.,, j,-.-.,,. .,,.. ,„..,. „. . ., . ,, j, -.-. ,,. . ,, .., „..,. „. . .,. PROBE FOR KISH DECOMPRESSION
The utility model relates to medicine, namely to abdominal surgery, and serves to decompress the intestines and remove stagnant contents.
When performing operations on the abdominal organs in the presence of stagnant contents in the intestinal lumen by a surgeon (assistant), probes of various designs are used to aspirate the latter. At present, many probes have been developed: Baker type with an inflated cuff, an intestinal aspirator, a device for decompression (Gataullin N.G., Baykov B.A., 1972); probe Yu.L. Shalkova (Zaitsev V.T. et al., 1977); antireflux probe Freson (987). All currently available medical probes for intestinal intubation (catalogs of instruments of the Vorsma, Nizhny Novgorod, Kazan Medical Instrument Plant) have the following disadvantages: an increase in the duration of the operation due to the coordination of the work of the anesthesiologist and surgeon, multi-stage, and trauma when using the probe.
The prototype of the proposed device is a double-lumen probe (Tamazashvili T.Sh. On the issue of simultaneous decompression of various sections of the digestive tract. // Klin, surgery.-1986.- No. 8, -s.7778). The use of the prototype is associated with trauma during intubation and multi-stage aspiration of congestive intestinal discharge,
TECHNICAL RESULT - expanding the technical capabilities of the device, the possibility of decompression of the intestine with the least tissue trauma and the simultaneous removal of stagnant intestinal contents, reducing the duration of the operation. IPC A 61 M 25/00 TEXTBOOK.
The device proposed for use is a double-lumen PVC or silicone tube 1 or 2, 2.0 m long and 8 and 15 mm in diameter, respectively. At the distal, working end, the probe has holes 3 with a diameter of 4 mm, located on the inner and outer tube. Inside the probe along one of the walls there is an additional third tube 4 with a diameter of 2 mm, inside of which there passes a braided steel cable 5 with a diameter of 1.8 mm, ending outside the tube with olive 6 with a diameter of 12 mm. At the proximal end of the probe, a steel cable 5 is connected to a steel wire 7 with a diameter of 1.8 cm and a length of 15 cm, which is located outside of the tube 4 and ends with a ring 8. Due to the design features of the probe, it moves portionwise along the intestine and simultaneously sucks the stagnant intestinal contents .
1,2).The proposed device is a double-lumen tube, in the distal working part it has holes with a diameter of 4 mm, in the inner tube there is an additional third tube in which there is a steel cable, in the proximal part outside the tube it is connected to a steel wire with a ring, and in the distal part it ends in olive with a diameter of 12 mm (Fig. 1.2).
The proposed device is used as follows: during surgery, this probe is carried out by an anesthetist through the mouth, esophagus, stomach and duodenum. To advance the probe through the gut, the anesthesiologist, using the ring 7 and steel wire 6, moves the steel cable 4 and olive 5 along the gut 10-15 cm, then, as the conductor passes through the steel wire 6 and cable 4, probe 1 is distal to the intestine, at the same time aspiration takes place stagnant intestinal contents. After liberation from the contents in this section of the intestine, the probe is advanced according to the method described above to its distal parts. CLINICAL RESULT.
