Abstract
Description
The invention relates to medicine, namely to orthopedic dentistry, and can be used to prepare a toothless prosthetic bed before the manufacture of removable dentures.
With a prolonged absence of teeth, atrophy develops not only in the bone tissue of the jaws, but also in the soft tissues of the prosthetic bed. This is expressed, first of all, in a decrease in the volume and compliance of the mucous membrane of the prosthetic bed. The lack of a sufficient volume of keratinized mucous membrane in the area of the prosthetic bed during prosthetics with removable prostheses leads to their unsatisfactory stabilization on the jaws. The alveolar process, covered with a thin layer of the mucous membrane, experiences significant pressure from the base of the removable prosthesis. As a result of the functional overload of the tissues of the prosthetic bed (mucous membrane, periosteum, bone of the alveolar bone), the progression of destructive processes in the bone tissue occurs. An increase in the thickness of the mucous membrane of the prosthetic bed contributes to a better perception of chewing pressure due to the cushioning ability and, as a result, to improve stabilization of the removable prosthesis. In addition, trauma and soreness of the mucous membrane under the base of a removable prosthesis are reduced.
The most widely used for unstable and thinned mucous membranes was the technique of manufacturing removable dentures with a soft pad (Kopeikin VN, Mirgazizov MZ Orthopedic dentistry. - 2001. - P. 438-441). A removable denture with a double base more evenly distributes chewing pressure on the tissue of the prosthetic bed, and therefore reduces the progression of atrophy, improves fixation of the prosthesis on the jaw and reduces pain under the basis of the prosthesis.
The disadvantage of this method is the lack of a strong chemical bond between the material of the soft pad and the base of the prosthesis, which leads to its peeling in a fairly short period of time. After a few months, the soft pad gradually begins to harden, thereby losing its elastic properties. The use of removable dentures with a soft pad does not suspend atrophic processes. In addition, the soft pad is unhygienic due to the high porosity of the material and lasts an average of 6 months.
С. Д. В. С. - 2011. - №3. - С. 19-24).The prototype of the present invention is a method for transplanting a full-layer connective tissue transplant on a site with keratinized mucous membrane (Smbatyan B.S. Experimental biomodeling to assess the possibility of changing the morphotype of the oral mucosa / B.S.Smbatyan, G.D. Kapanadze, M.V. Lomakin, A.S. Aleinikov et al. // Biomedicine. - 2011. - No. 3. - S. 19-24).
The method consists in the fact that they produce a full-layer graft in the sky. Then, on a sterile napkin moistened with saline, de-epithelialization is carried out, after which it is ready for use. Next, preparation of the perceptive bed on the periosteum is carried out by peeling of the muscular-muscular attachment, lowering its level of fixation, thereby creating an open periosteal surface on which the connective tissue graft is fixed with sutures. The aim of the method is to obtain a layer of connective tissue with an epithelium that is histologically similar to the epithelium of neighboring sites.
The disadvantage of this method is that when using a free mucous flap, we are faced with some tasks: the need to take it from the donor site and the associated discomfort in the postoperative period. In addition, we cannot take from the donor site (for example, from a hard palate) a free epithelial flap of large size and arbitrary shape. Therefore, the closure of large defects on the mucous membrane is not possible. Low predictability and high likelihood of postoperative complications, the complexity and complexity of surgical intervention. Transplanted grafts do not change their cellular composition after a long period of time. Epithelial tissue does not have its own genetically defined differentiation, since it is determined by the underlying connective tissue. Often, the mucous membrane to be transplanted is thinned and atrophic, which does not allow using it as a complete transplant.
Collagen preparations have been widely used to stimulate regenerative processes, reduce inflammatory reactions, and shorten the healing time of wounds. They are based on a biodegradable dermal filler containing native unreconstructed collagen material, similar in structure to human collagen. Material for the biological product is extracted from the dermis of cattle. Collagen is a protein that forms the basis of the connective matrix, which forms and supports the natural framework of the face and other parts of the body. The speed of skin regeneration, its elasticity and strength directly depend on its quantity and quality. Collagen materials are used in patchwork operations in periodontology, in the treatment of cysts, in maxillofacial surgery, implantology, in reconstructive operations, in traumatology, cosmetology and other fields of medicine.
