Abstract
Description
The invention relates to the field of medicine, in particular to obstetrics and gynecology, and can be used to determine the risk of developing preeclampsia (PE) in pregnant women in the first trimester of pregnancy.
Preeclampsia is a complication of pregnancy, childbirth and the postpartum period, characterized by an increase in systolic blood pressure (BP) >140 mmHg after the 20th week of pregnancy. Art. and/or diastolic blood pressure >90 mm Hg. Art. regardless of the level of blood pressure in the history in combination with proteinuria or at least one other parameter indicating the addition of multiple organ failure [Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and the postpartum period. Clinical guidelines of the Ministry of Health of the Russian Federation, 2021].
There is a known method for forming a risk group for PE based on identifying high and moderate risk factors in a woman, which include: hypertensive disorders during a previous pregnancy, chronic kidney disease, systemic lupus erythematosus (SLE), antiphospholipid syndrome (APS), diabetes mellitus 1 or 2 types, chronic hypertension; first pregnancy, age 40 years and older, interval between pregnancies more than 10 years, body mass index (BMI) at registration more than 35 kg/ m2 , family history of PE [Hypertensive disorders during pregnancy, childbirth and the postpartum period. Preeclampsia. Eclampsia. Clinical recommendations (treatment protocol). Letter of the Ministry of Health of the Russian Federation dated 06/07/2016 No. 15-4/10/2-3483. M., 2016. 73 p.]. Disadvantages of this method: a large number of false positive results; the ability to predict only one pregnancy complication.
10% [Tan M.Y., Wright D., Syngelaki A. et al. Comparison of diagnostic accuracy of early screening for pre-eclampsia by NICE guidelines and a method combining maternal factors and biomarkers: results of SPREE. Ultrasound Obstet Gynecol. 2018. № 6. Other models based on maternal factors make it possible to predict PE at any time in 30.4-40.3% of cases, PE before 37 weeks - in 41.5-47.6%, PE before 34 weeks - in 51-53, 5% with a false-positive rate of 10% [Tan M.Y., Wright D., Syngelaki A. et al. Comparison of diagnostic accuracy of early screening for pre-eclampsia by NICE guidelines and a method combining maternal factors and biomarkers: results of SPREE. Ultrasound Obstet Gynecol. 2018. No. 6. RR. 743-750]. Disadvantages: low prognostic value, the ability to predict only one pregnancy complication.
First trimester preeclampsia screening and prediction. Am J Obstet Gynecol. 2020. № 2. Other authors consider a combination of maternal factors and mean blood pressure, pulsatility index of the uterine arteries (PI MA), pregnancy-associated plasma protein-A (PAPP-A), placental growth factor, alpha-fetoprotein, while PE at any stage can be predicted in 41.2-43% of cases, PE after 37 weeks - in 24-47%, PE before 37 weeks - in 50-75%, early PE - in 71.4-94.1% [Chaemsaithong P., Sahota D.S., Poon L.C. First trimester preeclampsia screening and prediction. Am J Obstet Gynecol. 2020. No. 2. RR. 1071-1097]. Disadvantages: the method is based on the use of laboratory diagnostic methods, which are not used when examining pregnant women in accordance with the order of the Ministry of Health of the Russian Federation dated October 20, 2020 No. 1130n “On approval of the procedure for providing medical care in the field of obstetrics and gynecology”; the ability to predict only one pregnancy complication.
There is a known method for predicting PE in the first trimester, based on genetic, immunological and other laboratory tests that are not included in the list of examinations of pregnant women according to the order of the Ministry of Health of the Russian Federation dated October 20, 2020 No. 1130n “On approval of the procedure for providing medical care in the field of obstetrics and gynecology” ( RF patent No. 2648872; RF patent No. 2697845; RF patent No. 2693412; RF patent No. 2775434). There are known methods for predicting PE in women with chronic arterial hypertension (CAH) (RF patent No. 2699974), gestational diabetes mellitus (GDM) (RF patent No. 2740852). The disadvantage of the presented methods is the use of the methods only in a certain group of pregnant women and the impossibility of their use as a screening method.
There is a known method for predicting PE, proposed by E.V. Kudryavtseva. et al. “Method for predicting severe preeclampsia” (RF patent No. 2739694), based on determining anemia in the first trimester, family history of cardiovascular diseases (CVD), APTT, AST, creatinine, INR, PAPP-A values. Disadvantages of the method: the ability to predict only severe PE.
2017; 8: 74-84. http://dx.doi.org/ 10.18565/aig.2017.8.74-84]. The closest way to determine the risk of PE is the method proposed by the Fetal Medicine Foundation [Kholin A.M., Muminova K.T., Balashov I.S., et al. Prediction of preeclampsia in the first trimester: validation of screening algorithms in the Russian population. Obstetrics and gynecology. 2017; 8: 74-84. http://dx.doi.org/10.18565/aig.2017.8.74-84]. The risk of PE is calculated based on the following parameters: multiple pregnancy, BMI, smoking during pregnancy, PE in the patient’s mother, method of conception, CAH, diabetes mellitus type 1 or 2, SLE, APS, history of childbirth, mean blood pressure, pulsatility index of the uterine arteries ( average value of measurements on the right and left uterine arteries). A high risk is 1:100 or more. Disadvantages of the method: low specificity, inability to predict severe and moderate PE.
