У больных с тромбозом глубоких вен проводится ультразвуковое исследование не только глубоких вен, но и паравазальной клетчатки для обнаружения мест ее воспаления. Лекарственная смесь готовится путем смешивания вышеперечисленных компонентов в шприце 20 мл, без учета какой-либо последовательности, в соотношении, рассчитанном на основании средних терапевтических дозировок препаратов. Полученная смесь вводится паравазально однократно на курс лечения под ультразвуковым контролем методом пункции под углом 30-45 градусов в зоны наиболее выраженного воспаления венозной стенки и паравазальной клетчатки с последующим ультразвуковым контролем через 1 сутки, 3 суток, 14 суток, 1 месяц, 6 месяцев. При угле введения шприца менее 30 градусов возможна безуспешная пункция из-за выраженного отека и подкожно-жировой клетчатки, при угле более 45 градусов повышается риск травматизации стенки вены. Если при ультразвуковом контроле сохраняется выраженное воспаление венозной стенки и паравазальной клетчатки, введение лекарственной смеси необходимо повторить.In patients with deep vein thrombosis, an ultrasound examination of not only deep veins, but also of paravasal fiber is performed to detect places of its inflammation. При поступлении больного с тромбозом глубоких вен производится ультразвуковое исследование глубоких вен и паравазальной клетчатки. Выявляются места наиболее выраженного воспаления венозной стенки и паравазальной клетчатки, которые впоследствии прорастают элементами соединительной ткани и ведут к окклюзии венозного сегмента. Под ультразвуковой навигацией в эти места вводится лекарственная смесь, состоящая из дексаметазона 4 мг, гепарина 5000 ЕД, 0,25% раствора новокаина до 20 мл. Больная Б., возраст 67 лет, госпитализирована в отделение сосудистой хирургии г. Орла 07.02.17 г. с жалобами на распирающие боли в правой нижней конечности до уровня паховой складки, резкий отек данной конечности, положительными симптомами Хомаса и Мозеса, цианозом правой нижней конечности, расширением подкожных вен. Клинический диагноз: тромбоз глубоких вен голени и бедра с переходом на подвздошную вену, длительность заболевания 9 суток. 07.02.18. при тщательном ультразвуковом исследовании выявлен окклюзионный тромбоз подколенной и бедренной вен. Наиболее выраженное воспаление венозной стенки находится в бедренной вене чуть проксимальнее впадения в нее глубокой вены бедра в паховой области и в месте слияния берцовых вен с образованием подколенной вены в подколенной ямке. В шприце смешали 4 мг дексаметазона, 5000 ЕД гепарина, 20 мл 0,25% раствора новокаина. После обработки паховой области антисептиком (5% спиртовым раствором йода) под ультразвуковым контролем паравазально в проекции бедренной вены введено 10 мл лекарственной смеси. Больную перевернули на живот, обработали подколенную ямку антисептиком, под ультразвуковой навигацией паравазально в проекции подколенной вены ввели 10 мл лекарственной смеси. Через 1 сутки проведено контрольное ультразвуковое исследование. Больная отмечает снижение болевого синдрома на бедре, уменьшение отека правой нижней конечности. Зарегистрировано уменьшение воспаления венозной стенки и паравазального отека. Через 3 суток проведено контрольное ультразвуковое исследование: воспаление венозной стенки и паравазальной клетчатки слабо выражено. Через 14 суток отмечен значительный регресс клинических симптомов тромбозов глубоких вен: болевой синдром купирован, отечность уменьшилась в 2 раза. При контрольном ультразвуковом исследовании воспалительные изменения венозной стенки и паравазальной клетчатки не регистрируются. Тромботические массы в просвете вены стали менее плотными. Больная выписана из отделения с назначением ривароксабана - непрямого антикоагулянтного препарата, согласно инструкции по его применению. Осмотрена через 1 месяц. Жалоб нет, выполняет домашнюю работу, отмечает незначительный отек правой стопы к вечеру. При контрольном ультразвуковом исследовании наличие тромбов в просвете глубоких вен правой ноги не определяется. After treatment of the inguinal region with an antiseptic (5% alcohol solution of iodine), 10 ml of the drug mixture was injected paravasally into the projection of the femoral vein under ultrasound guidance. The patient was turned on his stomach, the popliteal fossa was treated with an antiseptic, and under ultrasound navigation, 10 ml of the drug mixture was injected paravasally into the projection of the popliteal vein. After 1 day, a control ultrasound was performed. The patient notes a decrease in pain on the thigh, a decrease in edema of the right lower limb. A decrease in venous wall inflammation and paravasal edema has been reported. After 3 days, a control ultrasound was performed: inflammation of the venous wall and paravasal tissue is weak. After 14 days, a significant regression of the clinical symptoms of deep vein thrombosis was noted: the pain syndrome was stopped, swelling decreased by 2 times. During the control ultrasound, inflammatory changes in the venous wall and paravasal tissue are not recorded. Thrombotic masses in the lumen of the vein became less dense. The patient was discharged from the department with the appointment of rivaroxaban - an indirect anticoagulant drug, according to the instructions for its use. Examined after 1 month. There are no complaints, performs homework, notes a slight swelling of the right foot in the evening. During the control ultrasound, the presence of blood clots in the lumen of the deep veins of the right leg is not determined. Rivaroxaban is canceled, it is recommended to wear an elastic stocking.
Указанным способом при информированном согласии больного пролечено 10 пациентов с тромбозом глубоких вен нижних конечностей.
