Abstract
Description
The invention relates to medicine, namely to neurology and rehabilitation, to the rehabilitation treatment of patients with cerebrovascular pathology.
To date, in order to improve cognitive functions after a stroke, restorative methods of active and passive physical exercises, ideomotor training (when the patient mentally imagines how he makes a movement), aimed at activating neuroplasticity, are actively used.
It is known the positive influence of the development of fine motor skills of the hand in speech therapy practice when working with patients with impaired speech function and the rehabilitation program should include physical exercises of varying degrees of intensity.
A known method of rehabilitation of patients in the early recovery period of cerebral stroke (RF Patent No. 2513418, IPC A61N 1/00, A61M 21/00, A61N 1/32, publ. 2014), during which complexes of physiotherapy exercises are used, including breathing exercises and exercises on relaxation with elements of auto-training, dynamic propriocorrection using a reflex-loading device, four- and six-channel electrical stimulation of the neuromuscular apparatus, exercises in front of a mirror, during which the patient with a healthy hand performs exercises in all joints of the upper limb and in all planes, observing the reflection in the mirror , mentally imagining that he performs the movements with a sore hand.
The disadvantage of this method is the presence of restrictions associated with the use in patients with cardiac arrhythmias, as well as the difficulty of using this method of rehabilitation in patients with epilepsy and paroxysmal diseases.
There is a method of complex rehabilitation of patients with post-stroke disorders in the early recovery period (RF Patent No. 2642950, IPC A61N 3/00, A61B 5/11, A61N 1/36, publ. 2018), including training on a stabilometric platform with biocontrol with feedback on the support reaction using visual and auditory canals in the training process by performing game tasks with a mark on the monitor and correcting body position by an audio signal using a metronome, multichannel electrical stimulation with bipolar impulse currents with the application of electrodes to the upper limbs on the middle third of the shoulder on the outside at the motor point of the radial nerve, on the middle third of the outer surface of the forearm, on the inner surface of the middle third of the shoulder in the projection of the ulnar and median nerves, on the inner surface of the forearm; on the lower limb in the projection of the peroneal nerve and the muscles innervated by it and in the projection of the tibial nerve and the muscles of the lower leg innervated by it.
The disadvantage of this method is insufficient efficiency, since active motor rehabilitation is not carried out, which increases the recovery time, in addition, this method also has limitations of use in patients with heart rhythm disturbances.
The closest is the method of rehabilitation of patients with post-stroke disorders (RF Patent No. 2688723, IPC А61Н 1/00, А61Н 23/00, А61М 21/00, publ. 2019), including exercises on hand movements, training on a stabilometric platform with a biological feedback using the visual and auditory canals in the training process, for which the patient, being on the stabilometric platform in front of the monitor and moving his body relative to the feet, must align his center of pressure shown to him on the screen in the form of a cursor with the target, keeping the center of pressure in the center target or moving it, with a simultaneous session with a speech therapist, in which the patient first comments on the actions performed by him on the stabilometric platform, and then the same session continues in a question-and-answer form, the general course is 8-10 procedures.
The disadvantage of this method is insufficient efficiency in patients with mild and moderate paresis, since it is recommended to carry out recovery actions with the upper limbs only in a passive mode, which increases the recovery period. The method is difficult to use in patients with paroxysmal conditions.
The objective of the proposed invention is to eliminate the indicated disadvantages, to increase the efficiency of the rehabilitation process due to interrelated mechanisms that potentiate the action of each other, to shorten the rehabilitation period.
The method includes exercises on hand movements, training on a stabilometric platform with biofeedback using the visual and auditory canals during training, for which the patient, being on the stabilometric platform in front of the monitor and moving his body relative to the feet, must combine his center of pressure, shown to him on the screen in the form of a cursor with a target, holding the center of pressure in the center of the target or moving it, with a simultaneous exercise with a speech therapist, in which the patient first comments on the actions performed by him on the stabilometric platform, and then the same exercise continues in a question-and-answer form, general course of 8-10 procedures. What is new is that classes begin on a stabilometric platform with a power joystick for the hand, while the patient alternately, first with a healthy and then a sick hand, applies pressure with the joystick on the stabilometric platform, holding the cursor on the target on the monitor screen, for 2-3 minutes on each hand with a repetition of 2-3 times. After this, classes continue in the same mode with closing the eye opposite to the working hand, after a break of 20-40 minutes, classes are carried out on a stabilometric platform holding the center of pressure in the center of the target or moving it, without a speech therapist and with a speech therapist. Then, after a break of 30-40 minutes, the patient performs gymnastic exercises: axial rotational movements of the trunk, rotational and flexion movements in the shoulder, elbow, wrist joints, first with two hands at the same time, then alternately with each hand separately, starting with a healthy hand, for 5-7 minutes , gradually increasing the time of the gymnastic exercises up to 15-20 minutes.
