Изобретение относится к области медицины, а именно к офтальмологии, и касается техники операции передней глубокой послойной кератопластики.The invention relates to medicine, namely to ophthalmology, and relates to the technique of the operation of anterior deep lamellar keratoplasty.
Десцеметоцеле - это патология, которая представляет собой выпячивание задней пограничной мембраны роговицы. Образование имеет небольшие размеры. Формирование десцеметоцеле происходит в месте дефекта роговицы, там, где другие ее слои разрушены вследствие действия определенных повреждающих факторов. Наиболее часто данная патология образуется как результат язв роговицы. Медикаментозному лечению десцеметоцеле не поддается. Для его устранения применяют хирургическое лечение. Основными методологическими подходами являются различные варианты кератопластики: конъюнктивальная, ламеллярная или пересадка донорского роговичного лоскута.Descemetocele is a pathology that is a bulging of the posterior bordering membrane of the cornea. Most often, this pathology is formed as a result of corneal ulcers. Descemetocele is not amenable to drug treatment. Surgical treatment is used to eliminate it. The main methodological approaches are various options for keratoplasty: conjunctival, lamellar or donor corneal flap transplant.
Ближайшим аналогом является способ послойной кератопластики при язве роговицы. В этом способе при помощи режущей коронки трепана необходимого диаметра, смоченной раствором бриллиантовой зелени, на роговицу, в пределах здоровой зоны, наносят круговую метку, скарификатором со дна язвы удаляют детрит, затем, начиная от центра язвы, по меридианам 12, 6 и 8 часов расслаивателем отсепаровывают передние слои роговицы в направлении трепанационной метки, формируя три тоннеля, роговичными ножницами, начиная от центра, производят радиальные разрезы передних стенок этих тоннелей в направлении к метке, расслаивателем последовательно окончательно отсепаровывают сформированные три сегмента пораженной роговицы от глубоких слоев, доходя до метки, по которой окончательно их иссекают, в сформированное ложе помещают послойный роговичный трансплантат необходимого диаметра и толщины, узловыми швами подшивают его к роговице реципиента (патент РФ на изобретение № 2604771).The closest analogue is the method of lamellar keratoplasty for corneal ulcers. Технический результат достигается тем, что в способе выполнения передней глубокой послойной кератопластики у пациентов с язвой роговицы, включающем деление пораженной ткани роговицы и отсепаровывание ее от центра к периферии, согласно изобретению, при формировании ложа донорского трансплантата у язвы роговицы, осложненной десцеметоцеле, роговицу реципиента при помощи хирургического маркера размечают по границе здоровой и пораженной ткани; в интактной зоне лимба делают парацентез на 9 или 3 часах; офтальмологическим циркулем определяют необходимый диаметр трепана, пораженную ткань роговицы, за исключением десцеметоцеле, делят на 4-6 усеченных секторов (секторальных участка), помечая их маркером, далее шпателем производят просечение всех секторальных участков на -2/3 их толщины параллельно десцеметовой мембране (ДМ), от центра к периферии, затем отсепаровывают ДМ от задних слоев стромы до границы зоны трепанации, после этого поочередно вводят шпатель под каждый секторальный участок роговицы и рассекают их по шпателю; во время всей операции используют интраоперационную систему оптической когерентной томографии (иОКТ). Роговицу реципиента под контролем иОКТ при помощи стерильного хирургического маркера размечают по границе здоровой и пораженной (изъязвленной, инфильтрированной) ткани. Стерильным офтальмологическим циркулем определяют необходимый диаметр трепана. Далее в интактной зоне лимба делают парацентез, необходимый для снижения давления в передней камере на 9 или 3 часах (в зависимости от локализации дефекта). Далее пораженную ткань роговицы делят на 4-6 секторальных участка, помечая их маркером. После отметки зоны трепанации и частичного просечения роговицы на -2/3, шпателем параллельно ДМ от центра к периферии производят отсепаровывание ДМ от задних слоев стромы до границы зоны трепанации. Далее шпатель заводят под каждый из секторальных участков и производят разрез вдоль шпателя. Удаление остаточных слоев стромы в пределах сформированного ложа для трансплантата осуществляют под контролем иОКТ, что существенно снижает риски перфорации ДМ и травмы эндотелия. В последующем проводят сквозную трепанацию донорского материала трепаном нужного диаметра, отделяют ДМ и эндотелий от стромы донорской роговицы. Сформированный трансплантат укладывают в подготовленное ложе в роговице реципиента и фиксируют непрерывным обвивным швом (при большом диаметре дефекта) или узловыми швами (при малом), используя нейлоновую нить 10-0.The recipient's cornea is marked along the border of healthy and diseased (ulcerated, infiltrated) tissue under iOCT control using a sterile surgical marker. Next, a spatula is inserted under each of the sectoral sections and an incision is made along the spatula. Removal of residual stroma layers within the formed graft bed is carried out under the control of iOCT, which significantly reduces the risks of DM perforation and endothelial injury. Subsequently, end-to-end trepanation of the donor material is carried out with trepan of the required diameter, the DM and endothelium are separated from the stroma of the donor cornea. The formed graft is placed in a prepared bed in the recipient's cornea and fixed with a continuous twisting suture (with a large defect diameter) or with interrupted sutures (with a small one) using a 10-0 nylon thread.

