黃斑部水腫(macular edema,ME)為糖尿病患者視力喪失的主要原因,而視網膜前新生血管形成(preretinal neovascularization,PDR)為實際失明的主要原因。糖尿病的特徵在於持續的高血糖症,這引起各種器官的微脈管系統中可逆及不可逆的病理變化。因此,糖尿病性視網膜病變(DR)為一種視網膜微血管疾病,表現為一連串的階段,嚴重程度不斷提高,而視力預測也惡化。糖尿病性視網膜病變的主要危險因子包括糖尿病持續的時間、血糖控制,以及全身性高血壓的存在的狀況。糖尿病性視網膜病變(DR)通常按二個主要臨床階段進行分類:非增生性糖尿病性視網膜病變(non-proliferative diabetic retinopathy,NPDR)以及增生性糖尿病性視網膜病變(proliferative diabetic retinopathy, PDR),其中「增生」乙詞係指如上所述之視網膜前新生血管形成。
黃斑部病變,亦稱為老年性黃斑部病變(AMD或ARMD),為一種醫學狀況,其可能導致視野中心模糊或無視力。早期通常沒有任何症狀;然而,隨著時間過去,有些人的視力會逐漸惡化而可能影響一隻或兩隻眼睛。有一些預防方法,包括運動、良好的飲食習慣,以及不吸煙,但卻沒有治癒或治療方法可恢復已經失去的視力。以濕的形式注入眼睛的抗血管內皮生長因子(Vascular endothelial growth factor,VEGF)藥物或較少見的雷射凝結或光動力療法可減緩惡化的情形。補充劑對於已罹患該病的人可能會減緩病情。
目前尚無核准的用於治療老年性黃斑部病變(AMD)、糖尿病性視網膜病變(DR),及/或黃斑部水腫的藥理療法。仍需開發用於治療所述疾病的藥物。
在本發明中意外地發現化合物,例如白殭菌素(beauvericin,在本發明中命名為GYT-088),在治療眼疾方面是有效的。
於一方面,本發明提供一種治療眼疾的方法,該方法包含對一有此需求的個體施用一包含一治療有效量的具有以下結構式的白殭菌素的組合物:。
於另一方面,本發明亦提供一種白殭菌素在製備用於治療眼疾的藥物中之用途。
於另一方面,本發明提供一種用於治療一眼疾的醫藥組合物,包含與一種或多種醫藥上可接受的載體以及一治療有效量的白殭菌素的組合。
於一具體實施例中,該眼疾係由新生血管形成的眼睛退化造成的。
於本發明之一些實施例中,該眼疾為老年性黃斑部病變(AMD)、糖尿病性視網膜病變(DR),或黃斑部水腫(ME)。於本發明之較佳實施例中,該眼疾為非增生性糖尿病性視網膜病變(NPDR)或濕性老年性黃斑部病變(濕性AMD)。

在本發明中,首先發現一些化合物在治療例如老年性黃斑部病變(AMD)、糖尿病性視網膜病變(DR),或黃斑部水腫(ME)等眼疾中具有顯著效果。
本發明提供一種治療眼疾的方法,該方法包含對一有此需求的個體施用一包含一治療有效量的白殭菌素的組合物,該白殭菌素係由三個L

根據常規技術或方法,可以從真菌例如大蟬花(Cordyceps cicadae
)、蟬蕈(Cordyceps sobolifera
)、冬蟲夏草(Cordyceps cicaicola
),以及玫煙色棒束孢(Isaria fumosorosea
)合成或分離本發明之化合物,例如白殭菌素。
於本發明中,該化合物被證實透過抑制VEGF誘導的血管生成而有效治療眼疾。
於一具體實施例中,該眼疾係由新生血管形成的眼睛退化造成的。該眼疾的一些實例包括老年性黃斑部病變(AMD)、糖尿病性視網膜病變(DR),以及黃斑部水腫(ME)。於本發明之較佳實施例中,該眼疾為非增生性糖尿病性視網膜病變(NPDR)或濕性老年性黃斑部病變(濕性AMD)。

本文所用之「醫藥上可接受的載體」乙詞係指在與製劑的其他成分相容且對與醫藥組合物一起施用的個體無害的意義上,可接受的載體、稀釋劑或賦形劑。根據藥物製劑的要求,本領域中通常已知或使用的任何載體、稀釋劑或賦形劑均可用於本發明。
根據本發明,該醫藥組合物可適於透過任何適當的途徑施用,包括但不限於口服、直腸、鼻、局部、陰道,或腸胃外途徑。在本發明之一特定實施例中,將該醫藥組合物配製成用於口服給藥。這樣的製劑可透過藥學領域中已知的任何方法來製備。

