Abstract
Description
The invention relates to medicine, namely to dentistry, is intended for use in assessing regional blood flow in periodontal tissues using Doppler ultrasound.
- 57 с.).The Doppler ultrasound method uses the effect of changing the frequency of a reflected moving object of a signal by an amount proportional to the speed of the reflector, discovered in 1842 by Doppler. In the absence of movement of the studied medium, the Doppler signal does not exist, since the ultrasonic wave passes through the tissues without reflection, which makes this method of studying moving structures the most objective. The presence of a reflected signal indicates the presence of blood flow in the ultrasonic location area. The propagation and reflection of ultrasonic vibrations are two main processes on which the operation of all diagnostic ultrasound equipment is based. Ultrasonic Doppler ultrasonography in comparison with laser has a number of significant for the researcher indisputable advantages: sound and visual control of the installation of the sensor at the location point, the ability to determine the type of vessels (arterial or venous) according to the shape of the curve, and the distribution of blood particles with different velocities over the cross section of the studied vessels, assessment of the direction of blood flow. The fastest have a darker color and are farthest from the isoline. Slow particles run along the isoline and characterize the parietal region of the vessel. In contrast to a laser Doppler, the readings of which largely depend on the degree of pressing the emitter to the surface of the investigated area, in an ultrasonic Doppler, the clamp does not affect the measurement results, since it is practically not required to receive a signal - the acoustic contact is provided through the gel. We receive the maximum signal in sound and amplitude in a large vessel at an angle between the direction of blood flow and the sensor beam of 45-60 degrees (Artyushenko N.K., Girina M.B., Shalak O.V., Monastyrenko A.A., Akhlakova PM , Egorkina A.A.Ultrasound Doppler ultrasound of the vessels of the macro- and microcirculatory bed of the tissues of the oral cavity, face and neck .: Textbook. Edition 2e, revised and supplemented. - SPb .: SP Minimax, 2016. - 57 p.).
Известно средство для фиксации десневых электродов при проведении реопародонтографии, представляющее собой силиконовую оттискную стоматологическую массу с перфорациями (Патент РФ 2290862 от 10.01.2007).Known means for fixing gingival electrodes during rheoperodontography, which is a silicone impression dental mass with perforations (RF Patent 2290862 dated 01/10/2007).
Известна система электродов для определения регионарной гемодинамики глубоких сосудов альвеолярного отростка челюстей, состоящая из шести электродов, расположенных по касательной линии к внутренней поверхности индивидуального силиконового слепка исследуемого участка челюсти в шести сформированных на расстоянии 8 мм друг от друга отверстиях, три из которых располагаются на вестибулярной поверхности, а три других - напротив на противоположной (оральной) поверхности слепка, причем измерительный электрод расположен по центру между двумя токовыми электродами каждой поверхности слепка, что дает возможность увеличить чувствительность и точность получаемых результатов (Патент РФ 2546402 от 10.04.2015).A known system of electrodes for determining the regional hemodynamics of deep vessels of the alveolar process of the jaws, consisting of six electrodes located tangentially to the inner surface of an individual silicone impression of the investigated area of the jaw in six holes formed at a distance of 8 mm from each other, three of which are located on the vestibular surface , and the other three are opposite on the opposite (oral) surface of the impression, and the measuring electrode is located in the center between the two current electrodes of each surface of the impression, which makes it possible to increase the sensitivity and accuracy of the results (RF Patent 2546402 dated 04/10/2015).
Изобретение позволяет создать полноценное электрическое поле в зоне исследуемого участка межкорневой костной перегородки нижней и верхней челюстей с возможностью мониторинга и оценкой результативности лечения пародонтита (Патент РФ 2659130 от 28.06.2018).A device for assessing intraosseous blood flow in periodontal tissues is known from the prior art, which contains an individual positioner aligner, two measuring spherical electrodes and an apparatus for rheoperiodontography. These electrodes are made of cobalt alloy, are located opposite each other on opposite sides of the alveolar bone in the projection of the interdental bone septum with the possibility of regulating movement in an individual dental aligner-positioner fixed on the teeth. The individual dental aligner-positioner is made by vacuum heat-pressing 4 mm thick and consists of two combined layers of transparent polymer, an inner soft and an outer hard one. In the area of the study, there is a 2 mm gap between the soft tissues of the gums and the inner surface of the aligner-positioner to avoid pressure on the mucous membrane of the measured area. The electrodes fit snugly to the gum and are connected by soldering to a common flexible cable, into which the current wires of the tetrapolar system are also soldered behind the said wires of the measuring electrodes. The invention allows you to create a full-fledged electric field in the area of the investigated area of the inter-root bone septum of the lower and upper jaws with the possibility of monitoring and evaluating the effectiveness of the treatment of periodontitis (RF Patent 2659130 dated 06/28/2018).
