【發明所屬技領域;J 發明領域 本發明係有關一種清潔裝置及方法,特別係有關清潔 5 受污染的手術器材外部之方法及裝置。已經發展出清潔裝 置供清潔剛性内視鏡或可撓性内視鏡外部,主要將參照該 項用途作說明。 I:先前技術3 發明背景 10 内視鏡是一種細長管狀器材,内視鏡可為剛性或可撓 性,内視鏡結合光學系統或視訊系統及光源。典型地,内 視鏡係組配成其一端經由切開或孔口而插入手術切開或體 腔内部的某種深度’讓外部觀察者可觀視位在内視鏡或接 近内視鏡内部插入端表面。常用内視鏡是一種稱作為結腸 15 鏡之可撓性内視鏡,用於人類腸診斷過程》結腸鏡之結腸 插入部長約二米’有一或多個中空通道或管腔。 由於内視鏡成本高,且需防止不同病人間的交叉感染 ,内視鏡於每次使用後須徹底清潔與消毒或滅菌。此種程 序耗時且構成重大困難’原因在於内視鏡顯然受到來自病 2〇 人的生物物質污染,例如受到黏液、糞便、血液、組織小 塊等生物物質的污染。 典型清潔裝置涉及於流動水下方清洗及擦栻内視鏡外 部經歷一段時間來去除巨觀污染物,隨後將内視鏡浸泡於 適當清潔浴槽内2至10分鐘,清洗器材,乾燥器材,然後消 5 毒器材。某些情況下’器材外部的巨觀污染物於消毒前 ’藉清洗以及隨後藉清潔浴槽的去除不充分。此種外部清 潔不足’將危害消毒步驟或滅菌步驟,無法以再現性方式 消毒或清潔污穢的器材。 由於清潔與消毒或滅菌所需時間經常比内視鏡使用的 醫藥程序的時間更長’因此須耗費成本投資於多件器材, 或耗費成本於清潔,變成醫藥程序進行速率的決定性因 素°此外’耗用相當大量時間及材料在清潔刷子以及其它 用來清潔内視鏡包括清潔内視鏡管腔的裝置上。晚近注意 力集中在改良管腔内部清潔方法(例如參考發明人之共同 審查中之專利申請案PCT/AU99/00669),但該方法不適用於 外部表面。外部的清潔仍然是有關重要性、困難且勞力密 集的工作。多種複雜的化學產品及生化產品例如酶/清潔劑 組成物已經發展用作為前置浸泡’來將污染物由外表面鬆 脫,以及辅助污染物的隨後藉擦洗去除。此項處理的最終 效率大為依據下列因素決定:(a)進行清洗及擦洗之勤奮程 度如何;(b)檢查後表面殘留多少碎屑,以及此等碎屑的本 質;以及(c)被玷污的器材於清潔前是否乾燥,若經乾燥 則進行乾燥之溫度為何。此外,本案申請人發現使用刷 子清洗與擦洗,經常只單純造成沉積物於器材表面的重排 ,而非去除沉積物。此外,使用刷子可能形成污染的氣溶 膠’危害附近醫藥照護工作人員的健康。 全文說明書有關先前技術討論絕非視為承認此種先前 技術為眾所周知或構成業界普通常識的一部分。本發明之 45% w/w。也健若_子性界面活性齡在於組成物’
則其存在量為5至15%W/W。較佳’組成物之界面活性劑之 總含量為15至45% w/w。 W 希望該組成物適合接觸醫藥器材管狀部分周面的實質 弧度’且適合沿器材管狀部分之長度於軸向方向滑動,、俾 擦拭表心較佳該裝置接合器材管形部分外周面高達細度 弧度,且於軸向方向可彈性變形。更佳,該裝置係由親水 纖維製造。 根據本發明之裝置之較佳具體實施例可藉機械擦拭作 用去除大部分附著於外部之污物,但更要緊地,清潔裝置 係用來重新分g&任何親之外部污物,讓未被去除的污染 可分佈成又薄又均勻之薄膜。