Abstract
Description
The invention relates to restorative medicine, adaptive physical culture, rehabilitation of patients with cerebral palsy. The invention can be used for the formation of compensatory motor skills and coordination of the body and limbs in the process of movement in patients with cerebral palsy who have the skill of walking, but there is spasticity of the muscles and there is no correct biomechanics of movement. To do this, systematic classes are carried out with female patients using a set of special exercises created on the basis of the Argentine tango dance technique (10 basic movements and 5 dance structures made up of basic movements), in sequence. With male patients, systematic training is carried out using a special exercise No. 10 from the complex of basic movements. The movements of patients are controlled by an instructor-methodologist who ensures the interaction of three levels of the body: shoulder girdle and upper limbs - pelvis - lower limbs. Sequential rotational movement of each part of the body from the shoulder girdle to the lower limbs allows you to induce the desired installation reflexes and free movement of the legs. With regular exercise (daily - for exercise No. 10) and repetition, the correct coordination of the movement of the body and lower limbs is fixed. The method ensures the acquisition by patients of the motor skills necessary for the formation of the correct biomechanical movement stereotype.
A61N 1/31, 1981).Known methods and devices for the treatment of cerebral palsy by exposure to pulsed current. (Semenova K.A. Cerebral palsy. M., "Medicine", 1968, p. 136-139; Author's certificate of the USSR No. 8395 54, class A61N 1/31, 1981).
The disadvantage of this method is the increased complexity and complexity of the equipment used.
A known method of treating cerebral palsy by physical effects on a child by using various simulators (Polsky E.V. "Infantile cerebral palsy", M., 2004, 173 p.).
The possibility of using this method is limited by the design complexity and high cost of the simulators used, the need for highly qualified personnel to maintain the simulators, and increased labor intensity.
Критериями эффективности являются возраст ребенка, выраженность двигательных нарушений и степень сохранности психики (www.cerebral-palsy.ru).There is a known method of imperative corrective gymnastics and rehabilitation technologies of E.E. All stages of treatment - preliminary, preparatory, main - are carried out exclusively in a specialized center. The criteria for effectiveness are the age of the child, the severity of movement disorders and the degree of preservation of the psyche (www.cerebral-palsy.ru).
The use of this method is limited by the impossibility of carrying out exercises at home by parents, since they require author's simulators. The implementation of the method requires expensive treatment in a specialized center, which limits the availability of this method of rehabilitation.
(RU) https://findpatent.ru/patent/253/2539401.html ©, 2012-2020)A known method for correcting posture and walking disorders in cerebral palsy (Patent authors: Mikhailova Olga Viktorovna (RU), Khromushin Akim Nikolaevich (RU) https://findpatent.ru/patent/253/2539401.html ©, 2012-2020)
The use of this method provides for pneumomechanical stimulation of the support zones of the feet of the right and left legs, which is carried out with the help of a plantar support load simulator using mechanical pressure in a pulsed mode on the calcaneal and metatarsal support zones. EFFECT: method provides normalization of the processes of excitation and inhibition in the CNS, with a decrease in rigidity, muscle spasticity, the development of functional connections in the brain, the development of new conditioned motor reflexes, the development of compensatory devices, and improved coordination of movements.
Using the method with the support stimulation procedure requires an equipped room, the presence of a device - a support load simulator and a specialist working with this device, which makes it impossible for parents to carry out rehabilitation procedures at home.
RU https://findpatent.ru/patent/265/2651902.html ©, 2012-2019)There is also known a method of hippotherapy for patients with cerebral palsy in the conditions of the Black Sea coast (Patent authors: Nikitina Natalia Valerievna (RU) Komlatsky Vasily Ivanovich RU https://findpatent.ru/patent/265/2651902.html ©, 2012-2019)
The method includes therapeutic riding with the development of passive-active postures on a horse without a saddle, treatment in three stages with a gradual increase in the course of the child's participation in each posture and a decrease in the participation of the instructor and parent in the conditions of the Black Sea coast. As a result of the combination of passive horseback riding with swimming in the sea, the therapeutic effect obtained during hippotherapy is consolidated.
The use of this method, as well as other methods of hippotherapy for patients with cerebral palsy, is also limited by the impossibility of doing exercises at home by parents, since they require horses and qualified instructors-methodologists who know the method of hippotherapy.
The objective of the invention is the physical rehabilitation of patients with cerebral palsy who have the skill of walking, due to the formation of the correct biomechanical stereotype of linear forward movement and the achievement of muscle relaxation in the process of conducting a set of physical exercises based on the use of Argentine tango dance techniques, by one person in any room, including at home.
