Изобретение относится к медицине, а именно к хирургии, и предназначено для профилактики эвентраций и послеоперационного грыжеобразования при срединных лапаротомиях.The invention relates to medicine, namely to surgery, and is intended for the prevention of events and postoperative hernia formation in median laparotomies.
При анализе отечественной и зарубежной литературы по данной проблематике авторами, разрабатывающими вопросы профилактического эндопротезирования используются надапоневротический и интраперитонеальный способы размещения.When analyzing domestic and foreign literature on this issue, authors developing prophylactic endoprosthetics use supraponeurotic and intraperitoneal placement methods.
1. Наиболее простым является размещение сетчатого имплантата над апоневрозом. Сетка фиксируется к апоневрозу непрерывным полипропиленовым швом за его край с длиной шага 1,5-2 см на границе с подкожной жировой клетчаткой. Рана дренируется в течение 2-3 сут с активной аспирацией раневого отделяемого [Суковатых Б.С., Нетяга А.А., Валуйская Н.М., Жуковский В.А. Превентивная пластика брюшной стенки эндопротезом «Эсфил» при операциях на органах брюшной полости // Вестник хирургии. 2006. №3. С. 61-66]. Однако надапоневротическое расположение имеет свои недостатки. Необходимо отметить, что такое расположение эндопротеза в контакте со слабоваскуляризированными тканями (подкожная клетчатка и апоневроз) не способствует надежной интеграции протеза в слои передней брюшной стенки, что иногда приводит к его отслоению и формированию субпротезной грыжи. Так же надапоневротическое размещение сетчатого импланта связано с частым развитием сером и раневой инфекции.1. The simplest is the placement of a mesh implant above the aponeurosis. Nadaponeurotic placement of the mesh implant is also associated with the frequent development of seromas and wound infections.

При данной методике не всегда удается полностью изолировать петли кишечника от сетчатого имплантата и впоследствии могут возникнуть осложнения. При контакте полипропилена со стенкой кишечника есть вероятность развития кишечных свищей. Интраперитонеальная пластика на современном этапе предусматривает использование композитных сеток с различными свойствами поверхностей [Куликова Н.А. Предлагаемый метод отличается преперитонеальным расположением сетчатого импланта. Техническим результатом настоящего изобретения является снижение риска развития послеоперационных вентральных грыж и эвентраций путем повышения прочности послеоперационного рубца.The proposed method is characterized by a preperitoneal arrangement of the mesh implant. Для достижения поставленной цели разработана методика профилактического преперитонеального размещения сетчатого импланта задачами которой являются:To achieve this goal, a prophylactic preperitoneal placement of the mesh implant has been developed, the tasks of which are:

Краткое описание чертежей.A brief description of the drawings.
Для реализации поставленных задач хирургическая сетка из полимерного материала шириной 3 см помещается в преперитонеальное пространство с таким расчетом, чтобы ее длина превышала длину разреза апоневроза не менее чем на 1 см с каждой стороны (Фиг. 1). Первым узловым швом (1) сетка фиксируется у верхнего края раны (Фиг. 2). Второй фиксирующий шов (2) накладыватся в 5 см от первого по ходу разреза (захватывает края апоневроза и 2-3 нити центра сетки (Фиг. 2). Последующие фиксирующие швы накладываются каждые 5 см. Завершающий фиксирующий шов накладывается в нижнем углу раны, фиксируя сетчатый эндопротез в 1 см от ее нижнего края. Далее апоневроз ушивается поверх сетчатого эндопротеза непрерывным обвивным швом (3) с соблюдением правила 4:1, с захватом в шов сетчатого импланта каждые 2,5 см (Фиг. 3).To achieve the objectives, a surgical mesh made of

Больная П., 53 года, поступила в экстренном порядке 02.01.2016 года с диагнозом: Желчнокаменная болезнь. Острый калькулезный холецистит.Patient P., 53 years old, was admitted on an emergency basis 02.01.2016 with a diagnosis of Gallstone disease. Acute calculous cholecystitis.
После предварительной подготовки 02.01.2016 произведена холецистэктомия, которая выполнялась из срединного лапаротомного доступа. С учетом наличия факторов риска развития послеоперационных вентральных грыж (возраст старше 50 лет, ожирение 3 степени, наличие анатомо-функциональной недостаточности передней брюшной стенки 1 степени) оперативное вмешательство решено завершить превентивным эндопротезированием.After preliminary preparation, on 02.01.2016, a cholecystectomy was performed, which was performed from the median laparotomy access. Пациентка получала антибактериальную терапию, инфузионную терапию, спазмолитики, местное лечение. Послеоперационный период протекал без осложнений - раны зажили первичным натяжением. Дренажи удалены на 3-й сутки, швы сняты на 10-е сутки. Осложнений со стороны раны за все время наблюдения не выявлено, что подтверждено проведением УЗИ передней брюшной стенки в области раны на 3 сутки - жидкостных образований не обнаружено. Больная З., 67 лет поступила 28.10.2016 г. в экстренном порядке с клиникой острой спаечной кишечной непроходимости. Больна в течение 2 суток. Из сопутствующей патологии - ИБС, гипертоническая болезнь, ожирение 3-й степени. Через 2 часа от поступления, после проведения предоперационной подготовки, больная оперирована (операция №715). Выявлена острая спаечная тонкокишечная непроходимость. Выполнен адгезиолизис, назоинтестинальная интубация произведено дренирование брюшной полости.Patient Z., 67 years old, was admitted on October 28, 2016 on an emergency basis with a clinic of acute adhesive intestinal obstruction. Учитывая характер основного заболевания, возраст, сопутствующую патологию, повышенное внутрибрюшное давление вследствие пареза кишечника, для профилактики эвентрации и грыжеобразования срединная рана ушита с применением хирургической сетки согласно предложенной методике. Ранний послеоперационный период протекал без особеностей. Перистальтика с 3х суток, отхождение газов на 4 сутки. Самостоятельный стул на 6 сутки. Больная выписана в удовлетворительном состоянии. Осмотрена через 3 месяца. Жалоб не предъявляет, ощущения инородного тела в области оперативного вмешательства нет. Послеоперационной грыжи нет, дефектов апоневроза по данным УЗИ нет. Сетка не дислоцирована.Drainage removed on the 3rd day.