Technical result: early prediction of preeclampsia in pregnant women in the first trimester of pregnancy, increasing the specificity and accuracy of the method, accessibility and ease of implementation with high sensitivity; no additional material costs.
This result is achieved using a mathematical calculation of the risk of developing preeclampsia at 11-14 weeks based on medical history, laboratory parameters and average blood pressure.
In our invention, we focus on anamnestic, laboratory indicators and physical examination data (history of induced preterm labor, infertility for 5 years or more, Rh factor, blood pressure at 11-14 weeks), which are assessed in all pregnant women in 1st trimester of pregnancy in accordance with the order of the Ministry of Health of the Russian Federation dated October 20, 2020 No. 1130n “On approval of the procedure for providing medical care in the field of obstetrics and gynecology.” For the first time in obstetrics, the proposed method combines anamnesis data, laboratory and ultrasound indicators, and proposes unique formulas for determining the risk of PE.
Several logistic regression equations have been proposed, among which the one that predicts the development of complications most accurately has been selected.
The method is carried out as follows: at 11-14 weeks of pregnancy, in the antenatal fetal care office, a woman’s medical history is taken by questioning in order to assess anamnestic risk factors: induced birth defects, a history of infertility of 5 years or more. According to the pregnant woman’s exchange card (form No. 113/u), the results of laboratory tests performed in the conditions of the antenatal clinic are assessed, namely the results of determining the Rh factor; measure blood pressure according to the rules recommended by the Russian Society of Cardiology [Russian Society of Cardiology. Diagnosis and treatment of cardiovascular diseases during pregnancy. National recommendations. Russian Journal of Cardiology. 2018; 3 (155): 97-98]. Mean blood pressure was calculated as the arithmetic mean of systolic and diastolic blood pressure in the right and left arms.
Each indicator is assigned a score indicating the presence of a risk factor - 1 point or its absence - 0 points. Based on the data obtained, the value of the regression function beta is determined using the equation:
beta = 35,7444 + (-6,3048)*X1 + (-8,0696)*X2 + (-6,4490)*X3 + (-0,2619)*X4; beta = 35.7444 + (-6.3048)*X 1 + (-8.0696)*X 2 + (-6.4490)*X 3 + (-0.2619)*X 4 ;
ст. where X 1 is the Rh factor: negative - 1, positive - 0; X 2 - history of induced PRs: presence - 1, absence - 0; X 3 - infertility for 5 years or more: presence - 1, absence - 0; X 4 - the value of average blood pressure at 11-14 weeks, mm Hg. Art.
Then the risk of developing PE is calculated:
где e - основание натурального логарифма, beta - значение регрессионной функции; и при величине p менее 0,5 прогнозируют низкую степень риска развития ПЭ, при величине p равном 0,5 и выше - высокую степень риска развития ПЭ.where e is the base of the natural logarithm, beta is the value of the regression function; and with a p value of less than 0.5, a low risk of developing PE is predicted; with a p value of 0.5 and higher, a high risk of developing PE is predicted.
1,2%.When performing ROC analysis, the area under the ROC curve (AUC) was determined. An AUC of 0.9946 corresponds to a high quality model. At the same time, the most effective model is one that allows one to correctly classify 71.4% of patients with PE and 80% of patients with severe PE into the risk group with a false-positive rate of 1.2%.
Examples of specific implementation.

The risk of PE was calculated using the proposed method:
betaПЭ = 35,7444 + (-6,3048)*0 + (-8,0696)*1 + (-6,4490)*0 + (-0,2619)*107 = -0,6695betaPE = 35.7444 + (-6.3048)*0 + (-8.0696)*1 + (-6.4490)*0 + (-0.2619)*107 = -0.6695
Risk of PE = 1 - 2.7 -0.6695 /(1+2.7 -0.6695 ) = 0.66 (high risk)
Considering the high risk of PE, as well as the presence of high-risk factors according to clinical guidelines (chronic pyelonephritis), we recommended that the patient take acetylsalicylic acid at a dose of 75 mg daily (according to the clinical guidelines in force at the time of observation of the pregnant woman), however, the patient began taking the drug during pregnancy 18 weeks, which is a late onset and is not considered correctly carried out prevention of PE.
The pregnancy ended with induced pregnancy loss at 36 weeks and 2 days due to severe PE. During a cesarean section, a live premature girl weighing 2650 g, length 48 cm, with an Apgar score of 8/8 points was removed.
We correctly determined the risk of PE: it was calculated as high, and at 36 weeks of pregnancy the patient developed this gestational complication, which was the cause of induced PE.
Example 2. Pregnant K., 33 years old. This is the third pregnancy, unplanned, desired: the first pregnancy ended in spontaneous premature birth at 36-37 weeks, during pregnancy there was a recurrent threat of miscarriage, the postpartum period proceeded without complications; the second pregnancy ended with induced premature birth at 35-36 weeks due to severe preeclampsia, cesarean section, pregnancy proceeded against the background of high blood pressure, the postpartum period proceeded without complications.
