HPV-kapsiden består av 72 kapsomerer som var och en innehåller S kopior av HPV-huvudkapsidproteinet L1. Ett mindre kapsidprotein, L2, föreligger i mycket mindre mängder i kapsiden (l:l2 jämfört med Ll-proteinet) och lokaliseringen av L2 är osäker (2).
Ett antal små virus uttrycker kapsidproteiner som när de uttrycks samlar sig själva för bildning av virus- liknande partiklar (VLP) (d v s partiklar som morfo- logiskt liknar viruspartiklar men saknar det virala genomet). HPV-huvudkapsidproteinet Ll är bland de bäst studerade (2). HPV-VLP som endast innehåller Ll liknar morfologiskt VLP som innehåller både Ll och L2 (2). Både partiklar med enbart Ll och partiklar med Ll/L2 är hög~ effektiva när det gäller framkallande av ett högtitrat neutraliserande antikroppssvar i flera djurmodellsystem (kaniner, kor, hundar och rhesusapor) även när de inji- ceras i frånvaro av adjuvans (2).
Vaccination med papillomavirus-VLP har visat sig vara högeffektiv för skydd, medierat av neutraliserande antikroppar, mot efterföljande smitta med både kutana och mukosala papillomavirus men endast på ett typspecifikt sätt (2). Denna starka typspecificitet är överraskande, eftersom huvudkapsidproteinet i HPV är ett evolutionärt högkonserverat protein med mycket få aminosyraföränd- ringar mellan genetiskt besläktade men inte korsneutrali- serande HPV-typer.
De vanligast onkogena HPV är HPV16, 18, 31 och 45.
HPV16 återfinnes i ungefär 50% av cervicalcancerfallen, HPVl8 ca 20%, och dessa fyra typer tillsammans motsvarar >80% av alla cervicalcancerformer. Därför är en vanlig övervägd strategi att tillverka vacciner innehållande HPV-kapsider av de fyra vanligast HPV-typerna tillsammans (2).
Trots att denna strategi sannolikt verkar fungera för uppnående av signifikant reduktion av cancer, har den lO 15 20 25 30 35 514 982 3 vissa distinkta nackdelar. Beredningen av vacciner som innehåller fyra aktiva komponenter blandade tillsammans inbegriper en väsentlig ytterligare kostnad vid tillverk- ning och effektivtetstestning och kvalitetskontroll av varje komponent.
Vidare förorsakas ungefär 10-20% av cervicalcancer- formerna av HPV-typer som inte inbegripes i de för när- varande tillverkade vaccinkandidaterna. Bortsett från det faktum att vaccinet omöjligt kan skydda mot dessa typer, föreligger även möjligheten att eliminering av de fyra vanligaste onkogena HPV-typerna kan förorsaka en ökning av utbredningen av de andra onkogena HPV-typerna och där- igenom ytterligare minska de cancerpreventiva vinning- arna. Detta senare scenario blir sannolikt följden om det föreligger interferens mellan olika virala typer, vilket kan förutsägas från biologiska populationsstudier. Flera linjer med indirekt bevis indikerar att det verkligen existerar interferens mellan HPV-typer.
Flera andra HPV-typer förorsakar signifikant mor- biditet och mortalitet, mest noterbart är att HPV6 och 11 förorsakar genitala kondylom och återkommande andninge- papillomatoser och att HPV 5 och 8 förorsakar kutan hud- cancer hos den immunsuprimerade värden. Trots de uppen- bara fördelarna med brett korsreaktiva vacciner har möj- ligheten att alstra ett brett korsreaktivt vaccin genom modifiering av Ll-proteinet till att inte innehålla immundominanta typspecifika epitoper inte föreslagits.
