Abstract
Description
Предлагаемый способ относится к медицине, в частности к хирургии, и может быть применен для дифференциальной диагностики содержимого жидкостных образований поджелудочной железы и определения дальнейшей тактики лечения пациентов. Метод заключается в том, что больным с кистами поджелудочной железы выполняют чрескожное наружное дренирование под контролем ультразвукового исследования, после чего проводят электрохимическое исследование полученного содержимого.The proposed method relates to medicine, in particular to surgery, and can be used for differential diagnosis of the contents of fluid formations of the pancreas and for determining further tactics for treating patients. The method consists in the fact that patients with pancreatic cysts perform percutaneous external drainage under the control of ultrasound, after which an electrochemical study of the contents obtained is performed.
Кистозные образования поджелудочной железы являются наиболее частыми осложнениями деструктивных форм острого панкреатита, заболеваемость которым растет с каждым годом. Наличие кист поджелудочной железы у пациента определяет показания к выполнению того или иного вида оперативного вмешательства, выбор которого напрямую зависит от наличия связи кисты с протоковой системой поджелудочной железы. Такие методы диагностики как магнитно-резонансная томография, компьютерная томография, ультразвуковое исследование далеко не всегда дают необходимую информацию о связи с протоковой системой поджелудочной железы, что влечет за собой неправильный выбор тактики лечения пациентов.Cystic formations of the pancreas are the most frequent complications of destructive forms of acute pancreatitis, the incidence of which is increasing every year. The presence of pancreatic cysts in a patient determines the indications for performing one or another type of surgical intervention, the choice of which directly depends on the presence of a cyst connection with the pancreatic duct system. Diagnostic methods such as magnetic resonance imaging, computed tomography, and ultrasound do not always provide the necessary information about the connection with the pancreatic duct system, which entails the wrong choice of treatment tactics for patients.
Известен способ диагностики кист поджелудочной железы. Под ультразвуковым контролем выполняют прокол мягких тканей брюшной стенки и полости кисты полым элементом и подсоединяют к нему промывную систему с введением и выведением жидкости из полости кисты. При этом через прокалывающий полый элемент в полость кисты вводят оптический элемент и через него после заполнения жидкостью полости кисты диагностируют наличие каналов сообщения полости кисты с другой кистозной полостью или протоком поджелудочной железы, определяя характер канала сообщения и содержимого сообщающейся полости [1]. Недостатками существующего способа являются: сложность выполнения, низкая диагностическая ценность при связи кисты с мелкими ветвями протоковой системой поджелудочной железы, высокий риск осложнений при проведении исследования, труднодоступность способа в связи с необходимостью использования дорогостоящего оптического элемента с высокой разрешающей способностью.The disadvantages of the existing method are: the complexity of implementation, low diagnostic value when the cyst is connected with small branches of the pancreatic duct system, a high risk of complications during the study, the inaccessibility of the method due to the need to use an expensive optical element with high resolution.

Сущность предлагаемого способа заключается в том, что определяют электрохимические свойства содержимого кист поджелудочной железы методом джоульметрии. Согласно предлагаемому способу с помощью устройства для диагностики состояния биологических объектов осуществляют оценку вольтамперных характеристик содержимого кист поджелудочной железы и составление кривых зависимости работы тока от времени для каждого образца. Полученное при чрескожном наружном дренировании под контролем ультразвукового исследования содержимое кист поджелудочной вводят в жидкостной проточный электрод джоульметрического прибора, где за короткий промежуток времени на него действует ток различной силы, определяют зависимость работы тока от времени для каждого образца, оценивают динамику вольтамперных характеристик: при работе тока от 9,6 до 18,3 мкДж (в среднем 12,47±2,66 мкДж) отмечают наличие связи кисты поджелудочной железы с протоковой системой; при работе тока в диапазоне от 0,92 до 4,25 мкДж (в среднем 2,52±1,03 мкДж) связи кисты поджелудочной железы с протоковой системой нет. На основании полученных данных можно сделать вывод о связи кист поджелудочной железы с протоковой системой. The contents of the pancreatic cysts obtained during percutaneous external drainage under the control of ultrasound examination are injected into the liquid flow electrode of a joulemetric device, where a current of different strength acts on it in a short period of time, the dependence of the current work on time for each sample is determined, the dynamics of the current-voltage characteristics is estimated: from 9.6 to 18.3 μJ (on average 12.47 ± 2.66 μJ), the presence of a connection between the pancreatic cyst and the duct system is noted; when the current operates in the range from 0.92 to 4.25 μJ (on average 2.52 ± 1.03 μJ), there is no connection between the pancreatic cyst and the duct system. Based on the data obtained, it can be concluded that the cysts of the pancreas are connected with the duct system.
Такое сочетание новых признаков с уже известными позволяет достоверно оценить электрохимические свойства содержимого кист поджелудочной железы на основании изменения джоульметрических параметров за короткий промежуток времени и определить дальнейшую тактику лечения пациентов с данной патологией. Способ осуществляется следующим образом. Полученное при наружном дренировании содержимое кисты поджелудочной железы больного берется в количестве 10 мл. Затем содержимое кист поджелудочной железы вводят внутрь жидкостного проточного электрода джоульметрического прибора. За время 8 секунд через проточный жидкостной электрод джоульметрического прибора пропускается ток 0,01, 0,04, 0,08 и 0,1 мА. В программе IPC 2000 оцениваются полученные зависимости, которые представляют собой кривые со строго определенными значениями изменения потенциала во времени, кроме того прослеживается динамика изменения вольтамперных характеристик содержимого кист поджелудочной железы в разные периоды лечения больных. По полученным зависимостям рассчитывается работа для каждого значения тока, строятся графики, позволяющие судить о связи с протоковой системой.The method is carried out as follows. Затем, в процессе лечения исследуемых больных, отбирали 1 группу пациентов с доказанной связью кисты поджелудочной железы с протоковой системой (по данным фистулографии или по интраоперационным данным) и 2 группу пациентов, у которых связь кисты поджелудочной железы с протоковой системой не была установлена. Сопоставляя результаты электрохимического исследования с полученными данными, обнаружили, что наличие связи кисты поджелудочной железы с протоковой системой отмечается при работе тока от 9,6 до 18,3 мкДж (в среднем 12,47±2,66 мкДж); отсутствие связи кисты поджелудочной железы с протоковой системой – при работе тока в диапазоне от 0,92 до 4,25 мкДж (в среднем 2,52±1,03 мкДж).The data obtained were saved. В последнее время присоединилось чувство тяжести в желудке и тошнота поле приема пищи. 07.02.2019 обратилась к хирургу по месту жительства, обследована. При УЗИ: кита хвоста поджелудочной железы диаметром 8 см.