實施例
1
體內基質膠栓塞血管生成測試
基質膠栓塞由Engelbreth-Holm-Swarm (EHS)小鼠肉瘤細胞(由Corning Life Sciences公司與BD Biosciences公司生產及銷售)分泌的凝膠狀蛋白混合物製成。基質膠類似於在許多組織中發現的複雜細胞外環境,並且被細胞生物學家用來作為培養測試的基質(基底膜基質)。
試驗動物為5至8週齡的B57l/6小鼠。將基質膠與PBS以4∶1的比例混合以獲得基質膠栓塞。對小鼠施用含有30 IU肝素以及VEGF 150 ng/ml (500 ul/小鼠)的基質膠栓塞,然後分成四組(每組四隻小鼠),以白殭菌素進行腹腔注射或口服(GYT-088組),以索拉非尼(索拉非尼組)作為陽性對照,單獨使用VEGF (載劑組),以及未接受任何治療(對照組)。一天後,對動物以腹腔注射30 mg/kg劑量的索拉非尼,或腹腔注射2.4 mg/kg劑量的白殭菌素(0.1 c.c./小鼠/天),或口服施用24 mg/kg劑量的白殭菌素,每天一次,持續6天。在第7天,犧牲動物並取出基質膠栓塞以觀察血管生成。並以Drabkin氏試劑套組測量每隻小鼠的血紅素。
結果如圖1及圖2中所提供的。如圖1所示,在腹腔注射白殭菌素的組別(GYT-088組)或以索拉非尼(索拉非尼組)作為陽性對照的組別,VEGF誘導的血管生成被抑制。如圖2所示,顯示所測得的血紅素含量,白殭菌素在抑制VEGF誘導的血管生成方面提供了較高的功效。
總結,白殭菌素提供抑制血管生成的功效,甚至優於索拉非尼。
實施例 2
評估不同劑量的白殭菌素對改善恆河獼猴天然發生的糖尿病性黃斑部水腫60天的療效,為臨床試驗劑量設計與安全性研究提供基礎。
於評估中使用恆河獼猴(Rhesus Macaque)。

測量視網膜厚度以量化治療前後的視網膜厚度。
視網膜厚度透過兩種方法使用光學干涉斷層掃描(Optical coherence tomography,OCT)進行評估:(1) 在中央凹處,以及在中央凹的任一側1.5 mm (三點法),以及 (2) 使用52點柵格決定不同厚度分組(250-300、301-350、351-400 µm等)中黃斑部的厚度。這種方法是由Primed團隊與Case Western Reserve University著名的糖尿病性視網膜病變專家Timothy Kern博士合作開發的。請參閱ARVO 2018海報:自發性糖尿病恆河獼猴的黃斑部增厚評估

次要 終 點與安全參數:
(1) 眼底攝影(Fundus photography,FP)。檢測治療前後的出血、滲出液或血管病變的變化;以及
(2) 藥物對新陳代謝、脂肪輪廓、肝腎功能的影響。
非人類靈長類動物(Non-human primates,NHP)在生理功能及眼睛結構方面與人類極為相似。PriMed具有獨特大量的具有自發性代謝疾病,具有5年的歷史臨床/代謝數據的非人類靈長類動物(NHP)群體(n = 約1500),包括空腹血糖(fasting plasma glucose,FPG)、胰島素抗性、身體質量指數(body mass index,BMI)、三酸甘油酯(triglyceride,TG)、總膽固醇(total cholesterol,TC)、低密度脂蛋白膽固醇(low-density lipoprotein cholesterol,LDL-c),以及高密度脂蛋白膽固醇(high-density lipoprotein cholesterol,HDL-c)。彩色多普勒超音波、Kowa眼底照相機、海德堡光學干涉斷層掃描(Heidelberg OCT)、裂隙燈,以及檢眼鏡被用於診斷非人類靈長類動物(NHP)的糖尿病狀態,顯示恆河獼猴的自發性糖尿病性黃斑部水腫(diabetic macular edema,DME)的疾病特徵與臨床患者非常相似,表現出血管滲漏、視網膜增厚、滲出,或出血。患有自發性糖尿病性黃斑部水腫(DME)的恆河獼猴為人類糖尿病性黃斑部水腫(DME)/糖尿病性視網膜病變(DR)研究與新藥開發提供了理想的模型。
入選標準
(1) 雌性,年齡13-18歲(相當於40-60歲的人類成人),體重6-7公斤;
(2) 糖尿病病程:3年以上;
(3) 單眼/雙眼患有糖尿病性黃斑部水腫(DME);
(4) 糖尿病性黃斑部水腫(DME):最近半年由海德堡頻域式光學干涉斷層掃描(Heidelberg Spectralis OCT)測量並透過眼底攝影(FP)/螢光素眼底血管造影(Fluorescein Fundus Angiography,FFA)以Kowa VX-20眼底照相機診斷的視網膜厚度;
(5) 中央凹厚度(Central Foveal Thickness,CRT) ≥ 260 μm及/或早期糖尿病性視網膜病變治療研究(Early Treatment Diabetic Retinopathy Study,ETDRS)區域的平均厚度 ≥ 319 μm。(視網膜增厚由正常獼猴的CRT平均值及標準差(standard deviations,SD)與ETDRS厚度所定義)。視網膜增厚定義為厚度超出正常平均值2個標準差。非糖尿病彌猴:非糖尿病動物視網膜的中央凹平均厚度為192±11 μm,在距離顳側與鼻側中央凹1.5 mm處測得的分別為307±15與328±13。這種方法是由Primed團隊與Case Western Reserve University著名的糖尿病性視網膜病專家Timothy Kern博士合作開發的。請參閱ARVO 2018海報:自發性糖尿病恆河獼猴的黃斑部增厚評估