The technical result is to increase the efficiency of rehabilitation of patients with cerebral palsy by forming compensatory motor skills and acquiring natural coordination of the body and limbs in the process of movement.
The technical result is achieved by the fact that for the method of rehabilitation of patients with cerebral palsy based on the use of the Argentine tango dance technique, a set of special exercises has been developed that allow patients to restore the natural, biomechanically correct movement technique, and acquire the skill of free plastic movement in the Argentine tango dance technique.
The novelty of the proposed method lies in the combination of movements of the Argentine tango dance technique, stimulating muscle relaxation, free movement of the legs and developing the correct coordination of the movement of the body and legs of patients, with the rhythm of Argentine tango music, which contributes to the emotional and physical rehabilitation of patients and their successful integration into the community of healthy peers . The novelty of the proposed method is also in the relative simplicity of a set of exercises of 10 basic movements and 5 dance structures made up of basic movements, and above all, a special exercise No. 10 from a complex of basic movements that can be mastered by parents, relatives, friends of patients and, thus, to become home instructors-methodologists for patients. The mastering and application by parents, relatives, friends of patients of a set of exercises will allow the rehabilitation process to be carried out without the significant cash costs that are usual today, which is important for families with low incomes.
Movement to music also has a corrective effect on the physical development of patients with cerebral palsy and creates a favorable basis for improving mental functions such as thinking, memory, attention, and perception. Rhythmic structure, dynamic coloring, tempo changes in Argentine tango music stimulate constant concentration of attention, memorization of the conditions for performing dance exercises, a quick reaction to the change of musical phrases characteristic of Argentine tango. Dance improves the psychodynamic functions of a person. Rhythmic movements strengthen different muscle groups and improve joint function, as well as affect abilities such as speed, accuracy and synchronization of movements.
The method includes sequential training of patients in a set of dance-oriented exercises in Argentinean tango technique with the help of an instructor-methodist, which may be one of the patient's parents or friends. The method is carried out by conducting systematic dance classes for female patients and classes using a special exercise No. 10 from the complex of basic movements for all patients in compliance with the sequence, correct execution, and also taking into account the patient's motor capabilities and physical culture experience.
The method of using the dance technique of Argentine tango during the rehabilitation process is indicated for patients who have had cerebral palsy, starting from adolescence (exercise No. 10 - for patients from any age), who have the skill of walking, with the presence of spasticity of the muscles and lack of coordination of the movement of the body and legs.
Regular classes contribute to the formation of a new stereotype of the movement of patients. Therefore, depending on the capabilities of the parents, relatives or friends of the patients, as a result of training, acting as an instructor, dance classes are held at intervals of once a day, but at least once a week. The duration of the lesson - from 20-30 minutes in the first lessons to 30-40 minutes in subsequent lessons, is due to the fact that during this period of time the most effective relaxation of the patient's muscles occurs and the patient develops previously absent motor skills. A longer session will cause fatigue in patients. Exercise No. 10 from the complex of special exercises (Linear forward movement of the patient or patient - see below), depending on the capabilities of the patient's parents, relatives or friends, can be performed daily during the entire time of physical activity (from moving around the apartment to walking on the street) with breaks any duration upon the onset of fatigue. All exercises of the complex are performed smoothly, at a slow and medium pace with the control of the trajectory of movement by the instructor and the instructor applying adequate efforts to the hands and body of the patient.
A contraindication for the use of a set of special exercises based on the use of the dance technique of Argentine tango is the presence of deformities of the joints, spine, autonomic manifestations of the cardiovascular and respiratory systems in the patient.
A set of special exercises for the rehabilitation of patients with cerebral palsy based on the use of Argentine tango dance techniques.
The stage of mastering the basic movements of Argentine tango.
1. The instructor, taking the body of the patient with his right hand in an embrace and the right hand of the patient with his left hand, makes joint sliding movements of the body successively to the right and to the left, collecting weight on the right, then on the left leg.
2. "Eight back" (the term of the Argentinean tango is "ocho atras"). The instructor moves the body to the right and left, alternately collecting weight on the right or left leg. The partner makes a movement to the right and left together with the partner, but at the same time, at the extreme right and extreme left points, she turns the body in such a way that the trajectory of her legs is the number "8". The peculiarity of this movement is that the partner turns her hips and legs not with the efforts of the muscles, but successively pushing off with her right or left hand from the partner’s left hand or right shoulder. At the same time, she does not initiate a step with her foot, but allows her foot to follow the movement of the body.
