Abstract
Description
The invention relates to medicine, namely to the field of phlebology, and can be used to treat varicose veins of the lower extremities.
2016. №4. Р. 119-125]. Зарубежные популяционные исследования показывают распространенность варикозного расширения вен 23% - 35%, в эти данные часто включаются пациенты с хронической венозной недостаточностью (классы С3-С6 по СЕАР) [Evans C.J., Fowkes F.G., Ruckley С.V, Lee a J. Prevalence of varicose veins and chronic venous insufficiency in men and women in the general population: Edinburgh Vein Study. // J Epidemiol Community Heal. 1999. Vol. 53, №3. P. 110-121].According to Russian population studies, the prevalence of chronic venous disease (CVD) C2 (CEAP) in the population is from 10% to 19.3% [Mazayshvili K.V., Chen V.I. The prevalence of chronic diseases of the veins of the lower extremities in Petropavlovsk-Kamchatsky // Phlebology. 2008. No4. P. 52-54], and the share of class C2 in the structure of HZV is about 30% [Zolotukhin I., Seliverstov E., Shevtsov Yu., Avakyants I., Nikishkov A., Tatarintsev A., Kiriyenko A. The prevalence of chronic venous diseases : Results of a population-based epidemiological study // Phlebology. 2016. No4. R. 119-125]. Foreign population studies show a prevalence of varicose veins of 23% - 35%, and patients with chronic venous insufficiency (CE3 classes C3-C6) are often included in these data [Evans CJ, Fowkes FG, Ruckley C.V., Lee a J. Prevalence of varicose veins and chronic venous insufficiency in men and women in the general population: Edinburgh Vein Study. // J Epidemiol Community Heal. 1999. Vol. 53, No. 3. Prevalence of chronic venous insufficiency in men and women in Poland: multicenter cross-sectional study in 40095 patients // Phlebology. 2003. Vol. 18. P. 110-121].
Endovenous ablation therapy (laser or radiofrequency) or foam sclerotherapy versus conventional surgical repair for short saphenous varicose veins // Cochrane Database Syst Rev. / ed. Paravastu S.C.V. Chichester, UK: John Wiley & Sons, Ltd, 2016. Vol. 11. P. CD010878]. Performing sclerotherapy is recommended to eliminate varicose saphenous veins [Paravastu S.C.V., Horne M., Dodd P.D.F. Endovenous ablation therapy (laser or radiofrequency) or foam sclerotherapy versus conventional surgical repair for short saphenous varicose veins // Cochrane Database Syst Rev. / ed. Paravastu S.C.V. Chichester, UK: John Wiley & Sons, Ltd, 2016. Vol. 11. P. CD010878]. Sclerotherapy of the saphenous veins is a safe and effective method of treating varicose veins of the lower extremities, the most cosmetic in comparison with surgical methods.
The task of sclerotherapy is significant damage to the endothelium and subendothelial structures, which leads to the formation of a specific blood clot in the lumen of the vein, the cessation of blood flow through it and, subsequently, the transformation of the vein into a fibrous cord. The goal of sclerotherapy is not to achieve vein thrombosis, which can be recanalized, but to fibrosis (sclerosis) of the vein. In this case, the result of sclerotherapy is equivalent to surgical treatment.
- 83 с.].There is a method of sclerotherapy for varicose transformed saphenous veins by introducing a sclerosing preparation of microfoam form with subsequent immediate and constant elastic compression [Fegan D. Varicose disease. Compression sclerotherapy, M., 1997 - 83 S.].
