Abstract
Description
The invention relates to medicine, namely to physiotherapy, dermatology, medical rehabilitation, and is intended for the treatment of focal scleroderma.
The pathogenesis of focal scleroderma is based on several basic mechanisms: vascular changes, activation of the immune system and impaired regulation, as well as changes in the metabolism of collagen and fibroblasts. Modern principles of therapy for local scleroderma involve the appointment of various groups of drugs (methotrexate, glucocorticosteroids, tacrolimus, cyclosporin, imatinib, mycophenolate mofetil, retinoids, etc.), as well as physiotherapy.
- 2011 г. - №3. - С. 42-45).Nevertheless, the variety of treatment methods in this case does not always lead to the proper clinical effect. Thus, various schemes for the use of drugs for the treatment of focal scleroderma are known from the prior art: methotrexate is the most widely used drug; glucocorticosteroids used in both oral and external forms. As topical therapy, methylprednisolone aceponate, betamethasone, clobetasol propionate in the form of applications are most often prescribed, the course of treatment is 4-12 weeks, or occlusive dressings, the course is 2-3 weeks. (Shostak N.A. et al. Localized (focal) scleroderma in general medical practice. Medical business. - 2015 - No. 4. - P. 45-52). It is possible to use antihypoxants and antioxidants (Starovoitova MN Experience in the use of actovegin in focal scleroderma // Modern rheumatology. - 2011. - No. 3. - P. 42-45).
Thus, exclusively medicinal methods for treating focal scleroderma do not provide a stable therapeutic effect; they can also be complicated by the “withdrawal syndrome”, allergic reactions and other side effects of drugs, especially hormonal ones.
Another promising area of treatment for focal scleroderma is physiotherapy in combination with drugs, taking into account the form of the disease and the age of the patients.
There is a well-known method for the treatment of focal scleroderma, including the use of hyaluronidase in foci of scleroderma by ultraphonophoresis (Didenko I.G. Therapeutic efficacy of ultrasound and phonophoresis of lidase in various forms of scleroderma. Bulletin of Dermatology and Venereology. - 1978 - No. 6. - P. 76. - 1978. -79) or electrophoresis (K. Ananyeva et al. A new method for the treatment of various forms of scleroderma by electrophoresis with lidase. Methods for the diagnosis, treatment and prevention of diseases, developed and improved in MONIKI named after MF Vladimir r., Moscow: 1970 - S. 148-149). However, the use of hyaluronidase is not always highly effective in this pathology: the clinical effect is limited to one or two months, and allergic reactions are also possible. Currently, there is a high increase in the number of allergic reactions, which is partly associated with the use of drugs, including enzyme ones (according to the summary data for a number of countries, drug allergy occurs in 8-12% of patients). Mortality from complications of drug therapy came out on the 4th place in the world. Many countries have national drug safety control centers coordinated by WHO. Its database contains about 2 million reports of undesirable side effects of drugs. About 200,000 messages from different countries are received annually, and according to many authors, this number is constantly growing. When using longidase such reactions have also been observed. Given the important role of microcirculatory disorders of a systemic nature in the development of the pathological process with focal scleroderma, the effects of longidase are insufficient. All this limits the duration of remission after the course of treatment proposed in this and similar methods to one to two months.
The prior art method for the treatment of focal scleroderma (Shostak N.A. et al. Localized (focal) scleroderma in general medical practice. // Medical business. - 2015 - No. 4. - P. 45-52), including PUVA therapy (from the English. PUVA (Psoralen and Ultraviolet A: psoralen and ultraviolet rays of spectrum "A"). In this case, the authors suggest that ultraviolet therapy of the far long-wavelength range (UVA-1, wavelength 340-400 nm) is one of effective methods of treatment, including plaque and linear scleroderma with shallow skin lesions and PUVA therapy with both oral and external use of a photosensitizer can significantly improve the condition of the skin in the foci of scleroderma, however, the effect of PUVA therapy is short-lived, and side effects such as dyspeptic, phototoxic, pigmented, are not excluded and risk of skin cancer.
Oral administration of photosensitizing drugs is limited in patients with severe liver and kidney dysfunction, cataracts, aphakia. Moreover, the external use of psoralen in the form of alcohol solutions and creams causes difficulties in the presence of widespread skin lesions in patients.
The prior art method for the treatment of focal scleroderma with the help of ultrasound therapy (V. Volukhin. Federal clinical guidelines for the management of patients with localized scleroderma. - M .: 2013 - 21 p.), Including the sounding of lesions with a vibration frequency of 880 kHz, intensity 0.05-0.8 W / cm 2 , exposure time 5-10 min per field, according to the labile technique in continuous or pulsed mode. The course is 10-15 daily procedures. Ultrasound treatment is effective in the initial stages of the disease, but with intensive sclerosis of the focus, it is necessary to increase the power of exposure, which can lead to suppression of skin regeneration, therefore this method of exposure cannot be considered optimal.
