Abstract
Description
The invention relates to medicine, namely to surgery, and can be used to unload portal blood flow in cirrhosis of the liver with compensated and subcompensated portal hypertension syndrome.
Portal hypertension is a common and most formidable complication of cirrhosis. The treatment of patients with portal hypertension syndrome presents a certain complexity and is an urgent problem of surgical hepatology. There are many methods for surgical correction of portal hypertension syndrome, the essence of which boils down to one thing - the creation of a portocaval anastomosis between the portal and one of the vena cava (usually the inferior vena cava). However, all these methods are complex, have a rather high mortality rate, and require the knowledge and skills of the “vascular sutures” of surgeons.
A known method of treating children with portal hypertension syndrome is that during surgery, including omentohepatophrenopexy, 1/3 of the liver surface in contact with the omentum is subjected to cryotherapy, for which the temperature is reduced to 0.5 cm in the liver parenchyma to + 20 ° C (patent RU 2033759, 1995). The invention allows to reduce portal hypertension.
A known method of creating a portocaval shunt, which can be used in surgical treatment of portal hypertension syndrome that occurs with cirrhosis of the liver (patent RU 2062050, 1996). The distal end of the umbilical vein, previously mobilized together with a round ligament and awakened along its entire length, is sutured into the side wall of the inferior vena cava mobilized according to Kocher, and the suture area of the anastomosis is covered with the peritoneum of the round ligament of the liver. This method is technically simple to implement, can significantly reduce the invasiveness of the operation and reduce the time of surgery.
There is a method of treating children with portal hypertension syndrome, which consists in performing cryomentohepatofrenopexy, and exposing the portion of the omentum intended for pecia to cryogenic exposure, lowering the temperature to -5 ° C in the anterior plate of the omentum to a depth of 0.1 cm (Patent RU 2,128,951, 1999). The method allows to restore the pital blood flow through the liver parenchyma.
- М. - 2007].The prototype of the invention is a method for creating a collateral vascular bed between the portal and inferior vena cava by suturing a large omentum to a scarified capsule of the liver - omentohepatopexy in patients with diffuse liver diseases, in particular with liver cirrhosis. The method is carried out as follows, a patient under general anesthesia has an upper median laparotomy, liver capsules are scarified on the diaphragmatic surface of the liver, then a strand of a large omentum is pulled and the catgut is stitched between them. After this, the surgical wound is sutured in layers. Subsequently, due to nonspecific inflammation and granulation processes, adhesions are formed in this area with the formation of vascular connections between the liver and omental vessels, and roundabout blood flow is formed, which can lead to a certain unloading of the portal pool. However, the formed capillary and venulo-venular anastomoses do not provide sufficient relief of portal hypertension and the technique becomes ineffective [Diagnosis and treatment of diffuse liver diseases. Ivashkin V.T., Yushchuk N.D. / Methodological manual for doctors. - M. - 2007].
The objective of the invention is to develop a method of omentohepatopexy in cirrhosis of the liver with compensated and subcompensated portal hypertension syndrome.
The technical result when using the invention is the decompression of the portal blood flow system due to the formation of stable vascular connections between the liver and omental vessels.
The specified technical result is achieved due to the fact that in the method of treating portal hypertension syndrome with liver cirrhosis, including laparotomy, omentohepatopexy, a bed is formed in the liver by ultrasonic cavitation of the liver tissue with the release of blood vessels, and a lock of large omentum is laid in the formed bed.
The proposed method is as follows. Under anesthesia, a mid-median laparotomy is performed, a capsule is pre-incised on the diaphragmatic surface of the liver and ultrasound cavitation of the liver tissue is performed with a Byron Medical Ly Sonix 3000 apparatus. Inc. secrete hepatic vessels. Next, a strand of a large omentum is laid in the formed bed and stitched with vikril between them. After this, the surgical wound is sutured in layers. Subsequently, due to nonspecific inflammation and granulation processes, adhesions are formed in this area with the formation of vascular connections between the liver and gland vessels, and roundabout blood flow is formed, which leads to unloading of the portal pool.
The proposed procedure is high-tech and does not present technical difficulties. Thus, within one month, patients have decompression of the portal blood flow system and normalization of the function of the organs of the hepatobiliary and cardiovascular systems.
The authors in the available scientific, medical and patent literature did not find information about the fame of bed formation by ultrasonic cavitation of liver tissue, the allocation of large hepatic vessels, laying in the bed strands of a omentum when performing omentohepatopexy. Thus, the claimed invention meets the criterion of "novelty."
The studies of the authors found that the combination of the distinguishing features of the method provides decompression of the portal system of blood flow due to the formation of stable vascular connections between the hepatic and omental vessels.
Example. Patient I. 48 years. The patient underwent a clinical and laboratory examination and the patient was diagnosed with “Cirrhosis of the liver complicated by portal hypertension syndrome”. After appropriate preoperative preparation under general anesthesia, a mid-mid laparotomy was performed, a capsule was pre-incised on the diaphragmatic surface of the liver and ultrasonic cavitation of the liver tissue was used with Byron Medical Ly Sonix 3000. Inc. and isolated the largest hepatic vessels. Then a strand of a large omentum was pulled into the formed bed and flashed between them with a catgut. After that, the surgical wound was sutured in layers. The postoperative period was uneventful, the patient was discharged home for 15 days.
In this way, 10 patients with cirrhosis of the liver complicated by portal hypertension syndrome were treated.