The technical result of the proposed method for preparing a toothless prosthetic bed before prosthetics is aimed at increasing the thickness of the mucous membrane of the prosthetic bed, restoring hemocirculation, elasticity and strength of the mucous membrane of the prosthetic bed, reducing atrophy of the tissues of the prosthetic bed, and improving stabilization of the removable prosthesis on the full or partial toothless jaw.
The essence of the invention lies in the fact that in the method of preparing a toothless prosthetic bed before prosthetics, which includes taking a patient’s autograft, introducing it under the mucous membrane of the alveolar bone of the toothless jaw, ready-made collagen material is used as an autograft in the form of a gel that is injected from the syringe under the mucosa a toothless prosthetic bed on top of the alveolar process of the upper or lower jaw, and on the upper jaw also in the seam of the hard palate, based on 1 cm 2 malopod of the mucous membrane of the prosthetic bed 0.1-0.2 ml of collagen and finger massage of the mucous membrane of the less pliable zones for 5-6 minutes after its introduction, while at the 1st degree of compliance of the mucous membrane according to the Supple, 1 procedure is performed, and at 2 degrees of compliance, 3 procedures are performed with an interval of 2 weeks.
Далее коллагеновый материал разогревали до температуры 36-40°C (на водяной бане или в термостате).A method of preparing a toothless prosthetic bed before prosthetics is as follows. Patients with a thinned, slightly or poorly flexible mucous membrane of the prosthetic bed 14 days before the introduction of a biodegradable dermal filler in the form of a gel (Kollost) performed an intracutaneous test test with the same material. 0.1 ml of gel (allergic sample) was injected intracutaneously into the forearm using a 27 1/2 G gauge needle. In the absence of any allergic reaction, the tissues of the toothless prosthetic bed were anesthetized using application or infiltration anesthesia. Then collagen material was heated to a temperature of 36-40 ° C (in a water bath or in a thermostat).
After that, prepared 7% Kollost gel was injected locally under the mucous membrane of the toothless prosthetic bed along the top of the alveolar process of the upper or lower jaw, and on the upper jaw also in the seam of the hard palate using a syringe (0.1-0.2 ml each) per 1 cm 2 of the mucous membrane). Further, for a more uniform distribution of the gel in the mucous membrane of the prosthetic bed, finger massage was performed for 5-6 minutes. At the 1st degree of compliance of the mucous membrane according to Supple, 1 procedure was performed, and at the 2nd degree of compliance, 3 procedures were performed with an interval of 2 weeks.
The method allows you to increase the thickness of the mucous membrane by an average of 2 mm in a fairly short time (with grade 1 compliance of the mucous membrane of the prosthetic bed according to Supple, the effect is noticeable after 10-12 days, with grade 2 the effect is achieved after 34-36 days). In addition, hemocirculation, elasticity and strength of the mucous membrane of the prosthetic bed are restored. Atrophy of the bone tissue of the alveolar processes is reduced.
An increase in the thickness and compliance of the mucous membrane of the prosthetic bed using the method we proposed was performed in 17 patients with a complete absence of teeth. Of these, 11 patients with a complete absence of teeth in the lower jaw had a thinned, malleable mucous membrane of the prosthetic bed, which corresponded to the 2nd class according to Supple. In the remaining 6 patients with a complete absence of teeth in the upper jaw, the state of the mucous membrane corresponded to the 1st class according to Supple. Made 17 full removable laminar dentures. The results are good. According to measurements of the mucosal compliance of the prosthetic bed using the LIRA-100 apparatus (Yekaterinburg), its thickness increased by an average of 2 mm. Patients did not need correction of the bases of removable prostheses due to the absence of trauma to the mucous membrane of the prosthetic bed. Significantly increased stabilization of removable dentures on the toothless jaws. In addition, according to rheography, hemocirculation of the mucous membrane of the prosthetic bed normalized. Patients were observed by us for 8 months after orthopedic treatment. There were no complications from the oral mucosa.