- 1997 г. - №1. - С. 12-15).The optimal choice is the factor of physiotherapeutic effects, devoid of the above complications. So, it is known that low-intensity laser therapy can persistently improve microcirculation in the foci of scleroderma. Some studies have shown the effectiveness of the use of low-intensity laser therapy in the complex treatment of limited forms of localized scleroderma (Volnukhin V.A. et al. Experience in using combined methods of low-intensity laser therapy in the treatment of diseases with manifestations of sclerosis and skin atrophy // Laser Medicine. - 1997. - Volume 2. - No. 1. - S. 12-15).
As a result of the patent information search, the method of treating focal scleroderma closest in technical essence to the present invention was identified (Gaydina T.A. Longidase photophoresis in the rehabilitation treatment of limited scleroderma // Cand. Sc. Medical Sciences - 2011 - 107 sec. .), which consists in exposing the lesion sites to low-energy laser radiation in the infrared range, a pulse repetition rate of 1500 Hz, a pulse duration of 110-160 ns, a pulse power of 4-6 W / imp, 1-3 minutes per field, the total duration of exposure no more than 15 minutes, per course - 15 daily procedures.
The use of low-intensity laser radiation allows you to achieve anti-inflammatory effect, improve peripheral circulation; activation of metabolic and trophic processes, metabolism of biologically active substances, stimulation of a number of protective mechanisms (phagocytosis, etc.), but, in general, this method also has significant disadvantages. So, it is known that low-energy laser radiation is recommended for patients with focal scleroderma at the stage of stabilization of the process (Gaydina T.A. Photophoresis of longidase in the rehabilitation treatment of limited scleroderma. // Cand. Sc. Medical Sciences - 2011 - 107 pp.) , while to achieve stabilization, the use of only a low-energy laser is not enough.
Thus, there is a need for a method for the treatment of focal scleroderma, devoid of the disadvantages of the above analogues.
The technical result of the present invention is to increase the effectiveness of treatment of patients with focal scleroderma due to the combined effects of low-energy laser therapy with applications of NOLLA naturelle® body gel and paraffin therapy with optimal parameters.
This technical result is achieved by the fact that in the proposed method for treating patients, including exposure to lesions with low-energy laser radiation in the infrared range, the pulse repetition rate is 1500 Hz, the pulse duration is 110-160 ns, the pulse power is 4-6 W / imp, 1- 3 minutes on the field, with a total exposure time of not more than 15 minutes, immediately before the exposure to laser radiation, scleroderma lesions are applied with NOLLA naturelle® body gel applications and alternated every other day the laser exposure m radiation with applications of paraffin on the foci of scleroderma, temperature 48-52 °, time 20 min; a course of treatment of 8-10 complex procedures.
The proposed method is as follows.
е. Before exposure to laser radiation on scleroderma foci previously cleaned with a mild detergent, they apply the NOLLA naturelle® body gel applications. They have an effect on the lesions with low-energy laser radiation of the infrared range using a low-energy laser exposure, a pulse repetition rate of 1500 Hz, a pulse duration of 110-160 ns, a pulse power of 4-6 W / imp, 1-3 minutes per field, the total duration of exposure no more than 15 minutes, alternating every other day the effect of laser radiation with paraffin applications on scleroderma foci, a temperature of 48-52 ° C, a time of 20 minutes, a course of treatment of 8-10 complex procedures. One treatment in a complex treatment includes low-energy laser exposure through application of NOLLA naturelle® gel and exposure to paraffin applications, i.e. takes 2 days.
In addition to the pronounced defibrosing effect, comparable and significantly exceeding the effectiveness of longidase, paraffin applications have a thermal effect, which passes into other types of energy in the patient's body, which improves its bioenergetic potential.
Temperature is an important natural stimulating factor necessary to maintain a person’s heat balance, and receptors that receive it are located both on the periphery and in the central nervous system. This can explain the effect of paraffin applications on some systems of the body that are not involved in the heat perception process, such as immune, endocrine, etc. The temperature range of paraffin applications - 48-52 ° С - optimally promotes an increase in intradermal, muscle blood flow, as well as an increase in oxygen transport that activates skin repair processes.
Paraffin applications have a pronounced mechanical effect (paraffin crystallizes upon solidification, and its volume, decreasing, promotes compression of surface tissues), which effectively enhances lymphatic drainage, contributing to detoxification of the body.
