Abstract
Die Erfindung betrifft eine Vorrichtung zur Injektion in oder zur Punktion von Gelenkhohlräumen, in den Gelenkhohlraum einführbaren Hohlkanüle (11), die mit einer Injektions- oder Punktionsspritze (9) verbunden ist. Um bei einer derartigen Vorrichtung Verletzungen der den Gelenkhohlraum begrenzenden Knorpel oder Knochen zu vermeiden, schlägt die Erfindung vor, daß die Spritze (9) in der Einstichrichtung verschiebbar in einer Stützvorrichtung (1) gelagert ist, die im Bereich der Einstichstelle an der Körperoberfläche (4) des Patienten abstützbar ist und im Bereich der Abstützfläche einen Ultraschall-Meßkopf (3) einer Sonar-Meßvorrichtung (6) aufweist, die ein der zulässigen Einstichtiefe entsprechendes Steuersignal erzeugt, welches eine der Stützvorrichtung (1) zugeordnete Einrichtung zur Begrenzung der Einstichtiefe steuert.
Description
Vorrichtung zum Injizieren oder Punktieren von Gelenkhohlräumen
Die Erfindung betrifft eine Vorrichtung zur Injektion in oder zur Punktion von Gelenkhohlräumen, mit einer in den Gelenkhohlraum einführbaren Hohlkanüle, die mit einer Injektions- oder Punktionsspritze verbunden ist.
Bei der Applikation von Medikamenten in der orthopädi¬ schen oder chirurgischen Praxis ist es oft erforderlich, das Medikament unmittelbar in Gelenkhohlräume zu inji¬ zieren. Ebenso ist es häufig notwendig, sich ansammelnde Körperflüεsigkeit durch Punktion aus Gelenkhohlräumen zu entfernen. In beiden Fällen wird eine mit einer Spritze (Kolbenpumpe) oder einer sonstigen geeigneten Pumpe verbundene Hohlkanüle durch den Gelenkspalt hindurch in den Gelenkhohlraum eingeführt. Dabei ist es wegen der Verschiedenheit in der Anatomie der Patienten oft schwierig, den Gelenkhohlraum exakt zu treffen und insbesondere zu vermeiden, daß die Spitze der Injek¬ tionsnadel oder der Punktionskanüle durch zu tiefes Eindringen die den Gelenkhohlraum begrenzenden Knorpel oder Knochen verletzt. Um solche iatrogenen Schäden zu vermeiden, ist beim Injizieren von Medikamenten in Gelenkhohlräume oder beim Punktieren von Flüssigkei¬ ten aus Gelenkhohlräumen das Geschick des Arztes in besonders hohem Maße gefordert.
Es ist Aufgabe der Erfindung, dem behandelnden Arzt ein Hilfsmittel an die Hand zu geben, welches es ihm erheblich erleichtert, ohne Beschädigung von Knochen oder Knorpeln eine Hohlkanüle in einen Gelenkhohlraum einzμführen. Insbesondere soll durch dieses Hilfsmittel vermieden werden, daß durch zu tiefes Einstechen die den Gelenkhohlraum begrenzenden Knorpel oder Knochen verletzt werden.
Gegenstand der Erfindung ist eine Vorrichtung zur Injek¬ tion in oder zum Punktieren von Gelenkhohlräumen, mit einer in den Gelenkhohlraum einführbaren Hohlkanüle, die mit einer Injektions- oder Punktionsspritze verbun¬ den ist, wobei sich diese Vorrichtung dadurch kennzeich¬ net, daß die Spritze in der Einεtichrichtung verschieb¬ bar in einer Stützvorrichtung gelagert ist, die im Bereich der Einstichstelle an der Körperoberfläche des Patienten abstützbar ist und im Bereich der Abstütz- fläche den Ultraschall-Meßkopf einer Sonar-Meßvorrich- tung aufweist, die ein der zulässigen Einstichtiefe entsprechendes Steuersignal erzeugt, welches eine der Stützvorrichtung zugeordnete Einrichtung zur Begrenzung der Einstichtiefe steuert.
Die Vorrichtung gemäß der Erfindung benutzt ein in der medizinischen Praxis vielfach bewährtes Diagnoseverfah¬ ren, nämlich die Sonographie, zur automatischen Limi¬ tierung der Einstichtiefe beim Einstechen einer Hohlkanüle in einen Gelenkhohlraum. Der vom Ultraschall-Meßkopf der Sonar-Meßvorrichtung erzeugte Ultraschall wird an der Knorpelgrenze bzw. an der Knochengrenze reflektiert. Das Reflektionεsignal wird von dem Ultraschall-Meßkopf aufgenommen, zur Bestimmung der Entfernung ausgewertet und in ein Steuersignal umgewandelt, welches eine der
Stützvorrichtung zugeordnete Einrichtung zur Begrenzung der Einstichtiefe steuert. Auf diese Weise wird verhin¬ dert, daß der Knorpel oder der Knochen durch zu tiefes Einstechen der Hohlkanüle verletzt wird oder die Hohlka¬ nüle am Knochen verbogen wird. Außerdem wird sicherge¬ stellt, daß die Öffnung der Hohlkanüle in den Gelenk¬ hohlraum ausmündet und nicht etwa in einem Fettkörper oder einer ligamentären Struktur.
