Известен способ пластики дефекта грудной стенки после резекции грудины, предполагающий применение лоскута большого сальника. Данный вариант пластики сочетает в себе положительные эффекты в виде устранения остаточной полости в средостении и профилактики рецидива инфекции в грудной стенки (патент RU №2757374, А61В17/11, 14.10.2021). In addition, the full-thickness cutaneous-subcutaneous-fascial-muscular flap of the right rectus abdominis muscle is a massive and voluminous flap with good axial blood supply, which allows it to be placed as tightly as possible in the chest wall defect without the risk of developing a deficiency of blood supply in it, as well as to create conditions for minimizing pathological mobility of the chest wall in the early postoperative period. The use of a polypropylene prosthesis at the level of the anterior abdominal wall helps prevent the development of postoperative ventral and thoracoabdominal hernias by strengthening the donor zone of the full-thickness skin-subcutaneous-fascial-muscular flap and the laparotomy area for harvesting the greater omentum flap.
На практике способ многослойной пластики срединного дефекта грудной стенки после ее обширной резекции осуществляют следующим образом. После выполнения окончательного дебридмента раны, удаления нежизнеспособных мягких тканей, участков рукоятки, тела и мечевидного отростка грудины, хрящей ребер, производят вскрытие брюшины в нижней части раны, выделяют и мобилизуют прядь большого сальника на левой желудочно-сальниковой артерии. Затем перемещают лоскут большого сальника в дефект грудной стенки. Далее, в проекции правой прямой мышцы живота формируют полнослойный кожно-подкожно-фасциально-мышечный лоскут на правой верхней надчревной артерии длиной и шириной, соответствующий размерам дефекта грудной стенки. Производят выделение, пережатие, перевязывание и пересечение нижних надчревных сосудов в нижней части раны передней брюшной стенки. Затем пересекают и перевязывают все перфорантные сосуды, отходящие от нижней поверхности полнослойного лоскута. Интраоперационно с помощью ультразвуковой допплерографии подтверждают наличие артериального кровотока в верхней надчревной артерии и перемещают полнослойный кожно-подкожно-фасциально-мышечный лоскут в дефект грудной стенки и укладывают его на лоскут большого сальника. The peritoneal defect in the lower part of the chest wound is sutured with a continuous absorbable suture. At the level of the anterior abdominal wall, a polypropylene prosthesis of the appropriate size is sewn into the diastasis between the edges of the outer sheet of the sheath of the right rectus abdominis muscle, fixing it along the periphery in the medial part of the wound to the midline of the abdomen, in the lower and lateral parts - to the outer sheet of the sheath of the right rectus abdominis muscle, in the upper part - to the posterior layer of the sheath of the right rectus abdominis muscle with mandatory strengthening of the area of the performed laparotomy by fixing the prosthesis to the outer layer of the sheath of the left rectus abdominis muscle. Предложенный способ с положительным эффектом выполнен у пациента Р. 67 лет с диагнозом: Постстернтомный медиастинит 2а тип. Сахарный диабет 2 тип, инсулинотерапия. Пациент поступил в хирургическое отделение №17 ГБУЗ «Самарская городская клиническая больница им. Н.И. Пирогова» через 14 дней после стернотомии и протезирования восходящей дуги аорты с клинической картиной гнойного медиастинита. Интенсивная терапия в течение 10 дней, включающая дебридмент с тремя сеансами вакуумной терапии раны, антибактериальную, антикоагулянтную и симптоматическую терапию, позволила в короткие сроки подготовить пациента к реконструктивно-восстановительной операции. В результате подготовки раны к пластике была полностью удалена грудина и резецированы хрящевые части II-VI ребер с двух сторон. Площадь раневого дефекта составила 220 см2. Острый инфекционный процесс в средостении был купирован, что подтверждалось стерильным результатом микробиологического исследования. Проведено оперативное вмешательство. Выполнен окончательный дебридмент раны, удалены нежизнеспособные мягкие ткани, участки рукоятки, тела и мечевидного отростка грудины, хрящей ребер. Произведено вскрытие брюшины в нижней части раны, выделена и мобилизована прядь большого сальника на левой желудочно-сальниковой артерии с последующим перемещением лоскута большого сальника в дефект грудной стенки. Затем, в проекции правой прямой мышцы живота сформирован полнослойный кожно-подкожно-фасциально-мышечный лоскут на правой верхней надчревной артерии длиной и шириной, соответствующий размерам дефекта грудной стенки. Выделены, пережаты, перевязаны и пересечены нижние надчревные сосуды в нижней части раны передней брюшной стенки; пересечены и перевязаны все перфорантные сосуды, отходящие от нижней поверхности полнослойного лоскута. Интраоперационно с помощью ультразвуковой допплерографии подтверждено наличие артериального кровотока в верхней надчревной артерии. Полнослойный лоскут в дефект грудной стенки перемещен и уложен на лоскут большого сальника. В средостение под лоскут большого сальника и под полнослойный лоскут прямой мышцы живота установлены перфорированные дренажные трубки, которые выведены через отдельные разрезы в левом подреберье. Дефект брюшины в нижней части раны груди ушит непрерывным рассасывающимся швом. На уровне передней брюшной стенки в диастаз между краями наружного листа влагалища правой прямой мышцы живота вшит соответствующего размера полипропиленовый протез, фиксированный по периферии в медиальной части раны к апоневрозу срединной линии живота, в нижней и латеральной части – к наружному листку влагалища правой прямой мышцы живота, в верхней части – к заднему листку влагалища правой прямой мышцы живота с обязательным укреплением зоны выполненной лапаротомии путем фиксации протеза к наружному листку влагалища левой прямой мышцы живота. Установлен перфорированный дренаж на полипропиленовый протез и выведен в нижней части раны через