Abstract
Description
The group of inventions relates to medicine, namely to traumatology, orthopedics and surgery, in particular to methods of reposition and fixation of injuries of the musculoskeletal system.
Known methods for restoring the integrity and functionality of the human calcaneus with submersible retainers for compression fractures of the calcaneus [1]. The disadvantage of using submersible structures is the high invasiveness for quick access tissues, which often leads to the occurrence of purulent-necrotic complications and the need to introduce drugs that contribute to their prevention. In addition, scars formed in the soft tissues after open reduction of bone structures often require the wearing of special orthopedic shoes that prevent the development of scuffs, wounds and other changes in this area.
A known method of transosseous osteosynthesis of the calcaneus [2]. The disadvantages of the method include the cumbersome design of restoring the integrity and functionality of the calcaneus with the need for through introduction of a large number of knitting needles, the difficulty of determining the hinge establishment zone, insufficient controllability of the fragments during reposition due to the small angle of the crosses of the needles in the calcaneus, the difficulty, and sometimes the impossibility of elimination of compression of fragments with their reposition. Dosed distraction in the apparatus promotes the formation of soft tissue regenerate between the fragments, which, in turn, prevents their closed reposition. In addition, a large number of structures closes the fracture zone, prevents the determination of the degree and accuracy of its reposition, as well as the presence of bone callus. For the same reason, the elimination of open fractures is difficult, since a wound is often located where knitting needles are needed, and often with a soft tissue defect.
A known method of treating comminuted intraarticular fractures of the calcaneus [3], which to restore the integrity and functionality of the calcaneus provides reposition by conducting knitting needles through the bones of the lower leg, the calcaneus tubercle and its anterior section, fixed in the rings and threaded rods of the compression-distraction apparatus. However, this method has limited repositioning capabilities, insufficient structural stability, and the lack of support in the forefoot does not exclude dorsiflexion in the ankle joint, which makes reposition difficult. Cross-cutting of knitting needles in the heel region eliminates the possibility of performing an open intervention on the calcaneus due to the high risk of infectious complications.
Closest to the proposed invention is a method of treating compression fractures of the calcaneus [4], including the use of external structures for transosseous osteosynthesis: rings, arches, connecting rods and spokes, the introduction of two intersecting spokes in the lower third of the leg at an angle of 45 ° to the front and at an angle of 15 ° to the sagittal planes, one of the knitting needles being drawn through both bones, the third knitting needle being drawn at an angle of 15 ° to the sagittal plane through the calcaneal tubercle at the site of attachment of the Achilles tendon so that the bones of the base and repositional supports coincided, and the spokes are fixed in the ring of the base support and the arch of the repositional support eccentrically with removal to the ventral side by the amount of compression of the calcaneus, and the supports are connected with threaded rods, and the apparatus for treating calcaneus fractures used to implement it.
The disadvantages of the method selected as a prototype include the cumbersome design of the apparatus used for the treatment of calcaneus fractures with the need for through-insertion of spokes, the biomechanical scheme of controlled elimination of displacement along the width and length of the calcaneus does not allow to calculate the ratio of the elongation of the connecting rods and their angular displacement by rotation their nuts, which requires continuous dynamic x-ray control of the reposition process, through the conductivity of the knitting needles soft tissues and calcaneus exclude the possibility of performing an open intervention in the calcaneal region due to the high risk of infectious complications during its implementation, distraction of fragments by connecting rods inevitably leads to back flexion of the foot, which further complicates the reposition of fragments, limited repositional capabilities and the empirical nature of the actions during reposition of fragments, it practically makes it impossible to eliminate the displacements of fragments that form the articular surface, the absence of fixation of the ankle on the joint can not provide the immobility of the fragments during functional treatment.

The objective of the present invention is to develop a method for restoring the integrity and functionality of the human calcaneus with fractures of the calcaneus and the creation of an apparatus for restoring the integrity and functionality of the calcaneus of a person in fractures of the calcaneus.
