Abstract
Description
Recently, around the world, work is actively underway to study the therapeutic and prophylactic properties of grape wines. The results of numerous studies indicate the positive role of grape wines in the prevention of stress, atherosclerotic lesions of blood vessels and heart attacks, which play a significant role in the pathogenesis of coronary heart disease (CHD). As a result of multicenter epidemiological studies, it was found that moderate consumption of grape wine (from 200 to 600 ml of table wine per day) significantly reduces the risk of mortality in cardiovascular diseases.
A method of rehabilitation of patients with coronary heart disease relates to medicine, namely to physiotherapy and balneology, and can be used for rehabilitation treatment of patients with coronary artery disease in resort and out-of-resort practice in the treatment of coronary artery disease, functional angina pectoris. Despite the significant success achieved during treatment and medical rehabilitation, the treatment of coronary heart disease remains a difficult task, which is associated with the need for a comprehensive effect on pathogenetic factors. Drug therapy is characterized by insufficient effectiveness and the development of adverse reactions in the form of individual intolerance to drugs, allergic and adverse reactions, which significantly reduces the effectiveness of treatment. These disadvantages of drug therapy dictate the need to search for new effects that have a broad pathogenetic orientation and a smaller spectrum of side effects. Physical healing factors meet these requirements. Traditionally, the structure of the rehabilitation treatment of coronary artery disease consists of techniques that are based on the use of natural resources (climate and balneotherapy), kinesitherapy and physiotherapy. In recent years, the arsenal of methods has been supplemented with biologically active substances (BAS) of plant origin as part of diet, aroma and herbal medicine. The most attention is drawn to biologically active substances with antioxidant effect. These compounds take part in the regulation of oxygen-dependent energy exchange and the activity of redox metabolic reactions, counteracting the negative consequences of free-radical oxidation reactions on the structure of substances and biological membranes.
As a prototype, a method for the rehabilitation of patients with coronary heart disease was selected (Patent No. 2204403, Russian Federation, IPC A61K 35/78, A61P 9/00. Means and method of treatment of post-infarction cardiosclerosis / Korsun V.F .; Chuyko T.V .; Korsun E.V. ; Zakharov Yu.A. - Application 2002100053/14. Publish. May 20, 2003), which includes the use of climatotherapy, medical gymnastics, outdoor health food, diet with reduced cholesterol, salt and carbohydrates and with a high content of vitamins and proteins of plant origin, as well as the use of drug collection plants that have a diuretic, anti-sclerotic and sedative effect.
В основу заявленного способа положена задача усовершенствования способа-прототипа путем дополнительного применения красного столового вина «Каберне» в качестве препарата растительного происхождения, который имеет высокую биологическую активность, которая позволяет достигнуть ожидаемый технический результат, а именно повысить эффективность восстановительного лечения пациентов с ИБС и, соответственно, повысить качество жизни и восстановить (уменьшить нарушение) функции данных пациентов по критериям «Международной классификации функционирования, ограничений жизнедеятельности и здоровья» (МКФ).Signs that coincide with the main features of the prototype are: the use of climatotherapy, medical gymnastics, outdoor exercise, diet food and a herbal preparation. Signs that impede the achievement of the expected result (increasing the adaptive capacity of the body and reducing stress, improving the rehabilitation of patients) are: a low level of polyphenols in the applied collection of medicinal plants and, accordingly, insufficient antioxidant therapeutic effect, which would improve blood circulation, blood coagulation properties and lipid metabolism. The basis of the claimed method is the task of improving the prototype method by the additional use of red table wine "Cabernet" as a herbal preparation, which has high biological activity, which allows to achieve the expected technical result, namely to increase the effectiveness of rehabilitation treatment of patients with coronary artery disease and, accordingly , improve the quality of life and restore (reduce impairment) the functions of these patients according to the criteria of the "International Classification f functions, disability and health ”(ICF).
The problem is solved in that in the method of rehabilitation of patients with coronary heart disease, which includes the use of climatotherapy, therapeutic exercises, outdoor health paths, diet food and the use of a herbal preparation, red table wine Cabernet is prescribed as a herbal preparation once in a dose of 200 ml, 5-10 minutes after eating, while the course is 14-21 days.
