При введении препаратов гиалуроновой кислоты улучшается смазка суставных поверхностей, уменьшается механическое повреждение хряща в момент функциональной нагрузки. Однако существующие в настоящее время препараты являются экзогенным субстанциями, а для их производства используются полисахариды различной молекулярной массы и происхождения, а также отличающиеся концентрацией гиалуроната натрия, в связи с чем всегда существует риск нежелательных побочных эффектов, таких как различные аллергические реакции, острый асептический артрит, острый синовит, развитие острого подагрического и псевдоподагрического артрита. Клинически недоказанное воздействие этих препаратов на регенераторные способности хрящевой ткани сустава, непродолжительный клинический эффект, высокая стоимость препаратов, содержащих гиалуроновую кислоту, а также довольно высокая частота побочных явлений создают необходимость поиска более эффективных и доступных методик лечения остеоартроза коленного сустава.With the introduction of hyaluronic acid preparations, the lubrication of the articular surfaces improves, the mechanical damage to the cartilage at the time of the functional load is reduced. Наиболее близким к предлагаемому способу лечения является «Способ лечения деформирующего остеоартроза крупных суставов», при котором в сустав вводят нативную аутоплазму в количестве 12-18 мл 2-3 раза на курс лечения с интервалом 4-5 дней (Патент RU №2136249 С1, 1999 г.). Для получения богатой тромбоцитами аутоплазмы за 1-2 часа перед манипуляцией производят забор крови из кубитальной вены в специальные одноразовые пластиковые мешки гемаконы емкостью 450 мл, содержащих 63 мл антикоагулянта на основе цитрата. Содержание тромбоцитов в которой составляет от 162 до 358 тыс./мкл (в среднем 256 тыс./мкл). Кровь центрифугируют на роторной центрифуге PC - 6-7 минут в режиме 2500 об/мин. Полученную в супернатанте аутоплазму подвергают второму центрифугированию в течение 30 мин в режиме 3000 об/мин. После чего бедную тромбоцитами аутоплазму удаляют, с извлечением 15-20 мл богатой тромбоцитами аутоплазмы. Содержание тромбоцитов в полученной аутоплазме составляет от 725 до 3029 тыс./мкл (в среднем 1354 тыс./мкл). Полученную эритроцитарную массу возвращают в кровяное русло. Полученную богатую тромбоцитами аутоплазму смешивают с 0,25% раствором CaCl2 в соотношении 3:1. Наружным доступом, после местной анестезии кожи, производят пункцию коленного сустава с введением в полость сустава 10 мл богатой тромбоцитами аутоплазмы. Затем 4-5 мл богатой тромбоцитами аутоплазмы вводят в наиболее болезненные зоны параартикулярных тканей, которые определяют при пальпации. Больной Золотарев В., 45 лет, поступил в травматологическое отделение 16.08.2009 г. с жалобами на боли в правом коленном суставе, ограничение движений. Боли возникают при ходьбе, при длительном стоянии, при подъеме по лестнице. В анамнезе травма - в возрасте 43 лет на производстве получил внутрисуставной оскольчатый перелом мыщелков болыпеберцовой кости, с дефектом костной ткани. По поводу чего была произведена операция - закрытая репозиция, чрескостный компрессионно-дистракционный остеосинтез по Илизарову. Боли в суставе стали беспокоить через 5 месяцев, сразу после демонтажа аппарата внешней фиксации и началом осевой нагрузки на конечность. Нарастание болей отмечает после выхода на работу, особенно после длительного стояния, так как пациент работает станочником на бетонном производстве. Систематически проходил курсы лечения амбулаторно в поликлинике по месту жительства с применением анальгетиков, физиопроцедур и неоднократным введением в полость сустава дипроспана, лечение имело непродолжительный клинический эффект, полного исчезновения болей не отмечалось. При объективном осмотре отмечается сглаженность контуров сустава справа. При пальпации отмечается болезненность, больше по наружной поверхности. При пальпации надколенника отмечается крепитация, смещение в стороны вызывает боль. Объем движений в суставе: разгибание 180° при вертикальном положение тела, сгибание до 95°. На рентгенограмме имелись изменения, соответствующие II стадии остеоартроза с переходом в III стадию. 19.08.10 произведен забор крови в объеме 450,0 мл из кубитальной вены в специальный одноразовый пластиковый мешок гемакон емкостью 450 мл, содержащий 63 мл антикоагулянта на основе цитрата. Содержание тромбоцитов в которой составило 194 тыс./мкл. Кровь центрифугировали на роторной центрифуге PC - 6-7 минут в режиме 2500 об/мин. Полученную в супернатанте аутоплазму подвергли второму центрифугированию в течение 30 мин в режиме 3000 об./мин. После чего бедную тромбоцитами аутоплазму удалили, а извлекли 20 мл богатой тромбоцитами аутоплазмы. Содержание тромбоцитов в полученной аутоплазме составило 2677 тыс./мкл. Полученную эритроцитарную массу возвратили в кровяное русло. Полученную богатую тромбоцитами аутоплазму смешали с 0,25% раствором CaCl2 в соотношении 3:1. Через 2 часа после забора крови наружным доступом, после местной анестезии кожи, произвели пункцию правого коленного сустава и ввели 15 мл богатой тромбоцитами аутоплазмы, активированной 0,25% раствором CaCl2. Оставшиеся 5,0 мл богатой тромбоцитами аутоплазмы ввели в параартикулярные ткани по наружной поверхности коленного сустава в точки наибольшей болезненности. 