Изобретение относится к области медицины, в частности к акушерству, а также к области научных исследований – патологической физиологии, и предназначено для прогнозирования риска развития анемии у беременных женщин с COVID-19 пневмонией.The invention relates to the field of medicine, in particular to obstetrics, as well as to the field of scientific research - pathological physiology, and is intended to predict the risk of anemia in pregnant women with COVID-19 pneumonia.
Коронавирусная инфекция COVID-19, вызванная SARS-CoV-2, в настоящее время является всемирной проблемой здравоохранения. Особую группу населения, требующую повышенного внимания, составляют беременные вследствие того, что пневмония занимает третье место в структуре непрямых причин материнской смертности. В зоне повышенного риска инфицирования оказались беременные женщины, иммунная система которых изменена. Иммуносупрессия и другие физиологические изменения во время беременности способствуют высокой восприимчивости организма женщины к патогенам, вызывающим острую респираторную вирусную инфекцию и пневмонию, что может потребовать госпитализации в отделение интенсивной терапии и проведения респираторной кислородной поддержки. При этом состояние беременности ограничивает применение некоторых методов инструментальной диагностики и антикоронавирусной терапии [2, 6, 10].The COVID-19 coronavirus infection caused by SARS-CoV-2 is currently a worldwide public health problem. Immunosuppression and other physiological changes during pregnancy contribute to a woman's high susceptibility to pathogens that cause acute respiratory viral infection and pneumonia, which may require hospitalization in the intensive care unit and respiratory oxygen support. At the same time, the state of pregnancy limits the use of some methods of instrumental diagnostics and anticoronavirus therapy [2, 6, 10].

С другой стороны, по мнению исследователей, беременность обостряет характерное для COVID-19 острое воспаление, повышая риск «цитокинового шторма», который, в свою очередь, увеличивает риск потери беременности и способствует формированию полиогранной патологии у беременной и плода. В частности, деструкция эритроцитов вследствие острого воспаления приводит к гипоксии и бесконтрольному перераспределению железа между тканями. As a result, conditions are simultaneously created both for the formation of hemosiderosis of the lungs and other tissues, and for increased iron loss by the body, which exacerbate iron deficiency anemia (IDA) in pregnant women [9].

In particular, in women during pregnancy, ESR values are often increased (more than 20 mm/hour), which corresponds to a decrease in the number of functional erythrocytes that carry out gas exchange [13]. A decrease in the number and quality of erythrocytes is facilitated by deficiencies of folates and other B vitamins [8]. Thirdly, in pregnant women (especially by the time of delivery), the iron depot is depleted and IDA is formed [9].

У беременных в I триместре нормальная концентрация сывороточного ферритина должна превышать 63,0 нг/мл, во II триместре – 43,0 нг/мл, а в III триместре – 12,0 нг/ мл. Снижение концентрации сывороточного ферритина нижеуказанных значений означает снижение концентрации гемоглобина. На следующем этапе пациенткам с анемией проводится исследование уровня интерлейкина-6, при значении которого 1,75 пг/мл и выше пациентка относится к группе высокого риска анемии хронических заболеваний и проводится исследование концентрации гепсидина. Пациентка, 28 лет, поступила в пульмонологическое отделение Амурской областной клинической больницы (г. Благовещенск) с диагнозом: беременность 34 недели. Короновирусная инфекция, вызванная SARS-CoV-2, подтверждена лабораторно.The patient, 28 years old, was admitted to the pulmonology department of the Amur Regional Clinical Hospital (Blagoveshchensk) with a diagnosis of pregnancy 34 weeks. Coronavirus infection caused by SARS-CoV-2 has been laboratory confirmed.
Пациентка предъявляла жалобы на насморк, заложенность носа, сухой кашель, першение в горле, повышение температуры до 37,5 °С.The patient complained of a runny nose, nasal congestion, dry cough, sore throat, fever up to 37.5 °C.
При поступлении состояние пациентки расценено как средней степени тяжести.On admission, the patient's condition was assessed as moderate.
Проведены обследования. Выполнено КТ органов грудной клетки – объем выявленного поражения легочной паренхимы 8%, мониторинг витальных функций SpO2 – 99%, ЧД – 25 в минуту, ЧСС – 79, АД – 125/80, температура тела – 37,8 °С. Гематологические показатели: Hb – 123 г/л, общее количество Er – 4,0×1012/ л. На основе чего был выставлен диагноз: левосторонняя нижнедолевая пневмония, легкой степени тяжести. По заявленному способу прогностический индекс, рассчитанный по формуле ПИ=-140,961+(8,916×Hb)-(8,747×SpO2), составил – (89), что свидетельствовало об отсутствии риска развития анемии. Данный способ прошел клиническую апробацию. С его помощью проведено прогнозирование угрозы развития анемии у 54 беременных с COVID-19 пневмонией в третьем триместре. Правильный прогноз определялся в 100% случаев, что подтверждает высокую эффективность заявляемого способа.This method has been clinically tested.