Eine vorteilhafte Weiterbildung der Erfindung sieht vor, daß die Sonar-Meßvorrichtung zusätzlich ein die optimale Einstichstelle lokalisierendes Signal erzeugt. Hierbei wird die Sonar-Meßvorrichtung zugleich dazu benutzt, den für die Einführung der Hohlkanüle erforderlichen Gelenk¬ spalt sicher zu finden.
Zweckmäßig ist die Sonar-Meßvorrichtung mit einem Dis¬ play verbunden, auf welchem die zulässige Einstichtiefe und/oder die optimale Einstichstelle digital und/oder als Bild darstellbar sind. Ein solches Display erleich¬ tert es dem behandelnden Arzt, sich eine genaue Informa¬ tion über die zulässige Einstichtiefe und/oder die optimale Einstichstelle zu verschaffen, ohne in erster Linie auf Erfahrung und Fingerspitzengefühl angewiesen zu sein.
Zweckmäßig ist der Ultraschall-Meßkopf mit einem Durch¬ trittskanal für die Hohlkanüle versehen. Hierdurch ist es möglich, den Ultraschall-Meßkopf im unmittelbaren Umgebungsbereich der einzustechenden Hohlkanüle zu plazieren. Dies hat den besonderen Vorteil, daß die Meßrichtung des Ultraschall-Meßkopfes nahezu genau mit der Einstichrichtung der Hohlkanüle übereinstimmt.
Zweckmäßig weist die Stützvorrichtung oberhalb des Ultraschall-Meß öpfes eine die Hohlkanüle umgebende, durchsichtige Griffhülse auf. Diese Griffhülse erleich¬ tert es dem behandelnden Arzt, die Vorrichtung sicher zu greifen und zu führen und die Hohlkanüle während des Einstechvorganges zu beobachten.
Der Einstechvorgang selbst kann von Hand oder mittels eines motorischen Antriebes vorgenommen werden. Für den zuletzt genannten Fall sieht eine besonders bevor¬ zugte Ausführungsform der Vorrichtung gemäß der Erfin¬ dung vor, daß die Stützvorrichtung mit einer motorisch angetriebenen Vorschubeinrichtung für die verschiebbar gelagerte Spritze versehen ist, wobei der Vorschubweg dieser Vorschubeinrichtung durch das Steuersignal der Sonar-Meßvorrichtung gesteuert ist.
Ein Ausführungsbeispiel der Erfindung wird im folgenden anhand der Zeichnung näher erläutert, die die Vorrich¬ tung schematisch in perspektivischer Ansicht zeigt.
In der Zeichnung ist die Stützvorrichtung in ihrer Gesamtheit mit dem Bezugszeichen 1 bezeichnet. Die Stützvorrichtung 1 weist einen langgestreckten, hohl ausgebildeten Stützkörper 2 auf, dessen Hohlraum zug¬ leich als Leitungskanal für die elektrischen Signal- und Energieleitungen dient.
Der Stützträger 2 ist an seinem unteren Ende mit einem Ultraschall-Meßkopf 3 versehen, der auf die Körperober¬ fläche 4 des Patienten oberhalb eines Gelenkspaltes 5 aufεetzbar ist. Der Ultraschall-Meßkopf 3 ist über durch den Hohlraum des Stützträgers 2 verlaufende Kabel mit einer Sonar-Meßvorrichtung 6 verbunden. Dieses Sonar-
Meßvorrichtung 6 ist zugleich an ein Display 7 ange¬ schlossen, welches zur digitalen oder bildlichen Wieder¬ gabe der von der Sonar-Meßvorrichtung 6 ermittelten und ausgewerteten Daten dient.
Am dem Ultraschall-Meßkopf 3 gegenüberliegenden Ende ist der Stützträger 2 mit einem feststehenden Führungsring 8 verbunden, in welchem vertikal verschiebbar eine aus¬ wechselbare Einweg-Spritze 9 gelagert ist. Die Einweg- Spritze 9 ist am unteren Ende mit einer Hohlkanüle 10 versehen, die entweder als Injektionskanüle oder als Punktionskanüle ausgebildet sein kann. Die Hohlkanüle 10 verläuft durch einen in der Mitte des Ultraschall-Me߬ kopfes 3 befindlichen Durchtrittskanal 11 derart, daß sie unterhalb des Ultraschall-Meßkopfeε nach unten auεtreten kann, um in den Gelenkεpalt 5 einzudringen.