Не было отмечено различий между двумя препаратами в отношении достижения исчезновения сосудов или побочных явлений [Tisi P.V, Beverley С., Rees A. Injection sclerotherapy for varicose veins // Cochrane Database Syst Rev. / ed. Tisi P.V. Chichester, UK: John Wiley & Sons, Ltd, 2006. №4. P. CD001732]. Пенная форма склерозирующего раствора вызывает более сильный спазм стенки сосуда и тем самым увеличивает эффективность процедуры путем уменьшения вероятности смешивания склерозирующего раствора с элементами крови [, Estadella В., Serra-Prat М., Marinel-lo J. Efficacy and Safety of Sclerotherapy Using Polidocanol Foam: A Controlled Clinical Trial // Eur J Vase Endovasc Surg. 2006. Vol. 31, №1. P. 101-107]. С. Varicose veins: optimum compression after surgery and sclerotherapy. // Ann R Coll Surg Engl.. 1989. Vol. 71, №6. P. 402-404, Hamel-Desnos С.М., Guias B.J., Desnos P.R., Mesgard A. Foam Sclerotherapy of the Saphenous Veins: Randomised Controlled Trial with or without Compression // Eur J Vase Endovasc Surg.. 2010. Vol. 39, №4. P. 500-507, , Stephens J., Parkin D., Earnshaw J.J. Randomized clinical trial of different bandage regimens after foam sclerotherapy for varicose veins // Br J Surg.. 2010. Vol. 97, №5. P. 650-656].In order to conduct sclerotherapy, it is recommended to use preparations from the group of detergents (polydocanol, sodium tetradecyl sulfate). There were no differences between the two drugs in terms of achieving vascular disappearance or side effects [Tisi PV, Beverley C., Rees A. Injection sclerotherapy for varicose veins // Cochrane Database Syst Rev. / ed. Tisi PV Chichester, UK: John Wiley & Sons, Ltd, 2006. No. 4. P. CD001732]. Preparations from the group of detergents are recommended to be used in the form of a solution or foam. Some works reveal the advantages of foam over the liquid form of the solution during sclerotherapy of varicose inflows. The foamy form of the sclerosing solution causes a stronger spasm of the vessel wall and thereby increases the effectiveness of the procedure by reducing the likelihood of mixing the sclerosing solution with blood elements [ , Estadella B., Serra-Prat M., Marinel-lo J. Efficacy and Safety of Sclerotherapy Using Polidocanol Foam: A Controlled Clinical Trial // Eur J Vase Endovasc Surg. 2006. Vol. 31, No. 1. P. 101-107]. After sclerotherapy, the use of medical compression hosiery or compression bandage is recommended [Shouler PJ, Runchman R.S. Varicose veins: optimum compression after surgery and sclerotherapy. // Ann R Coll Surg Engl .. 1989. Vol. 71, No. 6. P. 402-404, Hamel-Desnos S.M., Guias BJ, Desnos PR, Mesgard A. Foam Sclerotherapy of the Saphenous Veins: Randomized Controlled Trial with or without Compression // Eur J Vase Endovasc Surg .. 2010. Vol. 39, No. 4. P. 500-507, , Stephens J., Parkin D., Earnshaw JJ Randomized clinical trial of different bandage regimens after foam sclerotherapy for varicose veins // Br J Surg .. 2010. Vol. 97, No. 5. P. 650-656].
[Kanter A., Thibault P. Saphenofemoral incompetence treated by ultrasound-guided sclerotherapy. // Dermatol Surg.. 1996. Vol. 22, №7. P. 648-652].It is recommended to use ultrasound guidance during venous puncture and during the introduction of sclerosant (ECHO-controlled sclerotherapy). In the presence of visually detectable and palpable varicose veins, the introduction of sclerosant is acceptable without ultrasound monitoring [Kanter A., Thibault P. Saphenofemoral incompetence treated by ultrasound-guided sclerotherapy. // Dermatol Surg .. 1996. Vol. 22, No. 7. P. 648-652].
Preliminary experience with a new sclerosing foam in the treatment of varicose veins. // Dermatol Surg. 2001. Vol. 27, № LP. 58-60]. С., Frullini A. European guidelines for sclerotherapy in chronic venous disorders // Phlebology. 2014. Vol. 29, №6. P. 338-354].The closest analogue of the invention is a method for sclerotherapy of varicose veins, which consists in introducing a foamy form of sclerosant prepared according to the Tessari method (using two syringes connected through a three-way adapter) by mixing a solution of sclerosant and atmospheric air in a ratio of 1 + 4 or 1 + 5 [ Tessari L., Cavezzi A., Frullini A. Preliminary experience with a new sclerosing foam in the treatment of varicose veins. // Dermatol Surg. 2001. Vol. 27, No. LP. 58-60]. The introduction of the foam form of sclerosant is carried out within a minimum short period from the moment of its preparation. During one session, it is recommended to administer no more than 10 ml of a foamy form of sclerosant [Rabe E., Breu F.X., Cavezzi A., Smith P.S., Frullini A. European guidelines for sclerotherapy in chronic venous disorders // Phlebology. 2014. Vol. 29, No. 6. P. 338-354].
М., Avery L.L., Sims J.R. Intra-arterial Air Thrombogenesis after Cerebral Air Embolism Complicating Lower Extremity Sclerotherapy // Neurocrit Care. 2009. Vol. 11, №1. P. 97-100]. There is evidence to relate the amount of foam injected to the incidence of serious neurological disorders [Leslie-Mazwi T.M., Avery L.L., Sims J.R. Intra-arterial Air Thrombogenesis after Cerebral Air Embolism Complicating Lower Extremity Sclerotherapy // Neurocrit Care. 2009. Vol. 11, No. 1. P. 97-100]. Thus, 10 ml was accepted by international and Russian experts as the maximum amount of the foam form of sclerosant, which ensures safe administration in one session.
The disadvantage of the prototype is the limitation of the maximum amount of injected sclerosing drug to 10 ml, which leads to a decrease in the volume of effective treatment in one session and an increase in the number of necessary procedures.
The objective of the invention was the development of a method for sclerotherapy of saphenous veins, providing a therapeutic effect with a smaller amount of sclerosant in microfoam form and its sequential combination with a liquid form of sclerosing substance.