該薄膜適合藉酶消化而更有 效且更迅速達成污物的去除。 根據本發明之第二方面,提供一種用於清潔需要清潔 之管狀内視鏡外表面之裝置,該裝置包括—親水纖維襯勢 ,親水纖維有切槽由襯墊—端至襯墊相對端,且適合弹性 接合内視鏡部分外表面,該襯墊適合用於單獨或結合互補 襯墊來實質包圍該部分,且該襯墊可彈性變形,因而接合 被包圍部分表面,藉此當清潔裝置係沿内視鏡之管縱向方 向滑動時,該襯墊可均勻擦栻管狀内視鏡外表面。 較佳襯墊係由一種針刺氈製成,襯墊有二彼此隔開之 平行切槽,各切槽具有拱形截面,可對摺而校準管軸對側 來形成管狀隧道’其彈性接合内視鏡外部套住内視鏡周面。 較佳具體實施例中,根據第二方面之清潔裝置係由朴 1363639 根據本發明之裝置之另一項優點為酶作用始於刷洗期 ,換言之酶作用於清潔過程比過去更早期,因此延長整體 酶處理時間,又轉而提供清潔效率。 根據本發明之第三方面,提供一種清潔手術器材外表 5 面之方法,包括下列步驟(1)擦拭該外表面,同時(2)讓該表 面接受酶及界面活性劑處理。 圖式簡單說明 第1圖為示意圖,以平面圖顯示根據本發明之裝置之第 一具體實施例。 10 第2圖為示意圖,以剖面圖顯示第1圖之具體實施例。 第3圖為示意圖,以剖面圖顯示第1及2圖之裝置包裹套 住内視鏡管周邊。 【實施方式3 較佳實施例之詳細說明 15 現在僅參照附圖說明較佳具體實施例。1及2圖舉例說 明之具體實施例提供一種呈襯墊1形式之清潔裝置,襯墊1 係由非織針刺氈聚酯或黏膠纖維織物1於一表面黏合至織 造背襯所組成,織造背襯可由聚酯纖維或聚丙烯纖維3製 成。襯墊1實質上為矩形形狀,具有寬度「w」約133微米, 20 長度「1」約9厘米,最後纖度「t」厚約15毫米。襯墊之周 邊經過熱熔封,以及具有兩個約略成半圓形截面之拱形切 槽4,5,切槽4,5係藉加熱成形製成,且於縱向方向延伸, 取中於距離一緣6約1/4「w」以及3/4「w」之間隔。切槽各 自具有半徑約7毫米。此外有一摺疊切槽8於中線之縱向延 10 1363639 伸,換言之距離側緣6約1/2「w」距離。 使用時’内視鏡管被加壓入切槽4内部,讓襯墊接觸内 視鏡截面的半圓。然後如第3圖示意顯示,襯墊反摺,讓切 槽5接觸其餘截面。第3圖顯示第1及2圖之襯墊沿切槽8摺 5 疊’讓切槽4,5校準’而形成圓柱形隧道套住内視鏡管9, 本例中,内視鏡管9之外徑約為14毫米。聚酯或黏膠纖維針 刺氈結構具有某種程度的彈性,襯墊可維持套住内視鏡 管,壓迫襯墊,施加彈力朝向内視鏡管軸之徑向方向加壓。 如此襯墊壓迫内視鏡管外表面’清潔裝置沿縱向方向滑動 10 來以機械方式去除巨觀污物,以及同時,襯墊將任何殘餘 物重新均勻分配於表面’換言之實質以均勻厚度環繞周面 且順著長度分佈。須了解襯墊及切槽之尺寸可改變來適合 不同内視鏡,但本設計之優點為襯墊彈性可配合某個範圍 之内視鏡直徑。 15 適當襯墊例如係由兩種不同纖維組成,其中第一纖維 為3丹尼及長51毫米之聚酯纖維或黏膠纖維,以及第二纖維 為2丹尼長51毫米之聚丙烯纖維。兩種纖維之比為70%聚酯 纖維或黏膠纖維比30%聚丙烯。