Zum Verschieben der Einweg-Spritze in der Einstichrich¬ tung der Hohlkanüle 10 dient eine bewegliche Spritzen- halterung 12, die unverschiebbar mit der Einweg-Spritze
9 und in vertikaler Richtung verschiebbar mit dem Stütz- träger 2 der Stützvorrichtung 1 verbunden ist. Zur Verschiebung der beweglichen Spritzenhalterung 12 an dem Stützträger 2 dient eine Schubεtange 13, die von einem Antriebsmotor 14 angetrieben wird, der seinerseits an dem Stützträger 2 befestigt ist. Der Motor 14 wird von der Sonar-Meßvorrichtung 6 so gesteuert, daß der Vorschub der Einweg-Spritze 9 und damit der Hohlkanüle
10 beendet wird, venn die vom Ultraschall-Meßkopf 3 und der Sonar-Meßvorrichtung 6 ermittelte maximale Einstichtiefe erreicht ist. Ist die richtige Einstich¬ tiefe erreicht, wird auf herkömmliche Art und Weise z. B. von Hand oder mit Hilfe eines nicht näher darge¬ stellten Motors der Kolben der Einweg-Spritze 9 bewegt, um das in der Spritze befindliche Medikament zu injizie¬ ren oder über die Hohlkanüle 10 Körperflüssigkeit aus dem Gelenkhohlraum abzusaugen.
Zum Zwecke der sicheren Handhabung der Vorrichtung und um die Hohlkanüle 10 während des Einstechvorganges genau beobachten zu können, ist der Stützträger 2 ober¬ halb des Ultraschall-Meßkopfes 3 mit einer durchsichti¬ gen Griffhülse 15 versehen, die die Hohkanüle 10 umgibt. Bei der Vorrichtung gemäß der Erfindung ermittelt die Ultraschall-MeßVorrichtung nicht nur die maximal zuläs¬ sige Einstechtiefe, sondern ermittelt auch die genaue Lage des Gelenkspaltes 5. Diese Lokalisierung des Ge¬ lenkspaltes 5 wird ebenfalls auf dem Display 7 digital oder durch ein Bild dargestellt.
An die Stelle der Einweg-Spritze 9 können auch andersar¬ tig ausgebildete Injektions- oder Punktionspumpen tre¬ ten. In jedem Falle muß jedoch dafür Sorge getragen werden, daß der Vorschub der Hohlkanüle 10 mittels einer Einrichtung zur Begrenzung der Einstichtiefe und ge¬ steuert durch die Sonar-Meßvorrichtung daran gehindert wird, zu tief in den Gelenkhohlraum einzudringen.

Claims
1. Vorrichtung zur Injektion in oder zur Punktion von Gelenkhohlräumen, mit einer in den Gelenkhohlraum einfüfcrbaren Hohlkanüle, die mit einer Injektionε- oder Punktionsspritze verbunden ist, d a d u r c h g e k e n n z e i c h n e t , daß die Spritze (9) in der Einεtichrichtung verschiebbar in einer Stützvorrichtung (1) gelagert ist, die im Bereich der Einstichstelle an der Körperoberfläche (4) des Patienten abεtützbar iεt und im Bereich der Abεtützflache einen Ultraschall-Meßkopf (3) einer Sonar- Meßvorrichtung (6) aufweist, die ein der zuläsεigen Einstichtiefe entsprechendes Steuersignal erzeugt, welches eine der Stützvorrichtung (1) zugeordnete Ein¬ richtung zur Begrenzung der Einstichtiefe steuert.
2. Vorrichtung nach Anspruch 1, dadurch gekennzeich¬ net, daß die Sonar-Meßvorrichtung (6) zusätzlich ein die optimale Einstichstelle lokalisierendes Signal erzeugt.
3. Vorrichtung nach den Anεprüchen 1 und 2, dadurch gekennzeichnet, daß die Sonar-Meßvorrichtung mit einem Diεplay (7) verbunden iεt, auf welchem die zuläεsige Einstichtiefe und/oder die optimale Einstichstelle di¬ gital und/oder als Bild darstellbar sind.
4. Vorrichtung nach Anspruch 1, dadurch gekennzeich¬ net, daß der Ultraschall-Meßkopf (3) mit einem Durch¬ trittskanal Cll) für die Hohlkanüle (10) versehen ist.
5. Vorrichtung nach Anspruch 1, dadurch gekennzeich¬ net, daß die Stützvorrichtung (1) oberhalb des Ultra¬ schall-Meßkopfes (3) eine die Hohlkanüle (10) umgebende, durchsichtige Griff ülse (15) aufweiεt.
6. Vorrichtung nach einem oder mehreren der Anεprüche 1 bis 5, dadurch gekennzeichnet, daß die Stützvorrichtung (1) mit einer motorisch angetriebenen Vorschubeinrichtung (13, 14) für die verschiebbar gelagerte Spritze (9) ver¬ sehen ist, wobei der Vorschubweg dieser Vorschubeinrich¬ tung (14, 15) durch das Steuersignal der Sonar-Meßvor¬ richtung (6) gesteuert ist.
