Abstract
Description
The invention relates to medicine, in particular to rehabilitation, and can be used in restorative medicine for the rehabilitation of children in the first years of life with the most frequent disorders of the nervous system and musculoskeletal system.
Currently, a large number of children are born with neurological or orthopedic pathology. Both medicinal and non-medicinal methods of rehabilitation of children in the first years of life with disorders of the nervous system and musculoskeletal system are known.
For the formation of motor activity, various massage options are used, which are not effective enough, since they are not able to activate all the available neurophysiological resources of the child, they do not purposefully form the motor functions characteristic of infancy.
In children of the first years of life, active exercises are used; when giving a certain position to the child's body, he is forced to perform a movement, for example, when laying on his stomach, he unbends the body and raises his head.
Passive exercises are when the second person performs any movement in the baby's joints.
Reflex exercises, when using reflex connections, one or another reflex is triggered in the child, and he makes movements (for example, the Bauer's crawling reflex). Massage, so often used in children, can also be attributed to the method of passive muscle gymnastics.
10.11.2007.).There is a known method for the formation of motor functions in infants with neurological or orthopedic pathologies, including the development of motor activity in a child through various positions and poses, and the child is held vertically with his back to the adult's chest, fixing his chest, and then the adult performs a set of physical exercises in the form of bends, turns and changes in the position of the child's body (see RF patent No. 2309720 according to IPC A61H 1/00, publ. 10.11.2007.).
20.02.2014.).There is also known a method for the rehabilitation of young children with the consequences of perinatal damage to the central nervous system, including a set of physical exercises that involve changing the position of the child's body with the help of an adult's hands, including, in particular, tilting and turning the child's body in different directions, swinging, lowering and raising the child's head downward, the child's lying on one and the other side, and during the course of such exposure, a course of physiotherapeutic procedures is also carried out (see RF patent No. 2 506 942 according to IPC A61H 1/00, publ. 20.02.2014.).
The known methods have common disadvantages.
When performing active and reflex exercises, the child's movement is uncontrollable, i.e. he performs the movement as best he can, and with the asymmetries of the musculoskeletal system that he has (and almost every child has them), these asymmetries become stronger and stronger in the child.
Passive movements, massage occur as if without the participation of the child himself, therefore they do not strengthen the muscles, do not stabilize a weak joint, for example, with dysplasia of the hip joint, and are performed, as a rule, on superficial long-arm muscles, and not on deep short-arm muscles. which are joint stabilizers.
The technical result is to simplify the method of influencing the muscles of the child's body, increasing their functionality with stable stabilization of weakened joints with obtaining a stable rehabilitation effect and increasing the number of rehabilitation cases with a positive result.
The technical result is achieved by the fact that the method of rehabilitation of children in the first years of life with disorders of the nervous system and musculoskeletal
Курс реабилитации повторяют с интервалом 1-3 месяца, до 5-7 курсов. apparatus, includes the performance of sessions within the framework of the rehabilitation course of passive physical exercises, acting on the short periarticular muscles participating in the stabilization of the joint in a counter response mode and repeated repeated at a slow pace of passive physical exercises, and before performing each passive physical exercise, light pressure is produced along the axis of the limb or the spine, which is preserved during the entire physical exercise, bringing the short periarticular muscles to the isometric mode, while the joint participating in the exercise is controlled by capturing and holding it in a physiological position during the entire exercise, the session is carried out for 40-60 minutes, daily , a course of at least 20 sessions with breaks for two days off, the number of repetitions of at least 30 of each physical exercise. The rehabilitation course is repeated at intervals of 1-3 months, up to 5-7 courses.
The proposed method is illustrated by a specific example.
Patient B., 5 months old, at the appointment with a rehabilitation therapist. On examination, there is a limitation of the right hip joint, rough asymmetry of the gluteal folds in height and length. According to ultrasound and X-ray studies, there are signs of dysplasia of the hip joint on the right (decentration of the femoral head, its hypoplasia, slope of the acetabulum).
The child received rehabilitation sessions of the rehabilitation course according to the proposed method, which included the implementation of passive physical exercises as part of the rehabilitation course, acting on the short periarticular muscles of the hip joint involved in stabilizing the joint in a counter response mode and repeated physical exercises at a slow pace.
The session was carried out for 50 minutes, daily, in a course of 22 sessions with breaks for two days off and with a repetition of 35 times of each physical exercise.
Against the background of the rehabilitation, positive dynamics was noted. The volume of motion in the right hip joint was fully restored, the asymmetry of the folds, which became equal in height and length, was corrected. According to the control X-ray examination, the femoral head was centered relative to the acetabulum, the ossification nuclei were equal in size on the left and right sides, the roof of the acetabulum became less sloping.
When rehabilitating children using the proposed method, while performing passive physical exercise, muscles are brought to isometric mode due to pressure along the axis of the limb or spine, which allows physical exercise to be performed on a statically tense muscle, leading to its strengthening, and therefore to stabilization of the joint to which it is attached.
The number of courses depends on the age of the patient and the complexity of the problem being solved. When performing passive exercises in the counter-response mode, they affect precisely the short periarticular muscles that are involved in stabilizing the joint, and not the long-arm muscles as in the usual passive exercise mode, this also leads to the strengthening of the joint, which is very important, for example, when it is weak , instability, dysplasia, subluxation or dislocation.
In the proposed method of rehabilitation, physical exercises are repeated many times and performed under conditions of the physiological position of the joint, which allows the brain to “remember” the correct position of the joint in case of deviations in its initial position and stabilize the joint in this position.
The proposed method affects not only the muscles of the child's body, increasing their functionality, but also forms the correct, symmetrical biomechanical work of the entire musculoskeletal system of the child as a single system.
