Description
A device for preventing rotational displacements of the tibia in the treatment of their fractures by skeletal traction"
The utility model relates to the field of medicine and medical technology, namely to devices designed to prevent rotational displacements of the distal fragment of the tibia in the treatment of their fractures by the method of constant skeletal traction.
Purpose of utility model:
1) устранение ротационных смещений костных отломков большеберцовой кости при лечении переломов костей голени методом постоянного скелетного вытяжения;1) the elimination of rotational displacements of bone fragments of the tibia in the treatment of fractures of the tibia by constant skeletal traction;
2) сохранение правильной оси травмированной нижней конечности, находящейся на шине Белера;2) maintaining the correct axis of the injured lower limb located on the Beler tire;
3) creating favorable conditions for bone tissue regeneration.
In traumatology, in the treatment of patients with closed diaphyseal fractures of the lower leg bones, the method of constant skeletal traction is widely used [1, 2, 3]. To do this, the injured limb is placed on a standard Beler tire, and the Kirchner needle is passed through the calcaneus. The tension of the spokes is carried out in a bracket, for example, CITO, to which a reponing or fixing load is suspended through a thin cable or fishing line.
When fracturing the tibia, it is important: 1) to achieve an ideal comparison of its distal and proximal fragments; 2) to exclude any possible movements, including micro mobility, of bones in the fracture area; 3) to exclude any possibility of rotational displacement of the distal fragment of the tibia.
It is known that when lying on the back of a healthy person, due to the severity of the foot, the foot always rotates outward, which in the specialized literature qualifies as a “beach foot”. If a distal fragment of the tibia rotates outward in a patient with skeletal traction, this can lead to bone fusion in a vicious position, which, in turn, will lead to a change in the biomechanics of the foot and a violation of the axis of the injured lower limb.
The medical industry of our country does not produce special devices designed to prevent rotational displacements of the foot, and, consequently, bone fragments of the tibia in the treatment of fractures by skeletal traction.
The objective of the utility model is to ensure the immobility of bone fragments of the tibia located on the Beler’s splint and to prevent rotational movements of the foot.
It is very important that the comparison of the distal and proximal fragments of the tibia and their retention for the entire period of regeneration (the formation of primary bone callus) is carried out with minimal trauma to the soft tissues of the fracture area. This will create the possibility of subsequent full outpatient treatment.
3.The problem is solved by using a technical device, which is a standard (for the thigh) Kirschner needle, the lower end 1 of which is bent in the form of a hook in such a way that it freely encompasses the Kirschner needle drawn through the calcaneus, stretched in the CITO bracket. The upper end of the Kirschner needle contains thread 2 with nut 3.
Figure 1 shows a General view of a utility model.
The utility model works as follows. A patient with a fracture of the leg bones is treated by the method of constant skeletal traction by hanging a reponing, and then a fixing load, on the bracket of the CITO, strengthened by a strained Kirschner needle held through the calcaneus. To prevent rotational displacements of bone fragments of the tibia for the Kirschner spoke held through the calcaneus at a distance of 1-2 cm on the sides of the calcaneus, the lower end of 1 Kirschner spoke is fixed. The second (upper) end of the Kirschner needle is drawn through one of the holes in the horizontal pillar of a standard Beler tire. Elimination of rotational displacements of bone fragments of the tibia and foot and their subsequent retention is achieved by using a nut 3 moving along the thread 2 of the Kirchner knitting needle. The presence of a number of holes on the horizontal rack of a standard Beler tire, freely passing the Kirschner spoke, makes it easy to fix it in the right place. The location of the lower end of 1 Kirschner needle at a distance of 1-2 cm from the calcaneus does not interfere with the replacement of dried alcohol balls (napkins) on the outer surfaces of the calcaneus with new sterile alcohol balls.
