Срединные стенозы гортани чаще всего развиваются после струмэктомии вследствие повреждения гортанных нервов: во время операции (натяжение, прижатие нерва); после операции в связи с воспалительными явлениями, отеком и гематомой (в первые дни после операции); после операции в связи с последующим рубцеванием (на поздних сроках).Median laryngeal stenosis most often develops after a stumectomy due to damage to the laryngeal nerves: during surgery (tension, nerve compression); after surgery in connection with inflammatory phenomena, edema and hematoma (in the first days after surgery); after surgery in connection with subsequent scarring (in the later stages).

Наиболее близким к заявленному изобретению аналогом является способ хирургического лечения стенозов гортани, заключавшийся в рассечении пластины щитовидного хряща спереди парамедиально на всем протяжении; пластины щитовидного хряща разводили в стороны, отслаивали от внутренней надхрящницы боковую перстнечерпаловидную мышцу до латерального края черпаловидного хряща. Последний захватывали и выводили в операционную рану. Рассекали боковую заднюю перстнечерпаловидные мышцы, поперечную и косые черпаловидные мышцы. Далее спереди назад до черпаловидного хряща наружную щиточерпаловидную мышцу отделяли от внутренней. Черпаловидный хрящ удалялся с блоком мышц, за исключением отделенного голосового отростка. Затем П-образными швами голосовую мышцу фиксировали на поверхности щитовидного хряща, последний ушивали по наружной надхрящнице, вводили валик-обтуратор над трахеостомой. При благоприятном течении заживления послеоперационной раны на 8-9 сутки валик-обтуратор заменяли Т-образным силиконовым эндопротезом, с которым больного выписывали из стационара (Лапченко С.Н., Гаджиев Р.Ш. Вестник оториноларингологии 1987; 4:44-48). Его недостатком является высокая травматичность, обусловленная необходимостью отделения слизистой оболочки гортани от голосовых отростков черпаловидных хрящей, зоны перстнечерпаловидных суставов. Удаление черпаловидного хряща с блоком мышц может нарушить защитную функцию гортани (Мареев О.В., Старостина С.В. Вестник оториноларингологии 2007; 4:52-55). Величина разреза слизистой оболочки весьма большая, а главное, он производится в непораженном участке гортани, раневая поверхность в результате реализации указанных действий получается чрезвычайно большой, что обусловливает болезнь слизистой, длительность ее приживления, реакцию перихондрия черпаловидных и перстневидного хрящей в виде воспаления (перихондрита) и, как следствие, избыточный рост рубцовой ткани, и, соответственно, рецидив заболевания. Этому также способствует длительное нахождение в полости гортани Т-образной трубки (3-4 месяца), не исключается развитие гнойно-некротических осложнений вокруг валика-обтуратора или эндопротеза в послеоперационном периоде. Практика показала недостаточную эффективность способа-прототипа.The closest analogue to the claimed invention is a method for surgical treatment of stenosis of the larynx, which consists in dissecting the plate of the thyroid cartilage anteriorly paramedially throughout; plates of the thyroid cartilage were bred apart, the late cricoid muscle was exfoliated from the inner perichondrium to the lateral edge of the arytenoid cartilage. Further, from front to back to the arytenoid cartilage, the external thyroid muscle was separated from the internal. The arytenoid cartilage was removed with a block of muscle, with the exception of the separated vocal process. Then, with the U-shaped sutures, the vocal muscle was fixed on the surface of the thyroid cartilage, the latter was sutured along the external perichondrium, an obturator roller was inserted over the tracheostomy. With a favorable course of healing of the postoperative wound for 8-9 days, the obturator roller was replaced with a T-shaped silicone endoprosthesis, with which the patient was discharged from the hospital (Lapchenko S.N., Gadzhiev R.Sh. Bulletin of Otorhinolaryngology 1987; 4: 44-48). Its disadvantage is the high morbidity due to the need to separate the mucous membrane of the larynx from the vocal processes of the arytenoid cartilage, the area of the cirrus joints. Removal of the arytenoid cartilage with a block of muscles may impair the protective function of the larynx (Mareev O.V., Starostina S.V. Vestnik of otorhinolaryngology 2007; 4: 52-55). The size of the section of the mucous membrane is very large, and most importantly, it is made in the unaffected area of the larynx, the wound surface as a result of these actions is extremely large, which causes mucosal disease, the duration of its engraftment, the reaction of the perichondria of the arytenoid and cricoid cartilage in the form of inflammation (perichondritis) and as a result, excessive growth of scar tissue, and, accordingly, relapse of the disease. This also contributes to the long-term presence of a T-shaped tube in the larynx cavity (3-4 months), the development of purulent necrotic complications around the obturator roller or endoprosthesis in the postoperative period is not ruled out. Practice has shown a lack of effectiveness of the prototype method.
