A method for predicting placental insufficiency in the second half of pregnancy in seropositive women to cytomegalovirus with the threat of miscarriage early terms
Scope: the invention relates to the field of medicine, namely, to the problem of creating new ways of predicting placental insufficiency in seropositive women to cytomegalovirus with the threat of miscarriage of early terms.
The level of technology
[6].The significance of the study is determined by the high frequency of gestational complications associated with cytomegalovirus infection. These include not only reproductive losses caused by miscarriage (15–25% of the general statistics of early miscarriages, 60% of late miscarriages), but also perinatal pathologies arising in the antenatal period [6].
One of the main reasons for the high frequency of perinatal pathologies in cytomegalovirus infection are placental disorders [12], which leads to the development of hypoxia, intrauterine growth retardation of the fetus with impaired function and structure of vital organs [4]. The lack of a unified clinical approach to the diagnosis of placental insufficiency, including associated with cytomegalovirus infection, dictates the need to create highly accurate methods for predicting the complications of the gestational period in order to timely prevent perinatal morbidity and mortality.
[2].Known methods for predicting the development of placental insufficiency in the second half of pregnancy, based on the determination in serum concentrations of DcR3 [1], the content of molecules of average weight [5], the content of interleukin 6 (IL-6), interleukin 8 (IL-8), soluble vascular intercellular adhesion molecules (sVCAM), vascular endothelial growth factor (VEGF) and big-endothelin, followed by calculating the prognostic index [2].
The above factors, both in isolation and in combination with each other, may indicate a violation of the formation of the placental complex. In this regard, the methods that allow for preclinical prediction of threatening conditions of pregnancy associated with the development of placental insufficiency, including infectious genesis, are of the greatest diagnostic value.
[1, 5].Analogs of this invention are the above methods for predicting placental insufficiency in the second half of pregnancy, the main disadvantages of which are the complexity [2] and nonspecificity [1, 5].
The prototype is a method for predicting placental insufficiency based on determining the serum VEGF content in women in the second and third trimesters of pregnancy [7], the main disadvantages of which are:
- метод позволяет прогнозировать плацентарную недостаточность и ее клиническое проявление - синдром задержки развития плода только в том триместре, в котором проводится исследование содержания VEGF в сыворотке крови;- the method allows to predict placental insufficiency and its clinical manifestation - fetal growth retardation syndrome only in the trimester in which the serum VEGF test is conducted;
- отсутствует интеграция показателей ангиогенного фактора и его растворимого рецептора с инфекционной патологией и связанного с ним угрожающего исхода беременности ранних сроков;- there is no integration of indicators of the angiogenic factor and its soluble receptor with infectious diseases and the associated threatening outcome of early pregnancy;
- отсутствуют данные о точности способа.- there is no data on the accuracy of the method.
These factors can be eliminated in the claimed method.
The technical result is to improve the accuracy of prediction of placental insufficiency in the second half of pregnancy in seropositive women to cytomegalovirus with the threat of miscarriage of early periods for timely treatment and the development of optimal management tactics for pregnant women.
90,9%.The claimed technical result is achieved by the fact that in women with identified cytomegalovirus infection in the first trimester of pregnancy (6-12 weeks) who had clinical signs of threatened abortion, the level of IgM antibodies to cytomegalovirus is determined in the serum, the content of VEGF and VEGFR1 is additionally calculated by their ratio and when the level of IgM antibodies to cytomegalovirus 2.0 U / ml and more and the ratio of VEGF / sVEGF-R1 to 7.46 or less, placental insufficiency in the second half of pregnancy is predicted to an accuracy of 90.9%.
The novelty of the proposed method lies in the fact that for the first time it is proposed to predict the development of placental insufficiency in the second half of pregnancy in seropositive women to cytomegalovirus with the threat of miscarriage of early terms on the level of IgM antibodies to cytomegalovirus in the serum and the ratio of VEGF / sVEGF-R1. Previously, the combination of these signs in the prediction of placental insufficiency was not used.
[3].Studies show that VEGF is a major angiogenic factor expressed by trophoblast cells, which provides for differentiation, migration and proliferation of endothelial cells, the formation of a new uteroplacental vascular network [13]. VEGF increases vascular permeability by promoting extravasation with plasma proteins and forming a window for the migration of endothelial cells. Changes in the balance of VEGF in the microenvironment of endothelial cells underlie pathological disorders of placentation, gestation at different periods of pregnancy and preterm birth [8]. The decrease in its content leads to a violation of the vascular development of the placenta, resulting in fetal hypoxia [10] and the delay in its development [3].
[11].VEGF exerts its effect through the VEGF-R2 receptor, and the exact purpose of VEGF-R1 remains a matter of controversy. Some people believe that VEGF-R1 is a kind of “trap” for preventing the binding of VEGF to VEGF-R2 [9], which blocks the further development of the vascular bed and the formation of vascularized tissues of the placenta [11].
However, there are no data on the integration of infection activity indicators recorded by the level of IgM antibodies to cytomegalovirus and the ratio of VEGF / sVEGF-R1 in seropositive women to cytomegalovirus with the threat of miscarriage of early terms in the development of placental insufficiency.
The foregoing was the basis for a seropositive women serum test for cytomegalovirus with the threat of miscarriage of the early periods of VEGF and sVEGF-R1 for the prediction of placental insufficiency in the second half of pregnancy. Our research allowed us to establish the magnitude of the ratio of VEGF / sVEGF-R1, which is crucial for the selection of women at high risk for the development of placental insufficiency in the second half of pregnancy during cytomegalovirus infection.

In pregnant women with detected cytomegalovirus infection in the first trimester, with clinical signs of threatened miscarriage of early periods, blood serum obtained from whole blood taken from the cubital vein in the morning on an empty stomach in an amount of 5 ml is examined. Determination of the level of IgM antibodies to cytomegalovirus and VEGF and sVEGF-R1 parameters are carried out by enzyme immunoassay using standard ELISA-CMV-IgM-BEST test systems (Russia), VEGF-IFA-BEST (Russia) and Human sVEGF-R1 / Flt- 1 Quantikine ELISA Kit (USA). Accounting for the reaction is carried out on an immunoassay analyzer Stat Fax-2100 (USA) according to the protocols for the production of enzyme immunoassay from the manufacturer in serum.
When the level of IgM antibodies to CMV 2.0 U / ml and more and the value of the ratio of VEGF / sVEGF-R1 of 7.46 and less, placental insufficiency is predicted in the second half of pregnancy.
Using the method with high accuracy (90.9%) allows to predict placental insufficiency in the second half of pregnancy in seropositive women to cytomegalovirus with the threat of miscarriage early terms, from 6 weeks of pregnancy.
Distinctive features of this method are: in the early stages of pregnancy (6-12 weeks), prognostic values of the level of IgM antibodies to cytomegalovirus and the ratio of VEGF / sVEGF-R1 are established, with a value of 2.0 U / ml or more and 7.46 and less, accordingly, placental insufficiency is predicted in the second half of pregnancy.