Flera ytexponerade och korsreaktiva epitoper är expo- nerade på papillomaviruspartiklar (WO 96/33737), men är inte immunogena i närvaro av den immundominanta typ- specifika epitopen (4). Genom att modifiera Ll till att avlägsna immundominanta typspecifika epitoper bör det därför vara möjligt att alstra ett korsreaktivt papillomavirusvaccin med användning av ett modifierat HPV-Ll-protein som bärare av ytexponerade HPV-härledda antikroppsepitoper. 10 15 20 25 30 35 514 982 4 Vidare framkallar VLP mycket effektivt ett cyto- (CTL-svar) rapporterats vara höggradigt effektiva (genom en toxiskt T-lymfocytsvar och VLP-vacciner har CD8+cell-beroende mekanism) vid förebyggande och behand- ling av transplanterbara cancerformer i flera musmodeller trots det faktum att immunisering görs med exogent pro- tein (5). Den höga immunogeniteten synes delvis bero pà bevarandet av en aktiv mekanism för infektion av cellen (betecknad pseudo-infektion eftersom inget viralt genom är infört), vilket resulterar i att kapsidproteinet be- arbetas och presenteras i presentationsvàgen för MHC klass I (6). bioteknologisk synpunkt eftersom de kan användas som en VLP är därför av allmänt intresse ur vaccin- bärare för effektiv immunogen avgivning av vilket antigen som helst (7).
Effektiv immunisering med användning av HPV-VLP av vildtyp, som uppbär främmande antigener, har påvisats i flera system, t ex MAGE-melanomantigenerna och de humana immunbristvirusantigenerna.
Ett potentiellt problem med användning av VPL som vehiklar för immunogen avgivning är blockering med typ- specifika neutraliserande antikroppar. I Sverige är 16% av den vuxna befolkningen seropositiv för HPV-16, vilket återspeglar problemets betydelse. Dessutom förväntas terapeutisk vaccinering kräva återkommande behandlingar, sannolikt för att inducera ett typspecifikt antikropps~ svar mot en VLP-bärare av vildtyp.
Genom att modifiera Ll-proteinet för avlägsnande av typspecifika epitoper som förorsakar produktion av neu- (8) , införande av antikropp eller T-cellsepitoper i denna traliserande antikroppar, såsom har beskrivits och bärare, bör det därför vara möjligt att alstra ett immunologiskt svar mot den införda peptiden utan hinder från typspecifika neutraliserande antikroppar riktade mot själva bäraren. 10 15 20 25 30 35 514 982 5 Sammanfattning av uppfinningen Ett ändamål med föreliggande uppfinning är att åstadkomma medel för förebyggande och behandling av virala, bakteriella eller parasitära infektioner, sär- skilt av humant papillomavirus, och utvecklingen av benigna eller maligna konsekvenser av sådana infektioner, såväl som medel för behandling och förebyggande av cancer.
Föreliggande uppfinning åstadkommer användningen av ett modifierat HPV-Ll-protein i avsaknad av typspecifika epitoper som förorsakar produktion av neutraliserande antikroppar, som bärare av en substans in i celler. Som resultat av modifieringen inducerar denna HPV-L1-protein- bärare inte produktion av öppet neutraliserande anti- kroppar mot själva bäraren. I synnerhet åstadkommer upp- finningen ett sådant HPV-L1-protein i fusion med en pep- tid.
Uppfinningen åstadkommer även en sådan bärare som kan ge upphov till ett skyddande antikroppssvar, vilket antikroppssvar kan vara korsreaktivt mot två eller flera serologiskt definierade subtyper av humant papilloma- virus.
Bäraren mäste vara fysikaliskt kopplad, d v s fuserad, till den peptid för vilken den verkar som en bärare, och därigenom skapa ett fusionsprotein.
Särskilt inbegripes peptider härledda från HPV- proteiner och som definierar linjära antikroppsepitoper och T-cellsepitoper.
Kombinationer av nämnda bärare med ett mindre höljesprotein från humant papillomavirus (HPV-L2-pro- tein), nativt eller modifierat, förutses även. Också detta HPV-L2~protein kan i sig självt vara fuserat till en eller flera ytterligare peptider.
Uppfinningen åstadkommer även en oligo- eller poly- nukleotid som kodar för nämnda bärare. Uppfinningen gör det möjligt att skapa en bättre grund för framkallande av ett MHC klass I-medierat svar, d v s skapande av cyto- lO 15 20 25 30 35 514 982 6 toxiska T-celler, utan att ge upphov till typspecifika neutraliserande antikroppar mot bäraren, eller utan att typspecifika neutraliserande antikroppar föreligger vid starten.
Det år också möjligt att använda ett HPV-Ll-protein, modifierat såsom beskrivs ovan, som en bärare för oligo- eller polynukleotider till celler.