Целью настоящего изобретения является разработка способа хирургического лечения срединных стенозов гортани, позволяющего снизить травматичность хирургического лечения стенозов и повысить тем самым эффективность лечения, резко уменьшить рецидивы заболевания, сократить длительность стационарного лечения.The aim of the present invention is to develop a method of surgical treatment of median stenosis of the larynx, which allows to reduce the invasiveness of surgical treatment of stenosis and thereby increase the effectiveness of treatment, dramatically reduce relapse of the disease, reduce the duration of inpatient treatment.
Предлагаемый способ осуществляют следующим образом: больного укладывают на спину. Операционное поле обрабатывают раствором антисептика, отграничивают стерильными простынями. Под общим обезболиванием (наркоз через естественные пути) проводят разрез мягких тканей шеи по средней линии, от уровня подъязычной кости, до уровня 3-го кольца трахеи. Мягкие ткани разводят в стороны, обнажают тело подъязычной кости, щитовидный хрящ, дугу перстневидного хряща и верхние три кольца трахеи. Во втором кольце трахеи устанавливают отверстие, формируют трахеостому и операцию продолжают с интубацией через трахеостому. Щитовидный хрящ рассекают по средней линии, вскрывают просвет гортани, затем также по средней линии рассекают печатку перстневидного хряща. Мобилизованные таким образом половины гортани разводят в стороны и в просвет гортани вставляют трубку-дилятатор. Для дополнительной и надежной фиксации половин гортани в заданном положении их фиксируют путем наложения лигатуры, проведенной через верхний край щитовидного хряща к большому рогу подъязычной кости на расстоянии 1 см от места отхождения малого рога. Тем самым передневерхний край гортани подтягивают к месту наложения лигатуры. Аналогично проводят наложение лигатуры и на противоположной стороне. В результате подъязычная кость выполняет своеобразную роль распорки между разведенными половинами гортани. Срок нахождения трубки-дилятатора в просвете гортани во временном интервале укорачивается.The proposed method is as follows: the patient is laid on his back. An opening is established in the second ring of the trachea, a tracheostomy is formed, and the operation is continued with intubation through the tracheostomy. The thyroid cartilage is dissected in the midline, the lumen of the larynx is opened, then the signet of the cricoid cartilage is also dissected in the midline. The halves of the larynx mobilized in this way are pulled apart and a dilator tube is inserted into the lumen of the larynx. For additional and reliable fixation of the halves of the larynx in a given position, they are fixed by applying a ligature drawn through the upper edge of the thyroid cartilage to the large horn of the hyoid bone at a distance of 1 cm from the place of discharge of the small horn. Thus, the anteroposterior edge of the larynx is pulled to the place of application of the ligature. Similarly, the ligature is applied on the opposite side. As a result, the hyoid bone performs a peculiar role of the spacer between the diluted halves of the larynx. The time spent by the dilator tube in the lumen of the larynx in the time interval is shortened.
Если при обычном методе трубка-дилятатор находится в просвете 3-4 месяца, при использовании данного метода трубку-дилятатор можно убрать через 3 недели. За это время между рассеченными половинками гортани в межхрящевом пространстве формируется интерпозиция тканей, после удаления трубки-дилятатора разведенные половины гортани остаются в заданном положении. В данном случае дополнительная фиксация гортани к подъязычной кости и использование ее в качестве распорки укорачивает сроки нахождения трубки-дилятатора до 3-х недель и укорачивает сроки излечения. После удаления трубки-дилятатора удаляется и трахеостомическая трубка и больной переходит на дыхание через естественные пути. Способ позволяет добиться снижения травматичности хирургического лечения, уменьшения рецидивов заболевания и сокращения длительности стационарного лечения.If in the usual method the dilator tube is in the lumen for 3-4 months, when using this method the dilator tube can be removed after 3 weeks. Хирургическое лечение заключалось в рассечении передней и задней стенок гортани по средней линии, разведении половин гортани в стороны и фиксации в этом состоянии при помощи трубки-дилятатора, фиксированной к коже. Рану послойно ушивали. Дыхание осуществлялось через трахеостому. Через 1,5-2 месяца трубку-дилятатор удаляли через рот при помощи прямой опорной микроларингоскопии. Одновременно удаляли и грануляции, появившиеся в гортани вследствие длительного ношения трубки. При малых сроках ношения трубки-дилятатора эффект лечения оказывался нестойким, половины гортани приближались друг к другу и стеноз возобновлялся. Так у 3 больных после удаления трубки-дилятатора через 3 недели после операции, стеноз возобновлялся в течение 6-8 месяцев, по поводу чего им повторно произведена операция.Surgical treatment consisted of dissection of the anterior and posterior walls of the larynx along the midline, dilution of the larynx halves to the sides, and fixation in this state using a dilatator tube fixed to the skin. С целью надежной фиксации половин гортани в расширенном состоянии и сокращения сроков ношения трубки-дилятатора, 4 больным вышеприведенная операция дополнена подшиванием пластин щитовидного хряща к большим рогам подъязычной кости в позе растяжения. У всех оперированных больных дыхание через естественные пути восстановилось, они деканулированы.
