Abstract
Description
Предлагаемое изобретение относится к медицине, а именно к хирургии, травматологии и ортопедии, и может быть использовано при лечении дефектов кожи и подлежащих тканей при глубоких пролежнях, пролежневых язвах, глубоких ожогах и обширных ранах.The present invention relates to medicine, namely to surgery, traumatology and orthopedics, and can be used in the treatment of skin defects and underlying tissues with deep bedsores, pressure ulcers, deep burns and extensive wounds.

Известен способ пластики мягких тканей методом дозированного растяжения (Амирасланов Ю.А. с соавт. // Врач. - 1993. - №2. - С.25-27). Способ заключается в том, что после полноценной ранней хирургической обработки выполняют мобилизацию краев раны с формированием кожно-жировых, кожно-фасциальных и кожно-мышечных лоскутов. Вдоль раны с двух сторон на расстоянии 1,5-2 см от края субдермально путем вкола проводят спицы Киршнера. Через кожу в полость раны, огибая спицы, проводят лигатуры таким образом, что, проходя над раневой поверхностью, лигатура на противоположном краю раны захватывает вторую спицу, выходит на поверхность кожи и равномерно подтягивает ткани, сближая края раны. Количество лигатур на спицах зависит от размеров раны. При необходимости лигатуры можно развязать и провести перевязку раны. Растяжение тканей продолжают постепенно до соприкосновения краев раны. В качестве ближайшего аналога принят способ кожной пластики, основанный на дерматотензии, описанный Гусейновым А.Г. и Гусейновым К.Г. (Гусейнов А.Г., Гусейнов К.Г. Способ закрытия ран при лечении открытых и огнестрельных переломов нижних конечностей. // Хирургия. - 2005. - №6. - С.51-54.). Способ заключается в следующем. Вдоль краев раны проводят пару спиц Киршнера таким образом, что кожу по краям раны дробно нанизывают на спицы (см. приложение, фиг.1А). Длина под- и надкожных отрезков спиц при этом должна быть приблизительно одинаковой. Концы спиц скусывают и загибают, а под их надкожные отрезки проводят капроновую нить (фиг.1Б). С помощью последующего натяжения концов нити осуществляют сближение спиц и краев раны (фиг.1 В). Во избежание трофических расстройств и прорезывания кожи на спицах натяжение нити должно быть умеренным. Концы нити связывают между собой провизорным узлом. Через некоторое время натяжение нити ослабевает из-за эластичных свойств кожи и эффекта Г.А.Илизарова, за счет которого происходит регенерация и рост тканевых структур под действием напряжения растяжения. Через каждые двое суток производят развязывание провизорного узла концов нити, повторное ее натяжение и завязывание с возобновлением первоначального натяжения, что приводит к поэтапному сближению краев раны и уменьшению ее площади. При достижении полного закрытия раны накладывают вторичные швы с сохранением натяжения нити еще на некоторое время (фиг.1Г). Сущностью предлагаемого способа лечения ран спицевым дозированным растяжением тканей является постепенное сближение краев раны и их сшивание путем проведения дугообразных отрезков спиц длиной 5-8 см вдоль всего протяжения краев раны, прошивание краев раны лигатурами с захватом швами проведенных отрезков спиц и их постепенное сближение поэтапным натяжением лигатур. При этом количество отрезков спиц определяется размерами раны, каждый отрезок спицы проводят через кожу, подкожно-жировую клетчатку, фасции, мышцы одним вколом в направлении снаружи - внутрь и выколом изнутри - наружу, с внутритканевым расположением всего отрезка спицы, кроме его концов, на расстоянии 2-3 см от края раны. Техническим результатом предлагаемого способа лечения ран спицевым дозированным растяжением тканей являются: равномерная тракция кожи, подкожно-жировой клетчатки, подлежащих фасций и мышц при закрытии глубоких дефектов покровных тканей, что обеспечивает условия для закрытия