Between the set of essential features of the claimed method and the technical result that is achieved, the following causal relationship is identified: the additional use of red table wine “Cabernet”, which has an adaptogenic and antioxidant effect, allows to achieve higher efficiency of medical rehabilitation and reduce impaired function, estimated according to the criteria of the "International Classification of Functioning, Disability and Health."
Table wine “Cabernet” (CB) is produced according to the classical technology of fermentation of pulp with a submersible “cap” from the Cabernet Sauvignon grape variety (clone C337 / S04C3). The alcohol content in wine was 10.4% of the volume, the content of titrated acids was 8.7 g / dm 3 , the mass concentration of total phenolic substances was 1717 mg / dm3, incl. anthocyanins - 393 mg / dm3 and procyanidins - 936.0 mg / dm 3 . The advantage of CB is that it provides easy assimilation by the human body of the whole complex of biologically valuable grape polyphenols in soluble form.
Polyphenolic compounds of grapes are among the most active bioantioxidants, which are an integral component of all cellular and tissue structures of living organisms, where they maintain a constant low level of free radical oxidation. Polyphenols in the human body are not synthesized; they come only with plant foods. Therefore, in the organisms of living beings, their metabolism and replenishment are balanced. The disappearance from the diet or from the body structures of any bioantioxidant leads to serious metabolic disorders, which are characterized by a wide nonspecific effect on the pathogenesis of many diseases. When assessing the severity of free-radical oxidation processes, i.e. oxidative stress, first of all, it is necessary to determine the risk factors for antioxidant deficiency. Alimentary risk factors for antioxidant deficiency are associated both with insufficient intake of antioxidants from food (ascorbic acid, vitamin A and E, beta-carotene, as well as polyphenols), and with excessive intake of prooxidants from food.
Polyphenols are an important group of phytocompounds, which are secondary metabolites of plants. Grape polyphenols are one of the most active bioantioxidants. Polyphenols are one of the many groups of natural phenolic compounds united by the general structure of C6-C3-C6. Currently, more than 8,000 polyphenolic compounds synthesized by plants are known. Grape wines, like no other drinks, are rich in phenolic compounds - monomeric, oligo - and polymeric. The highest concentration of polyphenols and flavonoids is found in the peel and seeds of grape berries. It is from them that these substances pass into wine; therefore, the technology for preparing red wine ensures its great biological value in terms of the content of polyphenolic compounds. The daily requirement of an adult for the total consumption of polyphenols (including phenolic acids, flavonoids, tannins and stilbenes) is about 1 g. Functional food products are defined as products that not only provide the body with energy and plastic material, but also affect certain physiological functions, biochemical and behavioral responses that help maintain good health, reduce the risk of illness, and speed up the healing process. National standard of the Russian Federation Functional food products GOST R 52349-2005 defines a natural functional food product as a functional food product consumed in processed form, containing natural functional food ingredients of the original plant and (or) animal raw materials in an amount corresponding to one servings of the product at least 15% of the daily requirement. The consumption of a single dose of 200 ml of red table wine more than 30% satisfies the daily need of the human body for polyphenols and flavonoids.
The method is as follows. Patients with coronary heart disease during the period of sanatorium-resort adaptation are prescribed climatotherapy, physiotherapy exercises. A diet with a reduced content of cholesterol, salt, carbohydrates and with a high content of vitamins and proteins of plant origin is also prescribed. The main treatment package also includes taking Cabernet red table wine once in a dose of 200 ml as a herbal preparation. The dose is taken 5-10 minutes after eating. The course of taking wine is 14-21 days. It was noted that taking a dose of the drug before or during meals did not lead to significant changes in the quality of life and restoration of functions of patients with coronary artery disease according to the criteria of the International Classification of Functioning, Functioning, Disability and Health ICF.

When studying the effect of red table wine “Cabernet” (CB), the comparison of the obtained data was carried out between two groups: “A” - the main one - with the use of CB against the background of complex treatment (21 patients); "B" - comparisons - with the use of complex treatment without the use of SV (67 patients). The average values of the initial parameters of both groups did not differ significantly. 17 patients had a habit of regularly drinking alcoholic beverages (on average, they consumed 6.96 ml of alcohol per day).