(6) 有/無血管滲漏或非灌注區域。
排除標準
(1) 第三期及以上高血壓;
(2) 進行性眼或全身發炎或感染;眼內高壓:眼壓(Intraocular pressure,IOP) > 21 mmHg;
(3) 實驗室檢查顯示腎臟或肝臟功能明顯受損;
(4) 任何其他可能影響療效的病史。
群組大小
將提供6隻自發性糖尿病性黃斑部水腫(DME)恆河獼猴(一年內有病歷),且在研究前期(4週)在治療前將該6隻猴子中的4隻進行眼底血管造影(FA)、眼底攝影(FP)、光學干涉斷層掃描(OCT),以及裂隙燈檢查,並進行血液生化檢查。光學干涉斷層掃描(OCT)會根據視網膜厚度將這4隻猴子分為兩組:
(1) 第一組,載劑組,N=1;
(2) 第2組,試驗組,N=3;
(給藥途徑:口服,每天一次;劑量:總基礎劑量(total basal dose,TBD))。
結果測量:
主要功效參數:
視網膜厚度包括中心視網膜厚度(central subfield thickness,CST)以及ETDRS厚度。給藥前與給藥後第14天(D14)、第30天(D30),以及第60天(D60)各一次; 根據D14與D30的結果確定是否在第45天(D45)增加檢測。
次要功效參數與安全性參數:
(1) 眼底照相:以給藥前為基準線,給藥後第30天(D30)、第60天(D60)各一次,共3次;
(2) 螢光素眼底血管造影(FFA):以給藥前一次為基準線;
(3) 眼壓與血壓測量:給藥前與給藥後各一次,共2次;
(4) 透過每日籠側觀察評估眼睛的臨床徵狀;
(5) 代謝參數、肝腎功能的測定:FPG、FRA、LDL、HDL、TC、TG:給藥前與給藥後第14天(D14)、第30天(D30)、第45天(D45),以及第60天(D60)各一次;
(6) 裂隙燈:給藥前1次,給藥後1次,共2次;
(7) 體重:給藥前與給藥後第30天(D30)、第60天(D60)各一次;
(8) 食物攝取/臨床觀察:包括異常徵狀、症狀,以及行為,每天一次。
眼底攝影 (FP) 與螢光素眼底血管造影 (FFA)
(1) 頻率:給藥前進行螢光素眼底血管造影(FFA)一次。
(2) 儀器:KOWA VX-20。
(3) 目的:檢查視網膜血管的損傷及滲漏。
(4) FFA影像採集程序:
在獲取影像之前,透過肌內注射氯胺酮:甲苯噻嗪混合物(1:1,8 mg/kg氯胺酮)麻醉動物。麻醉後,向每隻眼睛施用2滴托品醯胺(Tropicamide)苯腎上腺素滴眼液,以擴大瞳孔。麻醉後,將動物放在黑暗的房間裡,直到瞳孔直徑大於6毫米。將自保留眼瞼光譜置於眼睛中。攝影主眼的眼底後極聚焦良好。
(5) 當透過股靜脈以0.075 mL/kg的劑量注射10%螢光素鈉(Alcon Laboratories公司,美國)時開始計時。在最初的30秒中每秒對主眼拍攝一系列後極照片,在接下來的30秒中每2-3秒拍攝一次。在5分鐘與10-15分鐘為兩隻眼睛拍照。
結果
載劑組:#6189在給藥前顯示輕度非增生性糖尿病性視網膜病變(NPDR)。如表1所示,以眼底攝影(FP)分析給藥前後沒有發現明顯變化。GYT-088組:#251,#2003,以及#3536顯示出輕度或中度非增生性糖尿病性視網膜病變(NPDR)。以眼底攝影(FP)分析給藥後第28天(D28)改善了#2003中非增生性糖尿病性視網膜病變(NPDR)的特性。以眼底攝影(FP)分析在任何時間點給其他動物給藥前後均未見明顯變化,請參閱表1。

結果提供於圖4中。在給藥後第14天,在#2003實驗動物(NPDR組)中發現,中央黃斑部厚度(central macular thickness,CMT)從左眼與右眼的204 µm與210 µm變為197 µm與202 µm,分別減小了7 µm與8 µm;中央凹的厚度從左眼與右眼的257 µm與260 µm變為249 µm與251 µm,分別減小了8 µm與9 µm。另一方面,發現在施用載劑的動物#6189 (載劑組)中,與正常動物中正常發現的一樣,中央黃斑部厚度或中央凹厚度的變化在±2 μm的範圍內。在給藥後第56天,中央黃斑部厚度減少了6 μm與8 μm,左眼與右眼黃斑部厚度分別減少了3 μm與5 μm。因此,可以得出結論,GYT-088在糖尿病性黃斑部水腫中提供了功效。
