Definitions
- the invention relates to a knee joint prosthesis with a femur part and a tibia part, the plane of the joint gap between the parts being inclined according to the anatomical conditions of the natural joint to the vertical axis leading from the center of the femoral head to the ankle.
- Such an anatomical knee prosthesis is shown in DE 100 62 715 AI. It significantly improves knee joint prostheses that have been used hitherto, the joint gap plane of which extends perpendicular to the axis mentioned and which are therefore only imperfectly adapted to the actual anatomical conditions.
- the object of the present invention is to create a new knee prosthesis of the type mentioned at the outset which, compared to the known prosthesis of this type, can take over the function of the natural, healthy knee joint to an even greater extent.
- the knee joint prosthesis which achieves this object is characterized in that at least the tibia part has a space which creates space for the anterior cruciate ligament.
- BESTATIGUNGSKOPIE This solution of the invention advantageously makes it possible to use a knee joint prosthesis that is adapted to the inclination of the natural joint gap in combination with a cruciate ligament prosthesis or a preserved anterior cruciate ligament, and thus to further adapt the function of the prosthesis to the natural knee joint.
- the possibility of preserving the anterior cruciate ligament or inserting an appropriate cruciate ligament graft can prevent instabilities when bending the knee in particular.
- the tibia part has a plate forming the tibia plateau and the recess comprises a cutout in this plate through which the cruciate ligament or a cruciate ligament implant can be guided or which releases an area for connecting the implant to the lower leg bone.
- the plate cutout is preferably open toward the dorsal plate edge, so that a cruciate ligament which has been preserved can enter the cutout laterally during the implantation.
- the plate is connected to a hollow anchoring pin that is open to the plate cutout, and a passage opening for the cruciate ligament or cruciate ligament implant is formed in the wall of the anchoring pin.
- the cavity of the anchoring pin can be filled with bone cancellous bone.
- the plate cutout is arranged between two anchoring pins formed on the plate.
- the two anchoring brackets adjoin the plate cutout, so that they are arranged as centrally as possible with a view to stable anchoring in the shaft of the lower leg bone.
- jacket lines of the anchoring pins bordering the plate cutout are perpendicular to the edge of the cutout, the jacket lines running through the pin tips.
- a further anchoring element can be formed on the plate opposite the dorsal edge opening of the plate cutout, e.g. through a web protruding from the edge of the plate in the direction of the anchoring pins.
- the latter anchoring element serves on the one hand to stabilize the tibia part by reinforcing a plate part that remains between the ventral edge and the plate cutout.
- the bridge helps to stabilize the connection of the tibia part to the lower leg bone.
- FIG. 1 shows a first embodiment of a tibia part of a knee joint prosthesis according to the invention
- Fig. 2 shows a second embodiment of a tibia part of a knee joint prosthesis according to the invention in a view obliquely from above
- Fig. 3 shows the tibia part of Fig. 2 in a view obliquely from below
- FIG. 4 shows a further exemplary embodiment of a tibia part of a knee joint prosthesis according to the invention in an oblique view from below
- FIG. 5 shows a knee joint prosthesis according to the prior art
- FIG. 6 shows the leg skeleton to explain the function of the prostheses according to FIG 1 to 5.
- FIG. 6 where the upper and lower leg bones 2 and 3 connected to the knee joint 1 are shown in a front view.
- a vertical axis 5 leading through the center of the femoral head 4 runs through the ankle at 6.
- An axis 7 parallel to the plane of the tibial plateau and leading through the gap of the knee joint 1 intersects the axis 5 at an angle a which can deviate from a right angle by up to 10 ° and is on average 85 °.
- the tibial plateau rises laterally.
- the axis 5 also intersects an axis 8 parallel to the tibial plateau on the ankle, at an angle ⁇ which deviates from the right angle by 2 ° to 3 °, the tibial plateau on the ankle falling to the side.
- the axes 7 and 8 intersect at an angle ⁇ , which is on average around 7 ° to 8 °.
