Abstract
Спосіб перитонеального лаважу при загальному перитоніті включає видалення вогнища перитоніту, дренування черевної порожнини полівінілхлоридними дренажами: правої та лівої піддіафрагмальних ділянок, правого бокового каналу та малого тазу, промивання черевної порожнини антисептичним розчином інтраопераційно та дозоване введення антисептичного розчину у післяопераційному періоді через верхні дренажі та виведення розчину через нижні дренажі впродовж 3-4 днів після операції. Інтраопераційна санація черевної порожнини завершується введенням в черевну порожнину антисептичного гелевого розчину на основі гіалуронової кислоти та у післяопераційному періоді фракційне введення цього розчину у верхні дренажі кожні 8 годин по 250-500 мл впродовж 3-4 днів після операції.Method for peritoneal lavage for common peritonitis involves removal of peritonitis lesion, drainage of abdominal cavity by polyvinyl chloride drainage: right and left sub-diaphragmatic areas, right lateral canal and small pelvis, flushing of abdominal cavity with antiseptic solution through the lower drainage within 3-4 days after surgery. У зв'язку з цим актуальності набуває перитонеальний лаваж черевної порожнини, якому присвячено недостатньо уваги (Матвійчук О.Б., Погорецький Р.М., 2012). Класичні методики перитонеального лаважу з антисептиками, що використовуються для санації черевної порожнини: водний розчин хлоргекседину 0,02 95, декаметоксин 0,02 95, фізіологічний розчин є ефективними лише перші 24 години, адже у зв'язку з процесом адгезії у черевній порожнині антисептик протікає по каналах, що формуються, а у місцях застою розчину виникає патоморфологічний субстрат для формування міжпетельних абсцесів, не відбувається адекватного зрошення всієї черевної порожнини та внутрішніх органів.In this connection, the peritoneal lavage of the abdominal cavity is gaining relevance, to which insufficient attention is devoted (Matviychuk O.B., Pogoretskyi R.M., 2012). В основі даної корисної моделі поставлено задачу оптимізувати перитонеальний лаваж у хворих на загальний перитоніт шляхом проведення перитонеального лаважу з використанням антисептичного гелю на основі гіалуронової кислоти, що покращує результати лікування загального перитоніту.The basis of this useful model is the task of optimizing peritoneal lavage in patients with general peritonitis by performing peritoneal lavage using an antiseptic gel based on hyaluronic acid, which improves the results of treatment of general peritonitis.

Хворий М., 37 років, госпіталізований у відділення хірургічного стаціонару Київської міської клінічної лікарні Ме5 в ургентному порядку з діагнозом: Виразкова хвороба дванадцятипалої кишки. Гостра перфорація виразки цибулини дванадцятипалої кишки. Загальний фібринозно- гнійний перитоніт. ІТШ І ст. З анамнезу відомо, що з моменту початку захворювання пройшло близько 30 годин. Призначено оперативне лікування в ургентному порядку. Хворому виконали лапаротомію. Висічення перфоративної виразки ДІЖ, пілоропластику по Джаду. Інраопераційно провели санацію черевної порожнини розчином декаметоксину в об'ємі 200 мл, осушили, механічно видалили плівки фібрину. Черевну порожнину дренували з чотирьох точок: правої та лівої піддіафрагмальних ділянок, правого бокового каналу та малого тазу. Рану ушили пошарово. У верхні дренажі ввели 500 мл антисептика на основі гіалуронової кислоти. Повторне введення діалізату провели через верхні дренажі на 8, 16, 24 годину по 500 мл, а на 32, 40, 48, 56, 64, 72 годину по 250 мл, відтік діалізату відбувався через нижні дренажі. Верхні дренажі видалені на 4 добу, нижні - на б добу. Післяопераційний перебіг без ускладнень. Хворого в задовільному стані виписали на 7 добу.
