Abstract

FIELD: medicine.

SUBSTANCE: invention refers to medicine, specifically to reconstructive surgery. The first stage involves acute dermatesion to restore a chin cacitrices with normal mobilised skin of submaxillary region and anchoring a flap edges along the line of labiomental sulcus. At the second stage the cicatrices are restored within labium with a full-thickness skin graft.

EFFECT: method provides formation of labiomental sulcus, improved cosmetic effect of plasty.

Способ устранения рубцового выворота нижней губы осуществляют следующим образом. На первом этапе иссекают деформирующие рубцы на подбородке. Образовавшийся раневой дефект после мобилизации краев раны с подкожной клетчаткой в подчелюстной области закрывают неизмененными кожными покровами путем острой дермотензии. Мобилизованный лоскут укладывают на подбородке и край его подшивают глубокими швами по линии губно-подбородочной борозды. На втором этапе через 4-6 месяцев при благоприятном течении послеоперационного периода иссекают рубцы, деформирующие нижнюю губу, а образовавшийся раневой дефект закрывают полнослойным кожным трансплантатом.The way to eliminate cicatricial eversion of the lower lip is as follows. Клинический пример. Больная Л., 19 лет, и.б. № 209979, поступила в институт по поводу рубцовой деформации лица после ожога (гипертрофические рубцы нижней части лица и рубцовый выворот нижней губы). На первом этапе оперативного вмешательства 4/XII-03 г. выполнено иссечение рубцов, пластика местными тканями по поводу гипертрофических рубцов лица. В подчелюстной области по границе здоровой кожи и грубых гипертрофических рубцов произведен разрез. Мобилизованы края раны в области шеи до 6-8 см. Произведено иссечение рубцов шириной до 4 см. Образовавшийся раневой дефект закрыт мобилизованной по принципу острой дермотензии кожей, причем в подбородочной области лоскут уложен по губно-подбородочной борозде. 1/IV-04 произведен второй этап операции, на котором иссечен грубый гипертрофический рубец в области нижней губы размером 1×6 см, деформирующий красную кайму. Пересечены глубокие рубцы, губа выведена в положение гиперкоррекции. Образовался раневой дефект 4×6 см, который закрыт полнослойным кожным трансплантатом, взятым на внутренней поверхности правого плеча. Приживление трансплантата составило 100%. The eversion of the lower lip is eliminated.

Способ устранения рубцового выворота нижней губы после ожога дает возможность сформировать губно-подбородочную борозду, приближающуюся по внешнему виду к естественной. Использование кожной пластики путем острой дермотензии обеспечивает надежное закрытие подбородочной области. A method of eliminating cicatricial eversion of the lower lip after a burn, consisting in the removal of constricting scars and restoration of the skin, characterized in that the surgical procedure is carried out in two stages, the first replacing the scars in the chin area with unchanged mobilized skin from the submandibular zone by acute dermatension with hemming the edges of the flap along the labio-chin groove, and in the second stage, the scars in the lower lip are replaced by a full-layer skin graft.