26.08.2010 процедура повторена. После проведения процедур пациенту запрещали осевую нагрузку на конечность с ходьбой на костылях в течение 3 дней. Осложнений в процессе проведения процедур не отмечено. Через 10 суток после проведенного лечения состояние больного и функция сустава значительно улучшились. Полностью прекратились боли в суставе. При объективном осмотре отека нет, болей при пальпации не отмечает, сгибание в правом коленном суставе 75°. Выписан в удовлетворительном состоянии. Осмотрен через 4 и 12 месяцев, жалоб не предъявляет, работает на прежней работе.Patient Zolotarev V., 45 years old, was admitted to the trauma department on 08/16/2009 with complaints of pain in the right knee joint, restriction of movements. Joint pain began to be disturbed after 5 months, immediately after the dismantling of the external fixation apparatus and the onset of axial load on the limb. The increase in pain is noted after going to work, especially after prolonged standing, as the patient works as a machinist in a concrete production. He systematically underwent outpatient treatment at the clinic at the place of residence with the use of analgesics, physiotherapy and repeated administration of diprospan into the joint cavity, the treatment had a short clinical effect, there was no complete disappearance of pain. An objective examination indicates a smooth contour of the joint on the right. On palpation, pain is noted, more on the outer surface. On palpation of the patella, crepitus is noted, displacement to the sides causes pain. Range of motion in the joint: 180 ° extension with a vertical position of the body, flexion to 95 °. On the x-ray there were changes corresponding to stage II of osteoarthrosis with the transition to stage III. 08/19/10 blood sampling was performed in a volume of 450.0 ml from a cubital vein into a special disposable plastic bag hemacon with a capacity of 450 ml, containing 63 ml of an anticoagulant based on citrate. The platelet count in which amounted to 194 thousand / ál. Blood was centrifuged on a PC rotary centrifuge for 6-7 minutes at 2500 rpm. The autoplasma obtained in the supernatant was subjected to second centrifugation for 30 min at 3000 rpm. After that, platelet-poor autoplasma was removed and 20 ml of platelet-rich autoplasma was removed. The platelet count in the resulting autoplasma was 2677 thousand / μl. The resulting erythrocyte mass was returned to the bloodstream. The resulting platelet-rich autoplasma was mixed with a 0.25% CaCl 2 solution in a 3: 1 ratio. 2 hours after blood sampling by external access, after local anesthesia of the skin, a right knee joint was punctured and 15 ml of platelet-rich autoplasma activated with 0.25% CaCl 2 solution were injected. The remaining 5.0 ml of platelet-rich autoplasma was injected into the paraarticular tissue along the outer surface of the knee joint at the point of greatest pain. 08/26/2010 the procedure is repeated. After the procedures, the patient was forbidden axial load on the limb with walking on crutches for 3 days. There were no complications during the procedure. 10 days after the treatment, the patient's condition and joint function improved significantly. The pain in the joint has completely stopped. With an objective examination of the edema, there is no pain during palpation, flexion in the right knee joint 75 °. Discharged in satisfactory condition. Examined after 4 and 12 months, no complaints, works at the previous job.
Положительный эффект лечения объясняется тем, что при введении богатой тромбоцитами аутоплазмы в полость сустава происходит улучшение скольжения суставных поверхностей и уменьшение механического повреждения суставного хряща при функциональной нагрузке. Богатая тромбоцитами аутоплазма играет важную роль в хрящевой регенерации регулирующими клетками, протеиновым синтезом и факторами роста. Она стимулирует мезенхимальные клетки, что способствует пролиферации хондроцитов. Факторы роста, освобожденные из тромбоцитов, побуждают ткани сустава к секреции гиалуроновой кислоты, что способствует созданию гомеостатического окружения для восстановления ткани внутри сустава и параартикулярных образований. Преимущество богатой тромбоцитами аутоплазмы в том, что она содержит значительно большее количество тромбоцитов, а значит, и факторов роста. Она аутологична, апирогенна и нетоксична, что создает идеальную основу для клеточной инкубации. Отсутствует опасность внесения вируса гепатита, ВИЧ и других инфекций. The method is simple, does not require expensive equipment and special training of medical personnel and can be used in the treatment of osteoarthritis.
Описанный способ лечения остеоартроза применен у 19 больных. У всех больных удалось добиться купирования болевого синдрома от 7 до 18 месяцев, а также увеличения объема движений в пораженном суставе.