兩種纖維混合至均質,然 後藉針刺技術糾結’直到形成實質上不含飛離纖維之低密 20 度料片形成為止。 料片厚度係藉使用之纖維量及針刺裝置控制,但證實5 毫米至15毫米之料片用於該用途為理想。乾料片浸潰以吸 濕性酶清潔調配物,酶清潔調配物含有一或多種酶,一或 多種界面活性劑,酶穩定系統,且含有與使用之酶可相容 11 丄观639 定化學性質而異。但熟諳技藝人士熟悉重新調製具有不同 活性之酶,可根據特殊情況調整配合本發明之調配物。 31施例2 實施例2係與實施例i相同,但以* 5% w/w甘油取代氣 5 化每六水合物。 界面活性劑、保藏劑及酶之組成及用量可根據調配需 、、及相合& 4求改<。重要地,组成物含有濕潤劑。適當 濕潤劑例如為氯化妈、氯化鈉、甘油、删砂、乙二醇等。 减潤劑也可為界面活性劑。 1〇。-或多個襯塾可制封裝於—包裝内,該包裝可透水 。如此連同濕·之存在时轉組成_濕以防當包 裝開啟時酶之乾粒子釋放入空氣。選擇性地,調配物可含 有可與酶相容之消毒劑例如第四録化合物。 15 ^組成物於織物、擦巾或海綿上的負載量可視需要而改 5變°組成物可以任-種比例施用,由占織物、擦巾或海綿 之1至5。/。重量比,直至完全飽和襯墊或擦巾之數量。 使用時’根據本發明之清潔裝置由包裝中取出,可進 —步以水打濕,包裹套住内視鏡的一端,順著内視鏡長度 2〇方向擦栻來去除污物。然後襯墊以適當方式拋棄。酶即刻 開始進行消化分解作用。現在已經不含大部分附著污物之 内視鏡,進—步於適當清潔溶液内清潔,然後消毒或滅菌 因任何殘餘物現在已經分佈成為有均勻厚度之薄膜,因 匕隨後於浴槽内之處理比較内視鏡外部單純以刷子擦洗, 可於有效更短時間内完成處理。 13 1363639 雖然前文討論之具體實施例係採用黏膠纖維襯墊,但 涵蓋襯墊由聚合物泡沫體或適當織物紙或混成結構製成。 由於希望醫療器材維持濕潤,因此裝置不可太具有吸收性。 高度較佳之具體實施例係使用非織織物「擦巾」卷。 5 擦巾卷例如長8米,寬10厘米,以10厘米間隔穿孔,因此由 擦巾卷的自由端共可撕成80條10厘米χΙΟ厘米的擦巾。另 外,產物可呈冠狀含200條8米長度之擦巾卷供應。擦巾使 用之非織織物係由纖維束纖維(例如黏膠纖維)料片製成。然 後料片使用可撓性交聯丙烯酸系乳膠水性分散液處理。水 10 性分散液包含至多15%重量比且較佳約8%重量比乾丙稀酸 系聚合物。當添加適當比例交聯催化劑時,非織結構被乳 膠/催化劑分散液所飽和,過量分散液藉重力而由結構滴 乾,否則利用壓縮輥的辅助擠壓去除過量分散液,致使結 構於適合誘生交聯之溫度乾燥。當冷卻時獲得經黏合之非 15 織開放結構料片,具有如下規格: 基重(克/平方米):約42.5米 乾強度:(克/25毫米):225 厚度(微米/4層):1270 吸收性(克/5克):420 20 擦巾卷容納於配送罐内,配送罐較佳係由塑膠製成, 罐直徑僅略大於卷直徑,且具有可復位的封閉件來密封容 器。於可復位封閉件下方有罐蓋或罐壁設置有一或多個開 槽以卷軸為中心,通過該開槽可配送擦巾卷末端。如實施 例1或實施例2所示之調配物,且含有酶及濕潤劑以足量添 14
