Данная методика предусматривает нанесение аппликатов в шахматном порядке по всей поверхности РШ и в виде барьера по периферии РШ, что оказывает значительное травмирующее воздействие на сетчатку с риском возникновения осложнений (в частности, эпиретинального макулярного фиброза, снижения остроты зрения), а также прогрессированию уже имеющихся патологий. К тому же необходимость нанесения большого количества аппликатов увеличивает время процедуры и повышает интенсивность нежелательного энергетического воздействия лазерной энергии на внутриглазные структуры. Параметры нанесения аппликатов в один паттерн 2×2 с расстоянием между аппликатами 0,25 диаметра аппликата были определены эмпирическим путем и являются оптимальными для получения надежной хориоретинальной спайки, ускорения резорбции интраретинальной жидкости и прилегания РШ. При контактной биомикроскопии периферии глазного дна выявлен плоский распространенный РШ без четкой границы, захватывающий нижненаружный сегмент крайней периферии, зоны экватора и частично средней периферии. Пациентке была выполнена лазеркоагуляция сетчатки на НЛУ Navilas 577s с длиной волны 577 нм со следующими параметрами: мощность - 240 мВт, длительность импульса - 20 мсек, диаметр пятна 215 мкм. Был разработан план лечения, состоящий из нанесения лазерных аппликатов вдоль сосудов по обе стороны сосуда от центра РШ к периферии до бессосудистой зоны, т.е. на всем протяжении визуализации сосуда.