- 30 с. A comprehensive examination was carried out before and at the end of the course of sanatorium-resort rehabilitation. Assessment of the effectiveness of medical rehabilitation was carried out in accordance with the "Methodology for assessing the effectiveness of health resort medical rehabilitation for diseases of the cardio-respiratory system using the criteria of the" International Classification of Functioning, Disability and Health. " Guidelines. - Yalta, 2017 .-- 30 s.
All patients received an individually indicated complex of sanatorium-resort rehabilitation treatment, which will include the following procedures (on average for one treated): air baths - 13.0; sunbathing - 6.8; sea bathing and bathing in a pool with sea water - 6.7; Exercise therapy 18.4; massage - 7.6; inhalations and other types of aerotherapy - 4.9; hydropathy (healing baths and showers) - 7.9; electrotherapy (galvanization, electrophoresis and other types of currents) 7.1; hardware light therapy - 1.8; other physiotherapy methods - 8.8. Patients also received maintenance therapy, on average, but 51 daily doses of medication during the treatment period in the sanatorium.
In group A, in addition to the individually indicated rehabilitation complex, Cabernet red table wine (CB) was included in the diet of patients. SV intake was carried out once, after dinner, daily doses were 200 ml of wine (i.e. 20.8 ml of alcohol). On average, in the main group A, the exchange rates were 3447.619 (+105.021) ml of wine, taken during 17.238 (+0.525) procedures.
Evaluation of the effectiveness of sanatorium-resort rehabilitation was carried out by us on a generally accepted scale (significant improvement, improvement, without improvement and worsening), the opinion of the attending physicians and the patients themselves about the positive or negative effect of SV was additionally noted.
The research results were analyzed using methods of variation statistics using a standard computer program for mathematical analysis (Microsoft Excel). Reliable differences (p <0.05) of the studied parameters and their dynamics, as well as reliable values (p <0.05) of the correlation coefficients (r) of wine doses with the studied patient parameters, were taken as a criterion for assessing the effects of wine. The statistical nature of the distribution of the data obtained in both groups of the studied patients is close to normal.
As a result of the sanatorium-resort rehabilitation treatment, the overwhelming majority of patients showed a positive dynamics in their health status. During treatment with SV, a larger number of parameters changed for the better in group A patients, than in group B. Figure 1 shows the average values of those parameters for which a significant difference was established between the two studied groups, which indicates the influence of SW.
Note:! - indicates a positive effect of wine on the effectiveness of rehabilitation; ? - indicates the negative impact of wine on the effectiveness of rehabilitation.
2.For the purpose of a more lethal analysis of the influence of SW, a correlation analysis was performed. Statistically significant (at p <0.05) pair correlation coefficients r are included in the table. 2.
Note:! - indicates a positive effect of wine on the effectiveness of rehabilitation; ? - indicates the negative impact of wine on the effectiveness of rehabilitation.
The data obtained indicate that the intake of SV positively affects the dynamics of a number of functions according to the ICF criteria. Only one case of negative side effects of wine was noted - in 1 patient there were complaints of a feeling of heaviness in the epigastrium.
Comparison of the revealed effects of wine showed that the greatest positive effect is associated, above all, with more successful resistance to stress. First, the quality of life index, as an integral self-assessment by patients of the severity of stress, in the main group by the end of the course of treatment is significantly lower than in the comparison group. Secondly, all reactions of adaptation and stress according to Garkavi and Reader decrease under the influence of wine. Thirdly, taking SV also reduces the desire to drink strong alcoholic beverages.
The proposed method for the rehabilitation of patients with coronary heart disease can significantly reduce dysfunction and improve the quality of life of the patient according to 2 ICF domains - b 2401 “Dizziness” and b 280 “Sensation of pain” - and the quality of life index, and also allows you to positively affect the dynamics of functions 4 ICF domains.
A method for the rehabilitation of patients with coronary heart disease is aimed at increasing the effectiveness of medical rehabilitation and can be applied in sanatorium-resort and rehabilitation institutions.
