В основу изобретения положена задача, заключающаяся в разработке не инвазивного метода диагностики субклинически протекающей внутриматочной инфекции. Указанная задача достигается тем, что используется способ определения металлопротеиназы-8 (ММР-8) в цервикальной слизи у женщин во II триместре беременности.The invention is based on the task of developing a non-invasive method for diagnosing a subclinically occurring intrauterine infection. Актуальность проблемы определена причинно-следственной связью внутриматочной инфекции с преждевременными родами и такими неонатальными осложнениями как врожденная пневмония, сепсис, некротический энтероколит, церебральный паралич, аутизм. Частота интраамниального воспаления при родах в сроках до 34 недель достигает 84%. Гистологический хориоамнионит регистрируется у 94% женщин со спонтанными эктремально ранними преждевременными родами. Следует сказать, что клиницисты используют термин «хориоамнионит» для обозначения клинического синдрома, однако, верификация данного диагноза основывается на патологоанатомическом исследовании последа. Для того чтобы различать два этих понятия, в литературе используют термины «клинический хориоамнионит» и «гистологический хориоамнионит», которые не являются синонимами и не должны использоваться взаимозаменяемо. В тех случаях, когда антенатально имеются иммунологические или биохимические маркеры воспаления в амниотической или цервиковагинальной жидкости, но нет классических (явных) клинических признаков хориоамнионита, правомочно использовать термин «субклинический хориоамнионит» или «внутриматочная инфекция».The relevance of the problem is determined by the causal relationship of intrauterine infection with preterm birth and such neonatal complications as congenital pneumonia, sepsis, necrotizing enterocolitis, cerebral palsy, autism. Thus, the search for non-invasive markers of subclinical intrauterine infection during pregnancy continues. In this regard, the study of various immunological and biochemical markers of inflammation in cervical mucus is of scientific interest. The human cervical mucus plug is a unique barrier between the lower genital tract and the uterus, and is a complex mixture of secretions produced by endocervical, decidual, and chorioamnion secretory cells. The concentration of specific biomarkers, such as matrix metalloproteinases (MMPs), IL6, IL10, TNF, in cervical mucus may reflect the activation of inflammatory processes in the uterus.
Матричные металлопротеиназы представляют собой протеолитические ферменты, способные разрушать компоненты внеклеточного матрикса и активировать цитокины и антимикробные пептиды. ММР-8 продуцируется нейтрофильными лейкоцитами, в отличие от нелейкоцитарной ММР-2, а значительное их повышение свидетельствует об эндоцервикальном воспалительном процессе и высоком риске интраамниального воспаления и преждевременных родов. Nien J.K, Yoon В.Н, Espinoza J. рекомендуют определение ММР-8 в амниотической жидкости в качестве экспресс-теста на интраамниальное воспаление, что подтверждает высокую информативность данного метода. Статистическая обработка результатов осуществлялась с помощью редактора данных IBM SPSS Statistics 23.0 Учитывая несоответствие полученных данных нормальному закону распределения, количественные и порядковые показатели представлены в виде медианы (Me), 25 и 75 квартилей. Оценку различий между группами проводили при помощи непараметрического критерия Манна-Уитни. Для номинальных признаков приведены абсолютные значения и относительная частота встречаемости, различия в группах оценивались по критерию %2 Пирсона. Средний возраст пациенток составил: в I группе - 29,8±0,99; во II- 30,3±0,98, в группе контроля - 28,7±0,98, то есть статистически сопоставимы. Во всех группах преобладали повторнобеременные женщины, среди которых первородящие составили 18,8% в I группе, 32,1% - во II группе, в группе контроля - не было. У повторнобеременных из I и II группы акушерско-гинекологический анамнез был отягощен искусственными абортами и репродуктивными потерями. В I группе статистически чаще в анамнезе у женщин были спонтанные выкидыши - 31,3% (р=0,008) и преждевременные роды 21,9%, по сравнению с группой контроля (0%, р=0,035). Во II группе статистически чаще были повторные медицинские аборты 32,1% р=0,006) и неразвивающаяся беременность 35,7% (р=0,003), по сравнению с группой контроля. У беременных из II группы достоверно чаще гинекологический анамнез был отягощен воспалительными заболеваниями верхнего и нижнего отдела репродуктивного тракта. Так, воспалительные заболевания верхнего отдела репродуктивного тракта в анамнезе были у 60,7% женщин, что достоверно чаще, чем в группе контроля (0%, р=0,008). Воспалительные процессы нижнего отдела генитального тракта в анамнезе регистрировались практически у каждой четвертой пациентки из II группы (25%), в 6,3% у пациенток I группы, тогда как в группе контроля - в 5% (р=0,046), при этом, при межгрупповом сравнении различия не достигли статистической значимости. В I и II группах статистически чаще настоящая беременность осложнилась стойкой угрозой прерывания, по сравнению с группой контроля. Стойкая угроза прерывания на протяжении всей беременности имела место у 81,3% пациенток I группы и у 96,4%) - II группы, в группе контроля - 0% (р<0,001). На основании анализа ROC-кривых были выбраны точки разделения с максимальными значениями чувствительности и специфичности. При поиске точек разделения мы в большей степени ориентировались на показатель специфичности теста, величина которого более 70% указывает на возможность использования тестов в клинической практике. Мы определили, что в качестве значимого теста (площадь под кривой >0,7) для диагностики субклинической ВМИ у беременных женщин может быть использовано определение - ММП - 8>17 нг/мл в цервикальной слизи (чувствительность теста - 80%; специфичность - 70,2%).We examined the diagnostic value of MMP-8 using ROC analysis and calculated the sensitivity and specificity of the test. Для подтверждения разработанного способа диагностики ВМИ во время беременности проспективно была набрана группа из 30 пациенток, которые имели не менее 3-х критериев согласно шкале прогноза ВМИ на сроке 14-24 недели. Пациенткам был произведен забор цервикальной слизи для определения уровня ММП-8. Диагностически значимый уровень ММП-8 (>17 нг/мл) был выявлен у 22 пациенток, у которых после родов при морфологическом исследовании последов ВМИ гистологически подтверждена у 82,7% пациенток. Среди 8 пациенток с низким уровнем ММП-8 (<17 нг/мл) воспалительные изменения в последе были обнаружены в одном случае (12,5%).To confirm the developed method for diagnosing IUI during pregnancy, a group of 30 patients was recruited prospectively, who had at least 3 criteria according to the IUI prognosis scale for a period of 14-24 weeks. Учитывая отсутствие единых референтных интервалов для предложенного нами иммунологического показателя, рекомендуется использовать количественный критерий, предложенный нами, не как точный диагностический тест, а как значимый прогностический признак, способный указывать на возможный воспалительный процесс в матке. Данный критерий необходимо использовать только с учетом клинических маркеров ВМИ и в совокупности с другими видами исследования. Учитывая, что понятие «нормы» в медицине является дискуссионной проблемой, мы рекомендуем использовать найденный нами показатель в качестве ориентира для диагностики субклинической ВМИ, вместе с тем принимая во внимание, что данный уровнь показателя в небольшом проценте случаев может встречаться и у здоровых беременных.Thus, as a significant test for the diagnosis of subclinical IUI in pregnant women in the second trimester, the determination of MMP-8 (>17 ng/ml) in cervical mucus can be used.