глубоких ран полнослойным тканевым массивом; возможность корректного захвата отрезками спиц краев раны через все слои при сложной конфигурации раны; возможность дифференцированного силового тракционного воздействия на различные участки краев раны при их постепенном сближении, что позволит добиваться сближения краев на всем протяжении раны, независимо от ее конфигурации. Способ осуществляют следующим образом. Вокруг раны, на расстоянии 2-3 см от ее краев проводят отрезки дугообразно изогнутых спиц Киршнера длиной 5-8 см через кожу, подкожно-жировую клетчатку, подлежащие фасции и мышцы таким образом, что после проведения отрезки спиц оказываются в тканевом массиве и только их концы выступают над поверхностью кожи. Место вкола на коже каждого следующего отрезка спицы располагают примерно на середине длины ранее проведенного отрезка спицы. Концы отрезков спиц загибают (фиг.2Д). Отрезки спиц захватывают в мягких тканях подфасциально лигатурой из толстой хирургической нити, которую проводят с помощью крутой иглы. При этом каждая лигатура охватывает отрезок спицы путем введения ее со стороны стенки раны, проведения вокруг отрезка спицы и выведения наружу через ту же стенку раны в непосредственной близости от места ее введения (фиг.2Е). С помощью последующего натяжения концов лигатур, соответствующих противоположно расположенным отрезкам спиц, осуществляют сближение отрезков спиц и краев раны. Во избежание трофических расстройств и прорезывания тканей на отрезках спиц, натяжение лигатур должно быть умеренным. Концы нитей завязывают провизорными узлами. Через некоторое время натяжение лигатур ослабевает из-за эластичных свойств кожи и эффекта Г.А.Илизарова, за счет которого происходит регенерация и рост тканевых структур под действием напряжения растяжения. Через каждые двое суток производят развязывание провизорных узлов, повторное натяжение и завязывание концов лигатур с возобновлением первоначального натяжения, что приводит к поэтапному сближению краев раны и уменьшению ее площади (фиг.2Ж). При этом степень натяжения нитей и скорость сближения краев раны в различных ее участках может быть неодинаковой, что обусловлено различной эластичностью кожи и подлежащих тканей в отдельных участках раны, а также сложной конфигурацией раневого дефекта. При достижении контакта краев раны, на ее закрытых участках накладывают вторичные швы с сохранением натяжения лигатур, сближающих отрезки спиц еще на некоторое время (фиг.2, 3). По мере заживления и укрепления рубца на закрытых участках раны производят удаление соответствующих лигатур и отрезков спиц и снятие швов. Таким образом, постепенно раневой дефект оказывается полностью закрытым.The method is as follows. In order to avoid trophic disorders and the eruption of tissues on segments of knitting needles, the ligature tension should be moderate. The ends of the threads are tied with provisional knots. In this case, the degree of tension of the threads and the speed of convergence of the edges of the wound in its various sections may be different, due to the different elasticity of the skin and underlying tissues in individual sections of the wound, as well as the complex configuration of the wound defect. Upon reaching the contact of the edges of the wound, in its closed areas impose secondary sutures while maintaining the tension of the ligatures, bringing together the segments of the spokes for a while (Fig.2, 3). As the scar heals and strengthens in closed areas of the wound, the corresponding ligatures and segments of the needles are removed and the sutures are removed. Thus, gradually the wound defect is completely closed.
Предлагаемый способ апробирован нами в клинических условиях при лечении 8 больных.The proposed method has been tested by us in clinical conditions in the treatment of 8 patients.