The combined use of NOLLA naturelle® body gel applications and low-energy laser radiation potentiates their effects. The gel contains a large number of biologically active substances in the form of polysaccharides with a high spectrum of biomedical action. So, sodium alginate is a highly active and harmless natural sorbent, for example, cholesterol and fatty acids, which ensures its lipid-lowering and anti-atherosclerotic properties. At the same time, fucoidans included in this gel are known as activators of the immune system, radio and cryoprotectants, they exhibit anti-inflammatory and anticoagulant effects, increase the phagocytic activity of cells, therefore they are called “polyvalent biomodulators”.
For skin regeneration, the presence of micro and macro elements, amino acids, vitamins in a ratio and amounts close to the daily needs of the human body, without which the restoration of pathologically changed tissues is impossible, is important.
Thus, low-energy laser radiation procedures with applications of the NOLLA naturelle® body gel, alternating with paraffin therapy, which has an optimally selected temperature range, significantly increase the effectiveness of treatment of focal scleroderma. Such a multifactorial effect covers all the links of metabolic processes in pathologically altered fibrosed skin lesions. The humoral effect of biologically active substances will have a general effect on the patient's body, stimulating a number of protective mechanisms that are substantially potentiated by the active components of the gel. In addition, the components of the NOLLA naturelle® body gel are quite hypoallergenic.

During the examination, a pathological skin process of a sub-inflammatory nature was noted, widespread and asymmetric. It is localized on the skin in the areas of the abdomen, hips and buttocks and is represented by lilac-pink edematous spots 2-10 cm in size. In the center of some of them, yellowish-white areas of atrophy are visualized with a smooth, shiny surface. Examination revealed eosinophilia, while Scl-70 antibodies were not detected. Histological examination: perivascular inflammatory infiltrates in the reticular and subcutaneous layers of the dermis, edema of the upper layer of the dermis. The biopsy is dominated by cells of lymphocytes, histiocytes, as well as eosinophilic granulocytes.
Diagnosis: Plaque scleroderma, inductive-atrophic form, first detected.
Patient L., in addition to standard drug therapy, received treatment by the proposed method. Immediately before exposure to laser radiation, NOLLA naturelle® body gel applications were applied to the scleroderma lesions. In this case, the impact on the lesions was carried out by low-energy laser radiation of the infrared range, a pulse repetition rate of 1500 Hz, a pulse duration of 110-160 ns, a pulse power of 5 W / imp, 1-3 minutes per field. The total duration of exposure was 15 minutes. The exposure to laser radiation alternated every other day with paraffin applications on scleroderma foci, at a temperature of 48 ° C, for a time of 20 minutes. Thus, one complex procedure included low-energy laser exposure through NOLLA naturelle® gel applications and exposure to paraffin applications. The course of treatment was 8 complex procedures in 16 days.
After a course of complex treatment, patient L. noted an improvement in the form of resolution of rashes, almost complete resolution of the density in the foci, the patient does not bother with itching and constriction. The patient underwent a histological examination, while the edema decreased, the structure of the layers of the dermis is close to normal.
A follow-up examination after 6 months showed a stable remission of the disease.
Example 2. Patient M., 20 years old, sick 2 years. Complaints about the presence of dense plaques surrounded by a corolla of hyperemia on the skin of the back, abdomen and chin. Considers himself ill for two years, when he first noticed the appearance of a bluish-colored spot in the lumbar region, which has become denser with time. Within two years, the process progressed, dense plaques with patches of atrophy appeared on the skin of the back, abdomen. Subjectively worried about the feeling of “tightness” of the skin in places of rashes. I did not seek medical help, I did not receive treatment. A year ago, dense foci of waxy white appeared on the skin of the chin. Hospitalized in the Department of Dermatovenereology and Dermatology Oncology GBUZ MO MONIKI them. M.F. Vladimirsky, where the examination noted a pathological skin process of a sub-inflammatory nature, widespread and predominantly symmetrical, which was localized on the skin of the back, abdomen and chin. It is represented by plaques from two to six cm in diameter, rounded in shape with fuzzy borders of a brownish-cyanotic color, dense upon palpation with signs of sclerosis. On the skin of the chin are linear plaques with signs of ivory atrophy. Hypereosinophilia was noted during the examination; Scl-70 antibodies were not found. Histological examination: perivascular inflammatory infiltrates in the reticular and subcutaneous layers of the dermis, edema of the upper layer of the dermis. The biopsy is dominated by lymphocytes, plasma cells, eosinophilic granulocytes, as well as a pronounced thickening of bundles of collagen fibers parallel to the dermis.
Diagnosis: Linear scleroderma, first detected.
