Гипертензивные расстройства при беременности - одна из актуальных проблем в современном акушерстве, так как являются распространенной патологией при беременности и занимают ведущее место в структуре материнской и перинатальной заболеваемости и смертности. В большинстве случаев (70%) артериальная гипертензия встречается при гестозе (преэклампсии), реже - это хроническая артериальная гипертензия (ХАГ) (30%). Прогноз, тактика лечения и родоразрешения различается при различных гипертензивных состояниях.Hypertensive disorders during pregnancy are one of the urgent problems in modern obstetrics, as they are a common pathology during pregnancy and occupy a leading place in the structure of maternal and perinatal morbidity and mortality. Аналогами данного метода являются описанные в отечественной и зарубежной литературе методы клинической диагностики гестоза (преэклампсии), основанные на выявлении отеков, артериальной гипертензии и суточной протеинурии (МКБ - 10 [1], ACOG [2]). Однако классическая триада симптомов отмечается в 50-60% случаев, у остальных отсутствует один из симптомов, в основном отеки или протеинурия (Шалина Р.И. [3], Шифман Е.М. [4]). В России предложена бальная шкала, разработанная Савельевой Г.М. на основе шкалы Goeke [5], в которой наряду с клиническими симптомами учитывается длительность заболевания, состояние внутриутробного плода и сопутствующая экстрагенитальная патология. С целью выявления тяжелых форм гестоза (преэклампсии) предложены различные методы лабораторной диагностики: исследование коагуляционных свойств крови, гематокрита, печеночных ферментов, биохимических показателей крови и мочи, иммунологические и методы генетической диагностики (Макацария А.Д. [6], Шалина Р.И. и соавт. [3], Conde-Agudelo A. et al. [7], Parra М. et al. [8]). Все предложенные лабораторные тесты, основываются на единичных показателях гомеостаза, отражая отдельные звенья патогенеза и дисфункцию органов, однако не отражают глубины расстройств микроциркуляции, приводящей к нарушению доставки и потребления кислорода тканями.In order to identify severe forms of preeclampsia (preeclampsia), various laboratory diagnostic methods have been proposed: the study of the coagulation properties of blood, hematocrit, liver enzymes, biochemical parameters of blood and urine, immunological and genetic diagnostics methods (Makatsaria AD [6], Shalina R.I. . et al. Прототипом данного изобретения является классификация гипертензивных расстройств при беременности, предложенная Рабочей группой по высокому артериальному давлению при беременности Всероссийского научного общества кардиологов (ВНОК) [9]. ХАГ определяется при наличии систолического АД≥140 мм рт.ст. и/или диастолического АД ≥90 мм рт.ст. Сущность изобретенияSUMMARY OF THE INVENTION
Цель изобретения заключается в разработке способа дифференциальной диагностики гипертензивных расстройств при беременности, а именно хронической артериальной гипертензии (ХАГ) и гестоза (преэклампсии), на основании исследования основных параметров гемодинамики, кислородного статуса и уровня лактата крови женщины в третьем триместре беременности.The purpose of the invention is to develop a method for the differential diagnosis of hypertensive disorders in pregnancy, namely chronic arterial hypertension (CAH) and preeclampsia (preeclampsia), based on the study of the main hemodynamic parameters, oxygen status and blood lactate level of a woman in the third trimester of pregnancy.
Способ осуществляют следующим образом:The method is as follows:
Измеряют основные параметры гемодинамики - среднее артериальное давление (АД ср) и частоту сердечных сокращений (ЧСС) при поступлении пациентки в акушерские клиники или отделение интенсивной терапии. Проводят исследование кислородного статуса и уровня лактата в артериализованной капиллярной и венозной крови. Образцы крови забирают из предварительно согретого пальца [10, 11] и локтевой вены пациентки в количестве 0,2 мл в гепаринизированный капилляр. Исследования выполняют немедленно на автоматическом анализаторе газов крови "ABL-700" (Дания). Вычисляют коэффициент тканевой экстракции кислорода (КЭО2) по формуле: КЭО2 (%) = (CaO2-CvO2)/СаО2·100%, где СаО2 и CvO2 - содержание кислорода в артериальной и венозной крови (мл/л) (Золотокрылина B.C. Способ дает вероятность правильной принадлежности беременной к группе с хронической артериальной гипертензией в 88% и к группе с тяжелым гестозом в 92% случаев при уровне значимости Р<0,001. При исследовании плода установлена хроническая фетоплацентарная недостаточность (ХФПН), компенсированная форма. Проведено суточное мониторирование АД, установлена артериальная гипертензия I степени. Протеинурия - отрицательная. В сроке 39-40 недель произведено кесарево сечение, в связи с "незрелой" шейкой матки и возрастом первородящей. Новорожденный родился с оценкой по Апгар 8/9 баллов, масса тела 3650 г, рост 52 см. Поступила в стационар в сроке 33 недели беременности, при обследовании получены следующие данные: АД ср. - 104 мм рт.ст.; ЧСС - 69 ударов в мин.; уровень лактата в артериализованной крови - 1,2 ммоль/л; КЭО2 - 13,95%, протеинурия - 0,06 г/л.Example 2. Подсчитан диагностический индекс D=-0,64, что свидетельствует о принадлежности беременной женщины к группе с тяжелым гестозом. При ультразвуковом исследовании выявлена ХФПН, субкомпенсированная форма, задержка развития внутриутробного плода. По совокупности клинико-лабораторных показателей диагностирован гестоз тяжелой степени, по Г.М. Савельевой 13 баллов. Несмотря на интенсивную терапию отмечалось прогрессирование гестоза, и потребовалось родоразрешение в сроке 33-34 недели путем кесарева сечения. Ребенок родился с оценкой по Апгар 6/7 баллов, масса тела 2120 г, рост 44 см. Новорожденному потребовалась длительная интенсивная терапия, выписан на 24 сутки с диагнозом - ишемически-гипоксическое поражение центральной нервной системы тяжелой степени.The diagnostic index D = -0.64 was calculated, which indicates that the pregnant woman belongs to the group with severe gestosis. Таким образом, предлагаемый способ дифференциальной диагностики гипертензивных расстройств при беременности в третьем триместре позволяет достоверно определить группы пациенток с хронической артериальной гипертензией и тяжелым гестозом, своевременно начать адекватную интенсивную терапию и решить вопрос о пролонгировании беременности или необходимости родоразрешения.Thus, the proposed method for the differential diagnosis of hypertensive disorders in pregnancy in the third trimester allows one to reliably identify groups of patients with chronic arterial hypertension and severe gestosis, to start adequate intensive therapy in a timely manner, and to resolve the issue of prolonging pregnancy or the need for delivery.
Способ не требует дорогостоящей материально-технической базы, специальных реактивов, удобен в исполнении при использовании минимального количества исследуемого материала, обеспечивает получение необходимой информации в течение 15 минут с момента забора крови пациента, воспроизводим на любом уровне.
