Description

連接至少二骨骼固定元件用之桿狀植入元件及骨骼穩定裝置
本發明係關於脊柱手術和/或外傷手術上應用之桿狀植入元件,其中桿狀植入元件包括硬性段和相鄰之撓性段,延伸貫穿二段之腔孔,容納於腔孔內之芯管,其中芯管至少一端可在腔孔內自由運動。
本發明又關係到使用如此桿狀植入元件之穩定裝置。
遇到椎間損壞時,要穩定脊椎,迄今有若干措施,使相鄰椎骨間有某種程度的運動。
EP 0 669 109 B1揭示一種穩定裝置,可供穩定相鄰的胸椎,藉此可使受傷的椎間盤和椎間官節部份消除應力。此裝置包括二梗節釘,和利用夾釘固定於各梗節釘接納構件內之條帶,又含有支持元件,安裝在條帶上,設計成抗壓體。然而,此項穩定裝置的缺點在於扭力方面僵硬,不會產生脊椎運動節段的任何引導穩定性。
US 6,162,223號揭示例如手腕或膝蓋關節用之關節固定裝置。在一具體例中,此裝置包括撓性聯結器,包含長形彈簧線圈,各端有二夾具,以及撓性電纜或撓性桿,延伸貫穿撓性聯結器,並固定於夾具。此已知關節固定裝置由於其複雜和體積龐大結構,不適用於脊柱。
US 2003/0220643 A1揭示可伸縮元件,在裝置內用於防止在上、下脊椎體間完全延伸,可伸縮元件可例如為彈性索或彈簧。
US 2003/0191470 A1揭示一種植入裝置,有桿部,患者彎椎時,其中心段跟著屈曲。撓性段可為直線桿,有變化截面形狀,或非直線桿,有一部份以U形彎向軸線一側。
動態穩定裝置特別需用來處理只有部份退化的椎間盤。椎間盤應釋除應力,以支持其復原。若椎間盤的核仍然完好無損,則椎間盤在椎骨軸向仍然堅強(堅度約500至1500N/mm)。
參見第1至5圖,顯示治療退化椎間盤用的已知椎骨穩定裝置。二梗節釘2,3固定在鄰接於核41仍然堅實的退化椎間盤40之脊椎骨20,30。梗節釘2,3經桿狀植入元件100連接,植入元件100具有螺旋彈性段101和二硬性段102,103,用來與梗節釘連接。第1圖表示中性位置,其中梗節釘相隔距離為x,若患彎椎,此距離會變。以第2圖的彎曲或第3圖的伸張而言,核分別按朝後或朝前方向運動,造成梗節釘2,3間的距離,與中性位置時的距離x相較,增加約1.5mm,或減少約0.5mm。
因此,為使桿狀植入元件之撓性段可視需要伸縮,必須在縱軸線104方向有低度堅硬和高度可撓性。然而,若撓性段101在軸向的堅性低,亦有低度堅硬對抗彎曲和剪力下,如第5圖所示,若發生此力即會彎曲。在使用金屬彈簧,以及使用諸如彈性材料等其他塑膠材料製成的撓性段情況時,即會引起此問題。
DE 103 48 329 C1揭示一種長形桿狀植入元件,包括要連接於骨骼固定元件之第一硬性段,要連接於第二骨骼固定元件之第二硬性段,以及在二硬性段間延伸並與之形成一體之撓性段。桿狀植入元件包括延伸貫穿元件之共軸腔孔,和容納於腔孔內之芯管,芯管固定,故不在腔孔內滑動。
WO 2004/105577號揭示一種脊骨穩定系統,具有一或以上之撓性元件,壁上有開口或細孔。撓性段由第一和第二桿組成,組合成第二桿套入第一桿的縱向腔孔內,例如以加壓套合方式為之。
然而,此等撓性桿狀植入元件不能解決的問題是,桿狀植入元件在軸向充分撓性時,會遭到彎曲或扭曲。
所以,本發明之目的,在於提供一種桿狀植入元件,以及使用此等桿狀植入元件的骨骼或椎骨穩定裝置,在軸向具有堅性使骨骼固定元件彼此在軸向相對運動,但同時防止或減少在軸向之橫向彎曲,桿狀植入元件應容易製作、調節和使用。
此目的是以申請專利範圍第1項之桿狀植入元件,以及申請專利範圍第12項之穩定裝置解決。進一步發展則列在申請專利範圍附屬項內。
桿狀植入元件和穩定裝置的優點是,容許軸向壓縮和膨脹,使骨骼固定元件可彼此相對運動,同時防止彎曲和扭曲。特別有益於治療部份退化的椎骨節段。
本發明其他優點由附圖所示具體例之說明,即可明白。
由第6-13圖可見,本發明穩定裝置包括桿狀植入元件1和二梗節釘2,3,彼此利用桿狀植入元件1連接。
