Abstract
Description
The invention relates to medicine, specifically to therapeutic dentistry, can be used for topical treatment of lichen planus of the oral mucosa of a typical form.
- 320 с.;].Red lichen planus of the oral mucosa (CPL COP) is characterized by a polymorphism of clinical symptoms, a severe, prolonged, recurrent course, and torpidity to the traditional therapy [Maksimovsky, Yu.M. Rabinovich, A.L. Grigoryan, E.L .: Epeldimova // Institute of Dentistry - 2003. - No. 4. - S. 39.]. Despite the perfection of the methods for diagnosing and treating the disease, the number of patients with this keratosis has a tendency to constant growth [Diseases of the mucous membrane of the oral cavity and lips. - M .: MEDpress, 2001. - 320 p .;].
[Gosslau, A. Oxidative stress, age dependent (correction of age-related) cell damage and antioxidative mechanisms / A. Gosslau, L. Rensing // Z. Gerontol Geriatr. 2002. - №2. - P. 139-150; Regezi, J.A. Oral lichen planus: clinical and histopathological considerations / F.A. Sousa, L.E. Rosa // Braz. J. Otorhinolaryngol. - 2008. - Vol. 74. - P. 28492.]. Отмечаются случаи развития КПЛ в детском возрасте [Sandhu, K. Familial lichen planus / K. Sandhu, S. Handa, A.J. Kanwar // Pediatr. Dermatol. 2003. - Vol. 20, №2. - P. 186; Juvenile oral lichen planus: a report of 2 cases / V.L. Woo [et al.] // Pediatr. Dent. - 2007. - Vol. 29, №6. - P. 525-30.]. [Contact dermatitis due to eugenol used to treat oral lichen planus / Y. Fujita Y et al. // Contact Dermatitis. 2003. - Vol. 48, №5. - P. 285]. - 2009. - №4 (36). - С. 70-73.].According to the results of epidemiological studies, lichen planus (CPL) is found in 0.1-2% of the population, more often in women aged 40 to 65 [Gosslau, A. Oxidative stress, age dependent (correction of age-related) cell damage and antioxidative mechanisms / A. Gosslau, L. Rensing // Z. Gerontol Geriatr. 2002. - No. 2. - P. 139-150; Regezi, J.A. Oral lichen planus: clinical and histopathological considerations / F.A. Sousa, L.E. Rosa // Braz. J. Otorhinolaryngol. - 2008. - Vol. 74. - P. 28492.]. Recently, there has been a "rejuvenation" of those suffering from this disease [Usmanova I.N. Optimization of diagnosis, treatment and prevention of inflammatory diseases of the periodontium and oral mucosa in young people // The dissertation for the degree of Doctor of Medical Sciences / Bashkir State Medical University. Ufa, 2016. S. 267]. Perhaps this is due to environmental damage, increased stress, significant changes in the reactivity of the body. Cases of the development of CPL in childhood are noted [Sandhu, K. Familial lichen planus / K. Sandhu, S. Handa, A.J. Kanwar // Pediatr. Dermatol. 2003. - Vol. 20, No. 2. - P. 186; Juvenile oral lichen planus: a report of 2 cases / V.L. Woo [et al.] // Pediatr. Dent. - 2007. - Vol. 29, No. 6. - P. 525-30.]. The manifestation of CPL only in the oral cavity is observed in 30-35% of cases [Contact dermatitis due to eugenol used to treat oral lichen planus / Y. Fujita Y et al. // Contact Dermatitis. 2003. - Vol. 48, No. 5. - P. 285]. Simultaneous skin lesions and COPD were detected in 25% of patients, and isolated localization to COPD without skin lesion was detected in 75% of patients [Serviceaev, I.F. Modern aspects of the etiology, pathogenesis and treatment of lichen planus of the oral mucosa / I.F. Serving, E.B. Zagorodnaya, V.V. Sadovsky // Maestro of dentistry. - 2009. - No. 4 (36). - S. 70-73.].
И. А. - 1998. - №6. - С. 348-3521].The clinical picture and the manifestation of lichen planus on the mucous membrane of the mouth have distinctive signs in the manifestations of it on the skin. These features are primarily due to the structural features of the oral mucosa, as well as the specificity of the processes, the relationship with homeostasis, as well as the presence of fillings and prostheses from various or dissimilar metals, the composition and properties of the oral fluid, and the processes of the initial splitting of food products [Dovzhansky, S. AND. Clinic, immunopathogenesis and therapy of lichen planus / S.I. Dovzhansky, N.A. Slesarenko // Ross. honey. Journal. - 1998. - No. 6. - S. 348-3521].
According to the classification (1984), the following clinical forms of crane flat lichen of the oral mucosa were distinguished: typical, exudative-hyperemic, erosive-ulcerative, bullous, hyperkeratotic, atypical [Mashkillayson A.L. Lichen planus. In the book: Diseases of the mucous membrane of the mouth and lips. - M., - 1984. - S. 190-204]. In the oral cavity, most often the pathological process is localized on the mucous membrane of the cheeks along the line of closure of the teeth, the lateral and dorsal surfaces of the tongue, and is much less common on the mucous membrane of transitional folds, lips and the bottom of the oral cavity.
