Цель изобретени - исключение возможности вторичного смещени отломков в процессе дистракции путем регулировани рассто ни между спицами . Устройство состоит из опорной 1 и прижимной 2 пластин с пазами 3 дл спиц 4. Между пластинами располо жены шайбы 5, число которых зависит от длины отломков. Св зь спицедержателей каждого отломка осуществл ют двум резьбовыми стержн ми 6, проход щими через дополнительные отверсти 7 в опорных пластинах. Пазы дл спиц проход т под углом к другу по касательной к отверсти м с.болтами 8, вл ющимис направл ющими дл прижимной пластины. Закрепление спиц ос5гществл ют при завинчивании болтов. Это возможно благодар тому, что отверсти в прижимной пластине содержат резьбу, а в от версти х опорной дластины резьба отсутствует. При навинчивании гаек 9 на резьбовых стержн х, соедин ющих спицедержатели, осуществл ют их перемещение вдоль продольной оси. Устройство работает следующим образом. В соответствии с рентгенологически ми размерами отломков кости и их диа метром определ ют место предполагаемого проведени спиц. При этом важно путем подбора соответствующего числа шайб 5 располозЕить места проведени спиц 4 вдоль продольной оси так, чтобы обеспечить оптимальные биомеха нические услови дл стабильности остеосинтеза. Так, на коротком фрагменте спицы располагают ближе друг к другу, а на более длинном необхоДЮ4О , внос дополнительное число шайб 5, расположить у концов отломка . Смонтированный таким образом аппарат устанавливают на операционное поле, спицами 4 прокалыйают м гкие ткани до упора в поверхность кости. Убедившись в том, что остри спиц наход тс на кости, поочередно провод т их через оба кортикальных сло . При зат гивании болтов 8 спицы фиксируют в спицедержател х. Обычным способом производ т остеотомию, при расконтрагаенных гайках 9, убежда сь при этом в завершенности остеотомии . После этого на 6-7-й день приступают к дистракции отломков, подкручива перекрестно расположенные гайки. Использование дл дистракции гаек, расположенных у одной опорной пластины, нежелательно, так как из-за ее незначительной толщины возможно по вление подвижности в поперечном направлении по отношению к продольной оси кости. Дистракцию осуществл ют до достижени необходимой длины культи. Устройство имеет минимальные габариты и состоит из малого числа деталей . Конструкци его позвол ет повысить жесткость фиксации отломков в зависимости от длины отломков, уменьша или увеличива рассто ние межлу спицами, образующими перекрест, и избрать оптимальный уровень остеотомии . Кроме того, по вл етс возможность избежать остеотомию в диафизарных отделах кости и осуществл ть ее ближе к метафизарной части, например, при удлинении 1-й п стной кости. Отсутствие над костными фрагментами перекрывающего зону дистракции резьбового стержн обеспечивает оптимальные услови дл рентгеноконтрол состо ни репаративного костеобразовани удлин емых фрагментов п стных костей, особенно в пр мой проекции, когда четкость изображени в боковой проекции снижена из-за наслоени соседних костей.The invention relates to medical technology, namely to devices for the treatment of fractures and orthopedic diseases of the musculoskeletal system. The device consists of a support 1 and a clamping 2 plates with grooves 3 for the spokes 4. There are washers 5 between the plates, the number of which depends on the length of the fragments. The spit holders of each fragment are connected by two threaded rods 6, passing through additional holes 7 in the support plates. The grooves for the spokes are angled to each other along a tangent to the holes with bolts 8, which are guides to the pressure plate. The fastening of the spokes is done by tightening the bolts. This is possible due to the fact that the holes in the pressure plate contain a thread, and there is no thread in from the top of the base plate. When the nuts 9 are screwed on the threaded rods connecting the clip holders, they move along the longitudinal axis. The device works as follows. In accordance with the radiological size of the bone fragments and their diameter, they determine the location of the intended holding of the spokes. It is important, by selecting the appropriate number of washers 5, to place the locations of the spokes 4 along the longitudinal axis so as to ensure optimal biomechanical conditions for the stability of the osteosynthesis. So, on a short fragment, the spokes are positioned closer to each other, and on a longer one, it is necessary to add an additional number of washers 5, to position the fragments at the ends. The apparatus mounted in this way is installed on the operative field, with the spokes 4 soft tissues are punctured all the way to the bone surface. Making sure that the tips of the spokes are on the bone, they are alternately carried through both cortical layers. When tightening the bolts 8, the spokes are fixed in the spokes. In the usual way, an osteotomy is performed with the nuts released to the 9, while convincing that the osteotomy is complete. After that, on the 6th-7th day, the fragments are started to distraction, tightening the cross-arranged nuts. The use of nuts located on one support plate for distraction is undesirable, since, due to its small thickness, mobility in the transverse direction with respect to the longitudinal axis of the bone is possible. Distraction is carried out until the required length of the stump is reached. The device has minimum dimensions and consists of a small number of parts. Its construction allows one to increase the stiffness of fixation of fragments depending on the length of fragments, reducing or increasing the distance between the spokes that make up the crossing, and select the optimal level of osteotomy. In addition, it is possible to avoid osteotomy in the diaphyseal parts of the bone and to carry it closer to the metaphyseal part, for example, when lengthening the 1st puncture bone. The absence of a threaded rod overlapping the distraction zone over bone fragments provides optimal conditions for X-ray monitoring of the condition of reparative bone formation of elongated fragments of bones, especially in direct projection, when the clarity of the image in the lateral projection is reduced due to the layering of adjacent bones.