本發明桿狀植入元件1在第一具體例內,包括第一硬性段4,從其第一端5延伸跨越預定長度;和第二硬性段6,從其第二端7延伸跨越預定長度;以及在硬性段4,6間預定長度之撓性段8。在圖示具體例中,所有節段的外徑相同。預定直徑的共軸腔孔9延伸貫穿桿狀植入元件。撓性段8設計有螺旋長孔型開口10,從表面徑向延伸至共軸腔孔9,因而形成預定節距之匝圈11。螺旋形開口可有筆直或斜縮側面。撓性段8在桿狀植入元件的縱軸線A方向之匝圈11高度,在徑向界定匝圈11高度之共軸腔孔9直徑,以及節距,經選擇企求獲得對作用於桿狀植入元件1的軸向伸縮力有所需堅性。凡此專家對此等維度容易改變,以達成所需堅性,不需過當實驗。
桿狀元件1由生理相容性材料製成,諸如鈦、不銹鋼或鎳鈦合金(nitinol)。
特別由第7圖可見,桿狀植入元件1又包括芯管12,呈實質上圓柱形,長度較桿狀植入元件為短,直徑較共軸腔孔9直徑小,使芯管12可在腔孔9內自由運動和軸向滑動。芯管12長度至少等於撓性段8之長度。在圖示具體例中,芯管12延伸貫穿撓性段8和硬性段4,6之一部份。選擇長度使得撓性段8受到壓縮時,芯管12不會碰到硬性段的末端。芯管12由具有可抵抗彎曲力量的堅性之材料製成,其堅性較桿狀元件的撓性段8堅性大。芯管12以堅性較撓性段8為大的生體相容性聚合物或金屬物製成為佳。芯管12是利用桿狀植入元件各端5,7所設二封閉構件13,14,加以固設在腔孔9內。封閉構件13,14形成封閉腔孔9,例如利用螺合或壓套連接,固定於硬性段。
在第8圖所示較佳具體例中,芯管12塗佈塗料15,其材料使芯管可相對於腔孔9內壁以低摩擦力滑動。塗料例如由聚乙烯(PE)、鐵弗龍或減少摩擦之其他材料製成。塗佈材料可用例如UHM WPE型高分子量聚乙烯,其分子量介於2×106 至10×106 之間。選擇塗料15厚度,使在連續使用桿狀元件之際,不會完全磨損。
如第9和10圖所示,桿狀植入元件1的撓性段8長度的選擇是,在梗節釘於中性位置固定在相鄰椎骨內時,基本上相當於梗節釘2,3間之距離。穩定裝置之各梗節釘包括固定部21,21’,有骨骼螺紋,可供梗節釘固定於骨骼內,並有基本上圓筒形接納構件22,22’,可供接納桿狀植入元件1。在第9和10圖中,第一梗節釘2包括接納構件22,有U形凹部23,在接納構件於固定段對立之頂端開口,以便從頂部引進桿狀植入元件之硬性段4。梗節釘3的接納構件22’有二開口24取代U形凹部,供桿狀植入元件的另一硬性段6插入。桿狀植入元件1藉從頂端螺入接納構件內之內螺釘25,固定在接納構件22,22’內。
特別由第10圖可見,梗節釘2,3設計成多軸釘,釘頭26在接納構件22,22’內樞動,得以調節接納構件相對於固定段之角位。於內螺釘25旋緊時,釘頭26經由壓力構件27施壓於釘頭,而鎖定於位置。
在第9和10圖所示穩定裝置例中,硬性段4,6的長度宜大約相當於把桿狀植入元件固定於接納構件22,22’內的固定元件25直徑。如第10圖所示,第二硬性段6的直徑可縮小,得以容易插入接納構件之開口24內。在此情況下,供軸腔孔9不會延伸到第二端6,故第二硬性段7仍然足夠穩定到利用內螺釘25夾持在接納構件22’內。
使用時,先將梗節釘2,3固定在相鄰椎骨內。然後,把桿狀植入元件1插入接納構件22,22’內,藉旋緊內螺釘25加以固定。桿狀植入元件之撓性釘8在軸向具有堅性,可相對於中立形狀伸出約1.5 mm,和收縮約0.5 mm。脊椎在屈伸之際,彎曲內作用於桿狀元件,造成撓性段彎曲。然而,由於芯管12較桿狀植入元件的撓性段8為堅硬,芯管12會抵抗彎曲,而整個桿狀植入元件不會彎曲。所以,桿狀植入元件提供引導穩定性。
芯管12必須在腔孔9內至少一端自由運動,容許桿狀植入元件伸縮。