This method uses a biofeedback mechanism for the support response through the visual and sound channels, when the received sensory information is processed in the structures of the brain during training. At the same time, the cerebellum, its afferent and efferent connections, basal ganglia, and, ultimately, the motor cortex are turned on, since it is here that the final program of voluntary movement is formed. An important factor that determines the inclusion of late pre-adjustment, which is necessary to maintain balance and perform a directed movement by a patient on a stabilometric platform, is the presence of motor experience, an idea of the way to perform this movement. Therefore, with each subsequent session, it is already easier for the patient to perform the given exercise. Thus, the signal received through the sensory channels is converted into a motor response, and the feedback is working. In fact, the principle of "feedback" is carried out along the entire length of the motor path, with the help of various sensory switching, which is necessary for performing fast movements and for regulating slow movements and maintaining posture.
Thus, the work of several subsystems is simultaneously ensured, namely sensory (visual, auditory, vestibular, somatosensory afferentation), musculoskeletal (angle of motion in the joints, muscle spasticity, elasticity and flexibility of the spinal column, biomechanical interactions between segments of the spine and upper extremities) and the higher central part (motor memory). The motor act is corrected. Also, along with this, it seems possible to influence the training of cognitive functions after a stroke.
The method is carried out as follows.
The lesson begins with training the hands on a static stabilometric platform (ST-150 or A-150 with a power joystick for the hand) with biocontrol with feedback on the support reaction with a power joystick for the hands.
Special computer programs with biofeedback (BFB) are used, based on the visualization of the position of the target on the monitor screen by holding the cursor on it by the patient, holding the power joystick with his hand. Various sets of exercises on a stabilometric platform for hands to keep the cursor on the target have been developed and are practically used; the choice in favor of one of them is made depending on the tasks set, i.e. depending on the neurological deficit.
The patient should, clasping the power joystick with his hand, resting his elbow on the table, by changing the pressure of the hand, align the center of pressure shown to him on the screen in the form of a cursor with the target and move the cursor towards the target located in a certain area of the screen. The action on the joystick is carried out first with a healthy and then with a sick hand, alternately, for 2-3 minutes on each hand, repeated 2-3 times. After this, the exercises continue in the same mode with the closing of the eye opposite to the working hand. Closing the eye is carried out either by the currently idle hand or by a dark monocular bandage. In this case, the doctor can, by changing the scale, complicating or simplifying the task.
After the completion of the training, a rest of 20-40 minutes is given, depending on the patient's condition and concomitant pathology.
Further, training is carried out on a stabilometric platform with biofeedback with feedback on the support reaction using the visual and auditory canals during training by performing game tasks with a mark on the monitor and adjusting the position of the body by an audio signal, with your feet on the platform, without a speech therapist and with a speech therapist, in which the patient first comments on the actions performed by him on the stabilometric platform, and then the same exercise continues in a question-and-answer form. The objective of the exercise is to keep the center of pressure (CP) in the center of the target. The patient should, standing on a stabilometric platform or sitting on a chair in front of the monitor, resting his feet on the stabilometric platform, by moving the body relative to the feet, align his center of pressure, shown to him on the screen in the form of a cursor, with the target and move the target to a certain area of the screen or hold the CP in the center of the target.
At the beginning of training, the movements of a patient with cognitive impairments, as a rule, are excessive and require a large amount of energy. However, as cognitive function is restored, the patient will perform more accurate and timely movements, which will lead to a change in stabilometry characteristics.
During training on stabilometric platforms - for the hands and feet, various training tests can be used: "Target", "Shooting Range", "Flower", "Apple", "Sector", "Shooting on skeet", "Lights", "Ball + wall", "Skeet shooting in motion", etc., the principle of which is similar. The choice of the test to be performed is determined by the attending physician, depending on the severity of the patient's condition, the degree of neurological deficit, and the subjective tolerance of the training.
Then there is a break of 30-40 minutes. After that, the patient performs gymnastic exercises: axial rotational movements of the trunk, rotational and flexion movements in the shoulder, elbow, wrist joints, first with two hands at the same time, then alternately with each hand and separately, starting with a healthy hand, for 5-7 minutes, gradually increasing the time carrying out gymnastic exercises up to 15-20 minutes.
Be sure to monitor the heart rate and blood pressure before, during and after the end of the session. An increase or decrease in blood pressure values (against the usual for the patient), a violation of the heart rhythm, the appearance of subjective complaints of a worsening of the condition are the reason for the cancellation of classes. The general course of treatment is 8-10 procedures.
The criteria for the effectiveness of treatment are - clinical neurological examination, control of stabilometric indicators during and after the course of treatment.