Пациент Л., 29 лет, обратилась в ФГБУ «НМИЦ «МНТК «МГ» с жалобами на дискомфорт и чувство инородного тела. Из анамнеза: рецидивирующие кератиты невыясненной этиологии на протяжении 5 лет. Острота зрения OS 0,3 sph +1,0 cyl -4,0 ах 25=1,0. Объективно: роговица прозрачна, локально в парацентральной зоне на 5 ч визуализируется язвенный дефект диаметром около 2,5 мм, в центре дефекта - десцеметоцеле. Передняя камера средней глубины, радужка структурна, хрусталик прозрачный, диск зрительного нерва бледно-розовый, границы четкие, макулярная область - без видимой патологии. Учитывая данные анамнеза (рецидивирующий кератит), дооперационной диагностики и объективного исследования - был выставлен диагноз: OS Язва роговицы. Десцеметоцеле. Хронический рецидивирующий кератит, стадия обострения. Хирургическое лечение на OS включало в себя глубокую переднюю послойную кератопластику с разделением пораженного участка на четыре секторальных участка, используя рассечение по шпателю в каждом из них под контролем иОКТ по предложенному способу.Patient L., 29 years old, applied to the FSBI NMITs MNTK MG with complaints of discomfort and feeling of a foreign body. The anterior chamber is of medium depth, the iris is structural, the lens is transparent, the optic nerve head is pale pink, the boundaries are clear, the macular region is without visible pathology. Taking into account the data of the anamnesis (recurrent keratitis), preoperative diagnosis and objective examination, the diagnosis was made: OS Corneal ulcer. Descemetocele. Chronic recurrent keratitis, exacerbation stage. Surgical treatment on OS included deep anterior lamellar keratoplasty with dividing the affected area into four sectoral areas using a spatula dissection in each of them under the control of iOCT according to the proposed method.

Со слов пациентки, жалобы появились после перенесенного инфекционного заболевания, получала лечение в стационаре. Острота зрения OD 0,5 sph -0,5 cyl -1,5 ах 160=1,0. Объективно: роговица прозрачна, локально в парацентральной зоне на 12 ч визуализируется язвенный дефект диаметром около 3,5 мм, в центре дефекта - десцеметоцеле. Передняя камера средней глубины, радужка структурна, хрусталик прозрачный, диск зрительного нерва бледно-розовый, границы четкие, макулярная область - без видимой патологии. Учитывая данные анамнеза, дооперационной диагностики и объективного исследования - был выставлен диагноз: OD Язва роговицы. Десцеметоцеле. Хирургическое лечение на OD включало в себя глубокую переднюю послойную кератопластику с разделением пораженного участка на шесть секторальных участков, используя рассечение по шпателю в каждом из них под контролем иОКТ по предложенному способу.Patient I., 35 years old, applied to the FGBI "NMITs" MNTK "MG" with complaints of discomfort and feeling of a foreign body. Заявленным способом прооперировано 3 больных с язвой роговицы различного генеза, осложненные десцеметоцеле. Ни в одном из случаев не наблюдались интра- или послеоперационные осложнения.The claimed method operated on 3 patients with corneal ulcers of various origins, complicated by descemetocele. No intra- or postoperative complications were observed in any of the cases.