The technical result when using the invention is to reduce the number of medical procedures by increasing the volume of sclerotable veins in one procedure using a smaller volume of sclerotherapy in microfoam form and its sequential combination with the liquid form of sclerosant.
The invention is illustrated by a figure, which shows a diagram of the proposed method, where 1 and 2 are disposable plastic syringes with a microfoam solution of sclerosant; 3 - a disposable plastic syringe with a liquid solution of sclerosant; 4 - tee for infusion system; 5 - intravenous catheter; 6 - saphenous vein.
The proposed method of combined sclerotherapy of saphenous veins is as follows. After treating the puncture area with an antiseptic solution using an intravenous catheter 5 with a 23 G needle, a saphenous vein is punctured 6. A microfoam form of a 0.5% sodium sclerosant tetradecyl sulfate (fibrowain) solution is introduced in an amount of 2.0-5.0 ml, prepared using two disposable

Patient I. turned to the Clinic of Modern Phlebology LLC, Ufa on January 10, 2017, complaining of the presence of varicose veins of the right lower limb. Based on complaints, clinical examination and ultrasonography of the veins of the lower extremities, the diagnosis was made: Varicose disease Cl, 2, Ep, As, Pr1.5, LII on the right, Cl, Ep, As, LII on the left.
13.01.2017 г. 01/13/2017, combined sclerotherapy of varicose saphenous veins was carried out on an outpatient basis by sequentially introducing into the lumen of the microfoam form a 0.5% solution of sodium sclerosant tetradecyl sulfate in an amount of 2.0 ml, which caused a spasm of the vein, visualized by ultrasound control, immediately afterwards into the lumen of the same vein, a liquid form of sclerosant was introduced - a 0.5% solution of sodium tetradecyl sulfate in an amount of 2.0 ml. An eccentric pressure bandage (rollers) and a compression stocking of compression class 2 were applied to the right lower limb.
27.01.2017 г. 01/27/2017, a repeated examination of the patient was performed, which revealed complete sclerosis (the vein is not compressible) of the saphenous vein with minor hyperpigmentation during ultrasound examination. It is recommended to continue wearing the compression stocking.
15.03.2017 г. 03/15/2017, at the re-examination, no consolidations and hyperpigmentation were revealed, on the echocarticle, the saphenous vein was not visualized. The course of treatment was 1 procedure.
2.Example 2
Patient R. contacted the Clinic of Modern Phlebology LLC, Ufa on January 23, 2017. with complaints of the presence of varicose veins of the left lower limb, a feeling of heaviness in it at the end of the day. Based on complaints, clinical examination and ultrasonography of the veins of the lower extremities, the diagnosis was made: Varicose disease Cl, Ep, As, LII on the right Cl, 2, Ep, As, Pr1.5, LII on the left.
24.01.2017 г. On January 24, 2017, combined sclerotherapy of varicose anterior extra saphenous vein (PDBPV) was carried out on an outpatient basis by sequentially injecting a microfoam form of a 0.5% solution of sodium sclerosant tetradecyl sulfate in an amount of 4.0 ml, which caused an echo-controlled spasm of the vein, immediately after a liquid form of a sclerosant was introduced into the lumen of the same vein - a 1.0% solution of sodium tetradecyl sulfate in an amount of 5.0 ml. An eccentric pressure bandage (rollers) and a compression stocking of compression class 2 were applied to the left lower limb.
06.02.2017 г. 02/06/2017, a repeated examination of the patient was performed, which revealed partial sclerosis during ultrasound examination (the investigated vein is not compressible in the proximal part, in the distal part and its tributaries at the level of the lower leg, the lumen is partially compressible) of the saphenous vein, with minor hyperpigmentation effects. It is recommended to repeat the sclerotherapy session.
3.03.2017 г. March 3, 2017, outpatient combination sclerotherapy of varicose distal and inflows of PDBPV was performed by successively introducing into the lumen of the microfoam form a 0.5% solution of sodium sclerosant tetradecyl sulfate in an amount of 2.0 ml, which caused an echo-controlled spasm of the vein, immediately afterwards into the lumen of this the veins introduced a liquid form of sclerosant - a 0.5% solution of sodium tetradecyl sulfate in an amount of 2.0 ml. An eccentric pressure bandage (rollers) and a compression stocking of compression class 2 were applied to the left lower limb.
14.03.2017 г. On March 14, 2017, a repeated examination of the patient was carried out, which revealed complete sclerosis (the vein is not compressible) of the saphenous vein with minor hyperpigmentation during ultrasound examination. It is recommended to continue wearing the compression stocking.
10.04.2017 г. On April 10, 2017, at the second examination, patient R. does not reveal complaints of a feeling of heaviness in the left leg, during the clinical examination, isolated elements of hyperpigmentation were revealed, on the echocardinus, areas of hyperechoicity in the projection of the PDPVV. The course of treatment was 2 procedures.
