverture, et on procède aux derniers réglages.
Le chirurgien peut alors combler la cage d'ostéotomie de greffons osseux.
La dite cage est finalement sécurisée par un matériel d'ostéosynthèse de type plaque vis par exemple, de part et d'autre des pattes P.1 et P.2 durant le temps de la consolidation.
Claims
REVENDICATIONS
Revendication 1
Implant d'ostéotomie caractérisé en ce qu'il comprend un corps prothétique 1 matérialisé par une cage d'ostéotomie très ajourée permettant une rhéabitation osseuse, à ajustement réglable de l'angulation, pour la correction d'un axe de jambe affecté d'une déformation en varus.
Revendication 2
Implant d'ostéotomie selon la revendication 1 , caractérisé en ce qu'il comprend un réglage de l'angulation de la cage entre les faces FS et Fl, généré par le coulissement des pattes P.1 et P.2 l'une contre l'autre, ce système apportant une correction angulaire très précise.
Revendication 3
Implant d'ostéotomie selon la revendication 1 et 2, caractérisé en ce qu'il comprend une crémaillère crantée située au niveau de la patte P.1 (figure 1) et de la patte P.2 (figure 2), et apte à bloquer temporairement l'angulation des faces FS et Fl.
Revendication 4
Implant d'ostéotomie selon la revendication 1 , 2, 3 caractérisé en ce qu'il comprend une gorge GOC située au niveau de la patte P.1 (figure 1), et une autre gorge ORV située au niveau de la patte P.2 (figure 2), dans lesquelles sont introduites une vis cortico spongieuse vissée dans le tibia, ce système étant apte à bloquer le mécanisme actionnant les angulations possibles.
Revendication 5
Implant d'ostéotomie selon la revendication 1 , caractérisé en ce qu'il comprend un système d'articulation des charnières C, caractérisé par les orifices O.1 et O.2 situés sur la face supérieure FS, dans lesquels sont introduits les plots d'ancrage PLT.1 et PLT.2 solidaires de face inférieure Fl, et permettant l'articulation de la cage au niveau des charnières C.