3. "Eight forward" (the term of Argentine tango is "ocho adelante"). The instructor moves the body to the right and left, alternately collecting body weight on the right or left leg. Stepping back, the instructor leads the patient towards himself, also shifting the weight of her body alternately on the right and left legs in such a way that the trajectory of her movement is the number "8". The peculiarity of this movement is that with the right hand, the instructor gently pulls the patient's body to the next (right or left) turn point; in this case, the patient does not initiate the step with her leg, but allows the leg to follow the movement of the body.
4. "Eight back" with the instructor walking around the patient on the left or right. The instructor starts the “figure eight back” movement with the patient, but instead of moving with the partner to the right and left and providing her with her shoulder and left arm as points of support for a turn, she walks around the patient to the left or right, turns 180 degrees and, supporting her body with her right hand , leads the patient one step back, after which the patient turns her body with the effort of her right or left hand, pushing off from the partner’s left hand or right shoulder.
5. "Eight forward" with "displacement" (the term of the Argentine tango is "sacado") and turn to the right. The instructor starts the “figure eight forward” movement with the patient, but instead of moving together with the patient to the right or left turn point, directs the patient to the right turn point, but after the patient’s diagonal step, takes his step under the patient’s free leg left behind (this is the partner’s movement in Spanish the language is called “sacado” - “displacement, substitution”, when the partner puts his foot on the line connecting the partner’s feet and, as it were, pushes the partner out of his place); then, together with the patient, makes a turn to the right around the patient's supporting leg and leads her one step forward.
The movement of the relaxed leg of the patient (from the "eight back") - circular "boleo" (in Spanish voleo - "flying", in Argentine tango the term means free movement of the leg). Directing the patient into another turn in the "eight back" movement, the instructor goes into a counter-movement and gently blocks the patient's body, preventing her from putting her free leg on the floor. At the same time, the free leg of the patient is beautifully extended (“flying”).
7. The movement of the relaxed leg of the patient (from the “eight forward”) is a circular “boleo”. The beginning of the movement is “forward figure eight with sacado and turn to the right (from exercise 5), but instead of taking the final step forward with the patient after turning to the right, the instructor goes into a counter-movement and gently blocks the patient’s body with his shoulder, preventing her from putting her free left leg on the floor and doing step forward. At the same time, the patient's free leg stretches beautifully - as if "flying". After blocking, the patient's torso begins a reverse movement in the "eight back", which the instructor interrupts by blocking with his left hand, as a result of which the patient's free leg is extended back. This rotation of the patient around the supporting leg with a circular "boleo" is repeated several times.
Movement of a relaxed leg - linear "boleo" (in Spanish voleo - "flying", in Argentinean tango the term means free movement of the leg). With a vigorous movement of his body forward, the instructor directs the patient a step back, then he goes into a counter-movement back and with his outstretched hand gently blocks the patient's body, preventing her from putting her free foot on the floor and taking a step back. At the same time, the free leg of the patient is beautifully extended (“flying”).
9. Linear movement (leading the patient back and forth). To relax the leg and step back or forward, a key element of Argentine tango is used - “twisting” (in Spanish - “torcion”). Standing still (weight on the left leg), the instructor turns the shoulders and body to the right, as a result of which the free right leg involuntarily takes a step forward. At the same time, the instructor turns the patient's body to the right, as a result of which the patient's free left leg involuntarily takes a step back. A similar exercise is done for the patient's right leg.
10. Linear forward movement of a patient (or patient) experiencing (experiencing) significant difficulties with a normal step forward. The instructor, standing next to and slightly behind the patient (patient), takes the patient with his hands by the shoulders and “twisting” the body synchronously with his body to the right and left, leads the patient forward.
The stage of drawing up dance figures from the mastered basic movements of Argentine tango.
1. "Mill" (the term of the Argentine tango is "molinete") - a combination of movements "eight back" (from exercise 2) - movement to the side (from exercise 1) - "eight forward" (from exercise 3) - movement to the side ( from exercise 1). The instructor directs the patient to the "eight back" movement, then to the sliding movement of the body to the side, then to the "eight forward" movement, then again to the sliding movement of the body to the side. The figure "mill" can be repeated several times.
