Abstract
Description
The invention relates to medicine, namely to surgical otolaryngology, and can be used in the surgical treatment of patients with perichondritis of the auricle.
Т. М. А. 375-377).A known method of surgical treatment of the auricle in the treatment of perichondritis of its cartilage frame (V.T. Palchun, M.M. Magomedov, L.A. Luchikhin. Otorhinolaryngology. - M .: Medicine, 2007, pp. 375-377).
According to the known method, after local anesthesia, the skin of the auricle is dissected with a scalpel and necrotic cartilage tissue is removed with a spoon. The operation is completed by installing a rubber graduate and applying a bandage. The method is characterized by a significant number of painful manipulations in the wound, accompanied by significant bleeding, hemostasis is performed by applying a tight dressing. Due to the invasiveness of the operation, large blood loss, followed by the development of scars and gross deformities of the auricle, the number of complications in the postoperative period is from 8 to 37%. Complications are associated with excessive trauma to the tissues of the auricle, the formation of dense fibrous tissue in place of necrotic cartilage, bleeding from a large incision and persistent otology.
The invention is aimed at reducing the invasiveness of the operation, reducing the number of complications in the postoperative period, increasing the cosmetic effect of the treated auricle.
A method for surgical treatment of perichondritis of the auricle, including dissection of the skin of the auricle and removal of necrotic cartilage tissue, is claimed. The method is characterized in that an incision repeating its contour is made on the skin of the auricle, with a length of not more than 3.5 cm, purulent discharge and necrotic tissue of the cartilage are removed, a cryoprobe with a diameter of at least 3 cm is introduced into the tunnel formed to a healthy cartilage of the auricle, cooled to a temperature minus 187 degrees and perform at least 2 cycles of "freezing-thawing" on the cartilage and incision to completely remove purulent discharge and reduce inflammation of the auricle cartilage.
A less traumatic incision that reduces bleeding on the skin of the auricle is performed so that it repeats its contour. This technique prevents subsequent scars and gross deformities of the auricle in the postoperative period. To completely remove purulent discharge and reduce inflammation of the cartilage of the auricle, at least 2 cycles of “freezing-thawing” are performed on the cartilage and incision using a cryoprobe with a diameter of at least 3 cm, cooled to a temperature of minus 187 degrees. In this case, there is no excessive bleeding, healthy soft tissues of the auricle with a cryoprobe are not injured, despite the fact that its miniature size provides a sufficient overview of the surgical field, without limiting the actions of the surgeon and without affecting healthy tissues of the auricle. Due to the rapid subsidence of the inflammatory process, the number of painful manipulations in the wound decreases.
A new technical result achieved by the invention is the prevention of hematomas, bleeding, as well as scars and gross deformations of viable cartilage in the postoperative period.
The most convenient for cryoapplications is a probe with a diameter of 3 mm. The best working mode is the exposure of the probe, previously cooled to t -187 degrees, to the area of inflamed cartilage 10 s 2-3 cycle "freeze-thaw". The method is implemented during surgical treatment of the auricle while removing necrotic cartilage.
Example. Sick D-s, 25 years old. Complaints of pain in the right auricle, swelling of the right auricle, its hyperemia. Complaints appeared after an ear injury. On examination, it was found: sharp pain on palpation of the auricle, its hyperemia, infiltration and crepitus under the skin. General condition is satisfactory. The pulse rate is 80 beats per minute, blood pressure is 135780 mm Hg, body temperature is 37.8 g., There are no pathologies from the cardiovascular system, no parinchymal organs of the abdominal cavity, no chest organs were found.
Preparation for the operation is traditional. After local anesthesia and aseptic processing of the surgical field with a scalpel, a 3.5 cm incision was made on the skin of the right auricle. The incision follows the shape of the auricle. Using a spoon, purulent discharge and necrotic cartilage tissue are removed. A cryoprobe, cooled to a temperature of -187 g, was introduced into the formed tunnel to a healthy cartilage of the auricle. and cryoapplications were carried out. The procedure was repeated three times on the 3rd, 5th and 7th day of treatment of the patient in the hospital, until the purulent discharge was completely removed and the auricle cartilage inflammation was reduced. As a result of the operation, all pathological necrotic tissues were removed with minor bleeding. Since the inflammatory process tended to rapidly decay, the number of painful manipulations in the wound decreased, and a good cosmetic result was obtained. The operation was completed by applying a sterile cloth with an antiseptic solution.
According to this method, 20 people were operated on. Complications in the form of bleeding, adhesions, cicatricial deformities of the affected ear were not observed, which allowed to achieve rapid rehabilitation of patients.