To date, the problem of treating CPL of the oral mucosa is one of the important and unresolved tasks of clinical dentistry [Litvinov SL. Local treatment of lichen planus of the oral mucosa // Russian Journal of Skin and Sexually Transmitted Diseases. - 2003. - No. 3. - S. 4446 .; Petrova L.V., Kushlinsky N.E., Makarov Y.L. The use of the drug "Emparkol" in the treatment of erosive and ulcerative forms of lichen planus of the oral mucosa // Institute of Dentistry. - 2003. - No. 4. - Article 39.]. Violations of the humoral and cellular parts of the immune system [Tikhonova, E.V. Clinical and pathogenetic substantiation of immunomodulating therapy of lichen planus: Abstract. dis .. cand. honey. sciences. - Novosibirsk, 2006. 19 p.] Justifies the appointment in the complex treatment of drugs that promote immunocorrection, in violation of the psychoemotional status in patients with CPL of COPD [Anisimova, T.V. Neuropsychological status, hemorheology indicators in patients with lichen planus and their therapeutic correction: Abstract. dis .. cand. honey. sciences. - M., 2004. - 29 pp.] Pathogenetically substantiate the inclusion in the complex of therapeutic measures of means with anxiolytic, anti-stress, antidepressant effects.
The low effectiveness of local treatment in the complex of traditional therapy is also due to a deficiency in the capacity of the antioxidant potential of the oral fluid [Epimakhova, E.G. Features of the antioxidant potential of oral fluid in patients with red flat lichen mucosa of the oral cavity / E.G. Epimakhova, A.R. Antonov, P.A. Iron // Collection of articles on the 70th anniversary of NGMA. Novosibirsk, 2005. - P. 21-24], which explains the need to include drugs with antioxidant effects in the complex treatment regimen for COP of COPD.
In the light of the data presented, it is relevant to optimize the primary stage in the complex treatment of COP of SOR, which determines the need for the use of herbal medicines that would limit the pronounced antibacterial load, reduce the degree of dysbiosis, normalize the humoral factors of local immunity, improve the reparative process in the oral mucosa and provide complete detoxification.
However, these drugs have a number of drawbacks: it has weak or slight adhesion to the affected area, high solubility in the oral fluid, short duration of exposure, inconvenience to use (liquid forms in the form of applications on gauze napkins). When carrying out irrigation, installations or applications on the mucous membrane, these drugs dissolve very quickly and are washed out with oral fluid, and thus the therapeutic concentration is reduced.
The main problem in the complex of local treatment using various medicinal compositions is the problem of creating such an effective concentration in the focus of inflammation, as well as the long-term maintenance of such a concentration for a long time, which is necessary for the treatment of pathology of the oral mucosa in CPL.

Plates due to incoming micro and macro elements provide the necessary physiological, tissue and intracellular metabolic processes: due to magnesium (Mg), the normal course of redox reactions is ensured, normalization of iron (Fe) metabolism enhances oxygen transport, and redox reactions. This mechanism of local effects on the oral mucosa as a whole is of no small importance in the complex treatment of CPL COP of a typical form.
The application of the method allows to effectively increase the local treatment of CPL of COP in a typical form, to strengthen the reparative process of the mucosa and to prolong remission due to the complex effect on all links of the pathogenesis of this disease. The use of plates does not aggravate dysbiosis in the oral cavity and thereby contributes to the restoration of microbiocenosis.

The application of adhesive plates on the lesion ensures their uniform distribution on the affected mucosa. In this case, there is a pronounced stopping of the degree of inflammation, a decrease in the process of hyperkeratosis, normalization of the state of humoral factors of local immunity of the oral cavity and microflora, regeneration of the tissues of the mucous membrane due to the increased flow of biological substances into the chronic focus of inflammation.
The advantages of the proposed method of treatment are as follows:
1. The composition of the adhesive plates include natural components.
2. The prolonged action and high adhesion of the drug to the oral mucosa contribute to the rapid removal of inflammation.
3. The treatment time for a typical form of CPL is accelerated.
4. The consumption of the drug is reduced several times and its dosage is achieved.
4. There are no systemic side effects.
5. Adhesive plates are convenient and easy to use.
6. There are no allergic reactions in patients.
The inventive method of local treatment was used in 25 patients with lichen planus of the oral mucosa with a typical form. The effectiveness of treatment was evaluated in dynamics according to subjective signs and the clinical picture. In all patients, there was a decrease and disappearance of inflammation in the oral mucosa by 10 days, the mucous membrane of the oral cavity was pale pink in color, by 14 days without damage elements. When conducting a luminescent diagnosis of the affected area of the COP, a whitish glow of the keratinization sites. A positive effect of up to 12 months was observed in 99.6% of patients, in the remaining patients, the remission period was 9-10 months.
