Изобретение относится к области медицины, а именно к акушерству. Способ позволяет обеспечить повышение точности, информативности, чувствительности, объективности определения степени тяжести гестоза и оптимизации лечебной тактики ведения беременных с гестозом.The invention relates to medicine, namely to obstetrics. Актуальность проблемы состоит в том, что до настоящего времени существует проблема в прогнозировании степени тяжести гестоза, который является одним из тяжелых осложнений беременности, занимающее ведущее место среди причин материнской и перинатальной смертности. На современном этапе в патогенезе гестоза большое значение придается эндотелиальной дисфункции [1]. Одним из проявлений патологии эндотелия являются расстройства микроциркуляции, которые обуславливают развитие основных симптомов гестоза [2]. Расстройства микроциркуляции ведут к нарушению тканевого метаболизма, что, в свою очередь, уменьшает утилизацию кислорода тканями из крови. Это имеет определенное значение в повышении оксигенации венозной крови и в развитии тканевой гипоксии. Блокада системы микроциркуляции на фоне повышенного системного артериального давления приводит к сбросу крови через анастомозы из артериальной системы в венозную [4]. При этом по мере прогрессирования гестоза беременных наблюдается повышение цифр парциального давления кислорода в венозной крови. Поэтому определение степени тяжести гестоза, его прогнозирование играет важную роль в определении тактики ведения беременных с гестозом. Известен способ диагностики гестоза посредством измерения допплерометрических индексов сосудистой резистентности в системе маточных и пуповинных артерий. In this case, with the progression of gestosis of pregnant women, an increase in the figures of the partial pressure of oxygen in the venous blood is observed. Therefore, the determination of the severity of gestosis, its prognosis plays an important role in determining the management of pregnant women with gestosis. A known method for the diagnosis of gestosis by measuring the Doppler indices of vascular resistance in the uterine and umbilical arteries. In the spiral arteries, these changes were noted at 16-19 weeks [3].
Недостатком данного способа является недостаточная специфичность, обусловленная высокой частотой ложноположительных результатов, так как такие же изменения кровотока происходят и при других патологических процессах, например при артериальной гипертензии, антифосфолипидном синдроме. Предлагаемый способ оценки степени тяжести гестоза определен и выбран на основании комплексного динамического обследования беременных женщин в возрасте от 18 до 40 лет при сроке беременности 22-41 недели. У 54 женщин беременность протекала физиологически - они составили контрольную группу. У остальных 211 женщин беременность осложнилась гестозом. Парциальное давление кислорода в венозной крови в контрольной группе на протяжении всего периода наблюдения находилось в пределах от 27 до 35 миллиметров ртутного столба и в среднем составило 31±4,7 миллиметр ртутного столба. Значения парциального давления кислорода в венозной крови от 36 до 43 миллиметров ртутного столба, в среднем 39±4,2 миллиметр ртутного столба, оценивались как легкая степень гестоза. При содержании парциального давления кислорода в венозной крови от 43 до 55 миллиметров ртутного столба, в среднем 49±3,2 миллиметр ртутного столба, оценивают гестоз средней степени тяжести. Из приведенных данных следует, что определение парциального давления кислорода в венозной крови у беременных с гестозом можно использовать в качестве более корректного критерия определения степени тяжести гестоза, причем высокая чувствительность и точность метода, а также выделение стадии его доклинических проявлений позволит принять своевременные профилактические меры, пролонгировать беременность, снизить перинатальную и материнскую заболеваемость и смертность. В дальнейшем на фоне проводимой терапии, отеки нарастали, артериальное давление оставалось стабильно высоким - 140 и 100 миллиметров ртутного столба, женщина была родоразрешена оперативным путем в связи с отсутствием эффекта от терапии гестоза средней степени тяжести у повторнородящей с рубцом на матке после операции кесарево сечение.Clinical example 3.