2. "Forward figure eight" with a joint turn on opposite axes of the instructor and the patient. The instructor, remaining in place, directs the patient in the “forward eight” movement (from exercise 3) to the left. When the patient takes a step forward and is on the left turn point, the instructor, who is on the right point with his hand, gently pulls the patient's body to the right turn point, and at the same time moves to the left turn point, on which the patient has just been. At the end of the oncoming movement, the instructor makes a joint turn of his body (on the left point) and the body of the patient (on the right point); while the relaxed left leg of the patient gently twists around her axial right leg (this movement is called "screwing" (the term of the Argentine tango is "enrosque", an artistic way to twist and unwind around its axis during rotation). The joint movement is repeated several times successively to the right and to the left.
3. "Step forward with alternate turn of the instructor and the patient." The instructor takes a step forward with his right foot, makes a turn to the right by 90 degrees; leads the patient one step forward from her right foot, while simultaneously stepping with her free left foot under the patient's free left foot (sakado movement). The instructor then makes a 90-degree turn to the right on their left foot and steps forward with their right foot and simultaneously guides the patient's free left foot under her free left foot. The joint movement is repeated several times.
4. "Colgada" (the term of Argentine tango is "colgada"). Movement off the axis of the instructor and the patient with one axis in the middle, the partners, as it were, form the letter V. The instructor begins the “eight forward” movement with the patient, at the moment the patient turns around his right leg, he removes from himself (“weighs”) her body, balancing the pair with a displacement own body in the same way. Rotation is provided by the partner, stepping around the supporting leg of the partner.
5. "Volcada" (the term of the Argentine tango is "volcada"). The movement of the patient is off axis, the weight is given to the partner. The partners seem to form an inverted V. The instructor takes the weight of the patient on the forearm of the right hand, supports her body with the right hand and controls the patient's free left leg through the body, describing her foot in a circle on the floor.
The claimed method, which includes following the sequence of a set of special exercises created on the basis of the dance technique of Argentine tango from 10 basic movements and 5 dance structures, makes it possible to form the correct biomechanical stereotype of movement in patients with cerebral palsy, stimulate relaxation of the muscles of the legs, improve coordination of the body and legs, and also contributes to their successful integration into the community of healthy peers.
The invention is new - it is unknown in the field of restorative medicine and in the system of physical rehabilitation of patients with cerebral palsy who have impaired walking function and natural movement biomechanics.
Examples of application of the proposed method.
Patient 1. 23 years old. cerebral palsy
До реабилитации предложенным способом: уровень развития больших моторных функций пациентки по Системе классификации больших моторных функций GMFCS (Gross Motor Function Classification System) -Уровень II (ходьба с ограничениями).Before rehabilitation by the proposed method: the level of development of large motor functions of the patient according to the GMFCS (Gross Motor Function Classification System) classification system of large motor functions - Level II (walking with restrictions).
As a result of rehabilitation using a set of special exercises from 10 basic movements of the Argentine tango dance technique, a positive effect was achieved: 1) the patient's step became free, full, the pattern of movement formed over many years with short steps on the leg bent at the knees was overcome; 2) the function of the vestibular apparatus has improved, the patient's fear of not only a free linear step has disappeared, but the effect of muscle blocking during the implementation of other, including new, unfamiliar movements, has also been overcome; 3) decreased fatigue.
Patient 2. 13 years old. Diagnosis: cerebral palsy
До реабилитации предложенным способом: уровень развития больших моторных функций пациентки по Системе классификации больших моторных функций GMFCS (Gross Motor Function Classification System) -Уровень II (ходьба с ограничениями значительными).Before rehabilitation by the proposed method: the level of development of the patient's large motor functions according to the GMFCS (Gross Motor Function Classification System) classification system of large motor functions - Level II (walking with significant limitations).
As a result of rehabilitation using exercise No. 10 from the complex of special exercises (“Linear forward movement of the patient or patient”) of the Argentine tango dance technique, a positive effect was achieved: 1) after the first lesson, the “inverted” left leg began to relax and straighten - a decrease in spasticity was noted leg muscles; 2) improved coordination of movement of the body and legs; 3) with the support of the instructor, the patient stopped being afraid to take full, free steps.
The invention can be effectively used in rehabilitation centers, adaptive physical culture centers and at home for the formation of compensatory motor skills and coordination of the body and limbs in patients with cerebral palsy.
The claimed method of rehabilitation of patients with cerebral palsy does not guarantee complete restoration of the coordination of the body and limbs and complete overcoming of spasticity of the leg muscles. Nevertheless, the author proposes it as an effective way for patients to acquire conditioned motor reflexes necessary for the formation of the correct biomechanical movement stereotype, based on the combination of physical exercises with the rhythmic movement of the Argentine tango dance technique.