The described device refers to rheoparodontography - a functional study of the intraosseous blood flow of deep vessels. The proposed device is applicable for ultrasound Doppler, used both for the study of intraosseous blood flow of the vessels of the interalveolar septum, and for the study of the superficial vessels of the gums.
Наиболее близким к предлагаемому изобретению можно отнести устройство для определения подвижности зуба, представляющее собой прозрачную капу, по форме соответствующую индивидуальным особенностям анатомии зубных рядов и альвеолярных челюстей пациента, отличающееся тем, что каппа изготовлена методом компьютерного прототипирования, а именно 3D-печати, выполнена из полимера, имеющего толщину 2 мм, и смоделирована с возможностью образования зазора по периметру между поверхностью капы с исследуемыми зубами, размер которого в 1,5 раза превосходит амплитуду подвижности зубов в вестибуло-оральном направлении; при этом в проекции центра клинического экватора вестибулярной поверхности исследуемых зубов в капе имеются сквозные отверстия, в вестибулярном направлении от которых отходят монолитно соединенные с каппой конусы, внутренние диаметры которых соответствуют наружным диаметрам сопла датчика измерительного прибора, длина полимерных конусов индивидуальна и определяется поперечными размерами измеряемых зубов (Патент РФ 2626372 от 26.07.2017).The closest to the proposed invention can be attributed to a device for determining the mobility of a tooth, which is a transparent mouthguard, in shape corresponding to the individual characteristics of the anatomy of the dentition and alveolar jaws of the patient, characterized in that the mouthguard is made by computer prototyping, namely 3D-printing, made of polymer , having a thickness of 2 mm, and is modeled with the possibility of forming a gap along the perimeter between the surface of the mouthguard with the studied teeth, the size of which is 1.5 times greater than the amplitude of tooth mobility in the vestibulo-oral direction; at the same time, in the projection of the center of the clinical equator of the vestibular surface of the teeth under study, there are through holes in the mouthpiece, in the vestibular direction from which the cones monolithically connected to the mouthpiece depart, the inner diameters of which correspond to the outer diameters of the nozzle of the measuring device sensor, the length of the polymer cones is individual and is determined by the transverse dimensions of the measured teeth (RF patent 2626372 dated July 26, 2017).
The problem to be solved by the invention is a dynamic representative assessment of regional blood flow in periodontal tissues using Doppler ultrasound.
The technical result of the invention is to conduct ultrasound Doppler ultrasonography of periodontal vessels using an individual navigation device that allows you to obtain representative data at all stages of the treatment.
The technical result of the invention is achieved due to the fact that the device for monitoring the hemodynamics of periodontal tissues is made monolithically from a dental polymer by the additive method of 3D printing and is a two-jaw mouthguard that overlaps the teeth and marginal gums to the level of the transitional fold and has a horizontal platform with imprints in its design teeth of the upper and lower dentition in the position of the central occlusion separated by 3 mm, while between teeth 16, 21, 25, 36, 41, 45 on all surfaces of the mucous membrane of their interrootal alveolar bone to the level of the transitional fold and the splint there is a gap of 1.5 mm, and on the outer surface of the device in the projection of the inter-root alveolar septa there are conical holes with a diameter of the inner surface of the mouthguard corresponding to the diameter of the Doppler sensor, and the outer radius of the conical opening corresponds to the vector of movement of the Doppler sensor inside the mouthguard at 60 degrees, when This device has a handle in the anterior region for positioning it in the oral cavity.
The proposed device allows with maximum representativeness to monitor regional blood flow in periodontal tissues using Doppler ultrasound at various periods of observation of the patient.
Tapered holes in the device allow for high precision examination at angles recommended by the manufacturer in the range from 25 to 60 degrees.
The gap between the teeth tray and the mucous membrane of the alveolar bone of 1.5 mm prevents tissue ishimization in the area of the investigated tissues, ensuring the purity of Doppler sonography. Also, the accuracy of the study is ensured by the method of computer modeling and production used in production technology, namely, obtaining contactless optical impressions.