Пример: Больная С, 19 лет, госпитализирована 30.08.07 г.в нейрохирургическое отделение №3 ККБ №1 г.Краснодара с диагнозом: ПСМТ, поздний период, оскольчатый перелом Th5, нижний парапарез с нарушением функции тазовых органов, глубокий пролежень в проекции крестца. По данным рентгенографии, КТ и МРТ повреждение позвоночника имело нестабильный характер, сопровождалось посттравматической деформацией и стенозом позвоночного канала. Больной была показана декомпрессивно-стабилизирующая операция на грудном отделе позвоночника с коррекцией деформации на уровне Th4-Th6. Однако имеющий место глубокий пролежень в проекции крестца, размером 9,0×7,5 см, с наличием участков некротизированных тканей и гнойного отделяемого, требовал хирургической санации, что в позднем периоде ПСМТ является противопоказанием к выполнению декомпрессивно-стабилизирующей операции. 03.09.07 г. больная переведена в отделение гнойной хирургии, где было проведено лечение пролежня с использованием предлагаемого способа лечения ран спицевым дозированным растяжением тканей. 06.09.07 г. произведена операция - некрэктомия поверхности и краев пролежня, имплантация шести отрезков спиц через кожу, подкожно-жировую клетчатку, фасции, мышцы вдоль краев раны с наложением стягивающих лигатур. Количество отрезков спиц определялось размером раны. При этом отрезки спиц имели дугообразную форму и длину 5-8 см и были проведены вокруг раны на расстоянии 2-3 см от ее краев. Каждый отрезок спицы проводили через кожу, подкожно-жировую клетчатку, фасции, мышцы только одним вколом в направлении снаружи - внутрь и выколом изнутри - наружу, с внутритканевым расположением всего отрезка спицы, кроме его концов. Края раны были прошиты лигатурами из толстых нитей с захватом отрезков спиц, причем каждая лигатура охватывала отрезок спицы путем введения ее с помощью крутой иглы со стороны стенки раны, проведения вокруг спицы и выведения наружу через ту же стенку раны в непосредственной близости от места ее введения. В послеоперационном периоде, во время проведения перевязок, производили постепенное дозированное сближение отрезков спиц, имплантированных в противоположные края раны путем поэтапного натяжения лигатур. Темп сближения определялся мобильностью тканей вдоль краев раны и составлял примерно 6-8 мм 2-3 раза в неделю. В течение трех недель края раны были сопоставлены и адаптированы на всем протяжении, после чего наложены вторичные швы на кожу, а отрезки спиц и лигатуры через 12 дней после ушивания раны были удалены. В данном клиническом случае применение способа лечения ран спицевым дозированным растяжением тканей позволило добиться полноценного заживления обширного раневого дефекта кожи, подкожно-жировой клетчатки, фасций, мышц в проекции крестца в оптимальные сроки и получить хороший ближайший результат.Example: Patient C, 19 years old, was admitted to the neurosurgical department No. 3 of KKB No. 1 of Krasnodar on August 30, 2007 with a diagnosis of PSMT, late period, comminuted fracture Th 5 , lower paraparesis with impaired pelvic organs function, deep bedsore in the projection the sacrum. However, a deep bedsore in the projection of the sacrum, measuring 9.0 × 7.5 cm, with areas of necrotic tissue and purulent discharge, required surgical debridement, which in the late period of PSMT is a contraindication to performing decompression-stabilizing surgery. 09/03/07, the patient was transferred to the purulent surgery department, where a bedsore treatment was carried out using the proposed method for treating wounds with spoke dosed tissue stretching. 09/06/07, an operation was performed - necrectomy of the surface and the edges of the bedsore, implantation of six segments of the spokes through the skin, subcutaneous fat, fascia, muscles along the edges of the wound with the application of tightening ligatures. The number of spokes was determined by the size of the wound. Moreover, the segments of the spokes had an arcuate shape and a length of 5-8 cm and were held around the wound at a distance of 2-3 cm from its edges. Each segment of the knitting needle was passed through the skin, subcutaneous fat, fascia, muscles with only one injection in the direction from the outside - inside and inside from the inside out, with the interstitial arrangement of the entire segment of the spoke, except for its ends. The edges of the wound were stitched with ligatures of thick threads with the capture of segments of the knitting needles, each ligature covering a segment of the knitting needle by inserting it with a sharp needle from the side of the wound wall, passing around the knitting needle and leading out through the same wall of the wound in the immediate vicinity of the place of its introduction. In the postoperative period, during the dressings, a gradual metered approach of the segments of the spokes implanted in the opposite edges of the wound by phased tension of the ligatures was performed. The approach rate was determined by tissue mobility along the edges of the wound and was approximately 6-8 mm 2-3 times a week. Within three weeks, the edges of the wound were matched and adapted throughout, after which secondary sutures were applied to the skin, and segments of the spokes and ligatures 12 days after suturing the wound were removed. In this clinical case, the use of a method of treating wounds with spoke dosed stretching of tissues made it possible to fully heal an extensive wound defect of the skin, subcutaneous fat, fascia, muscles in the projection of the sacrum in optimal terms and get a good immediate result.