- tibial plateau slopes backwards by 3 ° to 7 °.
- Fig. 5 shows a known from DE 100 62 715 AI included here
- Knee joint prosthesis which takes into account the anatomical conditions explained above.
- the knee joint prosthesis has a tibia part 9 'according to FIG. 5a and a femur part 10 shown in FIG. 5b.
- the plate 11 formed with a metallic carrier part 13 and a plastic projection 14 on the plateau has concavities 15 and 16 corresponding to condyle parts 17 and 18 of the femoral part 10. It is understood that the axis 7 'represents an average course of a corresponding line on the tibial plateau surface.
- the polycentric radii of the condyle parts 17 and 18 differ in size and the medial condyle part 17 protrudes further in relation to the axis 5 than the lateral condyle part 18 in accordance with the tapering plateau inclined medially.
- the anchoring pin 12 ' is driven into the front of the lower leg bone, its longitudinal axis 5' coinciding with the axis 5 and the axis 7 'coinciding with the axis 7.
- the cap-like, at the Pl ⁇ nt ⁇ tion adapted to the femur part 10 is aligned with its axis 5 'to the axis 5.
- the femoral part 10 is arranged rotated by 180 ° in FIG. 5b.
- FIG. 1 where a tibia part 9 of a knee joint prosthesis with a plate 11 forming the tibia plateau is shown.
- a substantially cylindrical anchoring pin 12 extends centrally from the plate 11, the cylinder axis of which is to be aligned with the axis 5 and is at the angle ⁇ mentioned to the plane of the plate 11.
- a surface structure of the tibial plateau corresponding to the curvatures 15 and 16 of FIG. 5a is not shown in FIG. 1.
- the tibia part 9 has a recess at 19, which comprises a plate cutout 20 opening dorsally at the edge of the plate 11 and a wall cutout 21 of a cylinder wall 23 of the hollow anchoring pin 12.
- Part of the recess 19 is also an opening 22 in the wall 23 of the anchoring pin 12.
- the opening 22 extends over the entire length and approximately a quarter of the circumference of the anchoring pin 12.
- the anchoring peg has a structured surface coating which promotes the growth of bone tissue, which is indicated at 26.
- the wall 23 of the anchoring pin can be perforated like a grid, as shown at 27, in order to enable the ingrowth of bone tissue.
- a cutting edge is expediently formed at the free end of the anchoring pin 12.
- the recess 19 allows implantation of a plastic anterior cruciate ligament in the case of an implanted knee joint prosthesis, which runs obliquely to the plate 11 through the opening 22 in the anchoring part 12 and further through the plate cutout 20 and the wall cutout 21 to the thigh bone, where in the notch between the condyle parts 17 and 18 there is a connection to the bone tissue.
- the cavity in the anchoring part 12 can be filled with bone cancellous bone.
- the prosthesis largely takes over the function of the healthy knee by ensuring that the inner leg naturally rotates even when the knee is flexed.
- FIGS. 2 and 3 differs from the exemplary embodiment of FIG. 1 in that two decentrally arranged, approximately conical anchoring pins 12a and 12a 'are provided without a wall opening, between which one opens dorsally to the edge of a plate 11a Plate cutout 20a is arranged.
- a further anchoring element in the form of a web 24 is provided on the edge of the plate opposite the opening of the plate cutout 20a.
- the plate 1 1 a is stepped so that the central plate plane is at an angle a to the cone axis.
- the tibia part 9a can be implanted while the anterior cruciate ligament has been preserved by inserting the still present cruciate ligament laterally into the plate cutout 20a.
- two anchoring pins 12b and 12b ' are brought directly to a panel cutout 20b and have surface lines 25 and 25' perpendicular to the cutout edge.
- the closer anchoring pins 12b and 12b ' can be longer than the pins 12a and 12a' of the embodiment of FIGS. 2 and 3, since they are driven more centrally into the bone shaft of the lower leg bone.