отдельный кожный разрез. Рана передней брюшной стенки послойно ушита. Произведена фиксация пряди большого сальника к грудной фасции редкими рассасывающимися швами. Полнослойный кожно-подкожно-фасциально-мышечный лоскут правой прямой мышцы живота фиксирован к краям раны грудной стенки. После сопоставления краев раны наложены кожные швы. Послеоперационный период в течение 3-х суток был в отделении интенсивной терапии. Дренажи удалены на 7-е сутки. Швы сняты на 12-е сутки. Лечебный результат достигнут - полное заполнение дефекта грудной стенки аутологичными тканями без натяжения краев кожной раны, а также достигнута стабилизация грудинно-реберного каркаса. Пациент выписан на 16-е сутки после операции. Через 12 недель на контрольном осмотре пациент жалоб не предъявляет, патологической подвижности, грыжи, рецидива инфекции не выявлено.The proposed method was performed with a positive effect in patient R., 67 years old, with a diagnosis of poststernum mediastinitis type 2a. As a result of preparing the wound for plastic surgery, the sternum was completely removed and the cartilaginous parts of the II-VI ribs were resected on both sides. The area of the wound defect was 220 cm2 . The acute infectious process in the mediastinum was stopped, which was confirmed by the sterile result of a microbiological examination. Surgery was performed. The final debridement of the wound was performed, non-viable soft tissues, sections of the manubrium, body and xiphoid process of the sternum, and rib cartilage were removed. The peritoneum in the lower part of the wound was opened, a strand of the greater omentum on the left gastroepiploic artery was isolated and mobilized, followed by moving the greater omentum flap into the chest wall defect. Then, in the projection of the right rectus abdominis muscle, a full-thickness cutaneous-subcutaneous-fascial-muscular flap was formed on the right superior epigastric artery with a length and width corresponding to the size of the chest wall defect. The lower epigastric vessels in the lower part of the wound of the anterior abdominal wall were isolated, clamped, tied and crossed; All perforating vessels extending from the lower surface of the full-thickness flap were crossed and ligated. Intraoperatively, Doppler ultrasound confirmed the presence of arterial blood flow in the superior epigastric artery. A full-thickness flap was moved into the chest wall defect and placed on the greater omentum flap. Perforated drainage tubes are installed in the mediastinum under the greater omentum flap and under the full-thickness flap of the rectus abdominis muscle, which are removed through separate incisions in the left hypochondrium. The peritoneal defect in the lower part of the chest wound is sutured with a continuous absorbable suture. At the level of the anterior abdominal wall, a polypropylene prosthesis of the appropriate size is sewn into the diastasis between the edges of the outer sheet of the sheath of the right rectus abdominis muscle, fixed along the periphery in the medial part of the wound to the aponeurosis of the midline of the abdomen, in the lower and lateral parts - to the outer sheet of the sheath of the right rectus abdominis muscle, in the upper part - to the posterior layer of the sheath of the right rectus abdominis muscle with mandatory strengthening of the area of the performed laparotomy by fixing the prosthesis to the outer layer of the sheath of the left rectus abdominis muscle. A perforated drainage was installed on a polypropylene prosthesis and removed in the lower part of the wound through a separate skin incision. The wound of the anterior abdominal wall is sutured in layers. A strand of the greater omentum was fixed to the pectoral fascia using sparse absorbable sutures. A full-thickness cutaneous-subcutaneous-fascial-muscular flap of the right rectus abdominis muscle is fixed to the edges of the chest wall wound. After matching the edges of the wound, skin sutures are applied. The postoperative period was in the intensive care unit for 3 days. The drains were removed on the 7th day. The sutures were removed on the 12th day. The therapeutic result was achieved - complete filling of the chest wall defect with autologous tissues without tension on the edges of the skin wound, and stabilization of the sternocostal frame was achieved. The patient was discharged on the 16th day after surgery. After 12 weeks, at the follow-up examination, the patient had no complaints; no pathological mobility, hernia, or recurrence of infection were detected.

Продолжительность операции составила 140 минут. В послеоперационном периоде дважды были приступы мерцательной аритмии, купируемые медикаментозно. Осложнений со стороны зоны операции в раннем и отдаленном послеоперационном периоде не выявлено. Лечебный результат достигнут - полное заполнение дефекта грудной стенки аутологичными тканями без натяжения краев кожной раны, а также достигнута стабилизация грудинно-реберного каркаса. A perforated drainage was installed on a polypropylene prosthesis and removed in the lower part of the wound through a separate skin incision. The wound of the anterior abdominal wall is sutured in layers. A strand of the greater omentum was fixed to the pectoral fascia using sparse absorbable sutures. A full-thickness cutaneous-subcutaneous-fascial-muscular flap of the right rectus abdominis muscle is fixed to the edges of the chest wall wound. After matching the edges of the wound, skin sutures are applied. The duration of the operation was 140 minutes. In the postoperative period, there were two attacks of atrial fibrillation, which were controlled with medication. No complications from the surgical site were identified in the early and late postoperative period. The therapeutic result was achieved - complete filling of the chest wall defect with autologous tissues without tension on the edges of the skin wound, and stabilization of the sternocostal frame was achieved. The patient's follow-up period was 6 months after surgery.