第11至13圖表示桿狀植入元件的第二具體例。第二具體例的桿狀植入元件50與第一具體例的桿狀植入元件1不同,在於芯管12一端固定於硬性段。與第一具體例一致的組件賦予同樣參考數字,不再贅述。圖示具體例內的硬性段4,6之一,即第二硬性段6包括腔孔51,按縱軸線A的橫越方向延伸。腔孔51位於離第二端7一段距離處。芯管12在一端包括加厚段53,其直徑比腔孔9稍小,使加厚段53仍可引進腔孔9內。加厚段53在離芯管自由端一段距離處,設有橫向腔孔54。芯管12在插入桿狀植入元件內時,利用插入腔孔51和54內之銷55,固設於腔孔9內。在此情況下,不必封閉腔孔9,因為芯管12不會滑出腔孔9外。
芯管12長度使其延伸至少全部穿過撓性段8。
在此具體例中,芯管12亦可加塗,以方便在腔孔內滑動。
桿狀植入元件之使用,與第一具體例相同。
第14和15圖表示本發明桿狀植入元件的第三具體例。第三具體例的桿狀植入元件60與第一和第二具體例的桿狀植入元件不同,在於撓性段之設計。桿狀植入元件60有鄰接第一端62之第一硬性部61,鄰接第二端64之第二硬性部63,以及介於第一硬性部61和第二硬性部63間之撓性段65。第一硬性部61和第二硬性部63具有縱向共軸腔孔66。撓性段65亦有共軸腔孔67,故呈實質上管狀。撓性段65自由端分別連接於第一和第二硬性部之自由端。撓性段65外徑從兩端的第一直徑減到中央部的較小第二直徑,以方便在壓縮和伸張。硬性段由人體相容性堅硬材料製成,例如鈦,一如第一具體例。撓性段由人體相容材料製成。此種彈性材料有例如聚胺酯或聚矽氧烷類。撓性段例如利用硫化而固定於硬性段。
和第一具體例一樣,芯管12設在腔孔67內,其長度可在腔孔9內自由滑動。硬性段的自由端62,64利用封閉構件68,69加以封閉,以防芯管滑出腔孔外。
第三具體例桿狀植入元件之使用,與第一和第二具體例相同。第一和第二具體例的螺旋形彈簧伸縮,改為撓性部65的彈性材料在伸縮。
第16和17圖表示第四具體例,為第三具體例之改變。桿狀植入元件70與第14和15圖所示第三具體例之桿狀植入元件60不同,只在撓性段71之形狀。撓性段71的直徑從撓性段末端的第一直徑,變化成中央部之第二直徑,較第一直徑為大。此項桿狀植入元件較第三具體例為堅硬,故特別適用於只有小小伸縮。使用上與前述諸具體例相同。
可以改變,尤其是不同具體例的元件,可以彼此組合。上述組件之組合實屬可行,該項組合是特別設想。
在變化例中,芯管可額外設有內部強化組件,作為芯管中的芯管。例如生理相容性聚合物製之芯管,可設有金屬製內部強化組件,以增進芯管堅性。內部強化組件可僅設在芯管的一部份,例如其中央部。
桿狀植入元件可有若干硬性和撓性段。撓性段的直徑可與硬性段相同。芯管可有若干段,可視需要縮小直徑,以降低硬性。芯管亦可空心。再者,芯管不需為圓柱形,亦可例如為長方形截面,以具有定向彎曲堅性。芯管亦可經特殊表面處理,諸如磨光,以減少摩擦。若芯管一端固定於硬性段,則可利用任何機構固定,包含例如螺合或膠合方式。
穩定裝置可有其他骨骼固定機構,例如單軸釘或鉤。

Claims (8)

  1. 一種連接至少二骨骼固定元件(2,3)用之桿狀植入元件,各包括要固定於骨骼之固定段(21,21’)和要連接於桿狀植入元件之接納構件(22,22’),其中桿狀植入元件包括:縱軸線(A);第一硬性段(4;61)之至少一第一長度,構成與接納構件(22,22’)合作,並接納於其內;鄰接該第一硬性段之撓性段(8;65;71)之第二長度,該第二長度為撓性段(8;65;71)中立形狀之預定長度;第二硬性段(6;63)之第三長度,其中撓性段(8;65;71)有二相反端,第一硬性段(4;61)之第一長度鄰接於第一端,而第二硬性段(6;63)之第三長度鄰於第二端;腔孔(9;66;67)沿縱軸線延伸,貫穿第一和第二硬性段以及撓性段芯管(12),容納於腔孔內,芯管有二相反端;其中當撓性段按縱軸線方向,呈伸出形狀或收縮形狀時,全芯管可在腔孔內,按縱軸線方向自由運動,伸出形狀和收縮形狀係相對於中立形狀而言;其中芯管係由不鏽鋼,鈦,鎳鈦合金(nitinol)或堅硬塑膠材料製成;而芯管(12)表面塗佈利於滑動之材料者。
